성견 3면 골내낭에서 calcium sulfate를 calcium carbonate와 혼합이식 및 차단막으로 사용시 치주조직의 치유효과 (THE EFFECT OF CALCIUM SULFATE WITH CALCIUM CARBONATE GRAFT AND CALCIUM SULFATE BARRIER ON THE HEAUNGOF 3-WALL INTRABONY DEFECTS IN DOGS)

  • Various bonegraft materials and the technique of guided tissue regeneration have been used to regenerate lost periodontal tissue. Calcium sulfate has been known as a bone graft material because of good biocompatibility, rapid resorption and effective osteoinduction. It has been known that calcium sulfate works as a binder to stabilize the defect when it is used with synthetic graft materials. The effects on the regeneration of pericxiontal tissue were studied in dogs after grafting 3-wall intrabony defects with calcium carbonate and calcium sulfate and covering with calcium sulfate barrier. The 3-wall intrabony defectstdmm width, 4mm depth, 4mm length) were created in anterior area and treated with flap operation alone(contol group), with porous resorbable calcium carbonate graft alonetexperirnental group 1), with calcium sulfate graft alonetexperimental group 2) and with composite graft of 80% calcium carbonate and 20% calcium sulfate with calcium sulfate barriertexperimental group 3). Healing responses were histologically observed after 8 weeks and the results were as follows: 1. The alveolar bone formation was $0.59{\pm}0.19mm$ in the control group, $1.80{\pm}0.25mm$ in experimental group 1, $1.61{\pm}0.21mm$ in experimental group 2 and $1.94{\pm}0.11mm$ in experimental group 3 with statistically significant differences between control group and all experimental groups(P<0.05). There were statistically significant differences between experimental group 1 and group 2 (P<0.05). 2. The new cementum formation was $0.48{\pm}0.19mm$ in the control group. $1.72{\pm}0.26mm$ in experimental group 1, $1.43{\pm}0.17mm$ in experimental group 2, $1.89{\pm}0.15mm$ in experimental group 3 with statiscally significant differences between control group and all experimental groups (p<0.05). There were statistically significant differences between experimental group 1 and group 2, and between experimental group 2 and group 3(P<0.05). 3. The length of junctional epithelium was $1.61{\pm}0.20mm$ in the contol group, $0.95{\pm}0.06mm$ in experimental group 1, $1.34{\pm}0.16mm$ in experimental group 2, $1.08{\pm}0.11mm$ in experimental group 3 with statiscally significant differences between control group and experimental group 1. and btween control group and experimental group 3(p<0.05). There were statistically significant differences between experimental group 1 ,and group 2, and between experimental group 2 and group 3(P<0.05). 4. The connective tissue adhesion was $1.67{\pm}O.20mm$ in the control group, $1.33{\pm}0.24mm$ in experimental group 1. $1.23{\pm}0.16mm$ in experimental group 2, $1.08{\pm}0.14mm$ in experimental group 3 with statistically significant differences between control group and all experimental groups(p<0.05). There were nostatistically significant differences between all experimental groups. As a result, epithelial migration was not prevented when calcium sulfate was used alone, but new bone and cementum formation were enhanced. Epithelial migration was prevented and new bone and cementum formation were also enhanced when calcium carbonate was used alone and when both calcium carbonate and calcium sulfate were used.

치주 골내낭에서 anorganic bovine-derived hydroxyapatite matrix(ABM)/cell binding peptide(P-15)의 임상적효과 (Clinical effects of combination anorganic bovine-derived hydroxyapatite matrix(ABM)/cell binding peptide (P-15) in periodontal intrabony defects)

  • The ultimate goal of periodontal therapy is the regeneration of periodontal tissue which has been lost due to destructive periodontal disease. To achieve periodontal regeneration, various kinds of methods have been investigated and developed, including guided tissue regeneration and bone graft. Bone graft can be catagorized into autografts, allografts, xenografts, bone substitutes. And materials of all types have different biological activity and the capacity for periodontal regeneration, but ideal graft material has not been developed that fits all the requirement of ideal bone graft material. Intensive research is underway to identity, purify, synthesize a variety biologic modulators that may enhance wound healing and regeneration of lost tissues in periodontal therapy. The present study evaluates the effects of ABM/P-15 on the periodontal regeneration in intrabony defects of human. We used thirty four 2-wall or 3-wall osseous defects in premolars and molars of chronic peridontitis patient that have more than 5mm pockets and more than 3mm in intrabony defect. 12 negative control group underwent flap procedure only, 11 positive control group received DFDBA graft with flap procedure, and 11 experimental group received ABM/P-15 graft with flap procedure. The changes of probing pocket depth, loss of attachment and bone probing depth following 6months after treatment revealed the following results: 1. The changes of probing pocket depth showed a statistically significant decrease between after scaling and 6months after treatment in negative control(2.0${\pm}$0.9mm), positive control(3.0${\pm}$0.9mm), and experimental group (3.4${\pm}$1.5mm) (P<0.01). Significantly more reduction was seen in experimental group compared to negative control group (P<0.05). 2. The changes of loss of attachment showed a statistically significant decrease between after scaling and 6months after treatment in positive control(2.0${\pm}$0.6mm), and experimental group (2.2${\pm}$l.0mm) except negative control group(0.1${\pm}$0.7mm) (P<0.01). Significantly more reduction was seen in both experimental and positive control group compared to negative control group(P<0.05). 3. The changes of bone probing depth showed a statistically significant decrease between after scaling and 6months after treatment in positive control(2.7${\pm}$l.0mm), and experimental group (3.4${\pm}$1.3mm) except negative control(0.l${\pm}$0.9mm) (9<0.01). Significantly more reduction was seen in both experimental and positive control group compared to negative control group (P<0.05). The results suggest that the use of ABM/P-15 in the treatment of periodontal intrabony defects can reduce loss of attachment and bone probing depth more than flap operation only. It suggests that ABM/P-15 may be an effective bone graft material for the regeneration of periodontal tissue in intrabony defects.

