INTRA-ALVEOLAR TRANSPLANTATION OF COMPLETELY CROWN-ROOT FRACTURED TOOTH WITH DEMINERALIZED FREEZED DRIED BONE GRAFT (치은연 하방으로 파절된 치아의 탈회냉동건조골을 이용한 Intra-alveolar transplantation)

  • Incidence of crown-root fracture due to traumatic injury, have been reported 3% in the permanent dentiton, 2% in the deciduous dentition. There are two treatment methods for crown-root fractured teeth with pulp exposure, when the fracture line was located under the alveolar crest. One way is the extrusion by orthodontic force the other way is intra-alveolar transplantation which occlusally repositioning of apical fragment in the alveolar socket. Since intra-alveolar transplantation has introduced in 1970s, it was practiced as alternative to orthodontic extrusion. As the result, this method may thoughted that had a good prognosis. As a result of trauma, completely crown-root fracture was occured in the maxillary right central incisor in this case. We couldn't reposition the deepest fracture line above the alveolar crest by the conventional surgical extrusion, because apical fragment was too short. Thus, after extraction of apical fragment, we repositioned it to the socket following demineralized freezed dried bone graft, which possible to support the apical fragment. At the 15-month recall examination, the root still showed normal mobility and there was not observed any in flammatory or replacement root resorption in the periapical radiograph.

A CLINICAL AND RADIOGRAPHIC STUDY OF DENTIGEROUS CYSTS (함치성 낭의 임상 방사선학적 연구)

  • The purpose of this study was to obtain information on the clinical and radiographic features of the dentigerous cysts in the jaws. For this study, the authors examined and analysed the clinical records and radiographs of 233 patients who had lesions of dentigerous cyst diagnosed by clinical and radiographic or histopathological examinations. And the obtained results were as follows: 1. Dentigerous cysts occurred the most frequently in the 2nd decade(38.2%) and occurred more frequently in males(67.4%) than in females(32.6%). 2. The most common clinical symptom was swelling of the jaw(33.9%), and the lesions were treated by the method of surgical removal. 3. The type of lesions was mainly observed as central type(72.5%), and size of the lesion was most frequently observed 2 - 2.9cm in the widest length. 4. The lesions were most frequently observed well-defined outline with hyperostotic border(49.8%), and smooth margin(73.4%), and homogeneous lesional radiolucency(79.4%). 5. Cortical thinning and expansion of the lesions(82.0%) were observed, and their direction were most frequently observed toward buccal side(64.0%). 6. The effect on the causative tooth were observed as tooth displacement(41.2%) and delayed root development(l9.3%), and the distance between cemento-enamel junction and lesional wall attachment of the causative tooth was mainly observed as below 2mm(79.6%). 7. The effect on the adjacent tooth were observed as loss of lamina dura(66.8%), root resorption(33.9%), and tooth displacement(31.5%). 8. The effects on the adjacent anatomic structures were observed as displacement of the mandibular canal(46.5%) and maxillary sinus or nasal cavity(72.2%).

THE EFFECT OF PHOTODYNAMIC THERAPY ON THE VIABILITY OF STREPTOCOCCUS MUTANS ISOLATED FROM ORAL CAVITY (광역동 치료가 구강 내에서 분리한 수종의 Streptococcus mutans의 생존력에 미치는 영향)

  • Photodynamic therapy (PDT) is a technique that involves the activation of photosensitizer by light in the presence of tissue oxygen, resulting in the production of reactive radicals capable of inducing cell death. The aim of this study was to evaluate the effect of PDT on Streptococcus mutans in planktonic conditions, previously treated with different photosensitive concentrations of erythrosine, using halogen and LED curing unit as a light source. And we compared the effects of PDT on six strains of S. mutans isolated from oral cavity and reference strain. As a result, S. mutans was susceptible to the combination of hand held photopolymerizer (HHP) and erythrosine. The higher concentration of erythrosine in the presence of light irradiation induced greater effects in reduction of viability of S. mutans. Isolated S. mutans showed a significant reduction in bacterial counts of the groups submitted to PDT compared to the control groups. And they appeared to be similar or slightly lower antimicrobial effect compared with reference strain. However, the difference was not significant (p < 0.05). In conclusion, PDT using erythrosine as a photosensitizing agent and HHP as a light source could be an efficient option for diseases caused by S. mutans.

