Infraocclusion is defined as tooth whose relative occlusal movement was blocked during the period of active eruption due to ankylosis and so on. Then infraoccluded tooth remains under the occlusal plane composed by adjacent structures showing normal eruption patterns. Untreated infraocclusion may cause: prolonged retention of infraoccluded teeth; extrusion of apposed teeth; destruction of periodontal tissues by occlusal force and food packing; increased sensitivity for dental caries; and disturbances on eruption pathway of succedaneous teeth. Therefore, periodic check-ups and proper treatments are required. There are many treatment options on infraoccluded deciduous molars such as periodic observation, conservative method, restoration and space regaining with extraction of the teeth. The choice of treatment may depend on the presence of succedaneous teeth, time of diagnosis and degree of infraocclusion. In this case report, three patients showing displacement of the second premolars due to infraocclusion of upper second primary molars, were treated by means of space regaining with removable orthodontic appliances and extraction of ankylosed primary molars. All malpositioned permanent premolars in the 3 cases showed ordinary eruption pathways after treatment.

Orthodontic brackets often need to be bonded to porcelain such as porcelain fused to metal crowns and porcelain jacket crowns. The purpose of this study was to evaluate the clinical usability of direct bonding system on porcelain teeth by measuring shear bond strength according to various conditions and observing adhesive failure patterns. The specimens, 20 maxillary premolars and 80 porcelain teeth that were produced by duplication of the labial surface of a maxillary first premolar were used and randomly divided into four groups of twenty teeth each. The 5 different preparation procedures tested: (1) application of 37% phosphoric acid on natural teeth, (2) sandblasting on porcelain surfaces, (3) sandblasting and application of 9.6% hydrofluoric acid on porcelain surfaces, (4) sandblasting and application of silane on porcelain surface, (5) sandblasting and application of 9.6% hydrofluoric acid and silane on porcelain surfaces. The metal brackets were bonded with Transbond $XT^{(R)}$ bonding material. The shear bond strength was tested by the micro universal testing machine(Kyung-Sung, Korea) and the amount of residual adhesive on the tooth surface after debonding was examined by stereoscope and assessed with an adhesive remnant index. The results of this study suggest that the direct bonding system on porcelain teeth with sandblasting, HF and porcelain primer is clinically useful.

With a prevalence rate of 0.01%, the presence of eruption disturbances of mandibular first molar has rarely been in populations. Eruption disturbances of permanent molars have been usually manifested as impaction, primary retention, and secondary retention. The treatments of eruption disturbances are carried out by: periodic observation; surgical exposure; forced eruption after surgical exposure; forced eruption with luxation; surgical repositioning; and extraction. This case report show successfully erupted mandibular first molars by various treatment methods on five patients diagnosed with impaction, primary retention, and secondary retention, respectively. Eruption disturbances of the mandibular first molar can be properly diagnosed with impaction, primary retention, and secondary retention by clinical and radiographic examination at normal eruption time of the mandibular first molar. The treatment should be done synthetically, considering eruption state of affected tooth, the relationship between the affected tooth and the adjacent or opposite tooth, the patient's age, treatment compliance, and economic state.
Plaster models were constructed and orthopantomographs were taken for 86 male and 70 female primary school pupils, whose upper and lower permanent 4 incisors and 1st molars were completely erupted without crowding; whose deciduous canines and molars were found almost uniformly even without any visible tooth fractures, dental caries or restorations on proximal surfaces of the teeth. Certain reference points on the orthophantomograph were set up and measured and the values were compared with actual or predicted values from the models. The following results were obtained: 1. In regards to available space, the values from the orthopantomograph were greater than the values from the models by a mean of 3.24% on the upper and 10.06% on the lower for males; 3.05% on the upper and 10.01% on the lower for females. 2. In regards to total mesiodistal widths of permanent canine, 1st and 2nd premolars, the values from the orthopantomograph were greater than the presumed values based on the size of lower permanent 4 incisors from the models by a mean of 18.50% on the upper and 24.09% on the lower for males; 14.54 on the upper and 20.51% on the lower for females. 3. Comparing the magnified values of total mesiodistal widths of permanent canine, 1st and 2nd premolars with those of available space, the regression constants of regression equation (Y = a + bX) between them were a=3.2336, b=0.6533 on the upper and a=5.0138, b=0.3290 on the lower for males; a=2.5994, b=0.6521 on the upper and a=3.0113, b=0.6586 on the lower for females. 4. The correlation coefficients between the magnified values of available space and permanent canine, 1st and 2nd premolars were moderately positive as 0.6474 in the upper and 0.505 on the lower for males; 0.6493 on the upper and 0.6183 on the lower for females. 5. In regards to magnified values of the available space from the orthopantomographs there were no significant difference between sexes, (P>0.05) but of the total mesiodistal widths of permanent canine, 1st and 2nd premolars, a significant difference between sexes was found.(p<0.01).
