구강점막에 발생하는 종양성 병변으로는 대표적으로 구강점막암(oral mucous membrane cancer), 유두종(papilloma),혈관종(hemangioma), 임파관종(lymphangioma),섬유종(fibroma)등이 있는데 이들 병변들은 구강내 협점막, 치은, 경구개, 연구개, 구강저, 혀 등에 궤양이나 증식성 병소로 나타나서 동통, 출혈, 치아동요, 종창 등의 증상을 유발시킨다. 이러한 병변들에 대한 명확한 치료와 예후를 결정하기 위해서는 무엇보다도 이들 병변들에 대한 정확한 소견에 근거한 진단이 우선되어야 할 것이다. 따라서 본 장에서는 정확한 진단과 치료를 위해 이들 병변들에 대해 소개하고자 한다.
The aim of this study was to evaluate the effect a chewable tablet containing sodium fluoride and xylitol (DenPo, Hamizzle, Bucheon, Kyungki) on the oral hygiene state in the orthodontic patients.'rho subjects consisted of 30 adult orthodontic patients, who have worn the fixed orthodontic appliance at least 6 months. They were instructed to use the tablet forth a day, Gingival index (GI), plaque index (PI), and sulcus bleeding index (SBI) of upper and lower anterior and posterior regions were measured before using the tablet (T0), 2 weeks after using the tablet (T1), and 4 weeks after using the tablet (T2). The data were analyzed with Wilcoxon signed ranks tests. The results were as follows ; 1 PI was decreased significantly in the T1 and T2 stage compared with T0 stage in the both anterior and posterior regions (p<0.05). Although PI was decreased in the T2 stare compared with T1, there was not statistical significance (p>0.05). 2. GI and SBI showed similar results compared with PI. In the T1 and T2 stage, there was statistically significant decrease in the GI and SBI (p<0.05), This study showed that the DenPo tablet could be used as an adjunct to improve oral hygiene in the orthodontic patients. The decrease of the indices nay suggest that DenPo tablet could be effective in the plaque removal and prevention of gngival inflammation in the orthodontic patients.
The purpose of this study was to evaluate the efficacy of an electric toothbrush, with a specially designed orthodontic brush head, compared to a manual toothbrush on the periodontal health of patients with fixed orthodontic appliances Forty patients were randomly divided into two groups. the electric and the manual toothbrush groups, 1 month after attachment of fixed orthodontic appliances. Periodontal status was measured using a plaque index, a gingival index. a bleeding index, a pocket depth and a relative attachment loss, at baseline and after 3 months. 6 months, 9 mouths. and 12 mouths. The Braun Oral-B Plak Control with Ortho OD 15-1 brush head was used as the electric toothbrush while the Butler G.U.M 124 was given as the manual toothbrush. In the manual toothbrush group, the giugival and bleeding indices showed no statistically significant differences, but the plaque index was significantly decreased (P<0.001) and pocket depth as well as relative attachment loss were significantly increased (p<0.01). In the electric toothbrush group. the bleeding index, pocket depth and relative attachment loss showed no statistically significant differences. but the plaque and gingival indices were significantly decreased (p<0.001). In the case of the plaque. giugival and bleeding indices, there were no statistically significant differences between the electric and the manual toothbrush groups. On the contrary, in the case of pocket depth and relative attachment loss, there were statistically significant differences between the electric and the manual toothbrush groups: an increase in the manual toothbrush groups unlike the electric toothbrush group which kept the same state (P<0.05) These findings suggest that an electric toothbrush is useful for orthodontic patients with fixed appliances.

Patients with fixed orthodontic appliances frequently have increased levels of plaque accumulation leading to the possibility of gingivitis or enamel decalcification. Although many methods may be helpful in reducing dental plaque formation, the optimal mechanical removal of plaque is the most important factor during orthodontic treatment. The purpose of this study was to evaluate the efficacy of an electric toothbrush (with a specially designed orthodontic brush head) compared to a manual toothbrush in controlling plaque and gingivitis for patients with fixed orthodontic appliances. Oral hygiene status was measured in thirty-four patients using a plaque index, a gingival index and a bleeding index, before and four weeks after the attachment of fixed orthodontic appliances. Patients were randomly divided into two groups: electric and manual toothbrush groups. Oral hygiene instruction was given according to the type of toothbrush used. The Braun Oral-B D9511 with Braun Oral-B Ortho OD 15-1 brush head was used as the electric toothbrush while the Butler G.U.M. 124 was given as the manual toothbrush. After four md eight weeks, oral hygiene status was measured again. Through a comparison between the electric and the manual toothbrush groups, the following results were obtained. 1. All oral hygiene indices showed an increasing tendency after four weeks of fixed orthodontic appliance. 2. All indices presented a decreasing tendency four and eight weeks after oral hygiene instruction. 3. In case of the gingival index and bleeding index, the decreasing tendency did not show a statistically significant difference between the electric and the manual toothbrush groups. 4. The decreasing tendency of plaque index presented a statistically significant difference between the two groups, showing that the electric toothbrush was more effective in terms of oral hygiene. These findings suggest that an electric toothbrush is useful to orthodontic patients with fixed appliances.

