The Risk Factors of Recurrent Ischemic Stroke (허혈성 뇌졸중의 재발과 연관된 위험인자)

  • To eveluate the risk factors which are related to recurrence of ischemic stroke, we selected subjects who were admitted to YNUH due to recurrent stroke and compared their risk factors with non-recurred group who suffered from single ischemic stroke. In the subjects, 55 of them are men and 22 were women and in the non-recurred groups, 84 of them were men and 40 were women, Subject's age ranged from 29 to 85 years(Mean 62, 5years), and non-recurred group's age ranged from 27 to 90 years(Mean 60, 7years), Peak incidence of ischemic stroke is in the 7th decade in both groups. Age and sex are not, statistically significant for recurrence of ischemic stroke. The patient's history of diabetes mellitus, myocardial infarction, atrial fibrillation, transient attack and type or site of ischemic stroke had no significant effect statistically on the recurrent ischemic stroke. However, when the patient had previous history of hypertension or systolic blood pressure more than 160mmHg and diastolic more than 95mmHg, there was substantial difference(P<0.05) between the two group in the recurrence of ischemic stroke. According to the above results, hypertension is most likely significant risk factor of the recurrence of ischemic stroke within 2years after initial one. Therefore, adequate treatment of the hypertension is important for the prevention of ischemic stroke. Further study is required for searching other risk factors.

Correlation of Posterior Echo Patterns and Histopathologic Features in Invasive Ductal Carcinoma of Breast (유방 침윤성 관상피암의 초음파 소견 중 후방 에코 양상과 조직소견의 상호 비교)

  • Traditionally posterior shadowing is regarded as a malignant criterion in the evaluation of breast mass by sonogram. But on the basis of our clinical experiences of breast sonogram, we often met a breast mass without posterior shadowing later confirmed breast carcinoma through pathologic examination. For the focus of what character of pathologic breast tissue influence the posterior shadowing in breast sonogram, we analyzed retrospectively the sonographic findings of 26 histologically proven invasive ductal carcinomas. Even though invasive ductal carcinoma is the only one of the many breast cancers, it represents the greater part of breast malignancy. The posterior echo pattern was compared with various histologic characteristics, such as the amount of connective tissue, degree of elastosis, necrosis, gross circumscription, accompanying inflammation, histologic differentiation, and mitotic index. Nine breast masses (35%) demonstrated posterior echo shadowing, while 17 masses (65%) showed enhancement. The tumors with posterior echo shadowing had more abundant connective tissue, increased elastosis, and poorly demarcated margin (p<0.05). Other histologic characteristics are not influence in posterior shadowing with significant in stastically. On the basis of our study, the phenomenon of posterior shadowing by sonogram is difficult to accept as a specific criterion for malignancy. It is only a phenomenon influenced by the amount of connective tissue volume and elastosis.

Legal Issues in Clinical Trial on Minor (미성년자 대상 임상시험에 관한 법적 문제점)

  • All forms of Clinical trial should be fully equipped with protection systems for experimental subjects considering their uncertainty and various risks. Existing laws have some regulations in pharmaceutical affairs act and medical device act. Nonetheless, there is a limit to protect the subjects considering law objective to perform administration of medicine. Furthermore, the clinical trial on minor has no direct regulations in pharmaceutical affairs act, but prescribes certain portion in clinical trial assessment guideline on infants or medicine clinical trial management standard, however there is a limit because that is just recommendation not having legal effectiveness. The legislative solution would be possible for legal problems of clinical trial on minor by examining treatment system on minor in organ transplant act and clinical trial on minor in other foreign laws stronger than usual medical practice in terms of degree of human body invasion. I suppose that the control system of clinical trial being done focusing on the pharmaceutical affairs act, medical device act and other guidelines in existing laws system should be resolved by legislating 'trial subject protection law', in addition, this would be well balanced in organ transplant act on protection system of minor organ donors. Furthermore, the judgement on the consent ability and spontaneity in clinical trial on minor should be judged considering maturity and mentality of minor by clinical trial institutional review board based on legislative solution mentioned above.

