저자는 정상 치열을 가지며 두개하악장애의 증상 및 병력이 없는 성인 21명을 대상으로 computerized occlusal analysis system인 T-Scan system을 이용하여 교합력에 따른 치아접촉수와 총치아접촉시간을 측정한 후 정량적인 분석을 시도하여 다음과 같은 결론을 얻었다. 1. 최대 측두근 전부 근활성도의 약 20%, 50%, 80% 수준의 교합력에서 치아접촉수의 평균은 각각 1.6개, 8.8개, 16.7개로 교합력이 증가할수록 유의하게 증가하였다. 2. 최대 측두근 전부 근활성도의 약 20%, 50%, 80% 수준의 교합력에서 치아당 치아접촉수이 평균은 대구치에서는 0.2, 1.4, 2.2 개였으며 소구치에서는 0.1, 0.5, 1.1개였고, 전치에서는 0.1, 0.2, 0.6개로 교합력이 증가함에 따라 유의하게 증가하였다. 3. 치아접촉분포로 구분된 대칭군과 비대칭군간의 총치아접촉시간에는 유의한 차이가 없었다.
악구강계의 질환을 진단하고 치료후의 예후를 판정하기 위하여 maximal clenching시에 측정된 tooth contacts의 수와 분포가 이용되어 왔다. 그러나 maximal clenching시 반복된 tooth contacts의 측정은 저작근 피로등 문제점을 가지고 있다. 본연구는 건강한 성인 15명을 대상으로 maximal clenching 및 habitual clenching시 T-Scan system을 이용하여 치아접촉 수와 접촉시간을 측정, 비교 검토하여 habitual clenching을 이용한 방법이 Tooth contacts의 연구에 이용될 수 있는지를 확인 하는데 목적이 있으며, 연구결과는 다음과 같다. 1. Habitual clenching시 upright, semisupine position에서의 전측두근의 근활성은 maximal clenching시의 64.5%, 61.9% 및 73.3%로 나타났다. 2. Maximal clenching시 치아접촉 수와 접촉시간은 16.82개 및 0.429초로 나타났으며 habitual clenching시에는 11.65개 및 0.177초로 나타났다. 3. 치아접촉수와 접촉시간은 체위에 따라 크게 변화되지 않았다. 4. 치아접촉수와 접촉시간의 측정시에 habitual clenching을 이용하는 방법이 maximal clenching을 이용하는 방법보다 재현성이 더 높은 것으로 나타났다. 5. Tooth contacts의 연구시 habitual clenching을 기준으로 사용할 수 있다.
저자는 28명의 측두하악장애 환자와 31명의 정상인을 대상으로 최적기능교합의 개념에 입각하여 교합시의 치아접촉점을 동적이며 정량적인 방법으로 평가하여 교합안정장치의 사용으로 인한 교합안정성의 개선여부를 알아보고자 하였다. 이와 동시에 교근과 전측두근의 활성도를 측정하여 치료의 경과에 따른 근활성의 변화를 알아보기 위해 본 연구를 시행하였다. 측정항목은 개구범위, 두개하악장애지수, 치아접촉점 좌우균형치, 치아접촉점 전후균형치, 치아접촉점 평균시간간격, 치아접촉점의 개수, 접촉시간, 좌우측 교근과 좌우측 전측두근의 근활성, 근활성 비대칭 지수 등이었다. 이의 측정을 위해 T-Scan System, K-6 Diagnostic System 그리고 EM 2등을 사용하였으며 얻어진 자료에 대해 검정한 후 다음과 같은 결론을 얻었다. 1. 교합안정장치를 이용하여 측두하악장애 환자들을 4주간 치료한 결과 전반적인 임상증상이 호전되어 개구범위와 두개하악장애지수에 있어서 뚜렷한 개선이 있었다. 2. 측두하악장애 환자에서 최대 악물기시의 치아접촉점 좌우균형치및 평균시간 간격이 큰 것으로 나타나 치아접촉이 일어나는 순간의 교합안정성이 좋지 않은 것으로 평가되었다. 3. 치료 4주후 교하안정장치를 장착한 상태에서 측정한 치아접촉점 좌우균형치및 치아접촉점 평균시간간격은 치료전에 비해 개선된 것으로 나타났다. 4. 치료 4주후 치아접촉점의 전후방 분포가 구치부위로 이동되는 양상을 보였다. 5. 측두하악장애 환자에서 최대 악물기시의 좌우측 교근 및 전측두슨의 근활성은 정상인에 비해 낮에 나타났으며, 이는 치료기간 동안 감소되는 경향을 나타내었다. 6. 측두하악장애 환자에서 전측두근의 근활성 비대칭지수는 정상인에 비해 상당히 높게 나타났으며 이는 치료기간 동안 감소되는 경향을 나타내었다.