  • The stability of teeth is an important measure of the periodontal health. This study was designed to determine if there was a relation between the patterns of disclusion and a tooth mobility. An evaluation was made on 117 persons with Angle's class I occlusion who were free from histories of orthodontics, removable or fixed prosthesis, and anterior crowding or open contacts. The results were as follows: 1. In this study, 17.52% of the subjects were exhibited canine protected occlusion. 2. In protrusive movements, 75.21% of the subjects were exhibited incisor guided occlusion and the subjects of canine guided occlusion was the smallest. 3. The canine tooth of mouths having canine-protected occlusions had slightly lower mean tooth mobility scores than the canine of mouths having group function occlsion, but there was no significances. 4. The tooth mobility score of central incisor had higher than that of lateral incisor.

  • Congenital neutropenia or Kostmann syndrome is an inherited disorder manifesting in infancy and characterized by severe bacterial infections. The myelodysplastic syndromes(MDS) are a group of stem cell disorders characterized by a reduction in one or more elements of the peripheral blood. This paper reports a case of Kostmann syndrome and MDS with oral complications such as generalized gingivitis and periodontitis, oral mucosal ulcer, petechiae. The features of these syndromes are reviewed and their oral manifestations and significance to dental management outlined.

  • 교합의 부조화는 외상성교합을 야기시킬 뿐만 아니라 치태의 침착을 용이하게 하며 일단 침착된 치태의 제거에도 애로점이 있고 외과적으로 치주낭을 제거하기 위한 술식을 적용할 때에도 문제점이 있다. 따라서 교합의 부조화를 치료해주는 교합치료는 완벽한 치주처치를 위한 중요한 한 분야이다. 특히 치주과 영역에서 외상성교합의 처치는 필수적이며, 그 방법도 대단히 다양하다. 외상성 교합의 증상에 따라서 치료방법을 선택하자면 다음과 같다. 우선 치은조직에 치은퇴축 혹은 형태의 이상이 야기되었을 때에는 이에 따라 외과적인 처치가 선행되어야 함은 물론이며 다음 단계가 치아의 위치이상을 교정적인 처치를 통하여 정복하거나 동요치를 안정시켜줄 필요가 있으며 또한 정확한 교합조정술이 필요하다. 교합조정술에 대한 제반 사항과 술식에 대하여는 본지, 1982년도 6월호에 상세히 언급되었으므로 본 원고에서는 교정적인 처치방법과 치주고정장치의 적용에 대하여 기술 하고자 한다.

  • Recently, jawbone osteonecrosis has been largely reported as a potential adverse effect of bisphosphonate (BP)administration. Currently available published incidence data for BRONJ are based on retrospective studies and estimates of cumulative incidence range from 0.8 to 12%. The mandible is more commonly affected than the maxilla (2:1 ratio), and 60-70% of cases are preceded by a dental surgical procedure. The signs and symptoms that may occur before the appearance of clinical evident osteonecrosis include changes in the health of periodontal tissues, non-healing mucosal ulcers, loose teeth and unexplained soft-tissue infection. Tooth extraction as a precipitating event is a common observation. The significant benefits that bisphosphonates offer to patients clearly surpass the risk of potential side effects; however, any patient for whom prolonged bisphosphonate therapy is indicated, should be provided with preventive dental care in order to minimize the risk of developing this severe condition.

  • The successful results of the treatment using a double crown denture have been reported in several papers for some of the few remaining teeth in patient. In particular, double crown dentures may be an alternative to treatment in cases where the periodontal status is poor and clasp type removable partial dentures are inappropriate (crown/root ratio > 1). In this case, the patient visited clinic with a chief complaint of difficulty in mastication due to loss of posterior teeth and overall teeth mobility. After teeth with severe periodontitis were extracted, treatment plan of mandible is Kennedy class I removable partial denture (RPD) and treatment plan of maxilla is hybrid telescopic double crown RPD with a friction pin. Patient is well adapted after the prosthetic treatment and we report due to achieving satisfactory results in pronunciation, mastication, retention and aesthetics.