Diagnostic Sensitivity of Several Muscles in Repetitive Nerve Stimulation Test for Myasthenia Gravis (중증근무력증에서 반복신경자극검사시 각 근육들의 진단적 민감성)

  • Background: This study was undertaken to evaluate the diagnostic sensitivity of several muscles in repetitive nerve stimulation test (RNST) for myasthenia gravis (MG) patients. Materials and Methods: The study population consisted of 39 MG patients classified by modified Ossermann's classification. Using Stalberg's method, RNST was systematically performed in facial (orbicularis oculi and nasalis) and upper extremity (flexor carpi ulnaris, abductor digiti quinti and anconeus) muscles. Results: The significant electrodecremental response of RNST were noted in orbicularis oculi(58.9%), nasalis (51.3%), flexor carpi ulnaris(42%), anconeus(41%) and abductor digiti quinti muscles(27%). Among the 3 muscles of upper extremity(abductor digiti quinti, flexor carpi ulnaris and anconeus), the positive electrodecremental response of anconeus muscles was significantly higher than other two muscles(p<0.05) in type IIa, IIb and there were no statistical differences of the positive electrodecremental response between orbicularis oculi and nasalis muscles. The facial muscles showed more prominent decremental responses than upper extremity muscles in type I MG(p<0.05). In type IIa MG patients, there were no significant statistical differences between facial and upper extremity muscles but significant statistical differences among upper extremity muscles. In type IIb MG patients, there were no significant statistical differences in all tested muscles in spite of the increased positive electrodecremental response of RNST. Conclusion: On the basis of this study, RNST would be initially performed for the orbicularis or nasalis in type I MG and for the anconeus in type IIa or IIb MG.

Flight Test Hazard Identification (비행시험 위해요인 식별 연구)

  • Flight tests for aircraft development and certification have higher air accidents risk than other aerospace industries. Accordingly risk identification necessary for risk management in the safety management system is a very important factor in preventing similar air accidents during flight tests. But internationally issued accident statistics are usually confined to commercial transport aircraft and they do not reflect characteristics of flight tests, also databases to hazards identification during flight tests are not established or available in Republic of Korea. Therefore, we identified flight test procedures and traced the major causes of aviation accidents based on the statistics and preventive materials for risk management which were issued by international organizations and advanced countries of aviation. A total of 312 final reports classified as flight test among air accident/incident database held by U.S. NTSB were researched and about 200 flight test hazard were identified. The results of this study will be used as basic data for establishing the safety management system of the national comprehensive flight performance test site.

Evaluation of Treatment Response Using Diffusion-Weighted MAI in Metastatic Spines (척추 전이암에서 확산강조 자기공명 영상을 이용한 치료반응의 평가)

  • Background: The purpose of this study was to evaluate the usefulness of diffusion-weighted magnetic resonance imaging for monitoring the response to radiation therapy in metastatic bone marrow of the spines. Materials and Methods: Twenty-one patients with metastatic bone marrow of the spines were examined with MRI. Diffusion-weighted and spin-echo MRI were performed in 10 patients before and after radiation therapy with or without systemic chemotherapy, and performed in 11 patients after radiation therapy alone. Follow up spin-echo and diffusion-weighted MRI were obtained at 1 to 6 months after radiation therapy according to patients' condition. The diffusion-weighted imaging sequence was based on reversed fast imaging with steady-state precession (PSIF). Signal intensity changes of the metastatic bone marrows before and after radiation therapy on conventional spin-echo sequence MRI and diffusion-weighted MRI were evaluated. Bone marrow contrast ratios and signal-to-noise ratios before and after radiation therapy of diffusion- weighted MRI were analyzed. Results: All metastatic bone marrow of the spinal bodies were hyperintense to normal bone marrow of the spinal bodies on pretreatment diffusion-weighted MRI and positive bone marrow contrast ratios(p<0.001), and hypointense to normal spinal bodies on posttreatment diffusion-weighted MRI and negative bone marrow contrast ratios(p<0.001). The signal to noise ratios after treatment decreased comparing with those of pretreatment. Decreased signal intensity of the metastatic bone marrows on diffusion-weighted MRI began to be observed at average more than one month after the initiation of the radiation therapy. Conclusion: These results suggest that diffusion-weighted MRI would be an excellent method for monitoring the response to therapy of metastatic bone marrow of the spinal bodies, however, must be investigated in a larger series of patients with longer follow up period.

