초보간호사의 월경곤란증이 임상수행능력에 미치는 영향 (The Effects of Dysmenorrhea on Clinical Competence in Early Stage Nurses)

  • 본 연구는 종합병원에서 근무하는 초보간호사의 임상수행능력에 영향을 주는 요인을 알아보고자 실시하였다. 초보간호사 155명을 대상으로 2020년 9월 11일부터 9월 30일까지 설문조사 하였으며, 수집된 자료는 SPSS 프로그램을 이용하여 t-test, ANOVA, Scheffe test, Person's correlation coefficients, 다중회귀분석을 하였다. 월경곤란증은 2.77점, 임상수행능력은 3.44점 이었고 월경곤란증과 임상수행능력은 유의한 음의 상관관계를 보였다(r=-.226, p=.002). 임상수행능력에 유의한 영향을 준 것은 자율신경계 반응이고(𝛽=-.261, p= .001) 설명력은 22.1%였다. 따라서 초보간호사의 임상수행능력을 증진시키기 위해 월경곤란증의 자율신경계 반응 완화를 위한 중재 교육 프로그램이 필요하다.

병원감염관리에 관한 인지도와 수행도 - 중소병원 초보간호사를 중심으로 - (Awareness and performance about nosocomial infection management; a early stage nurse in small and medium hospitals)

  • 중소병원에서 근무하는 초보간호사의 병원감염관리에 대한 인지도와 수행 정도를 확인하고, 병원감염에 대한 기초자료를 제공하고자 실시된 서술적 조사연구이다. 2017년 9월15일부터 2017년 9월 30일까지 A시와 B시에 300병상 규모의 중소병원 2곳의 초보간호사 150명중 무응답 및 미흡한 응답자 11명을 제외한 139명을 대상으로 하였다. 본 연구에서 수집된 자료는 SPSS Win 22.0 프로그램을 이용하여 실수, 백분율, 평균, 표준편차, t-test, ANOVA, 사후검정은 $Scheff{\bar{e}}$ test로 분석하였다. 대상자의 일반적 특성에 따라 병원감염관리에 대한 인지도를 비교해 본 결과 요로 감염관리, 호흡기 감염 관리 그리고 소독 오염물품 관리 부분은 연령에서 유의한 차이가 있었고, 수행 정도는 손 씻기 영역에서는 근무부서와 직위, 수액요법영역은 감염관리 교육 필요성 여부(p<.001)에서, 요로감염관리 영역 및 호흡기 감염관리 영역(p=<.001)에서는 직위에 따라 차이가 있는 것으로 나타났다. 또한 병원감염관리에 대한 인지도와 수행 정도에 대한 차이를 살펴본 결과 손 씻기, 수액요법, 요로감염관리, 호흡기 감염관리, 감염성 폐기물 관리, 소독오염 물품관리영역의 모든 영역에서 인지도가 수행도 보다 모두 높게 나타났으며 모두 유의한 차이를 보였다. 경험과 지식이 부족한 초보간호사들이 병원감염관리 간호를 원활하게 수행하기 위해 이론적 지식을 바탕으로 수행도도 높일 수 있는 체계적인 병원감염관리 관련 교육프로그램이 구체적으로 개발 운영되어야 할 것이다.

상급초보 간호사의 재직의도 영향요인 (The Factors Affecting advanced Beginner-Stage Nurse's Intention of Retention)

  • 본 연구는 상급초보 간호사의 재직의도에 영향하는 요인을 파악하기 위해 시도된 서술적 조사연구이다. 자료수집기간은 2017. 5. 01~6. 30까지이며, D시와 C지역 및 K지역에 소재한 종합병원에 근무하고 있는 병원 경력 2~3년차인 상급초보간호사 150명을 대상으로 구조화된 설문지를 이용하여 자기기입 하도록 설문조사하였다. 수집된 자료는 IBM SPSS/WIN 20.0을 이용하여 서술통계, t-test, ANOVA, Pearson 상관관계와 다중회귀분석을 구하여 분석하였다. 연구대상자의 자아탄력성은 3.34점(${\pm}0.53$), 소명의식은 2.28점(${\pm}0.44$), 팀워크역량은 3.88점(${\pm}0.58$), 간호조직문화는 3.36점(${\pm}0.42$)으로 중위수보다 높았으며, 재직의도는 5.27점(${\pm}1.38$)으로 중위수보다 높았다. 재직의도와 자아탄력성(r=.211, p<.05), 소명의식(r=.338, p<.001), 팀워크역량(r=.284, p<.001), 간호조직문화(r=.215, p<.01)는 양의 상관관계가 있었다. 회귀분석 결과, 소명의식이 재직의도에 대해 11.4%(${\beta}=.284$, p<.001)의 설명력이 있었으며, 다음으로 팀워크역량이 4.2%(${\beta}=.212$, p<.001)로서 이 변수들은 재직의도를 총 15.6% 설명하였다. 결론적으로 상급초보 간호사의 재직의도를 향상시키기 위해서는 소명의식과 팀워크역량 강화가 필요함을 파악하였으며, 특히 소명의식 중 '친사회적지향'과 팀워크역량 중 '상호협력'을 강화할 다양한 프로그램 개발 및 실행전략이 요구되어진다.