Purpose: The importance of occlusal contacts of the natural dentition for durability of teeth, mandibular stabilization, and restorative dentistry is well known. The purpose of this study is to analyze the occlusal contact and guidance pattern of Koreans by evaluating the static occlusion on maximal intercuspal position and measuring dynamic occlusion during straight protrusion. Materials and methods: The occlusal contacts at maximal interincisal position and the occlusal guidance pattern during straight protrusion of 29 subjects were recorded with shimstock foil (Whaledent, Langenau, Germany), T-Scan III (Tekscan Inc., Boston, MA, USA), polyvinylsiloxane registration material (Genie Bite, Sultan Healthcare, Hackensack, NJ, USA) and compared. Occlusal registration procedures were repeated 3 times. The position was fixed to an upright position and the head position was fixed with the Frankfurt horizontal plane paralleling the horizontal plane. Fisher's Exact Test (R-General Public License, ver. 2.14.1) and Pearson's Test were used to assess the significance level of the differences between the experimental groups (${\alpha}=.05$). Results: When using shimstock foil, T-Scan III system, and polyvinylsiloxane registration material, most of the patients showed contact on anterior, premolar, and molar teeth during maximal intercuspal position. Approximately 51% of maximal intercuspal position showed anterior contact using shimstock foil. When examining the protrusive movement using shimstock foil and T-Scan III system, guidance pattern with the central incisor was the most common. Conclusion: During maximal intercuspal position, there were cases in which not all of the teeth showed occlusal contact. During mandibular protrusive movements, one or more maxillary central incisors frequently joined in straight protrusion and the posterior teeth were disoccluded. Therefore, the anterior teeth protect the posterior teeth, and vice versa. Thus, mutually protected occlusion should be applied when reconstructing occlusion.

Diagnosis and analysis of occlusal relationships are important factors in prosthetic treatment. A thorough occlusion analysis and evaluation should be performed before treatment to restore a stable interocclusal relationship. Analysis and evaluation are essential during the treatment process and at regular follow-ups. Recently, with the development of dental equipment and digital processing methods, new quantitative analysis methods that can record the patient's occlusal relationship have been introduced. Among them, the T-Scan Novus (Tekscan Inc., S. Boston, MA, USA) displays the strength of the initial contact point and the occlusal contact point of the teeth using a pressure sensor. With this, occlusal contact time of the teeth, anteroposterior and left-right balance of occlusal force can be compared. The Dental prescale II (GC Co., Tokyo, Japan) scans the occlusal contact point using a pressure-sensing film and analyzes the density of the contact point. It can measure the distribution and strength of the occlusal force of the teeth in the most natural occlusion state. Based on this, appropriate prosthetic treatment (four-unit fixed partial denture, removable partial denture, complete denture, and complete oral restoration cases) was performed according to the area and extent of the patient's tooth loss. The patient's occlusion at the first visit, treatment stage, right after treatment, and regular follow-up were compared and evaluated using a quantitative method for appropriate occlusion analysis using T-Scan Novus and Dental prescale II. This report enhances the understanding of occlusion analysis during prosthetic restoration. The results satisfied both the clinician and patients in terms of function and aesthetics.

Purpose: The purpose of this study was to compare the stress distribution of teeth and jaw on load by differentiating property of materials according to each layer of widely used mouthguard. Materials and methods: A Korean adult having normal cranium and mandible was selected to examine. A customized mouthguard was constructed by use of DRUFOMAT plate and DRUFOMAT-TE/-SQ of Dreve Co. according to Signature Mouthguard system. The cranium was scanned by means of computed tomography with 1mm interval. It was modeled with CANTIBio BIONIX/Body Builder program and simulated and interpreted using Alter HyperMesh program. The mouthguard was classified as follows according to the layers. (1) soft guard (Bioplast)(SG) (2) hard guard (Duran)(HG) (3) medium guard (Drufomat)(MG) (4) soft layer + hard layer (SG + HG) (5) hard layer + soft layer (HG + SG) (6) soft layer + hard layer + soft layer (SG + HG + SG) (7) hard layer + soft layer + hard layer (HG + SG + HG) The impact locations on mandible were gnathion, the center of inferior border, and the anterior edge of gonial angle. And the impact directions were oblique ($45^{\circ}$). The impact load was 800 N for 0.1 sec. The stress distribution was measured at maxillary teeth, TMJ and maxilla. The statistics were conducted using Repeated ANOVA and in case of difference, Duncan test was used as post analysis. Results: In teeth and maxilla, the mouthguard contacting soft layer of mandibular teeth presented lowest stress measure and, in contrast, in condyle, the mouthguard contacting hard layer of mandibular teeth presented lowest stress measure. Conclusion: For all impact directions, soft layer + hard layer + soft layer, the mouthguard with three layers which the hard layer is sandwiched between two soft layers, showed relatively even distribution of stress in impact.
