  • This study involved the effects of home-visit program with cognitive-stimulating activities on cognitive function and memory self-efficacy of an elderly with dementia special rating(5rates). The study was conducted from February 11 to April 26, 2019 using a 65-year old woman residing in G Metropolitan City. The individual subject study used an A-B-A design. The subject performed activities that required thinking and resolving in order to stimulate both her body and cognition. The activities chosen were easily accessible to those having a form of mild dementia, rates 5 dementia special rating, and repetitively stimulated the subject's intellectual capacity in an attempt to increase her degraded cognitive functioning. Specifically, the activities exercised both short-term memory and recollection using familiar letters, numbers, and bodily movements frequently encountered in daily life. The program positively affected the subject's memory and concentration. In addition, the program was found to be feasible for home use by those with mild dementia, obviating the need to travel to a location like a welfare center. The author presents specific activities for stimulating cognition that can effectively intervene to positively influence the cognitive functioning and memory of those with rates 5 dementia special rating.

The Effects of Emotional Labor, Burnout on Job Stress and Job Satisfaction of Nursing Home Workers : Focusing on Care Experience of Dementia Patients (요양원 종사자의 감정노동, 소진이 직무스트레스 및 직무만족에 미치는 영향 : 치매환자 돌봄 경험을 중심으로)

  • Patients with dementia are increasing steadily and becoming a social problem. Care workers in hospitals are increasingly interested in emotional labor. Therefore, this study examines the caregiving experience of dementia patients in the nursing home. The effects of emotional labor and exhaustion of nursing home workers on job stress and satisfaction were analyzed and the moderating effect of care experience was analyzed. The results of the study are as follows. First, the caregiver experience of the nursing hospital workers showed the initial mind, the perception of the early dementia patients, the difficulties, the physical and mental limitations, the satisfaction, the experience through nursing practice, and the change of mind about life. Second, emotional labor and burnout have positive effects on job stress. Care experience reduced the impact of emotional labor and exhaustion on job stress. Finally, the relationship between job stress and job satisfaction showed positive influence.

Development of a Smart Application for Protecting Dementia Patients (치매환자의 보호를 위한 스마트 앱 개발)

  • The applications for considering their position and daily patterns of dementia patients have been developed in an early stage even if the older patients who have weaker or serious symptoms has increased in various forms. In this paper, we develop an android-based application which displays positions and pathways of patients on maps and provide messages in the cases of dangerous situations. Guardians need to register schedules including safe areas and personal information of their patients. This system registers behavior status categorized as normal or abnormal each position which is sent to a database. In particular, the deviation status is assigned in case the patients are not within the safe areas that their guardians registered on their schedule. The wandering status is assigned in case the patients are repeatedly passed by their pathways. This smart application contains the modules such as patient position sending, guardian and patient information, patient schedule and safe zone registration, position and behavior status registration, pathway display and message sending, and rescue request. This system sends the notification and alarm service providing normal and abnormal behavior with deviation and wandering status of patients respectively.

Changes in cognitive function and functional disability in older adults - Comparison of groups converted and not converted to dementia among cognitively normal older adults - (노년기 인지기능과 일상생활기능장애 변화에 대한 연구 - 치매발생집단과 정상유지집단 간 차이 -)

  • This study aimed to analyze the trajectories of cognitive function and functional disability, and to identify the impacts of socio-demographic factors on the two variables. Based on the assumption that there are differences according to the progress of cognitive impairment, it focused on examining the differences in trajectories of cognitive function and functional disability between two groups: dementia conversion group and dementia non-conversion group among cognitively normal older adults. This study was conducted based on the data from a 3-wave panel study of a sample of 966 Yeoncheon elderly cohort aged 65 and over between 1997 and 2003. Latent growth curve model and multi-group structural equation modeling were used to examine the hypothesis. Results revealed that dementia conversion group exhibited faster rate of cognitive decline as well as lower initial cognitive level. Difference between two groups was not significant in the initial level of functional disability, yet dementia conversion group showed greater degree of deterioration in the functional disability over time than dementia non-conversion group did. In terms of the influence of socio-demographic factors on cognitive function and functional disability, the cognitive decline was more drastic in the female group, whereas functional decline was more apparent for the male group. The level of education in early life had a strong impact on the cognitive function in later life. Based on these findings, practical implication for high risk groups in cognitive function and functional disability were discussed.

Prediction Performance of Naming Tests for Differentiating Mild Cognitive Impairment and Mild Dementia (경도인지장애와 경도 치매의 감별을 위한 대면 이름대기와 범주 이름대기의 예측 성능 비교)

  • The present study identify the predictive power of confrontational naming and generative naming as screening tests for normal and early cognitive impairment. The subjects were analyzed for 203 healthy elderly, 106 mild cognitive impairment (MCI), 31 mild dementia. The confrontational naming was measured by the short-term Korean Boston Name Waiting Test, and the generative naming was measured by the Control Associative Word Test. As a result of polynomial logistic regression, both confrontational naming and generative naming had a significant effect on discriminating cognitive impairment (MCI, mild dementia) in general elderly (p<0.05). On the other hand, when distinguishing mild dementia from mild cognitive impairment, the generative naming-phonetic test had no significant odds ratio. The results of this study suggest that when discriminating mild dementia in mild cognitive impairment group, it is not meaningful to look only at the total score of generative naming test.

