Prevention of Dental Erosion Due to the Use of Calcium and Fluoride When Ingesting Lactic Acid Bacteria Fermented Milk (유산균발효유 섭취 시 칼슘과 불소 사용에 따른 치아부식증 예방효과)

  • The purpose of this study was to determine the incidence of dental erosion according to the type of lactic acid bacteria fermented oil and to identify a method for preventing dental erosion. For the lactic acid bacteria fermented milk, liquid fermented milk, condense-stirred type fermented milk, and condense-drink type fermented milk were used, and bovine tooth specimens used in the experiment were used. As a method to prevent dental erosion, the method of adding calcium to the lactic acid bacteria fermented milk, the method of applying high and low concentrations of fluoride to the teeth before exposure to the lactic acid bacteria fermented milk, and the method of applying these two methods together were measured to measure the preventive effect of dental erosion. As a result of immersing the specimen in the experimental beverage, the surface hardness of liquid fermented milk decreased the most. When comparing the difference in surface hardness before and after prophylaxis care, the Ca 2% group and the NaF 0.05%+Ca 0.5% group showed no significant difference from the negative control group, confirming that it is an effective method for preventing dental erosion. However, considering the change in taste and the stability of ingredients, a method of adding calcium at a low concentration rather than adding a high concentration of calcium is proposed. Therefore, it is recommended to use low-concentration calcium and low-concentration fluoride together to recognize the possibility of dental erosion when ingesting lactic acid bacteria and to prevent dental erosion caused by it.

  • In the oral cavity, the teeth undergo wear and corrosion throughout their lives. Progressive and constant tooth wear is a natural phenomenon of aging, but wear and corrosion due to specific factors are pathological factors. It can cause pathological damage of the occlusal surface, aesthetic problems, dimensional loss and jaw joint disorders. This case is a 26-year-old female patient with general tooth abrasion and erosion on the entire dentition. Diagnostic wax-up was fabricated based on the information including digital facial analysis, physiological stabilization, and evaluation of anterior crown length. Through the digital analysis, the necessary guides for crown lengthening were prepared and the mastication function and esthetics were evaluated by using temporary crowns. Definitive prosthesis was fabricated with the zirconia restorations. The results were satisfactory when they were observed 3 months of follow-up.

  • Extensive dental erosion and wear will cause serious loss of function and aesthetics in the mouth. In order to recover this condition, careful analysis of the patient's bite relationship is required. In particular, a treatment plan should be established considering the possibility of reproduction of the vertical dimension and centric relation, and appropriateness of the occlusal plane and anterior guidance. Also, the choice of prosthetic materials is an important consideration in patients with severe wear. In this case, patients with overall wear and erosion on tooth was established anterior guidance by orthodontic treatment and fully restored with monolithic zirconia, without increasing vertical dimension.

  • Dental erosion is defined as tooth structure loss by acidic chemical substance. It is caused by extrinsic factors such as acidic foods, drugs, and working environments, and also intrinsic factors such as gastric acid regurgitation in gastro-esophageal disorder or intensive vomiting in bulimia nervosa. These lesions can be treated with direct resin filling, laminate or full contour crown depending on the intensity of clinical problem. This is a case report about treatment of rare clinical case: labial erosion of anterior tooth caused by frequent intake of acidic fruit and palatal erosion of anterior tooth caused by intensive vomiting in bulimia nervosa.

Effects of Food Intakes on Dental Caries in Primary School Students (초등학교 아동의 식품섭취실태가 치아우식에 미치는 영향)

