Impact Factors and Validity of Blood Variables on Death in COVID-19 patient: Using Data of Korea Disease Control and Prevention Agency

  • 본 연구에서 우리는 COVID-19의 사망에 대한 영향요인과 혈액변수의 타당도를 제안하였다. 이 연구를 위하여 질병관리본부 중앙방역대책본부에서 제공하는 2020년 4월 30일까지 격리해제되었거나 사망한 COVID-19확진자 5628명의 임상역학정보를 활용하였으며, 자료는 R 3.4.1을 사용하여 분석하였다. 분석결과, 사망에 대한 위험요인으로 치매(HR 7.03), 많은 나이(HR 5.39), 높은 림프구(HR 4.66), 암(HR 4.27), 호흡곤란(HR 3.25), 만성폐쇄성 폐질환(HR 3.22), 의식변화(HR 3.13), 심장질환(HR 2.24), 높은 혈소판(HR 2.14), 비정상 이완기 혈압(HR 2.02), 열(HR 1.69)이 확인되었다. 사망에 대한 혈액검사의 타당도는 림프구, 헤모글로빈, 헤마토크리트, 혈소판, 백혈구 순으로 정확도가 높았다. 그러므로 COVID-19 환자를 치료할 때 사망의 위험요인으로 확인된 초기 임상적 특징 및 기저질환, 혈액검사의 소견을 고려하여 보다 효율적인 관리가 가능하도록 해야 할 것이다.

COVID-19 팬데믹 상황에서 초등학생 어머니의 변화된 양육 경험 (The Changed Parenting Experiences of Mothers of Elementary School Students in the Face of the COVID-19 Pandemic)

  • Purpose: The purpose of this qualitative study is to explore the changed parenting experiences of mothers of elementary school kids living in the aftermath of the COVID-19 outbreak in Korea. Methods: From July to August in 2020, 10 mothers parenting elementary school students under the COVID-19 were interviewed in depth. For data analysis, a phenomenological research methodology suggested by Colaizzi was used. Results: According to the analysis, the experiences of mothers consisted of four categories: the 'pain from the uncontrollable COVID-19', 'the problem of family relations due to the COVID-19 pandemic situation', 'standing firm against the drastic changes', and 'accepting the world changed by the COVID-19'. Conclusion: The findings in this study vividly represent the parenting experiences of mothers of elementary school students after the outbreak of COVID-19. As the pandemic persists, mothers parenting elementary school students feel complex emotions and experience exhaustion, but in the process, they found family members growing positively and trying to adjust to each other to overcome the crisis. To support positive adaptation to catastrophic situations, an institutional and political foundation is needed to develop a systematic crisis management program customized for mothers and families of elementary school students. The results of this study can be used as basic data when establishing national policies and support systems so that mothers parenting elementary school students can better adapt to and overcome crisis situations.

Related Factors to Korean Hospital Nurses in Burnout during the COVID-19 outbreak: A Systematic Review

  • 본 연구는 신종감염병인 COVID-19 팬데믹 상황에서 환자를 간호하는 국내 병원 간호사들의 소진과 관련요인들을 관리하기 위한 방안이 필요함을 제안하였고 COVID-19 유행 기간동안 국내 임상 간호사의 소진과 관련된 요인을 파악하기 위해 체계적 문헌고찰을 실시하였다. 체계적 문헌고찰을 위해 문헌 검색과 분석은 2019년 12월 1일부터 2022년 4월 30일까지 RISS, KISS, DBPIA, 교보스콜라의 데이터베이스를 사용하였고 '임상 간호사의 소진', '간호사의 소진'의 키워드를 사용하였다. 총 55편의 연구 논문이 검색되었고 자료선정기준에 따라 최종 7편의 논문이 선별되었다. 임상간호사 소진 정도와 소진과 관련된 요인을 살펴보면, COVID-19 동안 임상 간호사는 중등도 이상의 소진을 경험하였고 소진과 관련된 요인은 개인적 특성인 신체증상, 우울, 공감만족, 공감피로, 회복탄력성과 직업적 특성인 교대근무와 직무스트레스로 나타났다. 추후 임상 간호사 소진과 관련된 요인들을 조절할 수 있도록 역량 강화 프로그램 개발 및 적용하고 일관성 있는 간호인력 간호인력관리지관리지침이나 정책을 제시하여 간호사 부족 현상을 해소될 수 있기를 기대해 본다.

