IMPACTED MANDIBULAR CANINES; CASE REPORT (하악 매복 견치의 치과적 접근)

  • Impacted mandibular canines are less common than impacted maxillary canines. The proceeding studies show that impacted mandibular canines occur in approximately 0.1% of patients examined. The causes of impacted mandibular canines are inadequate space, premature loss of the primary dentition, excessive crown length, hereditary factors, functional disturbances of endocrine glands, tumors, and traumas. The basic choices for treatment of those cases are orthodontic migration or surgical removal. This is a case report about those two different treatment methods. One patient was 11 years old female who had treated with surgical approach and the other was 14 years old male who had received orthodontic treatment.

A Morphometric Study of Primary Anterior Zirconia Crowns in Korean Tooth Models (한국 유치 모델에서 유전치 지르코니아 크라운의 형태계측학적 연구)

  • The purpose of this study was to provide clinical recommendations for restoration with selection of the most similar zirconia crown by 3-dimensional analysis of the shape of the maxillary primary central and lateral incisors in Korean individuals and prefabricated zirconia crowns. The average shape of the sound maxillary primary central and lateral incisors in 300 children was reproduced by 3-dimensional scanning. Zirconia crowns of 4 manufacturers (NuSmile $ZR^{(R)}$ Crown, Cheng $Crowns^{(R)}$, Kinder $Krowns^{(R)}$, and EZ $Pedo^{(R)}$ Crown) were scanned 3-dimensionally, and coordinates for comparison of the shape were measured to evaluate the similarity between the teeth and crowns. The most similar crowns were selected by comparing the mesiodistal length, crown height, crown shape ratio, distance between the same coordinates of a tooth and crown, the radius of curvature of the labial surface, and the volume. As a result of analysis, Cheng $Crowns^{(R)}$ size 3 and NuSmile $ZR^{(R)}$ Crown size 2 were the most similar crowns in the maxillary primary central and lateral incisors, respectively. Scanning the inner surface of the crowns and evaluating the amount of tooth reduction required suggested that an overall lesser amount of tooth reduction compared to that presented by the manufacturer's guidelines should be performed.

  • 1. 목적 이 연구의 목적은 기존의 평가기준에 따라 관찰자에 의해 주관적으로 판단된 치은형과 실제 치은두께와의 상관관계를 규명하고 임상검사 시에 측정할 수 있는 변수들과 치은두께의 연관성을 평가하는 것이다. 2 방법 211명의 치과대학생(22-43세, 평균 24.7세)을 연구대상으로 하여 상악중절치부위에서 국소마취하에 근관 치료용 파일을 치은에 삽입하여 두께를 측정하고 임상검사를 통해 치주낭깊이, 치태지수, 치은지수, 치은퇴축, 체형 및 비만도, 피부형, 치경부의 형태, 관측자가 임의로 판단한 치은형을 기록하였다. 임상사진으로 치관의 폭경과 길이의 비율, 치은외형의 만곡정도를 조사하고 평행촬영법을 이용한 방사선사진으로 치아의 장평비율 및 치아의 치관 폭경과 치경부 폭경의 비율을 조사하였다. 전체 대상을 치은의 두께를 기준으로 평균두께보다 얇은 군과 두꺼운 군의 두 개의 군으로 분류하여 임상검사 사에 측정한 변수들이 각 군 간에 유의할 만한 차이를 보이는지와 임의로 판단한 치은형이 실제 치은두께와 연관이 있는지를 알아보았다. 통계처리는 Student t-test를 이용하였다. 3. 결과 치주낭 깊이, 치은지수, 체형 및 비만도, 피부형, 치아의 형태, 치경부의 형태, 치아의 장평비율의 경우 실제 측정하여 얻은 치은의 두께와의 상관관계는 통계적으로 유의성이 없었다. 치은의 형태는 치은의 두께와 상관성은 보이고 있으나 통계적으로 유의하지 않았다. 관측자가 임의로 평가한 치은형과 실제 측정치도 유의할만한 일치를 보이지 않고 있다. 4. 결론 치은의 형태는 치은의 두께를 예상하는데 약간의 도움이 될 수 있으나 실제 치은의 두께는 임상적으로 간단히 측정할 수 있는 겸사지수들과 직접적인 상관관계를 보이지 않았다. 따라서 치료결과의 예측에 있어서 치은형을 분류하여 예상하는 것은 큰 도움이 되지 않는다고 할 수 있다.

