Development of Device Management and Safety Diagnosis Manual for Electrical Stimulating Swallowing-Assist Equipments (전기적 자극방식 삼킴 보조 치료기의 장비관리/안전진단 매뉴얼 개발)

  • In Republic of Korea, there are considerable vulnerability in electrical stimulating swallowing-assist equipments even though they are professional medical equipments and have high penetration rate with heavy frequency of use. Therefore, in this article, we developed Device Management and Safety Diagnosis Manual for Electrical Stimulating Swallowing-Assist Equipments system. For achieving this, we collected and analysed international standard data from international organizations like International Electrotechnical Commission(IEC). After literature reviews, we build maintenance worksheet and equipment procedure manual for electrical stimulating swallowing-assist equipments according to evidences we collected. These manual and protocol would be used as standard process and contribute to sustain accuracy, effectiveness of the devices, also decrease failure rate and assist to save medical documents.

The Effects of Therapeutic exercise with Electrical Stimulation on Pain, Range of motion, Muscle strength in patients after Anterior Cruciate Ligament Reconstruction (전기자극치료를 동반한 운동치료가 전방십자인대 재건술 환자의 통증, 관절가동범위, 근력에 미치는 영향)

  • This study was to investigate the effect of therapeutic exercise with electrical stimulation on knee joint pain, range of motion and muscle strength in patients who underwent anterior cruciate ligament reconstruction. The subjects of this study were recruited who diagnosed with anterior cruciate ligament rupture and undergoing anterior cruciate ligament reconstruction, total of 20 patients were randomly divided to the 10 control groups and 10 experimental groups. The therapeutic exercise was carried out for 3 weeks with three exercises made by referring to the previous study. The measurement tools used were knee joint pain measurements were made using the visual analogue scale, range of motion was measured using a goniometer, and the muscle strength was measured using a handheld dynamometer. In the statistical analysis, to compare about pre and post test the difference in each same groups was accomplished by using the paired t-test, and compare the difference between the different each groups was accomplished by using the independent t-test. The results of the study showed that the experimental group showed significantly enhanced results than the control group(p<.05). Based on these results, it is concluded that it is effective for the recovery of the patient if the therapeutic exercise with electrical stimulation in parallel with knee joint therapy are performed for the patients with anterior cruciate ligament reconstruction.

Design of the Acupoint Discriminator(DM96A-2) by Improvement of Meridian Point Discrimination Parameter (경혈식별 파라메터의 개선에 의한 혈위식별시스템 DM96A-2의 설계)

  • 경혈을 대상으로 인체를 진단하는 의료기기들은 경혈점에 미세한 직류전류를 인가하고, 이 점에서의 전기저항과 세포의 분극에 의해 반응하는 경락체계의 균형상태를 측정하여 인체를 종합적으로 진단한다. 따라서 전 과정이 정확한 경혈의 위치에서 이루어져야만 치료효과와 진단의 신뢰성이 보장된다. 그러나 대부분의 경락관련 치료기들이 정확한 혈위식별에 어려움이 있고 사용자의 전문적 숙련을 요구한다. 따라서 선행연구에서 일정한 주파수로 교호되는 자극패턴(SPAC) 방식을 사용하여 식별률을 높인 혈위식별기 DM96A-1를 개발하였다. DM96A-1은 단일전원이 양방향으로 교호되는 전류로 자극펄스를 출력하고 측정범위가 0.5∼50㎂인 전류메터와 레벨메터를 이용하여 혈위의 전류량을 표시한다. 이에 따라 방향전환 소자의 두 채널 제어신호가 교호되는 과도기에서 두 자극패턴의 중첩 도통에 의해서 누설전류가 발생하는 경우가 있으며 이를 제거하기 위하여 정밀한 조정을 필요로 하는 불편이 있었다. 따라서 이와 같은 단점들을 보완하여 신뢰성 있는 출력 파라메터를 얻을 수 있도록 마이크로프로세서 i80c196kc를 사용하여 DM96A-2를 재 설계하였으며 임상실험에 의하여 개선된 최적 파라메터의 타당성을 검증하였다.