  • Objectives: The purpose of this study is to investigate changes in public health-related perceptions of residents of Gangwon province after the outbreak of Coronavirus disease-19(COVID-19) of the public. Methods: We performed paired T-test analysis to measure the change in public health-related perceptions before and after COVID-19. We also utilized generalized estimating equations to identify demographic factors correlated with public health-related perceptions. Results: The public perceived public health as 'All citizens can use medical care and protect/promote health.' The concept was the most popular, from 94.3% in 2019 to 95.5% in 2020. In addition, after COVID-19, residents of Gangwon province's satisfaction with medical services increased, but the overall level was not high. Among the eight essential healthcare needs after COVID-19, cardiovascular disease and injury services have emerged as preferred services. However, by sociodemographic factors, distinctive responses were detected. Conclusions: Through COVID-19, Gangwon residents' awareness of the public's health rights has increased. Those living in vulnerable areas or with unmet medical care, supported strengthening public health care. In addition, although medical satisfaction has increased, it is not satisfied, so listening to the voices of the population group with low satisfaction is essential. Lastly, since the necessity of essential health care may change due to specific events, the local government needs to plan health projects reflecting the needs of residents. Therefore, when designing the public health care strategy in Gangwon province, the local government should consider not only political factors but also environmental factors, demographic and conceptual factors.

코로나바이러스감염증-19 대유행 이후 응급실로 내원한 급성 일산화탄소 중독 환자의 특성 변화 (Changes in the characteristics of acute carbon monoxide poisoning patients who visited the emergency department during the COVID-19 pandemic)

  • Purpose: This study investigated the differences between patients with acute carbon monoxide (CO) poisoning who visited the emergency department (ED) before and during the coronavirus disease 2019 (COVID-19) pandemic. Methods: This was a single-center retrospective observational study. Patients with CO poisoning who visited the ED during the period from February 2020 to January 2023 were classified as the COVID-19 pandemic group, and those from February 2019 to January 2020 were classified as the non-pandemic group. Patients' medical records were reviewed, their demographic and clinical characteristics were compared, and the length of stay in ED was checked. The time from admission to the ED to the start of hyperbaric oxygenation (HBO) was defined as the door-to-HBO time, and this parameter was compared between both groups. Results: In total, 672 patients were included in this study. The proportion of intentional poisoning was significantly higher in the COVID-19 pandemic group than in the non-pandemic group (p=0.028). The proportion of intentional poisoning significantly increased in the 20- to 29-year-old age group during the COVID-19 pandemic (p<0.001). In addition, it took longer to initiate HBO in the COVID-19 pandemic group than in the non-pandemic group (p=0.001). Conclusion: These findings suggest that pandemics of infectious diseases, such as COVID-19, increase the proportion of intentional CO poisoning, and it may take longer to initiate HBO after visiting the ED. Efforts will be needed to decrease intentional CO poisoning and length of stay in ED.

코로나바이러스감염증-19 (COVID-19) 환자들의 사망관련 인자에 대한 연구: 체계적 문헌고찰 및 메타분석 (Predictors of Mortality in Patients with COVID-19: A Systematic Review and Meta-analysis)