초보간호사의 자아탄력성, 자기효능감, 간호전문직 자아개념의 관계 (Relationship between Ego Resilience, Self Efficacy and Professional Self-concept of Nurse in Early stage nurses)

  • 본 연구는 초보간호사의 자아탄력성, 자기효능감, 간호전문직 자아개념 정도를 파악하고 이들 간의 관계를 확인하여 초보간호사의 이직의도를 낮추기 위한 기초자료로 활용하기 위하여 시행되었다. 대상자 자료 수집은2015년 6월 1일부터 7월 7일까지 일 광역시 소재 3개의 종합병원에서 근무하는 경력 3년 이하의 초보간호사 145명을 대상으로 구조화된 설문지를 이용하여 자기기입 하도록 설문조사 하였다. 수집된 자료는 SPSS/WIN 18.0을 이용하여 서술통계, t-test, ANOVA, Pearson 상관관계를 구하여 분석 하였다. 연구결과 자아탄력성은 3.13점(${\pm}0.46$), 자기효능감은 3.64점(${\pm}0.64$), 간호전문직 자아개념은 2.95점(${\pm}0.50$)이었다. 초보간호사의 일반적 특성이 변수에 영향을 미치는 요인으로는 결혼유무가 자아탄력성에, 임상경력이 간호전문직 자아개념에 유의한 차이가 있었으며 그 외 항목은 유의하지 않았다. 자아탄력성과 간호전문직 자아개념은 정적인 상관관계 있었으며(r=.305, p<.001), 자기효능감과 간호전문직 자아개념은 정적인 상관관계가(r=.279, p=.001) 있었다. 이와 같은 연구결과를 기반으로 초보간호사의 자아탄력성, 자기효능감, 간호전문직 자아개념을 강화시켜줄 수 있는 병원 프로그램을 개발하고 활용하여 간호사의 직업 만족감을 높여주는 것이 필요하다.

상급초보 간호사의 소명의식, 직무만족, 조직몰입이 재직의도에 미치는 영향 (Effects of Advanced Beginner-Stage Nurses' Sense of Calling, Job Satisfaction and Organizational Commitment on Retention Intention)

  • Purpose: This study was designed to examine effects of sense of calling, job satisfaction and organizational commitment on retention intention in nurses who are at the advanced beginner-stage. Methods: Participants were 199 nurses with 13 to 36 months' clinical career at hospital located in Busan, Korea. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Scheffe test, Pearson correlation, and Multiple regression. Results: Retention intention correlated positively with sense of calling (r=.43, p<.001), job satisfaction (r=.31, p<.001) and organizational commitment (r=.31, p<.001). Factors affecting the participants' retention intention were found to be statistically significant (F=21.96, p<.001). Total factor score accounted for 24.1% of retention intention. Out of the related factors, the most influential factor was 'goals/meaning', belonging to the sub-areas of the sense of calling and explaining 18.2% of retention intention, followed by organizational commitment and average monthly income in that order. Conclusion: The results of this study suggest a need to develop a management program that helps promote retention intention for nurses at the advanced beginner-stage by both improving their monthly income and outside conditions and, more importantly, raising their sense of calling, especially in association with the goals/meaning of their life.

상급초보 간호사의 감정노동과 극복력이 재직의도에 미치는 영향 (Effects of Emotional Labor and the Resilience on Retention Intention among Advanced Beginner-Stage Nurses)

  • Purpose: The present study examined the psycho-emotional factors that affect the retention intention among advanced beginner-stage nurses. Methods: A cross-sectional study was conducted using a convenience sample of 118 nurses with a work experience of at least one to three years of work experience in a university hospital in Suwon, Korea. Structured questionnaires were used to assess retention intention, emotional labor, and resilience. Multiple regression analysis was used to examine factors affecting retention intention. Results: Retention intention had a moderate mean score (4.75 out of 8). Factors influencing participants' retention intention were founded in the order of resilience (β=.29, p<.001) and emotional labor (β=-.20, p=.011) after adjusting nursing satisfaction as a covariate. The multiple regression model accounted for 42% of the variance in retention intention (p<.001). Conclusion: Resilience and emotional labor may be primary factors influencing retention intention of advanced beginner-stage nurses. Therefore, this study finding suggested that psycho-emotional factors such as emotional labor and resilience should be considered while designing programs to increase nurses' retention intention.

