Review of Studies Using a Rhythm Task Intervention and the Rationale for its Formulation (국내 리듬 중재 연구의 중재 구성 및 논거에 대한 고찰)

  • With the recent emphasis on evidence-based research and practice, the field of music therapy has called for rigorous reviews of the scientific data and for therapeutic rationales for research procedures. The purpose of this study was to review studies that used a rhythm task as a therapy intervention in terms of whether they provided a rationale for the components related to the intervention. The components included activity rationale, music rationale, intervention validity, intervention format (individual or group), intervention development (contour), duration, and intervention provider. A total of 41 studies were selected: 22 studies were implemented by music therapists and 19 by professionals in other related fields. In terms of seven assessment categories, only 10 studies were found to report more than four items which are related to a rationale of interventions. The 10 studies identified were further examined to determine if their intervention rationale differed depending on whether the study included a researcher-formulated intervention or an existing music therapy protocol. Of the 10 studies, six used a researcher-formulated rhythm intervention, and four used an existing music therapy protocol. Those studies that used an existing music therapy protocol also provided a clear rationale for music selection, whereas those that used a researcher-formulated rhythm task tended to provide an activity rationale. The results of this study suggest that researchers need to be clearer about their interventions and provide an evidence-based rationale for why and how they use a rhythm task.

  • This article describes a clinical protocol for the conventional rehabilitation of patient diagnosed with partial anodontia. A combined dental therapy approach was used and included endodontic therapy and root capping on the maxillary central incisors, fabrication of a maxillary overdenture, and fabrication of mandibular konus overdenture supported by 3 konus abutments. Within this protocol, tooth-supported overdenture prostheses are used for 2 purposes: first, to obtain the most rigid retention and function at an established maxillary-mandibular relationship; and second, to continuously maintain function and esthetic appearance applying immediate dentures after teeth extraction. The idea behind this protocol and its associated clinical procedures is presented along with a discussion compared with implant therapy. In the case introduced, and after 7 years of observation, the therapy can be seen as a success. We increased the occlusal vertical height in this case, but it would be more appropriate to see this as recovering the occlusal vertical height that was lost. The process of increasing the occlusal vertical height, that is restoration of the face, modification of the extrinsic occlusion of the incisors, and retraction of the mandible is very difficult and important. Ultimately, class III malocclusion is fixed, adequate occlusal vertical height is gained, and the retracted posterior anodontial portion is restored by prosthodontic dentures based on the rigid support theory. The result of the therapy done on the later-achieved malocclusion with partial anodontia on the posterior portion must consider the following in order to maintain the safety of the esthetics of the tooth and face for a period of time: 1) occlusal restoration with an ideal occlusal vertical height, 2) allowance of the final occlusion induced by the functional relationship of the upper and lower jaw, 3)final occlusion functionally induced by the lip competence limit.

  • 목적 : 침(鍼)은 정신이상 및 약물중독과 같은 많은 기능상의 장애질환을 치료, 조정하는데 널리 쓰이고 있으며, 중추신경계에서 생화학적인 균형을 유지하는데 기여한다고 밝혀져 있다. 동물 연구에서 코카인의 강화 및 민감화 특성과 중추의 도파민 활성과의 관련 가능성이 높아지고 있다. 본 연구에서는 약물중독 치료에 이용할 수 있는 침의 효과를 규명하기 위하여 침자극 후 급성 코카인 투여에 의한 도파민 유리 및 자발적 코카인 섭취량에 미치는 침자극의 효과를 측정하였다. 방법 : 웅성 SD 쥐에 코카인 (1mg/kg, i.v.)의 주사 직전 양측의 신문혈(HT7)을 1분간 침자극 한 후, 측핵내 세포외액의 도파민 및 대사산물의 함량변화를 미세투석법을 이용하여 HPLC로 분석하였다. 침자극 후 two-bottle, free choice protocol을 이용하여 자발적 코카인 섭취량의 변화를 관찰하였다. 별도의 쥐를 이용하여 1회 동량의 코카인 정맥주사하고 2주간의 철회를 거친 다음 two-bottle, free choice protocol을 실시하여 자발적 코카인 섭취량과 급성 코카인 투여 후 측핵 중 도파민유리증가율 사이의 상관관계를 조사하였다. 결과 : 신문혈(HT7) 자침이 코카인 1회 주사 후에 유도된 도파민 증가를 대조혈(내관혈(PC6), 또는 꼬리)에 비해 유의하게 억제하였으며 two-bottle choice protocol에 의한 동물의 자발적 코카인 섭취량을 억제하였다. 아울러 자발적 코카인 섭취량과 급성 코카인 투여 후 측핵 중 도파민유리증가율 사이에는 상관관계가 있었다. 결론 : 본 실험결과 침자극에 의해 자발적 코카인 섭취량이 억제되는 것은 측핵에서의 도파민 유리억제에 의해 중개되는 것으로 보여지므로 침자극은 코카인중독과 같은 약물의 중독치료에 이용될 수 있을 것으로 사료된다.

