A 36-year-old male patient developed diffuse low back pain. His past medical history was unremarkable and had no family history of neuromuscular disease. He had no bladder and bowel problems. Creatine kinase was 172 U/L (normal < 170). Other fluid and blood chemistry tests were normal. Manual muscle test grades of extremities and sensory examination were normal. Muscle stretch reflexes were normal. Fasciculations and myotonia were not detected. Straight leg raising test was negative. There was no spinal root compression, spinal stenosis, or signal intensity change of spinal cord on magnetic resonance imaging (MRI). Fatty change and atrophy of the cervical, thoracic and lumbar paraspinal muscles were noted on MRI. Nerve conduction studies were normal. Electromyography showed 1+ positive sharp waves in the lumbar paraspinal muscles. Electromyography of upper and lower extremity muscles revealed no abnormal spontaneous activity. We report a rare case of severe paraspinal muscle atrophy with fatty degeneration in a Young Adult.

갈비뼈 병변 진단 과정은 방사선 전문의가 CT 스캐너를 통해 생성된 2 차원 CT 이미지들을 해석하며 진행된다. 병변의 위치를 파악하고 정확한 진단을 내리기 위해 수백장의 2차원 CT 이미지들이 세밀하게 검토되며 갈비뼈를 분류한다. 본 연구는 이런 노동 집약적 작업의 문제점을 개선시키기 위해 Biaxial Rib Segmentation(BARS)을 제안한다. BARS 는 흉부 CT 볼륨의 관상면과 수평면으로 구성된 2 차원 이미지들을 U-Net 모델에 학습한다. 모델이 산출한 세그멘테이션 마스크들의 조합은 서로 다른 평면의 공간 정보를 보완하며 3 차원 갈비뼈 볼륨을 재건한다. BARS 의 성능은 DSC, Recall, Precision 지표를 사용해 평가하며, DSC 90.29%, Recall 89.74%, Precision 90.72%를 보인다. 향후에는 이를 기반으로 순차적 갈비뼈 레이블링 연구를 진행할 계획이다.
Objective : The PDSQ is a brief and psychometrically strong self-report scale designed to screen for common DSM-IV Axis I disorders in clinical settings. In this study, the K-PDSQ was compared with the M.I.N.I.-Plus (Mini-International Neuropsychiatric Interview-Plus) for diagnostic validity and availability of the K-PDSQ as a part of standardization of the K-PDSQ. Methods : The 640 patients were evaluated with the K-PDSQ and the M.I.N.I.-Plus. Diagnosing with the M.I.N.I.-Plus, the diagnostic correspondence, administering time, sensitivity, specificity, ROC curve, and AUC of the K-PDSQ were evaluated. Results : For the diagnostic correspondence of the K-PDSQ, Cohen's kappa coefficient was .66 between the K-PDSQ and the M.I.N.I.-Plus. The administering time of the K-PDSQ was $18.2{\pm}11.80$ minutes. Both sensitivity and specificity of the K-PDSQ were higher: the mean sensitivity across 10 subscales of K-PDSQ was 86%; the mean specificity was 84%. All AUCs of each subscale were above .80, which were statistically significant. Conclusion : The K-PDSQ is valid and available as a diagnostic screening tool. It will be widely used in clinical settings for screening DSM-IV Axis I diagnosis because of its simplicity and high reliability.

With the aim of developing a smartphone-based point-of-care device that is small, inexpensive, and easy to handle by non-expert, we designed a fluorescence smartscope for counting particles and a DC motor-controlled particle positioning system. Our smartscope can count the number of fluorescent particles and fluorescently-stained white blood cells through a phone camera with an adaptor containing a LED, a ball lens and optical filters and an application running on a smartphone. The motor was controlled wirelessly via Bluetooth with an Android smartphone. We found that axial spinning of a helical microfluidic channel allows arrangement of particles having size similar to the white blood cells. The motor-controlled particle positioning system can minimize time-consuming manual processes and automate sample preparation process and thus, if integrated with the smartscope, it can be used for a point-of-care testing device based on a smartphone.
The two dimensional echocardiography is widely used to evaluate regional wall motion abnormality, because of its abilities to depict left ventricular wall motion. A number of researches have been processed for evaluation and quantitative analysis of left ventricular wall motion functions. In this paper, we proposed an algorithm which detects automatically and analyze quantitatively endocardial wall motion during systole. The echocardiograms were obtained in the short-axis views in normal subjects. Automated edge detection and endocardial contour tracking algorithm was applied to each frames, quantitative analysis based on segmentation was performed, pre-defined color overlays superimposed on the gray scale images, and the images was animated. The proposed algorithm provided automated, quantitative diagnosis of regional wall motion abnormality.