A study of the arch length discrepancy and the diagnostic analysis (치열궁내 공간 부조화와 진단적 평가에 관한 연구)

  • Predicting the arch length discrepancy by simply comparing the available arch perimeter with tooth materials is merely a 2-dimensional analysis of the teeth movement. However, the real teeth movement takes place 3-dimensionally and is affected by various factors such as, the arch fen the curve of Spee and the axis of the incisors. The purpose of this study is to clarify the relationship between the decrease in the arch perimeter and the horizontal positional change of the incisors after extraction of the 1st bicuspids, for more analytic evaluation of the arch length discrepancy at pre-treatment model analysis stage. In addition to that to evaluate the effect of the curve of Spee, teeth axis to the basal plane, and the incisional crowding to the treatment outcome. All patients were treated at the department of orthodontics, dental hospital, Yonsei university. Inclusion criteria for patients selection were as follows. $\cdot$ Angle classification I malocclusion with bialveolar protrusion $\cdot$ Extraction of 4 1st bicuspids $\cdot$ No tooth anomaly or prosthesis $\cdot$ No abnormal attrition $\cdot$ No ectopically erupted teeth $\cdot$ Angle classification I canine and molar relationship $\cdot$ Less than 3mm of crowding Model analysis of the above patients was performed and the following conclusions were obtained. 1. When the intercanine distance was maintained, the available space for the distal movement of the mandibular incisors after the extraction of the 4 1st bicuspids was larger than the space provided by the extraction of the 4 1st bicuspids. However the difference was less than 1mm. The more tapered the anterior arch form, the larger the difference. 2. Compared to the situation in which the intercanine distance was maintained, when the intercanine distance was expanded to meet the width of the Posterior teeth, the incisors could move about 3mm more distally. 3. The positional difference of the incisal tip was insignificant whether the central incisors were moved by tipping or bodily movement. 4. When the anterior crowding was solved without changing the intercanine distance, the larger the anterior arch length was, the more the anterior movement of the incisors. 5. When the curve of Spee was levelled, the increase in the arch perimeter was less than half of the deepest curve of Spee.

Dose Evaluation of Dental Artifacts Using MVCT in Head and Neck (두경부암 환자의 MVCT를 이용한 치아 인공물 보정에 따른 선량평가)

  • Purpose: Metals induce metal artifact during CT-image for therapy planning, and it occurs images distortion, which affects the volumetric measurement and radiation calculation. In the case of using megavoltage computed tomography(MVCT), the volume of metals can be measured as similar to true volume due to minimal metal artifact outcome. In this study, radiation assessment was conducted by comparing teeth volume from images of kVCT and MVCT of head and neck cancer patients, then assigning to kVCT image to calculate radiation after obtaining the similar volume of true teeth volume from MVCT. Also, formal IR image was able to verify the accuracy of radiation calculation. Material and method: 5 head and neck cancer patients who had intensity-modulated radiation therapy from Radixact® Series were of the subject in this study. Calculations of radiation when constraining true teeth volume out of kVCT image(A-CT) and when designated specific HU after teeth assigned using MVCT image were compared with formal IR image. Treatment planning was devised at the same constraints and mean dose was measured at the radiation assess points. The points were anterior of the teeth, between PTV and the teeth, the interior of PTV near the teeth, and the teeth where 5cm distance from PTV. Result: A difference of metals volume from kVCT and MVCT image was mean 3.49±2.61cc, maximum 7.43cc. PTV was limited to where the internal teeth were fully contained. The results of PTV dose evaluation showed that the average CI value of the kVCT treatment planning without the artifact correction was 0.86, and the average CI value of the kVCT with the artifact correction using MVCT image was 0.9. Conclusion: When the Treatment Planning was made without correction of metal artifacts, the dose of PTV was underestimated, indicating that dose uncertainty occurred.