In implant restorations, it is difficult for the patient to percept any symptoms. In addition, they are absent of shock absorbers, which can lead to mechanical failure if stress distribution is not considered. Since maxillary anterior multiple-implant restorations play a significant role in guiding the functional movement of the mandible by distributing lateral force, it is crucial to form appropriate occlusion. The use of the T-scan system is more advantageous in assessing 'dynamic occlusion', such as the change of occlusion over time, the amount of tooth contact during functional movement, and assessing the occlusion in the less-visible posterior teeth. The case is reported as it has satisfactory results in harmonious anterior guidance of a maxillary anterior multiple-implant restoration using T-scan analysis.

The purpose of this study was to analyze the area of occlusal contact points using visual method. One subject was selected who had Angle Class I, normal dentition, without dental caries, periodontal disease and temporomandibular disorders. Forty times PVS impressions were taken and 10 pairs casts were fabricated using dental super hard stone. After mounting the casts with customized loading apparatus, 78.9kg/f force was loaded as a maximum biting force. In T-Scan method, occlusal contact points measurement was repeated twice. Then, using Photoshop program (Adobe photoshop CS3, Adobe. San Jose, USA), the pixels which indicated occlusal contact points by color was recognized, and the distribution of recognized pixels were calculated to area. In Add picture method, polyether bite material applied to the occlusal surface of the casts. Then, the image of the translucent areas was recorded and classified $0{\sim}10{\mu}m$, $0{\sim}30{\mu}m$, $0{\sim}60{\mu}m$ area by the amount of transmitted light. To acquire occlusal surface, the numbers of pixels from the photograph of the contact area indicated cast converted to $mm^2$. The mean occlusal contact area by two methods was statistically analyzed (paired t-test). Part of the red and pink area in T-Scan image were almost equivalent to the $0{\sim}10{\mu}m$, $0{\sim}30{\mu}m$, $0{\sim}60{\mu}m$ area in Add picture image. The distribution of occlusal contact points were similar, but the average area of occlusal contact points was wider in T-scan image (P<.05). Pink and red area in T-scan image was wider than $0{\sim}10{\mu}m$, $0{\sim}30{\mu}m$ area in Add picture image (P<.05), but similar to $0{\sim}60{\mu}m$area in Add picture image (P>.05). Occlusal contact points in T-scan image did not indicate real occlusal contact points. Occlusal contact areas in T-scan method were enlarged results comparing with those in Add picture method.
The purpose of this study was to find the distribution and measurement of compressive and tensile stress when intrusi- on arch wire is forced engage with upper canine and to analysis stress at each section through FEM. And we compare compressive and tensile ratio at each section. The results were as follows. 1. At FA point and cemento-enamel junction of upper canine, compressive and tensile force ratio is about the same. 2. At apex, compressive force is the four times as tensile force. ; In intrusion, we show root resorption at apex. 3. At Cemento-enamel junction, the compressive and tensile force show the maximun value except FA Point.
이온 영동법은 전기력의 도움으로 이온화된 물질의 신체조직내 침투를 증가시키는 술식으로서 전신적 부작용은 줄어드는 반면, 국소부위의 약물농도를 증가시킬 수 있다는 장점 때문에 효과적인 국소요법으로 인정받고 있다. 치의학 분야에서는 과민상아질의 치료를 위해 불소 이온영동법이 빈번히 이용되어져 왔으며, 국소마취제나 항바이러스 제재의 도포시에도이용되었다.또,이온 영동법에 의한 스테로이드 투여로 피부나 구강점막의 염증성 질환의 효과적 치료를 보고한 많은 문헌이 있으나, 이온영동법에 의한 스테로이드의 구강점막에의 침투량이나 분포에 관해서는 거의 소개된 바가 없는 실정이다. 본 연구는 방사선 동위원소가 부착된 dexamethasone을 이온영동법을 이용하여 가토의 협점막에 침투시킨후 자기방사선 술식에 의해 그 침투량과 분포를 대조군과 비교 평가하였으며 다음과 같은 결과를 얻었다. 1. 이온영동법은 단순 국소도포에 비해 dexamethasone과 0.1M 인산소다 완충용액의 혼합액(dexamethasone in 0.1M sodium phosphate buffer solution)의 가토 협점막 침투량을 증가시켰으며, 양극을 사용하였을 때 더 효과적이었다. 2. Dexamethasone과 0.1M 인산소다 완충용액의 혼합액 투여 4시가, 24시간후 까지도 양극 잉온영동법이 효과적이었으며 은입자의 감소는 투여 4시간부터 24시간 후 사이에 주로 일어났다. 3. 인산소다 완충용액의 첨가는 양극 및 음극에 의한 이온영동법 모두에 효과적이었으며, 양극에 가장 효과적이었고 단순도포군에는 영향을 미치지 않았다. 4. 이온영동법에 의한 스테로이드 투여는 피부뿐만 아니라구강점막 염증성 병소의 효과적 치료술식으로 여거질 수 있다. 시와 maximal clenching시 사이의 치아 접촉시간에서도 유의한 상관관계를 보였다.