The purpose of orthodontic treatment is to achieve normal occlusion and good facial esthetics for individual patients. To produce harmonized facial balance, treatment planning for patient who require orthodontic treatment should include both a hard tissue and soft tissue cephalometric analysis. Author studied to derive the normal standards of soft tissue profile in Koreans by roentgenocephalometric analysis. For this study 12 soft tissue profile landmarks were plotted and 23 linear length, 9 soft tissue thickness, 8 vertical height length, 12 angles of soft tissue profile, and 3 vertical proportion were measured. The subjects consisted of 166 males and 209 females from 7 to 19 years with normal occlusion and acceptable profiles, and were divided into five groups according to age. The obtained results were as follows; 1. From the basis of N-Pog (Nasion-Pogonion) plane, the growth of facial soft tissue in the middle region especially nose area was greater than others facial region. 2. From the basis of G-Pog' (Glabella-soft tissue Pogonion) plane, the values of linear measurement of soft tissue Nasion and Inferior labial sulcus decreased and nose tip grew forward as growing older. 3. The growth of the facial soft tissue thickness was greatest in superior labial sulcus and the thickness of soft tissue nasion gradually became thinner as growing old. 4. The thickness of upper and lower lip was 14.47mm, 14.57mm in adulr male, 12.76mm, 13.78mm in adult female. 5. The soft tissue thickness of the lower lip was thicker than that of upper lip in all age groups and both sexes, 6. The vertical length of the upper and lower lips were 25.04mm, 49.97mm in adult male and 23.50mm, 48.39mm in adult female. 7. By the significant test, there were significant difference between male and female in fifth adult group on all vertical length measurements of lower face. 8. In fifth adult group, the perpendicular distance from LS, LI to Steiner's line and Ricketts' esthetic line were as follow; Steiner line to LS, LI were 7.98mm, 5.84mm in male. Steiner line to LS, LI were 6.71mm, 5.08mm in female. Ricketts' esthetic line to LS, LI were -0.40mm, 1.72mm in male. Ricketts' esthetic line to Ls, LI were -1.38mm 0.65mm in female. 9. In fifth adult group, the facial convexity angle and lower facial component angle were $171.17^{\circ}142.94^{\circ}$ in male and $172.5^{\circ}$, $144.41^{\circ}$ in female.
The purpose of this study was to evaluate the effects of the mandibular retractive force on the mandibular condyle of growing dog. The experimental animals were six mongrel dogs of two-month old. Their deciduous dentition were completed. Two of them was used as control group, and experimental group was composed of remaining four. Head band and chin cup were made of cotton tape, and hooks are fabricated on the chin cup and had band for closed coil. Mandibular retractive force was 100g/side and chin cap appliance was used for 14 hours/day during night. Experimental group were sacrificed at 2, 4, 6, 8 weeks from beginning of the experiment. Right TMJ was prepared for histologic study and left TMJ was examined grossly for disc, fossa, and condyle. The conclusions are: 1. Two-month old control animal showed active cartilaginous growth on the mandibular condyle, therefore showed thick proliferative and hypertrophic zones. Remodeling process in the condyle head was observed in which there were bone resorption on the anterior surface and bone apposition on the posterior surface. 2. Four-month old control animal showed marked reduction of hypertrophic zone but the condylar bone remodeling was more pronounced. 3. In experimental group, there are marked reduction of hypertrophic zone at 4 weeks from beginning of experiment, and hypertrophic zone disappeared at posterior-superior portion of condyle in 6-week experimental animal. 8 week experimental animal showed slight recovery of hypertrophic zone. 4. In experimental group, bone deposition was increased at anterior surface of condyle, and bone resorption was increased at posterior surface of condyle. 5. In control group, the glenoid fossa and surrounding bone showed mainly bone apposition. But experimental group showed bone resorption at anterior surface of articular eminence and increased bone apposition at posterior surface of postglenoid spine. 6. No marked traumatic change was seen but 4 weeks and 8 weeks experimental animal showed flattening of posterior surperior condylar surface. Bone marrow of condyle showed minute focal bleeding in 2 weeks and 4 weeks experimental animal, and congestion and depression of hematopoietic bone marrow during all experimental period.