  • The relation between food intakes and dental caries was investigated with 211 primary school students (83 boys and 128 girls) in Kumsan area. The results were as follows. The rate of dental caries of the subjects was 65.4%. 83.9% of the subjects had a snack, 69.5% of students having a snack and 44.1% of students having not snack was dental caries (p<0.001). For tooth brushing numbers per day, 38.4% of subjects was one time, and 61.6% was more than two times. For tooth brushing time, 31.8% had before meals and 68.2% after meals. The intake frequency rate of main food was cooked rice 33.30%, ramyun 14.81%, bread 9.57%, rice cake 6.16%, nodule 4.88% in good group, and cooked rice 27.04%, rice cake 12.04%, ramyun 8.50%, bread 8.11%, fried rice 5.49% in dental caries group. The intake frequency rate of a side dish was kimchi 13.88%, egg 6.21%, dried laver 5.51%, fish paste 5.51%, fish 5.03%, kongjaban 0.44% in good group, and potato soup 6.86%, cucumber 6.56%, sprout soup 5.78%, bean curd 5.33%, sesame leaf 5.21%, lettuce 1.23% in dental caries group. The intake frequency rate for snack was ice cream 9.85%, biscuit 7.35%, gum 6.17%, chestnut 6.16%, apple 5.81%, milk 5.56%, yogurt 4.86%, gypo 4.39%, candy 4.15%, chocolete 3.91% in good group, and biscuit 10.00%, ice cream 6.75%, candy 5.88%, fruit canned food 5.70%, milk 5.41%, corn 5.00%, banana 5.00%, peanut 4.42%, fritter 4.39% in dental caries group. The rate of cariogenic food, detergent food, protective food in a side dish were 4.0%, 40.8%, 55.1%o in good group, and 11.5%, 43.9%, 44.6% in dental group respectively (p<0.001). The rate of cariogenic food, detergent food, protective food in a snack 67.8%, 10.4%, 21.7% in good caries group, and 75.3%, 8.8%, 15.9% in dental caries group respectively (p<0.001).

Effects of conventional and self-etching adhesive systems on bond strength of orthodontic attachments bonded to erupted and unerupted teeth (치아 맹출 유무에 대한 자가부식 접착제에 의한 교정용 부착장치의 접착강도)

  • Objective: The aim of this study was to evaluate and compare the shear bond strength (SBS) and failure-mode of orthodontic buttons bonded to erupted and unerupted teeth with conventional and self-etching adhesive systems. Methods: Eighty-four erupted and 84 unerupted, human third-molar teeth were used. For both groups, the buccal surfaces of each tooth were assigned one of the following type of adhesive systems (n = 12). A, Conventional systems: 1, Transbond XT (3M Unitek, Monrovia, CA, USA); 2, Prime & Bond NT (Dentsply/Caulk, Milford, USA); 3, Single Bond (3M ESPE, Minnesota, USA); and B, Self-etching adhesives; 4, Clearfil SE Bond (Kuraray, Okayama, Japan); 5, Transbond Plus (3M Unitek, Monrovia, CA, USA); 6, Clearfil S3 (Kuraray, Tokyo, Japan); 7, G Bond (GC, Tokyo, Japan). The SBSs of the attachments and the adhesive remnant index (ARI) scores were recorded. Data were analyzed with analysis of variance (ANOVA), independent-sample t-test and chi-square tests. Results: When the SBSs of erupted and unerupted teeth were compared, only the Clearfil-SE Bond and G-Bond were significantly different. Bond strengths of all adhesive systems were higher in unerupted teeth than erupted teeth, except the Single-Bond system. Conclusions: When using conventional adhesives, bonding to erupted and unerupted teeth may not be significantly different. However, clinicians need to take into consideration the types of self-etching systems before usage.

SCANNING ELECTRON MICROSCOPIC STUDY OF THE EFFECT OF ACIDIC DRINK ON ENAMEL EROSION : A CASE REPORT (탄산음료에 의해 부식된 법랑질 표면변화에 대한 증례발표)

  • It is widely believed that the increase in the prevalence of dental erosion is due to soft drinks. The purpose of this study was to assess the influence of soft drinks and etchant on the erosion of tooth enamel and the effect of tooth brushing on the abrasion of eroded enamel, and the effect of milk, saliva and Tooth $Mousse^{(R)}$ on the remineralization. The changes in tooth enamel surface were observed by scanning electron microscophy. The results were as follows; 1. The pH of Coca-Cola and Chilsung-Cider were higher than the pH of FineEtch 37 and $Tyrian^{TM}SPE$. 2. The SEM photos revealed that the pattern of the surface eroded by Coca-Cola and Chilsung-Cider were more obvious than that eroded by etchants. 3. The results of remineralization by saliva, milk, Tooth $Mousse^{(R)}$ were not different from each other. 4. The remineralization by saliva decreased tooth brushing abrasion.

