Lens shielding block 제작시 업무개선에 관한 고찰

  • I. 목적 : 결막 림프종 환자의 전자선 치료를 위해 Lens shielding block의 제작시 외부 의뢰 제작과 SMC 자체 제작의 장${\cdot}$단점을 비교해보고 제작 과정에 있어 업무의 효율성을 평가하고자 한다. II. 대상 및 방법: Lens shielding block의 제작시 외부의뢰제작과정과 자체 제작 과정을 비교하여 업무의 효율성, 인력의 변화 및 두방법의 장?단점을 분석하였다. 모의치료후 두 방법의 제작 시간을 비교하였으며, 재료의 차이점과 이용방법을 비교하여 개선된 사항을 알아보았다. III. 결과 : Lens shielding block의 자체 제작시 치료 시작시기는 모의치료 실시 후 7일에서 3일로 단축되어 업무의 효율성과 그에 따른 환자의 불편함이 줄어들게 되었다. 또한, 외부의뢰 제작 과정에서 실시되었던 치과와 치과 기공실에 의뢰하던 과정이 없어지고 과내 모의치료실과 공작실에서 작업하게 되어 모든 작업을 과내에서 일률적으로 실시할 수 있게 되었다. 또한, 모의치료를 실시하면서 실시간으로 shielding block을 수정 할 수 있어 정확성이 재고되었고, 반복되는 제작 작업으로 인해 제작 공정이 보편화되고, 손쉬어졌으며, 재료 활용의 효율성이 증대되었다. IV. 결론 : Lens shieiding block의 자체 제작으로 시간이 단축되어 업무의 효율성이 증대되었고 그에 따른 인력소모와 환자의 불편함이 줄어들게 되었으며, 제작자가 모의치료를 통해 바로 block의 오차를 수정할 수 있어 치료의 안정성 및 정확성이 높아졌으므로 결론적으로 의료의 질 개선과 서비스 향상이 이루어졌다.

A Survey on Utilization of Dental Services by Foreign Children in Seoul (서울 거주 외국인 아동의 치과 이용 실태 조사)

  • This study aims to investigate the utilization status of dental services by foreign children living in Seoul and their level of satisfaction with the treatments. We developed a structured questionnaire with 35 questions, which comprised 14 questions (demographic characteristics) and 21 questions (oral hygiene and dental experience of the child). In this study, the distribution of nationality of 391 participants differed from the actual statistics in Korea. It consisted of high percentage of participants from African countries (23.6%), as well as North American countries (24.1%). In addition, the education status of most parents was above the level of college graduate. Despite the relatively high socioeconomic status of the participants, they showed minimal dental health knowledge regarding the aspect of oral hygiene. Utilization of dental health services and the types of dental clinics visited by them were similar before and after coming to Korea. The participants showed overall satisfaction with the care, but many of them expressed the need for improvement with regard to treatment fee and communication. In conclusion, foreign children living in Seoul require appropriate dental health education and governmental support to promote regular dental check-ups, to improve their overall oral health, and prevent the incidence of dental caries.

  • Objectives : The purpose of this study were to measure the effect of factors analyse the associated by the dental treatment fear of level middle school student's, and then to provide basic material which can help to improve adolescent's oral health and dental service. Methods : This subject of study consists of 342 middle schools each 1,2,3 grade Daegu city. The data were collected from July through August 2009, by way of the self-reported questionnaire. The data materials are analyzed by general characteristics and dental service behavior frequency analysis, dental fear of levels general characteristics, dental service behavior, the burden of dental visits, the pain experience of the past ${\chi}^2$-test and dental treatment fear relationship with multiple regression analysis. Results : General characteristics, 56.7% for boys and girls, with 43.3 percent of boys and girls high. dental service behavior in the dental visited was treatment purpose 76.5%, preventive purpose 23.5%. high-level dental treatment fear of the female 60.8%, male 33.0% was higher, The higher age also increased dental fear of the levels, dental services dental visit in 1-2 year the dental fear level of the higher, was statistically significant (p<0.05). Medical purpose 'preventive' than 'treatment' is a higher dental fear. Factors affecting dental treatment fear experiences directly from your pain (${\beta}$=0.116), inadequate anesthesia, treatment experience (${\beta}$=0.126), indirect painful experience (${\beta}$=0.125) in both a statistically significant positive relationship was found (p<0.05). Conclusions : Finally, it is necessary carry out further studies on the improvement of dental service of oral health of adolescent based on the dental treatment fear.