The Purpose of this study was to compare the degree of vortical head rotation and to evaluate the reproducibility of natural Head posture (NHP) according to two guiding methods, the head posture aligner (HPA) method and the solf balance posture (SBP) method. The subjects consisted of 30 adults. On the first day. lateral and frontal cephalometric radiographs were obtained through the two guiding methods. One mouth later. lateral and frontal cephalometric radiographs were obtained again through both guiding methods. The degrees of vertical head rotation of both guiding methods were compared and the reproducibility was evaluated for each guiding method. A comparison of the degrees of vortical head rotation for the two methods revealed that the vertical head posture was lower in the SBP method than in the HPA method by an average of $2.79^{\circ}$. All measurements obtained using the HPA and SBP methods with a time interval of one north did not show any significant difierence in lateral and frortal cephalometric radiographs. The results of the present study suggest that the SBP method may be used as an alternative to the HPA method in case the HPA method can not be applied.
This study aimed to investigate the effects of indomethacin on distribution and expression of COX-2 and IGF-1 in the mandibular condyle ofi growing dogs and to examine the number of chondroclasts around the mineralization zone indomethacin inhibits prostatlandin $E_2$ production in the tissue by inhibiting synthesis of cyclooxygenase 2. Prostaglandin $E_2$ stimulates insulin-like growth factor synthesis. Insulin-like growth factor stimulates growth of mandibular condylar cartilage. Eight mongrel dogs. aged 13-14 weeks, were divided into 4 groups. Group 1 and group 2 were administered indomethacin 2 mg/Kg/day orally two times a day for 7 days and 14 days respectively. Group 3 were administered indomethacin 8mg/Kg/day orally 2 times a day for 14 days, and 4he control group were administered a placebo. The mandibular condyle heads were sectioned in $5{\mu}m$ thickness The specimens were stained with H-E staining. COX-2 immunohistochemical staining and IGF-1 immunohistochemical staining and examined under microscope. After TRAP staining, the number of chondroclasts were calculated The observed results were as follows: Indomethacin inhibited expression and distribution of COX-2 and IGF-1 on the proliferative zone of condylar cartillage. Indomethacin decreased the number of chondroclastes on the mineralization zone by a time-dependent manner (P<0.05). Indomethacin inhibited expression and distribution of IGF-I by a dose and time-dependent manner. These results show that indomethacin inhibited expression and distribution of COX-2 and IGF-1 on the proliferative zone of condylar cartilage and decreased the number of chondroclasts and suggests that when indomethacin is administered for a long time, condyle growth could be delayed.
The purpose of this study was to redefine the cephalometric landmarks in three-dimensional (3D) images, which are used in orthodontic cephalometric radiography, and to evaluate the reproducibility of each landmark for 3D cephalometric analysis. Eighteen CT scans were taken at the Department of Diagnostic Radiology at Seoul National University Dental Hospital and manipulated with V works 4.0(Cybermed Inc., Seoul, Korea). The coordinate system was established using 7 reference points, with no more than 4 points on the same plane. These 7 points were generated as a volume model, the voxel size of which was 4 by 4 by 2 (threshold value=639). The cephalometric landmarks were selected at the multiplanar reformation (MPR) window on the volume mode of V works 4.0. The selected landmarks were exported to V surgery (Cybermed Inc., Seoul, Korea) for the calculation of coordinate values. All the data were taken twice with a lapse of 2 weeks by one investigator The reproducibility of each landmark was $0.17\~1.21mm$ in the x axis, $0.30\~1.53mm$. In the y axis, and $0.27\~1.81mm$ in the z axis. In all three axes, the range of error was similar. These error ranges were acceptable with regards to the pixel space and slice thickness. The most reproducible points were 1 points which were selected on the basis of the volume model. The least reproducible points were J points that were defined by sutures.
The present study was conducted to examine the morphometrics and function of the disk on both sides among patients with facial asymmetry (FA) and to elucidate plausible correlations between internal derangement (ID) and FA. The sample was composed of 10 males and 27 females with FA. The disk status of all subjects was evaluated by bilateral high resolution magnetic resonance scans in the sagittal (closed and open) and coronal (closed) planes. Five types of disk displacement were identified accordingly. The disk function was diagnosed as normal disk function, disk displacement with reduction, and disk displacement without reduction. The disk shape on sagittal MRI in closed position was classified as bi-concave, biplanar, funnel/hemiconvex, and deformed. The disk position, translation and rotation were also measured. The difference between the shifted side and non-shifted side was analyzed by statistical analysis. Approximately $70\%$ of the patients in the present study showed unilateral or bilateral ID. It was found that anterior disk displacements (ADD), especially rotational ADD, occurred more frequently in the shifted side, while normal disk position was observed mainly in the non-shifted side (p<0.01). The disk of the shifted side showed significantly deformed configuration and inferior-anterior disk position. However, the disk of the non-deviated side showed hyper-mobility during jaw opening movement. These results demonstrate that in FA patients, the disks status of the shifted side is different from that of the non-shifted side, a phenomenon that could be correlated to facial asymmetry.