A case of direct restore using 4-META/MMA-TBB resin containing organic filler in patients with severe occlusal surface wear and enamel fracture (심한 교합면 마모 및 교두 파절 환자에서 유기필러를 함유한 4-META/MMA-TBB 레진을 활용한 직접수복 증례)

  • Attrition is the loss of tooth hard tissue due to contact between teeth, and in severe cases, dentin is exposed, accompanied by selective corrosion and excessive wear of teeth, which is called cupping. If these lesions are left untreated, the size of the lesion gradually increases, breaking the unsupported enamel, resulting in a decrease in aesthetics and chewing function. In this case report, patients with cupping and enamel fracture due to severe attrition were directly restored using a resin with soft properties containing organic fillers. In the follow-up observation six years later, most of the filling of the occlusal surface was eliminated, but the filling on the buccal surfaces remained relatively intact, and it was confirmed that this type of resin was suitable for the area where the occlusal force was relatively weak rather than the area where the occlusal force was greatly applied.

  • 치아는 인체중에서도 혹한 환경에서 부분으로 높은 하중과 타액과 같은 강 부식성 매체로 그 환경이 상상을 초월한다. 즉 반복적으로 가해지는 하중(응력)과 침식을 유발하는 타액과 음식물 등이다. 따라서 치아가 쉽게 파괴되거나 썩는 현상이 나타나게 된다. 이렇게 사용되다가 치아의 역할을 다하게 되면 인공치아를 사용하게 되는데 그 재료가 바로 타이타늄(Ti)이다. 생체매식재로 사용되는 Ti는 반응성이 높아 산소와 쉽게 결합하여 표면에 TiO, $TiO_2$, 및 $Ti_2O_3$와 같은 산화피막을 표면에 형성함으로써 뛰어난 부식저항성과 생체적합성을 가지며 생체에 독성이 없고 탄성계수가 골과 비슷하여 골과 임플란트 경계면에서 응력분산에 유리한 성질 등 물리적, 기계적 성질이 뛰어나 외과용 임플란트 재료로 가장 좋은 재료이다. 금속 임플란트의 생체적합도는 임플란트 재료 자체보다는 생체 내 산화막이 화학적으로 불안정할 때 부식이 발생하게 되고 그 결과 금속이온이 주위로 유리되어 조직반응을 일으키므로 금속의 표면을 덮고 있는 산화막에 의해 좌우된다. Ti는 생체불활성재료로서 매식재료로 사용할 경우 뼈와 잘 융합되는 골유착을 나타내나 골과 화학적결합은 하지 않고 골형성을 적극적으로 유도하지 못함으로 환자의 치유기간이 길어지게 된다. 이러한 이유로 골조직내 임플란트의 접합을 개선하기위한 연구가 이루어져 골과의 결합을 높이기 위해 골유착을 일으키는 Ti에 골성장을 유도하는 뼈성분인 하이드록시 아파타이트(HA)라는 물질을 플라즈마 코팅법을 사용하던가 아니면 Hanks' solution내에서 침적 후 HA도금을 하는 방법 등으로 처리하고 있다. 그러나 플라즈마 코팅법은 고온에서 처리를 행하고 Hanks' solution내에 침적할 경우 Ti표면에 밀착도가 저하되거나 합금의 상변화 등으로 인하여 표면처리 과정 중에서 내식성이 크게 감소될 수 있다. 이러한 여러 가지 코팅법을 통하여 골 유착을 증진시키기 위한 연구는 계속되고 있지만 임상적으로 사용 후 문제가 단시일에 발생되는 것도 아니고 수년이 지나야 나타나게 된다. 이러한 방법으로 코팅을 하게 되면 골과 잘 유착이 되어 자연차아와 같은 기능을 하게 된다. 따라서 이러한 문제를 최소화하는 방법이 나노구조를 표면에 형성시켜 골유착을 쉽게 함으로써 이를 개선할 수 있을 것으로 생각되어 본 강의에서는 임플란트의 문제와 사용되는 재료에 대하여 고찰하여 자연치아를 대체 할 수 있는지 알아보았다.
