Comparative analysis of various corrosive environmental conditions for NiTi rotary files (니켈티타늄 파일의 부식에 영향을 미치는 다양한 환경 조건 비교)

  • The aim of the present study is to compare the corrosion tendency using two kinds of NiTi files in the various environmental conditions through the visual examination and electrochemical analysis. ProTaper Universal S2, 21 mm (Dentsply Maillefer, Ballaigues, Switzerland) and Hero 642, 0.06 tapers, size 25, 21 mm (Micromega, Besancon, France) rotary instruments were tested. The instruments were randomly divided into eighteen groups (n = 5) by the immersion temperature, the type of solution, the brand of NiTi rotary instrument and the presence of mechanical loading. Each file was examined at various magnifications using Scanning Electron Microscope (JEOL, Akishima, Tokyo, Japan) equipped with energy dispersive X-ray microanalysis (EDX). EDX was used to determine the components of the endodontic file alloy in corroded and noncorroded areas. The corrosion resistance of unused and used NiTi files after repeated uses in the human teeth was evaluated electrochemically by potentiodynamic polarization test using a potentiostat (Applied Corrosion Monitoring, Cark-in-Cartmel, UK). Solution temperature and chloride ion concentration may affect on passivity of NiTi files. Under the conditions of this in vitro study, the corrosion resistance is slightly increased after clinical use.

Dental Caries Suppression Effect and Other Physiological Properties of Erythritol (에리스리톨의 난충치성 및 기타 생리 특성)

  • Dental caries suppression effect and other physiological properties of erythritol were measured in comparison with those of sucrose, xylitol and sorbitol. The susceptibility test for dental caries by using Streptococcus mutans KCTC 3065 and Lactobacillus acidophillus indicated that erythritol was as effective as xylitol in suppressing dental caries. In lactic acid fermentation test, erythritol showed the least growth of bacteria among the tested sweeteners. The tolelerance test by using mice showed that diarrhea began by feeding once 1500 mg erythritol/kg b.w., 1500 mg xylitol/kg b.w. and 1000 mg sorbitol/kg b.w. respectively.

Assessment of dental erosion potential by the type of fermented milk (유산균 발효유 종류에 따른 치아부식 위험도 평가)

  • Objectives: The aim of this study was to evaluate the potential of dental erosion by three different types of commercial fermented milk on the enamel surface of bovine teeth. Methods: Forty bovine teeth (four groups of 10) were immersed in fermented milk (experimental groups: liquid type, condense-stirred type and condense-drink type) or mineral water (control group) for 1, 3, 5, 10, 15, 30, 60, 90 and 120 minutes. Enamel surface microhardness was measured before and after treatment with a microhardness tester, and Scanning Electron Microscope (SEM) was used to assess the enamel surface morphology. Results:Changes in enamel surface microhardness (before-after treatment; ${\Delta}VHN$) were significantly different among the four groups (p<0.05). ${\Delta}VHN$ was highest in the liquid type group, followed by the condense-stirred type, condense-drink type, and control groups. The ${\Delta}VHNs$ of three types of fermented milk groups were higher than that of the control group (p<0.05). The liquid type group showed higher ${\Delta}VHN$ than the other two types of condense fermented milk groups (p<0.05). However, there was no significant difference in ${\Delta}VHN$ between the two types of condense fermented milk (p>0.05). The results of SEM observation have shown the most severe surface damage in bovine teeth immersed in the liquid type of fermented milk. Conclusions: In this study, it appears that liquid type fermented milk causes greater development of dental erosion. The physical properties and pH of fermented milk types must be considered for prevention of dental erosion associated with ingestion of fermented milk.

  • Air abrasion technology can prepare enamel and dentin for bonding, similar to etching by acidic gels and solutions. Longer treatment can excavate pit and fissures, preparing the tooth for immediate placement of bonded resin materials. Although not appropriate for every clinical situation, the air abrasive technology minimizes heat, vibration and bone-conducted noise associated with conventional means of caries removal since the cutting is accomplished by air pressure. Also, patients treated with the air-abrasion technology rarely request anesthesia. Air abrasion technology was more effective in treating early carious lesions and stains compared to lesions where caries had already progressed to produce soft dentin and the strong air stream and noise caused by the evacuation system was a major discomfort to pediatric patients, and the experience and skillfulness of clinician should be required for accurate and proper tooth preparation.