강릉시 자활근로사업 참여자 대상 치위생 과정 사례보고 (Dental Hygienist-Led Dental Hygiene Process of Care for Self-Support Program Participants in Gangneung)

  • 본 연구는 강릉시에 소재한 자활센터 자활사업 참여자를 대상으로 하여 대상자의 복합적인 위험요인을 고려한 맞춤형 치위생 과정을 통해 구강건강상태 및 행태의 변화 과정 및 결과의 사례를 분석함으로써, 치과위생사 주도의 치위생 과정에 대한 임상적 근거를 마련하고 임상 현장 및 지역사회사업에서의 치위생 과정 적용 가능성을 검토하고자 수행하였다. 본 연구에서는 전체 모집단 80명 중 연구참여에 동의한 44명을 대상으로 치과위생사 면허를 소지한 교수 3인이 전신질환 여부(당뇨병, 심혈관계질환 등), 치은 상태 및 치석 여부, 구강위생상태 등의 기준으로 구강 내 사정을 실시하였고, 구강건강위험군으로 선발된 21명 중 개인적인 사정과 치과의사 치료 의뢰 등으로 인해 중도 탈락한 13명을 제외하고 치위생계획에 따라 모든 과정을 종료한 8명을 최종 연구대상자로 선정하였다. 본 연구에서는 수행자 간 일치도 평가를 거친 치과위생사 4인이 자활센터 자활사업 참여자 8인을 대상으로 치위생 과정을 수행하였다. 대상자별 전담 치과위생사를 선정하여 대상자 중심의 맞춤형 관리를 제공하였고 모든 치위생 과정 내용은 치위생관리 기록부에 기록하여 치위생 과정 전 후에 따라 회차별로 평가한 임상 지표 및 치위생 중재에 따른 행동 변화 등을 비교 분석하였다. 복합적인 건강위험요인을 동반한 취약계층인 자활사업 참여 대상자의 치위생 과정을 분석한 결과, 인간욕구이론에 기반하여 대상자 중심의 목표를 수립하고, 전담 치과위생사와의 지속적인 신뢰관계 형성에 기반하여 행동 변화를 위한 중재를 실시하며, 회차별 대상자의 구강건강상태에 따른 맞춤형 전문가구강위생관리를 실시함에 따라 긍정적인 행동 변화와 함께 유의미한 임상적 변화를 확인하였다. 범이론 모형에 근거한 행동 변화 중재에 따라 칫솔질 방법 및 빈도의 변화가 나타났고, 치간칫솔을 사용한 치간관리의 변화를 확인하였다. 개인별 치위생 과정에 따른 구강상태를 비교한 결과 치위생 과정을 수행한 후 탐침 시 치은출혈 치아 보유율과 4 mm 이상 치주낭 보유율은 감소하였고, 변형 O'Leary index는 향상되었다. 치위생 과정은 치과의료에서 환자/대상자 중심의 가치를 실현하고 질병의 예방과 구강건강증진을 지향함에 따라 임상 현장 및 지역사회에서 지속적으로 치위생 과정이 적용될 수 있는 시스템을 구축함으로써 구강건강증진을 통한 구강건강 관련 삶의 질 향상과 더불어 구강건강의 사회경제적 불평등을 줄이는 데 기여할 수 있을 것으로 생각된다.

랫드 각막에서 Vascular Endothelial Growth Factor(VEGF)로 유발시킨 신생혈관에 대한 $As_4O_6$의 혈관신생 억제효과 (Antiangiogenic Effect of $As_4O_6$ on the Angiogenesis Induced by Vascular Endothelial Growth Factor (VEGF) in the Rat Cornea)

  • 본 연구는 VEGF로 유발된 랫드 각막 미세낭 모델에서 As₄O/sub 6/와 As₂O₃ 혈관신생 억제효과를 비교하기 위해 실시하였다. 20ng VEGF가 함유된 펠렛을 이식하여 혈관신생을 유도하였다. 안과 검사후 정상적인 각막을 가진 Sprague-Dawley 랫드의 안구 60안을 이용하여 각 20안씩 무작위로 선정하여 대조군(비투약군), As₂O₃군(50mg/kg As₂O₃, 경구투여, 1일 1회), 그리고 As₄O/sub 6/군(50mg/kg As₄O/sub 6/, 경구투여, 1일 1회)으로 나누어 실시하였다. 이식이 완료된 후, 신생혈관의 개수, 길이 및 신생혈관의 각도를 미세수술현미경하에서 이식 3일째부터 9일째까지 매일 검사하였다. 신생혈관의 넓이는 신생혈관의 길이와 각도를 수학적인 공식에 적용하여 계산하였다. 실험 기간동안 As₄O/sub 6/군의 신생혈관 개수, 길이, 각도 및 넓이가 대조군과 As₂O₃군에 비하여 유의성 있게 억압되었다. (P<0.05). 이러한 결과 As₄O/sub 6/는 랫드 각막에서 VEGF에 의해 유도된 신생혈관에 As₂O₃보다 더 뛰어난 억제효과를 가지고 있음을 보여주었다.