  • This case study introduces the process of developing the technology rating evaluation model for investment. The technology evaluation rating model for investment is a project that the Financial Services Commission and the Ministry of Commerce, Industry and Energy collaborated to expand the scope of technology finance from loan to investment. The technology evaluation model for investment was developed with the aim of predicting high growth companies. The model consists of a statistical model and an expert model. Here, statistical models were modeled by using logistic regression analysis. Expert models gathered opinions of experts and identified the weight of each evaluation item and set the model. The rating system of the model is composed of 10 grades. The distribution of the model was consistent with KTRS grade distribution. Interestingly, the emphasis is on technology and marketability. In the technology valuation grade model for the goddess, there is a considerable difference from the emphasis on managerial competence or business performance.

A Study on the 3rd Generation CPTED Process through Double Diamond (더블 다이아몬드를 적용한 3세대 CPTED 프로세스에 관한 연구)

  • The shortcomings of the previous Crime Prevention Through Environmental Design (CPTED) theory in practice have led to the limitations of the conventional crime prevention design process. The purpose of this study is to analyze the graftability of the third generation CPTED theoretical framework and the Double Diamond design process from a design perspective. Thus, the third generation CPTED visualization design process is proposed to be grafted with the Double Diamond design process. In the study, the propositions, goals, strategic approaches, and liveability principles of the third-generation theoretical framework are clarified according to the relevant studies by scholars. In addition, the study identifies the limitations of conventional crime prevention design processes in terms of practicality and effectiveness, based on corrections from research in different scientific fields. On this basis, the Framework of Innovation of the British Design Council was used to clarify the benchmarks and foundations for the grafting of the third-generation theoretical framework and the Double Diamond design process. In addition, the details of the grafting are analyzed and explained in terms of phase outputs and phase activities. The significance of the study is that the third-generation CPTED visualization design process, which is grafted to the Double Diamond design process, can help residents to implement behaviors that reflect their self-worth and create a state of engagement. As well, it can inform the collaboration between design and non-design staff in CPTED projects.

Legal and Inferential Studies on Importer's Risk in Investigation of Origin on FTA (원산지조사에 대한 수입자의 통제불가능한 위험)

  • This study purpose to examine the importer's risks that may arise from origin investigation by Customs authorities. We have drawn the important factors affecting the application of FTA preferential tariffs and divided the stages from the conclusion of the contract for the importer to the undergoing origin investigation. In addition, we demonstrate empirically that the risks that arise in areas where importers are difficult to control exist. As a management method of the uncontrollable risk from the importer, we have provided the methods that the seller stipulated the seller's responsibility in the trade contract, prepared for situations in which no one was responsible, and formulated a friendly and cooperative supply chain. Even if the seller's liability is clarified in the contract for sale, the risk of the investigation into the origin of the imported goods is not completely eliminated. This is because, under the current agreement and system, there is no way for the customs authority of the contracting party of the FTA to claim compensation for damages incurred by importers due to breach of agreement such as not returning the result of the origin verification. Importers are subject to customs duties, but there may actually be situations in which no one is responsible for them.

A Study on the Inclusion of Standard Terms under the CISG (CISG상 약관의 계약편입에 관한 연구)

  • It becomes a common feature of business practices in International Trade to use a standard terms for the formation of their contracts. However, because of differences in legal systems, business practices and so on in their own countries, there have been many conflicts and disputes happening between parties concerned in International Trade. The CISG, which has long been used as the governing law in many cases of International Trade, could not be free from those conflicting issues in its usage and application. This study analyzes the "Black Letter Rules" which was adopted by CISG Advisory Council in 2013 to provide an effective way of resolving the conflicting issues regarding the inclusion of standard terms in International Trade Contracts under the CISG. This study scrutinizes, the relevant rules and requirements for the inclusion of standard terms into a contract. It also deals with the offeror's duty of making clear reference to the standard terms, transmitting the contents of standard terms to the other party. As the other rules for the inclusion of standard terms, this study reviews the principle of denying the inclusion of standard terms after the formation of contracts, exclusion of surprising or unusual terms, preference of individually negotiated terms to the standard terms, contra preferentum rule and preference of the "knock-out rule" to "last-shot rule" in resolving the issue of so called, "Battle of Forms." Lastly, on the basis of analyzed opinion, this study suggests the practical implications for the people working at International Trade-related business sector to facilitate International Trade.