Correlation of Left Mandibular Third Molar Development and Chronological Age (하악 좌측 제3대구치 발육과 연령 사이의 연관성)

  • The aim of this study is to evaluate the development of the left mandibular third-molar development with modified Demirjian method and its relation to chronological age. A total of 1653 digital panoramic radiographs of healthy individuals aged between 7 and 23 years who visited Yonsei University Dental hospital were selected. The developmental status of the left mandibular third molars was assessed using dental maturity scoring proposed by Demirjian et al. The mean age of the first appearance of mineralization, complete crown formation, and complete root formation were around 9, 14, 21 years respectively. Statistically significant differences between males and females in the development stage of D and G were revealed that crown formation and root length completion were attained earlier in males than in females. There was significant positive relationship between age and third molar development in both sexes, and new formula was presented to estimate age of children and adolescents based on their developmental stages of third molars. In this study, the use of left mandibular third molar as a developmental marker is appropriate, and age estimation can be attained with dental maturity stage.

Evaluation of retrievability using a new soft resin based root canal filling material (레진 계통의 근관 충전재의 제거 용이성에 대한 평가)

  • The aim of this study was to evaluate the retrievability of Resilon as a root canal filling material Twenty-seven human single-rooted extracted teeth were instrumented utilizing a crown down technique with Gates-Glidden burs and ProFile system. In group1 (n = 12) canals were obturated with gutta percha and AH-26 plus sealer using a continuous wave technique and backfilled. In group 2 (n = 15) Resilon was used as a filling material. Then teeth were sealed and kept in $37^{\circ}C$ and 100% humidity for 7 days. For retreatment, the samples were re-accessed and filling material was removed using Gates-Glidden burs and ProFiles. Teeth were sectioned longitudinally to compare the general cleanliness and amount of debris $({\times}\;75)$ using SEM. Chi-square test was used $({\alpha}\;=\;0.05)$ to analyze the data. The total time required for removal of filling materials was expressed as mean ${\pm}SD$ (min) and analyzed by the Student t-test $({\alpha}\;=\;0.05)$, Required time for retreatment was $3.25\;{\pm}\;0.32$ minutes for gutta percha/ AH 26 plus sealer and $3.05\;{\pm}\;0.34$ minutes for Resilon. There was no statistically significant difference between the two experimental groups. There was no significant difference between the groups in the cleanliness of the root canal wall. This study showed that Resilon was effectively removed by Gates-Glidden burs and ProFiles.

Retrospective study on survival and complication rates of posterior single implant according to the implant diameters, lengths and position (구치부 단일 임플란트의 직경과 길이, 식립 위치에 따른 생존율 및 합병증의 발생률에 대한 후향적 연구)

  • Purpose. This retrospective study was to investigate the survival and complication rates of posterior single implant according to the implant diameters, lengths and position. Materials and methods. Patients who had been restored single implant prosthesis in the posterior area by the three prosthodontists in the department of prosthodontics, Daejeon dental hospital of Wonkwang university, in the period from February 2014 to May 2018 were selected for the study. A total of 505 patients with 697 implants were observed. The survival and complications of implants were investigated using electronic medical records and radiographs. Fixture diameters, lengths, position, patient's sex and age were assessed as possible factor affecting the survival and complications of implants. Results. 3-year cumulative survival rates of posterior single implants were 98.5% and 5-year cumulative survival rates were 94.4%. 5-year cumulative survival rates were higher in implants with diameter > 4.0 mm (97.0%) than implants with diameter ≤ 4.0 mm (89.5%), and in females (98.8%) than males (92.4%). There were statistically significant differences (P < .05). The mechanical complication rate was 20.1% and the biological complication rate was 4.7%. Complications occurred in order of abutment screw loosening (7.5%), decementation (6.3%), proximal contact loss (3.7%) and so on. Abutment screw loosening occurred more frequently in the lower molar region (10.5%), in males (9.5%) than females (5.1%), and in patients aged < 65 years (9.4%) than patients aged ≥ 65 years (5.1%). There were statistically significant differences (P < .05). Conclusion. The 5-year cumulative survival rates were higher in implants with diameter > 4.0 mm than implants with diameter ≤ 4.0 mm and in females than males. Abutment screw loosening which was the most commonly occurring complication occurred more frequently in the lower molar region, in males than females, and in patients aged < 65 years than patients aged ≥ 65 years. There were statistically significant differences.

Pontic site development with an implant submergence technique for unaesthetic implant in the anterior maxilla (상악 전치부 임플란트의 비심미성 개선을 위한 임플란트 침수(submergence)를 동반한 치조제 증대술)

  • Improving implant esthetics is very difficult, especially in cases where unaesthetic problems are related to implants in the maxillary anterior dentition. A 69-year old male patient was referred by a prosthodontist for periodic pus discharge and an unaesthetic implant prosthesis (maxillary right lateral incisor). The implant was placed too deeply and showed soft tissue volume deficiency and a long clinical crown. After a clinical and radiographic examination, implant submergence and alveolar ridge augmentation were performed to enhance the aesthetics instead of an explantation. The treatment plan was as follows: extraction the adjacent teeth with tooth mobility, secondary caries, and poor prognosis; placement an additional dental implant with hard and soft tissue grafting; fabrication a fixed bridge using implant abutments. A fixed esthetic prosthesis using implants was fabricated, and the patient was satisfied with the prosthesis. A ridge augmentation with implant submergence may be an alternative for solving the problems of unaesthetic implant restorations in the esthetic zone.