Journal bearings support rotors using fluid film between the rotor and the stator. Generally, journal bearings are used in large rotor systems such as turbines in a power plant, because even in high-speed and load conditions, journal bearing systems run in a stable condition. To enhance the reliability of journal-bearing systems, in this paper, we study health-diagnosis algorithms that are based on the supervised learning method. Specifically, this paper focused on defining the unit of features, while other previous papers have focused on defining various features of vibration signals. We evaluate the features of various lengths or units on the separable ability basis. From our results, we find that one cycle datum in the time-domain and 60 cycle datum in the frequency domain are the optimal datum units for real-time journal-bearing diagnosis systems.
Z-axis automatic tube current modulation technique automatically adjusts tube current based on size of body region scanned. The purpose of the current study was to compare noise, and radiation dose of multi-detector row CT (MDCT) of lower extremity performed with Z-axis modulation technique of automatic tube current modulation with manual selection fixed tube current. Fifty consecutive underwent MDCT venography of lower extremity with use of a MDCT scanner fixed tube current and Z-axis automatic tube current modulation technique (10, 11 and 12 HU noise index, $70{\sim}450\;mA$). Scanning parameters included 120 kVp, 0.5 second gantry rotation time, 1.35:1 beam pitch, and 1 mm reconstructed section thickness. For each subject, images obtained with Z-axis modulation were compared with previous images obtained with fixed tube current (200, 250, 300 mA) and with other parameters identical. Images were compared for noise at five levels: iliac, femoral, popliteal, tibial, and peroneal vein of lower extremity. Tube current and gantry rotation time used for acquisitions at these levels were recorded. All CT examinations of study and control groups were diagnostically acceptable, though objective noise was significantly more with Z-axis automatic tube current modulation. Compared with fixed tube current, 2-axis modulation resulted in reduction of CTDIvol (range, $-6.5%{\sim}-35.6%$) and DLP (range, $-0.2%{\sim}-20.2%$). Compared with manually selected fixed tube current, 2-axis automatic tube current modulation resulted in reduced radiation dose at MDCT of lower extremity venography.
Purpose : To analyze the clinical characteristics, spontaneous resolution rate and predictive factors of resolution in children with primary vesicoureteral reflux(VUR). Methods : Between October 1991 and July 2003, 149 children diagnosed with primary VUR at Chonbuk National University Hospital were reviewed retrospectively. All of the patients were maintained on low-dose antibiotic prophylaxis and underwent radionuclide cystograms at 1 year intervals over 3 years after the initial diagnosis of VUR by voiding cystourethrogram was made. Results : The median time to resolution of VUR was 24 months and the total 3 year-cumulative resolution rate of VUR was 61.7%. The following variables were associated with resolution of VUR according to univariate analysis-; age<1 year, male gender, mild grade of reflux, unilateral reflux, congenital hydronephrosis as clinical presentation at time of diagnosis of VUR, absence of focal defects in the renal scan at diagnosis, absence of recurrent UTI, renal scars and small kidney during follow-up. After adjustment by Cox regression model, five variables remained as independent predictors of VUR resolution; age<1 yew, relative risk 1.77(P<0.05), VUR grade I+II 2.98(P<0.05), absence of renal scars 2.23(P<0.05), and absence of small kidney 5.20(P<0.01) during follow-up. Conclusion : In this study, spontaneous resolution rate of VUR, even high grade reflux, is high in infants during medical management, and it was related to age, reflux grade at diagnosis, absence of renal scars and small kidney during follow-up. Therefore early surgical intervention should be avoided and reserved for the selected groups.
임상진단에 있어서 종종 문제가 되고 있는 호흡에 따른 두뇌부분의 가상적인 상하이동을 고려해서 위상 엔코드 축인 Y 방향만의 강체의 평행이동을 취급한다. 종래의 발견적인 축차 근사 반복처리에 의한 제거방법과는 달리, 본 연구에서는 MRI 촬상과정과 화상특성의 해석에 근거한 MRI 신호중의 체동성분과 화상성분을 단순한 대수연산에 의해 분리할 수 있는 새로운 구속조건을 도출한다. MRI 신호에 대해서 X 방향의 1차원 푸리에 변환을 행한 후의 Y 방향의 스펙트럼 위상값은 화상자신의 성분과 체동성분의 합이 되고 있다. 한편, 두뇌부위 등의 단층상에 있어서 주위의 피하지방 부분의 밀도는 거의 균일한 것 알려져 있어 이 부분위의 Y 방향의 1 라인 밀도분포를 대칭이라고 간주할 수 있다. 밀도함수가 대칭인 경우 스펙트럼의 위상은 그 위치에 대하여 선형적으로 변화한다. 따라서 이 선형함수로부터 벗어난 성분을 체동성분으로 분리할 수 있다. 이러한 구속조건에 기초를 둔 근거가 명확한 아티팩트의 제거방법을 제안한다. 아울러 본 방법의 유효성을 확인하기 위해 호흡에 의한 주기성의 체동을 가진 MRI 화상에 대해서 시뮬레이션을 행하였다.
의료 영상에서 관심 있는 부위를 3차원으로 재구성 하여 보는 것은, 정확한 진단을 위해서 매우 중요하다. 이러한 3차원 재구성을 위해서는 관심 있는 영역의 분할이 필수적인 선행작업이다. 본 논문에서는 3차원적 정보를 이용한 영상 분할 방법으로 슬라이스 기반의 3차원 영역 확장법을 제안한다. 제안된 방법은 2차원 슬라이스 영상에서 영역 성장법에 의해 영역을 확장시키고, 그 이웃한 슬라이스들에 씨앗을 전달하여 재귀적으로 3차원 영역을 확장하여 영상을 분할한다. 이때, 이웃한 슬라이스 간의 영역의 크기를 이용하여 새나감을 방지한다. 제안된 방법을 튜브 형태의 기관의 분할에 적용한 결과, 새나감 없이 뽀족한 가지들까지도 성공적으로 분할 했으며, 튜브의 중심 축이 고차원 곡선인 경우에도 성공적으로 분할했다.