  • This study was conducted to provide basic data on the dental hygienists Happiness Index and identify factors influencing dental hygienists A self-administered questionnaire survey was conducted dental hygienists in Jeollabuk-do(Jeonju, Iksan, Gunsan) from October to November 2017. Data were analyzed using t-test, ANOVA and multiple regression. As the result of study, Happiness Index of socioeconomic level has increased when the more work career, and more monthly wage, and better interpersonal relationship. Happiness Index of psychological well-being has increased when younger group and more monthly wage, and better interpersonal relationship. Therefore, in order to increase the happiness index of dental hygienists, economic compensation such as incentives and if the education of the communication method of the cases where the most difficulty case of the communication situation in the human relationship is confirmed is preceded, the quality of the medical service provided to the patient due to the happy working life will be improved.

  • In this paper, we proposed of the results of the noise level and appropriate conversation distance by applying the noise characteristics generated during treatment at a dental clinic to the NR-curve and PSIL. As a result of analyzing the noise characteristics during treatment at a dental clinic, it was analyzed that the noise level exceeded 60dB(A), which is the health preservation limit value caused by noise, and the noise level increased as the frequency increased. the result of evaluation applying it to the NR curve, some treatment exceeded the workplace noise standard, and as a result of analyzing the level of conversational disturbance between the worker and the patient, it is desirable to have the conversation at a distance of less than 1M for accurate communication. In order to improve the quality of medical service in dental clinic and to reduce dental fear, it is judged that soundproofing protective equipment is provided to workers, and soundproofing measures are needed for noise sources (treatment devices used in treatment) and sound sources (patients and workers).

A Study on Network Hospital and the Ban on Opening and Operating the Muliple Medical Institution (네트워크병원과 의료기관 복수 개설·운영 금지 제도에 관한 고찰)

  • Our Constitution obliges the state to protect the health of the people, and the Medical Law, which embodied Constitution, sets out in detail the matters related to open the medical institution and one of them is to prohibit the operation of multiple medical institutions In the past, there was a provision stipulating the same purpose. But because the Supreme Court interpreted that several medical institutions could be opened if the medical treatment was not made at the additional medical instition which was opened in the another doctor,s license, multiple medical institutions could be opened and operated. However, some health care providers opened the several medical institutions to another doctor's license just by the excuse of the business management and then did illegal medical cares like the unfair luring of patients, overtreatment, and commition treatment for more profits. So, the health rights of the people came to be infringed on. Accordingly, lawmakers amended the Medical Law for medical personnel not to open and to operate more than one medical institution. As the amended medical law prohibited a medical personnel to open multiple medical institution, some medical personnels insisted that the amended medical law is unconstitutional under which they could not be able to open and operate medical institutions on based on free investment and bring out the benefits of network hospitals. But the regulation to prohibit multiple institutions does not apply only to a medical personnel. Many other experts like lawyer and pharmacist can open only one office under such a restriction. If the regulation goes out of force, the procedure that multiple medical institutions should be opened and operated in the capacity as a medical corporation or a non-profit corporation does not have to be followed. And we should keep in mind that the permission for medical personels to open multiple medical institutions could lead virtually to commercial hospital. If in the nation with a very low rate of public medical service, If only a few medical personnels with capital own many medical institutions and operate commercially them, this could cause a falling-off in quality of medical service, ultimately infringe on the health rights and the life right of the people.

The Effect of Internal Marketing on Customer Orientation of Dental Hygienists (내부마케팅이 치과위생사의 고객지향성에 미치는 영향)

  • The purpose of this study was to reveal association between internal marketing and customer orientation. Internal marketing was composed of empowerment, educational training, reward system, internal communication and management support. We thought these factors affect to the consumer orientation. For this study, 191 dental hygienists in Busan, Ulsan and Kyungnam are participated in this study. The data were analyzed with descriptive statistics, t-test, ANOVA, pearson's correlation coefficients, and stepwise multiple regression analysis with SPSS 18.0 program. In conclusion, we obtained the next results. First, the mean of internal marketing behavior was 3.22 out of 5. In terms of sub-domain, educational training (3.88) is the highest, followed by empowerment (3.35), internal communication (3.10), management support (3.05) and reward system (2.79). Second, the internal marketing factors of internal communication, reward system orientation, management support, empowerment, and educational training had positive correlations with customer orientation (r=0.189~0.381). Third, the influencing factor in customer orientation were educational training (${\beta}=0.277$) and empowerment (${\beta}=0.276$), adjusted $R^2=0.202$. As dental patients' desire to medical service quality becomes diversified, the analysis result is considered to help the future dental service management.