The comparison of microtensile bond strength with immediate and delayed dentin sealing (즉시 상아질 봉쇄와 지연 상아질 봉쇄에 따른 상아질 접착의 미세인장 결합강도 비교)

  • Purpose: The purpose of this study was to evaluate the effect of various methods of dentin bonding agent application on microtensile bond strength between dentin and resin, using a 2-step etch-and-rinse dentin bonding agent. Material and methods: Twenty freshly extracted human molars were obtained and divided into 4 groups of 5 teeth. 2-step etch-and-rinse dentin bonding agent was used for all groups. The control specimens were prepared using a direct immediate bonding technique. The delayed dentin sealing (A, C) specimens were prepared using an indirect approach with delayed dentin sealing. For group A, resin was built-up on uncured dentin bonding agent, and for group C, resin was built-up on pre-cured dentin bonding agent. Preparation of the immediate dentin sealing (B) specimens also used an indirect approach with immediate dentin sealing immediately following preparation. All teeth were prepared for a microtensile bond strength test. Specimens were stored in water for 24 hours. Ten beams (1.0 ${\times}$ 1.0 ${\times}$ 11 mm) from each tooth were selected for testing. Bond strength data (MPa) were analyzed with a one-way ANOVA test, and post hoc comparison was done using the Scheffe's test. Results: The mean microtensile bond strengths of control group, B and C (DDS with pre-cure) were not statisticaaly different from each other at 32.7, 33.3, 34.2 MPa. the bond strength for group A (DDS without pre-cure), 19.5 MPa, was statistically different (P < .01) from the other 3 groups. Conclusion: When preparing teeth for indirect bonded restorations, DDS with pre-curing dentin bonding agent and IDS results in the same bond strength between dentin and resin. On the contrast, the bond strength was decreased when DDS without pre-curing dentin bonding agent was used.

A 3-dimensional finite element analysis of tapered internal connection implant system (Avana SS $III^{(R)}$) on different abutment connections (경사형 내부연결 임플란트 시스템 (SS $III^{(R)}$)에서 지대주 형태에 따른 응력분포의 3차원 유한요소 분석)

  • Purpose: The purpose of this study was to compare the stress distribution characteristics of four different abutment connections on SS-$III^{(R)}$ fixture under occlusal loading, using 3-dimensional finite element method. Materials and methods: The fixture of SS-$III^{(R)}$ (Osstem, Korea) with 4 mm diameter and 11.5 mm length and 4 types of abutments were analyzed; Solid, Com-Octa, ComOcta Gold, and Octa abutment. The models were placed in the area of first molar in the mandible. The 4 loading conditions were; (1) the vertical loading of 100 N on the central fossa, (2) the vertical loading of 100 N on the buccal cusp, (3) the $30^{\circ}$ inclined loading of 100 N to lingual side on the central fossa, and (4) the $30^{\circ}$ inclined loading of 100 N to the lingual side on the buccal cusp. The 3G.Author program was used, the von-Mises stress was calculated and the stress contours were plotted on each part of the implant systems and the surrounding bone structures. Results: Regardless of abutment types and loading conditions, higher stress concentration was observed at the cortical bone. In cancellous bone, the highest stress was observed at apical portion and the maximum stress occurred at the implant neck. The higher internal stress was observed in the fixtures than in the bone. The lowest stress was observed at loading condition 1 and the stress concentration was also lower than any other loading conditions. Conclusion: Within the limitation of the result of this study, it seems that the abutment connection type does not affect much on the stress distribution of bone structure.

The effect of implant system with reverse beveled platform design on marginal bone stress distribution (임플란트 경부의 역사면 디자인이 변연골 응력분포에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effects of the surface morphology of the implant neck on marginal bone stress measured by using finite element analysis in six implant models. Materials and methods: The submerged type rescue implant system (Dentis co., Daegu, Korea) was selected as an experimental model. The implants were divided into six groups whose implant necks were differently designed in terms of height (h, 0.4 and 1.0 mm) and width (platform width, w = 3.34 + 2b [b, 0.2, 0.3 and 0.4 mm]). Finite element models of implant/bone complex were created using an axisymmetric scheme. A load of 100 N was applied to the central node on the top of crown in parallel with the implant axis. The maximum compression stress was calculated and compared. Results: Stress concentration commonly observed around dental implants did not occur in the marginal bone around all six test implant models. Marginal bone stress varied according to the implant neck bevel which had different width and height. The stress was affected more markedly by the difference in height than in width. Conclusion: This result indicates that the implant neck bevel may play an important role in improving stress distribution in the marginal bone area.