  • Background: Most meta-analyses of risk factors for severe or critical outcomes in patients with COVID-19 only included studies conducted in China and this causes difficulties in generalization. Therefore, this study aimed to systematically evaluate the risk factors in patients with COVID-19 from various countries. Methods: PubMed, Embase, and Web of Science were searched for studies published on the mortality risk in patients with COVID-19 from January 1 to May 7, 2020. Pooled estimates were calculated as odds ratio (OR) with 95% confidence interval (CI) using the random-effects model. Results: We analyzed data from seven studies involving 26,542 patients in total in this systematic review and meta-analysis. Among the patients, 2,337 deaths were recorded (8.8%). Elderly patients and males showed significantly higher mortality rates than young patients and females; the OR values were 3.6 (95% CI 2.5-5.1) and 1.2 (95% CI 1.0-1.3), respectively. Among comorbidities, hypertension (OR 2.3, 95% CI 1.1-4.6), diabetes (OR 2.2, 95% CI 1.2-3.9), cardiovascular disease (OR 3.1, 95% CI 1.5-6.3), chronic obstructive pulmonary disease (OR 4.4, 95% CI 1.7-11.5), and chronic kidney disease (OR 4.2, 95% CI 2.0-8.6) were significantly associated with increased mortalities. Conclusion: This meta-analysis, involving a huge global sample, employed a systematic method for synthesizing quantitative results of studies on the risk factors for mortality in patients with COVID-19. It is helpful for clinicians to identify patients with poor prognosis and improve the allocation of health resources to patients who need them most.

CoAID+ : 소셜 컨텍스트 기반 가짜뉴스 탐지를 위한 COVID-19 뉴스 파급 데이터 (CoAID+ : COVID-19 News Cascade Dataset for Social Context Based Fake News Detection)

  • 최근 전 세계적으로 COVID-19이 유행하는 상황 속에서 이와 관련된 가짜뉴스가 심각한 사회적 혼란을 야기하고 있다. 이러한 배경에서 가짜뉴스를 정확하게 탐지하기 위해, 뉴스가 소셜 미디어를 통해 파급되는 과정과 같은 소셜 컨텍스트 정보를 활용하는 소셜 컨텍스트 기반 탐지 기법들이 널리 사용되고 있다. 그러나 대부분의 기 구축된 가짜뉴스 탐지를 위한 데이터들은 뉴스 자체의 내용 정보 위주로 구성되어, 소셜 컨텍스트 정보를 거의 포함하지 않는다. 즉, 이 데이터들에는 소셜 컨텍스트 기반 탐지 기법을 적용할 수 없으며, 이러한 데이터의 한계는 가짜뉴스 탐지 연구 분야의 발전을 저해하는 방해 요소이다. 본 논문은 이러한 한계를 극복하기 위해, 기존의 저명한 가짜뉴스 데이터인 CoAID 데이터를 기반으로, 소셜 컨텍스트 정보를 추가적으로 수집하여, CoAID 데이터의 뉴스 내용 정보와 해당 뉴스들의 소셜 컨텍스트 정보를 모두 포함하는 CoAID+ 데이터를 구축한다. 본 논문에서 구축한 CoAID+ 데이터는 기존의 대부분의 소셜 컨텍스트 기반 탐지 기법들에 적용될 수 있으며, 향후 새로운 소셜 컨텍스트 기반 탐지 기법들에 대한 연구도 더욱 활성화시킬 수 있을 것으로 기대된다. 마지막으로, 본 논문은 다양한 관점에서 CoAID+ 데이터를 분석하여 진짜뉴스와 가짜뉴스의 파급 패턴 및 키워드에 따른 파급 패턴도 파악하여 소개한다.

코로나바이러스감염증-19와 일부 가축전염병 방역소독제품의 함유성분 및 유해물질 조사 (Investigation of Ingredients and Hazardous Substances in Disinfectants Used against COVID-19 and Some Livestock Diseases)