Evaluation of Electric Pulp Test (EPT) as a Tool for Measurement of Dentinal Hypersensitivity (상아질 지각과민 측정 도구로서의 전기치수검사에 대한 평가)

  • In this study, 40 hypersensitive teeth of 19 patients were investigated. The procedures performed were as follows: Before desensitization, EPT at occlusal third of buccal surface was done for the evaluation of pulp vitality and the EPT value was recorded for the reference value. And mechanical and thermal test was executed for the test of hypersensitivity. If the tooth responded to the above tests, we did EPT at the exposed surface, using toothpaste as a electrolite medium and recorded the EPT value at patient's response. After the tests had been done, desensitization procedures with Gluma(R) Desensitizer were performed according to the manufacturer's instructions. After desensitization, the same tests except EPT at occlusal third were repeated. All the 40 teeth responded positive before desensitization and negative after desensitization procedures. The EPT value at occlusal third ranged from 31 to 65 (48.9${\pm}$7.2). Before desensitization 34 teeth responded at EPT value of 2 and the remaining 6 teeth was in the range of 17 to 25. After desensitization all 40 teeth responded from 12 to 27 (19.6${\pm}$3.5). The 6 teeth responded at greater number than 2 before desensitization was in the range of 18 to 23. Within the limitations of this study we can conclude that: When a tooth with dentinal hypersensitivity responds to mechanical and thermal stimulation, the tooth shows very low resistance to electricity at the exposed surface while when a tooth is desensitized and doesn't show respond to mechanical and thermal stimuli, the tooth shows increased level of resistance to electric stimulation at the exposed surface. EPT can be used for the diagnosis of dentinal hypersensitivity. Furthermore EPT will be useful to evaluate the outcome of desensitization procedures. However, EPT is not a valid tool for measuring dentinal hypersensitivity.

Effects of Electrical Stimulation on Normal Soleus Muscle in Rat (전기자극이 흰쥐의 정상 가자미근 형태에 미치는 영향)

  • This study was carried out to determine effects of electrical stimulation on the soleus, target muscle of the sciatic newt, of white rat normal muscles. The biometric, histochemical, ultrastructural observations were made. The following results were obtained. A daily electrical stimulation of the skeletal muscle of the normally-functioning rat caused an increase of girth and weight of the muscle fibers for 2 weeks. No noticeable change was observed afterwards. More specifically, the density of volume of the red muscle fiber increased. whereas the density of the white muscle fiber decreased. The electrical stimulation group(experimental group) showed hypertrophy of the muscle fibers and narrowing of the space between perimysium and endomysium. Normally, glycogen granules are accumulated regardless of classification of muscle fibers. In addition, the NADH-TR reaction results were in agreement with the biometric findings, in that the red muscle fibers significantly increased. The ultrastructural observations revealed that mitochondria was formed in the red muscle fiber parallel to the muscle fibers of normal muscle, while mitochondria was observed in the sarcomere region of the white muscle fiber. However, activation of mitochondria took place in the sarcolemma region of the muscle fiber, and generation of mitochondria was observed in the sarcomere region of the white muscle fiber.

Effect of Electrical Stimulation using ABR and ECochG Analysis based on Jastreboff Tinnitus Mocel (Jastreboff 이명 모델에서의 ABR과 ECochG 신호분석을 통한 전기자극의 효과)

  • Many researches have been performed whether electrical stimulation could be used for diagnosis and treatment on the auditory system impairment. Unfortunately, there were no standard methods or theoretical background for choosing stimulus conditions because of the lack of understanding on the transmission of electrical stimulation through the auditory pathway. This research was conducted to observe the effect of electrical stimulation on the tinnitus-induced animals. Nine guniea pigs were used for the experment and divided into two groups, five animals for the experimental group(A) and four animals for the control group(B). Experimental conditions were divided into four steps, before tinnitus induction and 1, 6, 12 hours after tinnitus induction using salicylate based on the Jastreboff model. In each experimental condition, ABR and ECochG were obtained, and autocorrelation coefficients were calculated from normalized waveforms based on rms values. Sum of all the autocorrelation coefficients was extracted as a parameter to observe the changes between before and after the electrical stimulation. As a result, ABR parameter values were rapidly increased 6 hours after tinnitus induction, the gradually returned back to the initial state. On the other hand, when electrical stimulation was applied, parameter values did not change compared with the initial sate. Parameter values of ECochG showed that the effect of electrical stimulation appeared 12 hours after the tinnitus induction. It was concluded that an electrical stimulation to the tinnitus-induced model changes the correlation coefficients of ABR and ECochG waveforms.