Necrotizing ulcerative gingivitis(NUG), necrotizing ulcerative periodontitis(NUP), necrotizing stomatitis (NS), and Noma (cnacrum oris) are rapidly destructive debilitating and potentially serious oral infection which considered to be different clinical stages of the same process. These have been collectively referred to as: Vincent's infection, infectious oral necrosis, or necrotizing gingivostomatitis(NG). Prevalence of necrotizing gingivostomatitis is $0.19{\sim}0.5%$ and peak incidence is 2-6 years of age. The etiology and pathogenesis of necrotizing gingivostomatitis have been associated with virulent bacteria and impaired host defense and the primary diagnostic signs are pain, interdental ulceration or necrosis, and gingival bleeding. Secondary diagnostic sign is pseudomembrane. This case report was about oral conditions and treatment of the patient who referred from the Dept. of PED and diagnosed a necorotizing gingivostomatitis.

The general local cause of gingival bleeding is the vessel engorgement and erosion by odontogenic infection. Abnormal gingival bleeding is also associated with systemic causes. Bleeding disorders in which continuous gingival bleeding is encountered include the followings : vascular abnormalities, platelet disorders, hypoprothrombinemia and other coagulation defects. There are classic methods for gingival bleeding control, such as, direct pressure, electrocoagulation, suture, crushing and application of hemostatic agents. If the continuous gingival bleeding is not stopped in spite of the conventional methods, the life of patient is threatened owing to upper airway obstruction, syncope, vomiting and hypovolemic shock. Therefore, the rapid and correct hemostatic method is very important in the emergency condition. This is a case report of continuous gingival bleeding control by primary endodontic drainage & suture in a disabled patient with systemic bleeding disorders.
This study aimed to design and operate a complementary integrated health management program based on the connection between the hypertension and diabetes management programs and the oral health programs at a public health center. It also proposed to suggest the phased evaluation indicators. In this study, 48 adults registered in the hypertension and diabetes management program were selected from the Gangneung public health center. The clinic-specific programs were led by dental hygienists and operated for visitors twice every two weeks. The programs were designed based on the logical model, and indicators for evaluating the structure, process, and outcome were presented and applied to the input, process, output, and outcome. The evaluation indices consisted of quantitative and qualitative indicators, and the planning and operation, goal achievement, and effect of each program were assessed. The process evaluations were assessed by the appropriateness of the managers and the operating fidelity of the programs. Indicators for evaluating the outcomes were gingival bleeding, oral health knowledge, oral health awareness, and the satisfaction of the participant and the manager. The clinic-specific programs resulted in positive changes in the evaluated outcomes. The integrated health management of visitors to the hypertension and diabetes management program is important as the general and oral health has common risk factors. Furthermore, long-term operation and continuous monitoring of oral health programs are necessary to evaluate the common factors in chronic disease management.
The common local causes of active gingival bleeding are the vessel engorgement and erosion by severe inflammation and injury to hypervascularity lesion. Abnormal gingival bleeding is also associated with systemic bleeding disorders (liver disease, leukemia etc.). There are many conventional methods for gingival bleeding control, such as, direct pressure, packing, electrocoagulation, tight suture and application of hemostatic agents. If the continuous gingival bleeding is not stopped in spite of the all local application methods, the medical consultation should be obtained for systemic condition care and the major feeding arterial embolization. This is a case report of severe gingival bleeding and periodontitis control in a patient with liver cirrhosis and oral metastatic lesion of hepatocellular carcinoma. The bleeding lesion was placed in left buccal mucosa and gingiva of the left mandibular molars. The control methods were dental crown removal, primary endodontic drainage, gingival sulcus drainage and maxillary arterial embolization with medical consultation.
The aim of this cross-sectional study was to investigate the relationship between regular interdental cleaning and periodontal condition. A total of 319 pregnant women were recruited at 21 to 24 weeks of gestation. Information on demographics, health status and health behaviors including regulary use of interdental cleaning aids, that can influence periodontal condition was collected. Full mouth periodontal probing was performed. Periodontitis was defined as CAL of 4.0 mm or greater on 2 or more sites not on the same tooth. Gingivitis was defined as 15 percent and more sites showed bleeding on probing. Gingival crevice fluid sample was collected for bacterial analysis. We studied a total of 319 subjects, comprised of 116 subjects who use interdental cleaning device regularly and 203 subjects who didn't. Subjects who do not use interdental cleaning regularly had 2.3 times higher risk of gingivitis and 2.7 times of periodontitis, 2.3 times more amount of P. gingivalis. There was a significant relationship between regular interdental cleaning and periodontal condition in Korea.

다운 증후군(Down syndrome, Trisomy 21)은 가장 흔하게 나타나는 증후군으로 대략 1/1000의 확률로 태어난다. 세 개의 유전적 유형이 있지만, 95% 정도가 3개의 21번 염색체를 갖는 비분리 염색체 유형이다. 주된 구강 증상으로는 거대설, 균열설, 과잉치, 결손치, 왜소치, 유치와 영구치의 맹출지연 그리고 이에 따른 부정교합 등이 있다. 75%의 환자에서는 어린 나이에 치주질환에 이환되기 쉬우며, 치아우식증에는 낮은 이환율을 보인다는 보고가 있다. 급성 림프구성 백혈병(Acute Lymphoblastic Leukemia)은 주로 어린이에서 나타나며, 특히 다운 증후군 환자에서의 발병율은 정상인에 비해 20배 높다. 치은비대와 출혈의 양상이 주로 나타나며, 간혹 상악골과 구개골에 종괴가 관찰 되기도 한다. 본 증례에서는 서울대학교치과병원 소아치과에 내원한 환자 중 급성림프구성 백혈병을 앓고 있는 다운 증후군 환아가 있어 이를 보고하고자 한다.