Modified laser etching technique of enamel for bracket bonding (브라켓 부착을 위한 변형된 레이저 부식법)

  • Objective: Many studies have carried out research on comparisons between laser etching and conventional etching systems to investigate methods of reinforcing shear bond strength. The purposes of this study were to assess the efficiency of bonding with erbium, chromium doped: yttrium-scandium-gallium-garnet (Er,Cr:YSGG) laser etching combined with the conventional etching technique. Methods: Sixty-four sound premolars, extracted for orthodontic purposes, were randomly divided into 4 groups and treated in the following manner. First group, conventional etching of 37% phosphoric acid for 15 seconds (control); second group, 1.5 W laser etching for 10 seconds followed by conventional etching; third group, conventional etching followed by 1.5 W laser etching; fourth group, 1.5 W laser etching for 15 seconds only. We assessed the shear bond strength, the surface characteristics, and the adhesive remnant index scores between all groups. Results: Experimental groups showed higher shear bond strength than the control group. But no statistically significant differences were found between the second and third groups. Adhesive remnant scores were compared with the Kruskal-Wallis test, and no statistically significant differences were found between all groups. Conclusions: To obtain maximum shear bonding strength, a combined technique of Er,Cr:YSGG and 37% phosphoric acid is useful even though it may be inconvenient.

Full mouth rehabilitation of an asthma patient with severe tooth wear and occlusion disharmony (심한 마모 및 교합 부조화를 가진 천식 환자의 전악 수복 증례)

  • Objectives: Patients with excessive tooth wear should first be diagnosed for the etiology of the tooth wear. Causes of tooth wear include bruxism, clenching, and taking medications for systemic diseases. After identifying the cause of tooth attrition, the final prosthesis should be restored with an appropriate vertical dimension. Methods: A 79-year-old man with worn out teeth desired a whole dental treatment. He was on medications for high blood pressure and asthma. The treatment proceeded with a consultation with a medical doctor. The medications for asthma evoked multiple teeth wear and a loss of the vertical dimension. After recovery of 3 mm of vertical dimension, 2 months of evaluation was followed by an interim prosthesis. Results: The increased vertical dimension caused no problem in function and esthetics, and the final restoration was performed with a full monolithic zirconia crown. Group function, adequate anterior guidance, and the occlusal plane were determined. Conclusions: After the final restoration, the patient was both esthetically and functionally satisfied, and a night guard splint was delivered to prevent prosthesis fracture. The patient was informed about the potential tooth wear associated with asthma drugs and educated to visit the clinic regularly.

Full mouth rehabilitation of a patient with excessive worn dentition by increasing vertical dimension of occlusion: a case report (과도한 치아 마모 환자에서 교합수직고경 증가를 동반한 완전구강회복 증례)

  • Tooth wear refers to the loss of dental hard tissue caused by various physiological and pathological causes, and excessive pathological wear can cause complications such as pathological changes in dimensions, occlusal disharmony, loss of function, and aesthetic problems. The cause of tooth wear can be caused by attrition, abrasion, corrosion and abfraction, and it is known to act in a multifactorial etiology in interocclusal activity. In patients with excessive pathological wear, it is important to determine whether or not the vertical dimension of occlusion is reduced, and complete oral rehabilitation should be achieved with the adaptation of the neuromuscular and temporomandibular joint through accurate diagnosis and analysis. The patient in this case was a 63-year-old male patient, who presented discomfort to cold beverage due to severe tooth wear. After analysis of the patient's vertical dimension of occlusion, a full mouth rehabilitation was performed with increasing vertical dimension of occlusion. The goal of treatment was to improve the occlusal plane with the equal-intensity contact of all teeth, harmonious anterior guidance and immediate disclusion of all posterior contacts. After rehabilitation, the patient was satisfied with function and esthetic appearance.

The micro-tensile bond strength of two-step self-etch adhesive to ground enamel with and without prior acid-etching (산부식 전처리에 따른 2단계 자가부식 접착제의 연마 법랑질에 대한 미세인장결합강도)