  • The lower $2^{nd}$ molar eruption is beginning to mesiolingually, then rotate to distobuccally so it has a tendency to be tilted and impacted mesially. Signs and symptoms of impacted $2^{nd}$ molar are similar to impacted $3^{rd}$ molar's. However, treatment plan for impacted $2^{nd}$ molar is different from that of impacted $3^{rd}$'s. The former is the preservation and uprighting of $2^{nd}$ molar so that it could act to recovery of mastication, symmetrical facial growth, maintaining the symmetry of dental arch, stable occlusion, while the latter is the extraction of tooth. If the uprighting treatment is planned, most proper protocol of treatment and the additional treatment opition should be applied with consideration for it's crown exposure, present of $3^{rd}$ molar which interrupt the uprighting process, extrusion of opposite tooth. Although it could not improve the esthetic result, it could prevent many dental problems. Therefore, uprighting for impacted lower $2^{nd}$ molar is meaningful treatment.

Treatment of Locally Advanced Pancreatic Cancer (국소적으로 진행된 췌장암의 치료)

  • From January 1981 to December 1991, forty patients with localized advanced carcinoma of the pancreas were treated at the Department of Therapeutic Radiology, Seoul National University Hospital. The treatment protocol consisted of two split course external radiation therapy with each 2000 cGy over two weeks separated by two week rest period. Intravenous 5-fluorouracil (5-FU) was administered on the first three days of each radiotherapy course. Twenty three of these patients were treated by maintenance 5-FU or FAM (5-FU, adriamycin, mitomycin) chemotherapy. Median survival was 9 months and the 2-year survival rate was $10.0\%.$ Good prognostic indicators were good performance status, palliative bypass surgery and tumor located in the head of pancreas.

A Rehabilitation for Ankle Fracture in Korean Medicine: A Report of 4 Cases (발목 골절 수술 후 한방 재활치료 프로토콜 적용의 임상적 효과: 증례 보고)

  • The objective of this study was to propose a rehabilitation protocol for ankle fracture in Korean Medicine and to report its effectiveness. Four patients who had undergone ankle fracture by car accident were treated using the rehabilitation protocol in Korean Medicine. We estimated the outcome evaluating physical examination findings of the ankle joint, numeric rating scale, Olerud-Molander score, and walking state. After the treatment, we observed that ankle movement and muscular strength were improved. Moreover numeric rating scale, Olerud-Molander score, and walking state of patients were improved. A rehabilitation in Korean Medicine can be effectively used for ankle fracture patients. The limitation of this study was the insufficient number of cases. Further studies are needed to design a rehabilitation protocol in Korean Medicine.

Review of the Computerization Component for the Utilization of ICF as a Global Protocol (글로벌 프로토콜로서의 ICF 활용을 위한 전산화 구성요소 고찰)

  • PURPOSE: Computerization using ICF as a protocol can enhance the assessment, communication, and decision-making of various disciplines and cultures, individual functions, disabilities and health to promote communication and understanding among various professionals, organizations, and countries. The empirical foundation for these propositions was provided by delineating of six distinct computerization components. METHODS: This study analyzed 14 papers that combined the medical field and information technology to activate the ICF through computerization. From each of these papers, distinctive advantages were extracted to propose six computerization elements. The validity of these computerization elements was examined. These papers encompass various computerization elements, among which core elements were identified. In particular, six common core elements were extracted from these papers and assumed to be strategic computerization components for ICF activation. A heuristic methodology was employed to validate these components, representing IT technology maturity using four determining indices, which were then presented graphically for validation attempts. RESULTS: Four quantified indices were defined: reliability, cost-effectiveness, support and updates, and collaboration. Using these indices, this study identified elements that leverage existing IT technologies and require new development. The possibility of increasing utility was identified by applying computerization to ICF. CONCLUSION: This study examined the strategic elements of utilizing ICF by computerizing it using a protocol concept and discussed its potential for utilization. The potential to enhance the value of information in social, physical, and cultural contexts was presented by integrating various domains and data within the ICF framework.

  • This paper proposes a state machine design methodology in which an interactive robot that mimics discrete trial training (DTT protocol) can support social interaction training for children with autism. The robot applied to social interaction training uses the response to the provided training stimulus as a quantitative indicator by processing the data received from the sensors measuring the behavioral response of the child. In this process, the state machine is used as information that classifies the state of the acquired data and provides the subsequent stimulus for DTT protocol. Through the joint attentional training, it can be used as evidence-based treatment information by quantitatively classifying the data on the number of sustainable and DTT protocol and the child's response, as well as the current reaction status of the child to the observer performing remote monitoring. At the same time, it was confirmed that it is possible to properly respond to misrecognition situations.