A study on patients' choices over dental clinics and factors of complaint against medical service -focused on Seoul, Kyoungi, and Incheon areas (치과내원환자들의 의료기관 선택 및 의료서비스 불만요인에 관한 연구 - 서울, 경기, 인천지역을 중심으로 -)

  • In order to provide basic information to help dental medical service with supplying gratifying service for patients. the survey is conducted on the patients who frequently visited dental clinics located in the areas such as Seoul, Kyoungi, and Incheon from May of 2006 to June. The survey results are as following : 1. Judged from the factors to make patients choose one dental clinic over another. the low cost of treatment accounts for 32.8%. and followed by physician's academic background / career experiences with 20.5%. In case of recommending to their acquaintances, physician's proficiency in treatment is ranked high as 56.7%. 2. For most desirable facilities for dental hospitals. the results indicate that cutting-edge medical equipment holds 46.7% and followed by a comfortable waiting room with 32.8%. 3. Examined the questions of what patients are most satisfied with dental clinics. 43.1% of respondents point out kind and detailed explanation. 14.1% also responds to constant care after the treatment. This result suggests that after-care and detailed explanation also play an important role in dental care as well as the treatment itself. 4. For desirable services in the waiting room. 28.5% of the respondents choose a selection of magazines and newspaper and 18.7% say that they want to converse with the staff. As to how long can wait with patience. 38.2% of the participants agree on as long as 20 minutes. 5. Among unsatisfactory sources against dental clinics while visiting, the high cost of the treatment is scored high as 34.1% and followed by a long waiting time with 22.1%. The figure points to the fact that dental medical clinics should consider these two factors to solve in the future. In case where patients find the treatment discontent and they are asked what to do, 36.4% of the respondents respond that they go to a different hospital without saying anything. As to the enquiry about how the complains should be dealt, 46.% wishes that the matter should be taken care immediately upon complaining, 21.8% suggests that they like to feel understood and acceptable when presenting complaints.

The Effects of Oral Health Belief on Use of Dental Service of Students of Health and Medical Department of Universities (일부 보건의료계열 대학생들의 구강건강신념이 치과서비스 이용에 미치는 영향)

  • This study was conducted on 350 students of health and medical department of universities in Busan from June 3, 2013 to June 21, 2013, and a total of 350 questionnaires were researched. Among them 337 responses were used after 13 unfaithful responses were excluded. The effects of oral health belief on use of dental service of students of health and medical department of universities were examined and analyzed to provide basic data which can be used to expand understanding about oral health belief, to promote changes in behaviors regarding oral health and to improve oral health of the public. Collected data was analyzed through SPSS (Statistical Packages for Social Science 15.0. SPSS Inc. USA). For verification of differences of oral health belief depending on general matters and oral health behavior, t-test and ANOVA analysis were conducted, and for examination of the effects of oral health belief on use of medical service, logistic regression and regression analysis were conducted. The study results suggest that those who had higher sensitivity among oral health belief variables had higher probability of needing dental treatment and seeing the dentist immediately. And those who showed higher sensitivity and importance had higher frequency of seeing the dentist for six months. Future studies need to be conducted on the methods to change oral health belief for sustainable and systematic oral health enhancement in consideration of the factors affecting oral health belief and oral health behavior of the students of dental hygiene department.

Analysis of Business Performance in Dental Hygiene Process (ADPIE) in Dental Clinic (치과의료기관의 치위생과정(ADPIE) 경영성과 분석)

  • This study, the value of dental hygiene process and business performance among the dental clinics located in Gyeonggi province by comparing and analyzing the financial and non-financial results specifically in the department that provides and did not provide dental hygiene process (ADPIE). The collected data treated with percentage and t-test in utilization of IBM SPSS Statistics ver. 20.0. In terms of the medical cost per patient, the Department A (DA) that applied the dental hygiene process were 216,664 Korean Won (KRW) in 2013 and 324,810 KRW in 2014 whereas Department B (DB) which did not apply the dental hygiene process resulted in 184,655 KRW in 2013 and 225,698 KRW in 2014 (p<0.01). Regarding the number of daily patients, the DA showed increase of 8.08 (p=0.01) while DB showed increase of 2.42 patients (p>0.05). The medical consent rate was 89.17% in DA and 60.09% in DB in 2013 while showing 89.68% and 66.98% respectively in 2014 (p<0.001). The patients' revisit rate was 87.48% in DA and 44.92% in DB in 2013 and that of the DA and DB was 85.89% and 45.55% respectively in 2014 (p<0.001). The rate of regular check-up was 16.01% in DA and 2.53% in DB in 2013 and the same rate in 2014 showed 19.03% and 6.84% respectively in 2014 (p <0.001). The rate of referred patients was 38.46% and 29.98% respectively in DA and DB in 2013 whereas DA showed 47.59% and DB showed 30.77% in 2014 (p<0.05). According to the results, the medical system with dental hygiene process is verified to be a premium medical program that can improve satisfaction as well as management effectiveness in dental service.