  • Statement of problems: Self-etch adhesives exhibit some clinical benefits such as ease of manipulation and reduced technique-sensitivity. Nevertheless, some concern remains regarding the bonding effectiveness of self-etch adhesives to enamel, in particular when so-called 'mild' self-etch adhesives are employed. This study compared the microtensile bond strengths to ground enamel of the two-step self-etch adhesive Clearfil SE Bond (Kuraray) to the three-step etch-and- rinse adhesive Scotchbond Multi-Purpose (3M ESPE) and the one-step self-etch adhesive iBond (Heraeus Kulzer). Purpose: The purpose of this study was to determine the effect of a preceding phosphoric acid conditioning step on the bonding effectiveness of a two-step self-etch adhesive to ground enamel. Material and methods: The two-step self-etch adhesive Clearfil SE Bond non-etch group, Clearfil SE Bond etch group with prior 35% phosphoric acid etching, and the one-step self-etch adhesive iBond group were used as experimental groups. The three-step etch-and-rinse adhesive Scotchbond Multi-Purpose was used as a control group. The facial surfaces of bovine incisors were divided in four equal parts cruciformly, and randomly distributed into each group. The facial surface of each incisor was ground with 800-grit silicon carbide paper. Each adhesive group was applied according to the manufacturer's instructions to ground enamel, after which the surface was built up using Light-Core (Bisco). After storage in distilled water at $37^{\circ}C$ for 1 week, the restored teeth were sectioned into enamel beams approximately 0.8*0.8mm in cross section using a low speed precision diamond saw (TOPMET Metsaw-LS). After storage in distilled water at $37^{\circ}C$ for 1 month, 3 months, microtensile bond strength evaluations were performed using microspecimens. The microtensile bond strength (MPa) was derived by dividing the imposed force (N) at time of fracture by the bond area ($mm^2$). The mode of failure at the interface was determined with a microscope (Microscope-B nocular, Nikon). The data of microtensile bond strength were statistically analyzed using a one-way ANOVA, followed by Least Significant Difference Post Hoc Test at a significance level of 5%. Results: The mean microtensile bond strength after 1 month of storage showed no statistically significant difference between all adhesive groups (P>0.05). After 3 months of storage, adhesion to ground enamel of iBond was not significantly different from Clearfil SE Bond etch (P>>0.05), while Clearfil SE Bond non-etch and Scotchbond Multi-Purpose demonstrated significantly lower bond strengths (P<0.05), with no significant differences between the two adhesives. Conclusion: In this study the microtensile bond strength to ground enamel of two-step self-etch adhesive Clearfil SE Bond was not significantly different from three-step etch-and-rinse adhesive Scotchbond Multi-Purpose, and prior etching with 35% phosphoric acid significantly increased the bonding effectiveness of Clearfil SE Bond to enamel at 3 months.

The influence of moisture control on bond strength of composite resin treated with self-etching adhesive system (Self-etching adhesive system에서 수분 조절이 레진의 치질접착강도에 미치는 영향)

  • 최근에 많이 사용되어지고 있는 치과용 접착제는 산 부식 후 수분이 있는 상태에서 적용하는 wet-bonding 술식을 많이 추천하고 있다. 하지만 self-etching primer의 경우 산부식과 priming 과정이 동시에 시행되고, 제조자들은 건조된 치아표면에 적용할 것을 추천하고 있다. 그러나 건조된 정도에 대하여서는 별다른 추천사항이 없으며. 수분이 self-etching primer에 어떤 영향을 미치는지에 대하여서는 별다른 연구가 이루어져 있지 않은 상태이다. 이에 본 연구에서는 치질 삭제 후 남아있는 수분이 self-etching primer의 레진 접착 강도에 어떤 영향을 미치는 지를 알아보고자 하였다. 발거한 대구치 96개를 이용하여 물기가 있는 상태에서 #600 사포로 표면을 연마하고, 법랑질 면을 노출시킨 군과 상아질을 노출시킨 군으로 분류 후, 30분 공기 중 방치 군 (1군), 5초 공기 건조 군 (2군), 1초 공기 건조 군 (3군), 솜으로 약간의 물기를 제거한 군(blot dry) (4군) 등 총 8개의 군으로 나누었다. Self-etching adhesive system인 Clearfil SE Bond primer를 20초간 적용하고, bonding제 도포 후 10초간 광중합 시행하였다. 접착제 처리한 치아면에 몰드를 고정한 후 Clearfil AP-X 복합레진을 2mm 충전하고, 40초간 광중합을 시행하였다. 24시간 후 전단 응력 결합강도를 측정하였으며, 그 결과는 다음과 같이 나타났다. 법랑질과 상아질 모두에서, 30분 건조군과 5초 공기건조군이 1초와 blot drying 군보다 높은 결합강도를 보였으며 통계학적으로 유의한 차이를 보였다(p<0.05). 본 실험 결과에 의하면 self-etching adhesive system을 사용함에 있어서 법랑질과 상아질군 공히 건조된 상태에서 사용하여야 하며 수분의 존재시 치아와의 결합력이 감소하는 것으로 나타났다. 따라서 임상에서 접착제의 적용시 수분의 조절에 주의하여야 할 것으로 사료된다.

CORNELIA DE LANGE SYNDROME: A CASE REPORT (Cornelia de Lange 증후군 환아의 구내증상과 치과치료: 증례보고)

  • Cornelia de Lange syndrome(CdLS) is a well-described multiple malformation syndrome typically involving proportionate small stature, developmental delay, specific facial features, major malformations, and behavioral abnormalities. Dental issues include micrognathia, crowding of teeth, small teeth, absent teeth, poor oral hygiene, and periodontal disease. We present a case of CdLS in a 10-year-old girl. Micrognathia, crowding of teeth, and bifid uvula are the characteristic features of this case. Dental treatments for this child including preventive and restorative procedures were performed under outpatient general anesthesia.