Electrical Stimulation Parameters in Normal and Degenerate Rabbit Retina (정상 망막과 변성 망막을 위한 전기자극 파라미터)

  • Retinal prosthesis is regarded as the most feasible method for the blind caused by retinal diseases such as retinitis pigmentosa (RP) or age related macular degeneration (AMD). Recently Korean consortium launched for developing retinal prosthesis. One of the prerequisites for the success of retinal prosthesis is the optimization of the electrical stimuli applied through the prosthesis. Since electrical characteristics of degenerate retina are expected to differ from those of normal retina, we performed voltage stimulation experiment both in normal and degenerate retina to provide a guideline for the optimization of electrical stimulation for the upcoming prosthesis. After isolation of retina, retinal patch was attached with the ganglion cell side facing the surface of microelectrode arrays (MEA). $8{\times}8$ grid layout MEA (electrode diameter: $30{\mu}m$, electrode spacing: $200{\mu}m$, and impedance: $50k{\Omega}$ at 1 kHz) was used to record in-vitro retinal ganglion cell activity. Mono-polar electrical stimulation was applied through one of the 60 MEA channel, and the remaining channels were used for recording. The electrical stimulus was a constant voltage, charge-balanced biphasic, anodic-first square wave pulse without interphase delay, and 50 trains of pulse was applied with a period of 2 sec. Different electrical stimuli were applied. First, pulse amplitude was varied (voltage: $0.5{\sim}3.0V$). Second, pulse duration was varied $(100{\sim}1,200{\mu}s)$. Evoked responses were analyzed by PSTH from averaged data with 50 trials. Charge density was calculated with Ohm's and Coulomb's law. In normal retina, by varying the pulse amplitude from 0.5 to 3V with fixed duration of $500{\mu}s$, the threshold level for reliable ganglion cell response was found at 1.5V. The calculated threshold of charge density was $2.123mC/cm^2$. By varying the pulse duration from 100 to $1,200{\mu}s$ with fixed amplitude of 2V, the threshold level was found at $300{\mu}s$. The calculated threhold of charge density was $1.698mC/cm^2$. Even after the block of ON-pathway with L-(1)-2-amino-4-phosphonobutyric acid (APB), electrical stimulus evoked ganglion cell activities. In this APB-induced degenerate retina, by varying the pulse duration from 100 to $1200{\mu}s$ with fixed voltage of 2 V, the threshold level was found at $300{\mu}s$, which is the same with normal retina. More experiment with APB-induced degenerate retina is needed to make a clear comparison of threshold of charge density between normal and degenerate retina.

Convergence of Acupoint and Electrical Stimulation Therapy for Blood Flow and Pain Threshold (혈류량과 통증역치에 대한 경혈과 전기자극치료의 융합연구)

  • This study examined how the application of silver spike point (SSP) and acupuncture-like transcutaneous electrical nerve stimulation (A-TENS) on acupoints affects blood flow and pain threshold, using laser Doppler blood fluxes and the Commander algometer. Our study included 32 healthy men and women who were randomly divided into the SSP group (n=18) and the A-TENS group (n=14). The pain threshold and blood flow were measured at the Neiguan (PC6) of the Jueyin Pericardium Meridian of the hand. SSP was performed with a 2.8cm electrode at a fixed frequency of 3 Hz for 15 minutes. The change in blood flow and pain threshold after the intervention significantly differed between the two groups (p<0.05). We found that the application of SSP and A-TENS on an acupoint altered their blood flow and pressure pain threshold, with SSP resulting in significantly greater change than A-TENS. Based on these results, the convergence of acupoint and electrical stimulation therapy can be usefully applied as a method for various patients. Continued development of convergence interventions is necessary.

  • Alzheimer's disease (AD) is the most common cause of dementia, showing progressive neurodegeneration. Although oral medications for symptomatic improvement still take a huge part of treatment, there are several limitations caused by pharmacology-based real world clinic. In this respect, non-pharmacologic treatment for AD is rising to prominence. Transcranial direct current stimulation (tDCS) is a one of the non-invasive neuromodulation technique, using low-voltage direct current. In terms of safety, tDCS already has been proven through numerous previous reports. This review focused on behavioral, neurophysiologic and histopathologic improvement by applying tDCS in AD rodent models, thereby suggesting reliable background evidence for human-based tDCS study.

Development of Pains Therapy System Using Focused Variable Electromagnet (비침습 집중형 전자기장을 이용한 통증치료기 개발)

  • The effect of acupuncture that occurs at Meridians, can not be defined by western science. But we can find out the electrical effects when acupuncturing by measuring the charges meridians electric potential. So instead of using a needle to acupuncture, we used a electromagnet to give the same effect. We stimulated the meridians, with a electromagnet. Then we observed the pattern of the meridians potential, that occurred at ST-39 and ST-37. After doing clinical experiments with the acupuncture stimulation and the electromagnet stimulation. We found out the acupuncture stimulation and the electromagnet stimulations electric waves were similar. From this result, we knew it was possible to give similar effects to acupuncture stimulation by using electromagnets.