  • Objectives: The Coronavirus Disease 2019 (COVID-19) pandemic has caused the death of 740,000 people around the world as of August 12, 2020. Foot-and-Mouth Disease, Avian Influenza, and African Swine Fever are serious livestock diseases. Government agencies in Korea have provided ingredient information and usage instructions for disinfectants used to counter those infectious diseases. The purpose of this study was to provide information on the chemical ingredients in disinfectant products used against COVID-19 and certain livestock diseases. Methods: We collected information from the Korean government. The Central Disaster Management Headquarters and Central Disease Control Headquarters provided information on disinfectant products used against COVID-19. The Animal and Plant Quarantine Agency of Korea provided information on efficacy-certified disinfectant products for use against selected livestock diseases. Health hazard and environmental hazard information on the ingredients in the disinfectants was collected from the Korea Occupational Safety & Health Agency's Material Safety Data Sheets, and toxicity value information was collected from United States Environmental Protection Agency's CompTox Chemicals Dashboard. Results: There were 76 COVID-19 disinfectant products in use, and the most common ingredients were benzalkonium chloride (51%), alkylbenzyl dimethyl ammonium (30%), and ethanol (3%). There were 216 livestock disease disinfectant products comprised of 89 acidic, 88 oxidic, 30 aldehydic, three alkaline, and six other products. Among the 49 active ingredients used in the disinfectants that were investigated, health and environmental hazard information was provided for many of them, but only 20 chemicals had official toxicological information. Conclusion: Since the disinfectants included numerous chemicals, an understanding of their chemical characteristics could be critical to prevent unintended human or environmental exposure.

코로나바이러스감염증-19 (COVID-19) 백신 접종 후 재발한 대상포진 치험 1례 (A Case of Recurrent Herpes Zoster after Coronavirus Disease-2019 (COVID-19) Vaccination)

  • Objectives : This study reports a 42-year old female inpatient who visited for recurrent herpes zoster on the right hand and right side of the face on 5 days after coronavirus disease 2019 (COVID-19) vaccination. She already had 2 times of herpes zoster prior to this outbreak. Methods : During 10 days of hospitalization, both treatments were simultaneously applied to her. For Korean medical treatment, acupuncture, herbal medicine including Sipjeondaebo-tang, and Hominis Placenta pharmacopuncture were mainly used. Collaborating with internal medicine of our hospital, essential medications for herpes zoster including antihistamine, corticosteroid, and acyclovir were administered. To assess symptoms, taking photos and numerical rating scale(NRS) were used. Results : On the 3rd day of hospitalization, erythema on the right hand disappeared. On the 4th day, discomfort arisen by zoster lesions dropped from NRS 8, which was initially evaluated, to NRS 0. Facial lesion also steadily improved. At the moment of discharge, erythema on the right side of oral angle disappeared and mild traces of vesicular lesions remained on the right hand. Conclusions : Considering her medical history, the recent recurrent herpes zoster was suspected as an adverse event of COVID-19 vaccination. Unlike general course of herpes zoster, the skin lesions and discomfort were rapidly improved owing to the collaborated treatment during the 10 days. This study may be the first literature on herpes zoster following COVID-19 vaccination in Korea.

간호대학생의 비대면 수업 관련 자기주도 학습능력 영향요인 (Factors Influencing Nursing Students' Self-directed Learning Ability Related to Online Classes)

  • 코로나바이러스감염증-19로 인해 교육관련 디지털 콘텐츠에 대한 관심이 증가하고 있으며 학습자의 자기주도 학습능력의 중요성이 커지고 있다. 본 연구는 COVID-19로 인해 비대면 수업을 경험한 간호대학생의 자기주도 학습능력에 영향을 미치는 요인을 파악하기 위해 수행되었다. 연구대상자는 COVID-19로 인해 한 학기 이상 비대면 수업을 경험한 간호대학생 115명이며, 2020년 11월 18일부터 12월 1일까지 온라인설문을 통해 자료를 수집하였다. 대상자의 자기주도 학습능력에 통계적으로 유의한 영향을 미치는 요인은 감성지능의 하부요인 '감성활용' (β=.42, p<.001), 회복탄력성의 하부요인 '통제성' (β=.28, p=.001) 및 비판적 사고성향의 하부요인 '객관성' (β=.27, p<.001)이었고, 설명력은 62.0%이었다. 간호대학생의 감성지능 향상을 위한 연구와 중재가 지속적으로 필요하며 교육 컨텐츠 제작시에도 이를 고려하기를 제언한다.