초보간호사의 LIFO 행동유형에 따른 직무 스트레스, 직무만족도와 조직몰입 (Job Stress, Job Satisfaction, and Organizational Commitment according to Life Organization (LIFO) Behavior Type of Novice Nurses)

  • Purpose: This study was done to provide data that would contribute to effective task performance in clinical nursing and development of nursing professional by understanding job stress, job satisfaction and organizational commitment according to Life Organization (LIFO) behavior types of novice nurses. Methods: This study was a cross-sectional survey and the participants were 313 novice nurses who worked in one university hospital located in the city of Gwangju. Data were collected through the structured questionnaires that included general characteristics, job stress, job satisfaction, and organizational commitment scales. Collected data were analyzed using descriptive statistics, and ANOVA with SPSS/WIN 19.0. Results: Regarding job stress according to LIFO behavior types of novice nurses, there were statistically significant differences between the favorable situation and the unfavorable situation. There were also statistically significant differences regarding the organizational commitment according to LIFO behavior types. Conclusion: The results indicate a difference in behavior patterns of novice nurses that suggest a need for education in order to reduce stress from the job and enhance organizational commitment by developing each nurse's preferred pattern and compensating for unfavorable patterns, as well. There also needs to be nursing resources that take into consideration behavior patterns of novice nurses.

중환자실과 응급실 초보간호사를 위한 맞춤형 시뮬레이션 학습프로그램 개발 및 효과 (The Development and Effects of a Tailored Simulation Learning Program for New Nursing Staffs in Intensive Care Units and Emergency Rooms)

  • Purpose: The purpose of this study was to develop a tailored simulation learning (SL) program and to evaluate the effects of the program on the clinical competency, clinical decision-making competency, and communicative competency of new nursing staffs in intensive care units (ICU) and emergency rooms (ER). Methods: In this quasi-experimental study, fifteen nurses were assigned to the experimental group and fifteen to the control group. The experimental group was given the SL program of four sessions, whereas the lectures of four sessions were given to the control group. Data were analyzed using a $x^2$-test, Mann-Whitney U test, and Kruskal-Wallis test with an SPSS program. Analysis of covariance was used to treat the covariate of pre communicative competency between the experimental and control groups. Results: Based on the education needs of new nursing staffs in ICUs and ERs, three learning scenarios and one evaluating scenario were developed for the SL program. The score for clinical competency, clinical decision-making competency, and communicative competency were significantly higher in the experimental group compared with the control group. Conclusion: The SL program is an effective learning strategy for new nursing staffs in ICUs and ERs. These findings suggest that an SL program be offered as an alternative for new nurse orientation and continuing nurse education.

초보간호사의 대인관계능력, 직무 스트레스, 스트레스 대처유형이 이직의도에 미치는 영향: 직무 스트레스를 중심으로 (Impact of Interpersonal Relationship Ability, Job Stress, and Stress Coping Type on Turnover Intention of Nurses in the Early Stage of their Careers: Focusing on Job Stress)

  • Purpose: This study aimed to examine the effects of interpersonal relationship ability, job stress, and stress coping type on the turnover intention of nurses in the early stage of their careers. Methods: Data were collected from 136 early stage nurses from two general hospitals and one tertiary hospital in Chungbuk province. An early stage nurse was defined as the nurse with a work experience of 3 to 36 months. The collected data were analyzed using independent t-test, one-way ANOVA, Mann-Whitney U test, Person's correlation coefficient, and stepwise multiple regression. Results: It was seen that turnover intention had a negative correlation with interpersonal competency (r=-.20, p=.020) and a positive correlation with job stress (r=.56, p<.001). Job stress due to work over load, inadequate compensation and working environment, interpersonal relationship ability, and length of clinical career were significant factors influencing turnover intention (p<.05). These variables explained for 50.4% of the variance in turnover intention. Conclusion: It is necessary to develop an interpersonal relationship ability improvement program to decrease turnover intention among early stage nurses by helping them to successfully adapt to their job. Moreover, it is important to bring about continuous improvements in terms of provision of adequate workload and appropriate rewards, and creation of a suitable work environment to reduce the job stress of early stage nurses.

상급초보 간호사의 이차피해경험과 긍정심리자본이 재직의도에 미치는 영향 (The Influences of Second-victim Experiences and Positive Psychological Capital on Intention to Remain among Advanced Beginner Nurses)

  • Purpose: The aim of this study was to examine the influence of second victim experiences following patient safety incidents and positive psychological capital on the intention to remain among advanced beginner nurses working at tertiary hospitals. Methods: The participants were 179 nurses with a clinical experience of 13 to 36 months at two tertiary hospitals. We collected data using questionnaires related to second victim experiences, positive psychological capital, and intention to remain. Data were analyzed using descriptive statistics, independent t-test, a one-way analysis of variance (ANOVA), Scheffé test, Pearson correlation, and hierarchical regression analysis. Results: The intention to remain in advanced beginner nurses was negatively correlated with second victim experiences and positively correlated with positive psychological capital. A hierarchical multiple regression model with control variables (subjective job satisfaction and self-assessed work proficiency), psychological capital, and second victim experience accounted for 26% of the factors that influenced advanced beginner nurses' intention to remain. Among the related factors, the most influential was subjective job satisfaction. Second victim experience and positive psychological capital also influenced the advanced beginner nurses' intention to remain. Conclusion: The results of this study suggest a need to develop a program that helps promote intention to remain for nurses at the advanced beginner stage by minimizing second victim experiences and improving positive psychological capital and the work environment.