  • This study examined the relationships among dementia knowledge, attitude, and preventive behavior of an urban-rural complex city. In this study, 483 subjects using welfare and health centers were selected from one urban-rural complex city. The data were collected using the questionnaires for dementia knowledge, attitude and preventive behavior. Data analysis was performed by descriptive statistics, t-test and ANOVA using SPSS 19 software. In both urban and rural areas, elderly more than 70 years of age, no spouse, less educated, and lower income had higher scores of dementia attitude. In addition, the dementia attitude scores were higher when they were thinking more of their bad health. In conclusion, there is a need to strengthen the training and programs for dementia's knowledge, attitudes and prevention activities by a spouse or family member in early elderly period.

The effect of computer based cognitive rehabilitation program on the improvement of generative naming in the elderly with mild dementia: preliminary study (한국형 전산화 인지재활프로그램이 초기 치매노인의 생성 이름대기 수행에 미치는 효과에 관한 예비연구)

  • The purpose of this study was to investigate the effect of computer based cognitive rehabilitation program on the generative naming. Twenty - one patients were assigned to the CoTras program and eight were treated with traditional face - to - face language rehabilitation such as paper and table activities. The experimental group and the control group performed sequential language recall memory training, association memory recall training, language categorization memory training, and language integrated memory training for 12 weeks. The Welch's robust ANCOVA showed significant differences in mean fluency and MMSE-K changes (p<0.05). On the other hand, phonemic fluency increased significantly after 12 weeks of treatment compared to baseline in both experimental and control groups, but there was no statistically significant difference between treatment groups. The results of this study suggest that the computer based cognitive rehabilitation program may be more effective in improving the semantic fluency than the conventional cognitive-linguistic rehabilitation.

Design and Implementation of an IoT Device for the Effective Screening of Dementia and Mild Cognitive Impairment

  • Korea is rapidly becoming an aging society due to lower birth rate and longer life expectancy. As elderly diseases are of a serious concern, the number of patients with dementia has been increasing significantly. For now, it has become a nationwide problem, as dementia is difficult to manage and requires a lot of social costs. To solve this, an IoT device has been devised that efficiently screens Mild Cognitive Impairment(MCI), the early stage of dementia. To design and implement the device, the latest IoT technology is introduced, the optimized Korean Version of Hasegawa Dementia Scale Revised(K-HDS) is utilized, and convenient service scenario with user-friendly design is applied. As a result, the symptoms of MCI have been screened quickly with convenient operation, which lead to the healthier society by the prevention of dementia.

Differences of Tc-99m HMPAO SPECT Imaging in the Early Stage of Subcortical Vascular Dementia Compared with Alzheimer's Disease (초기 단계의 피질하 혈관성 치매와 알쯔하이머병에서 Tc-99m HMPAO SPECT 영상 소견 차이)

  • Purpose: The aim of this study is to assess the specific patterns of regional cerebral blood flow (rCBF) in patients with the early stage of subcortical vascular dementia (SVaD) and Alzheimer's disease (AD) using Tc-99m HMPAO SPECT, and to compare the differences between the two conditions. Materials and Methods: Sixteen SVaD, 46 AD and 12 control subjects participated in this study. We included the patients with SVaD and AD according to NINCDS-ADRDA and NINDS-AIREN criteria. They were all matched for age, education and clinical dementia rating scores. Three groups were evaluated by Tc-99m HMPAO SPECT using statistical parametric mapping (SPM) for measuring rCBF. The SPECT data of patients with SVaD and AD were compared with those of normal control subjects and then compared with each other. Results: SPM analysis of the SPECT image showed significant perfusion deficits on the right temporal region and thalamus, left insula and superior temporal gyrus, both cingulate gyri and frontal subgyri in patients with SVaD and on the left supramarginal gyrus, superior temporal gyrus, postcentral gyrus and inferior parietal lobule, right fugiform gyrus and both cingulate gyri in AD compared with control subjects (uncorrected p<0.01). SVaD patients revealed significant hypoperfusion in the right parahippocampal gyrus with cingulated gyrus, left insula and both frontal subgyral regions compared with AD (uncorrected p<0.01). Conclusion: Our study shows characteristic and different pattern of perfusion deficits in patients with SVaD and AD, and these results may be helpful to discriminate the two conditions in the early stage of illness.

Analysis of the Mental Images in Episodic Memory with Comparison between the patients with Dementia of Alzheimer Type and Healthy Elderly People (알츠하이머성 치매환자와 건강한 노인의 일화기억 이미지 비교 분석)

  • Episodic memory, i.e. memorization of information within a spatiotemporal environment, is affected Alzheimer's disease(AD), but its impairment may also be occurred in the normal aging process. The purpose of this study is to analyze and evaluate memory in with Dementia of Alzheimer Type by examining their cognitive skills in episodic memory using the technique. This new method involves assessing the mental images the subject's own past in the mind like projected and movies. Three patients in the early stage of Dementia of Alzheimer Type, one with mild depression, and 2 healthy controls for comparison were asked to retrieve their episodic memory of the previous day, week, month, and a day testing day. The answers were then analyzed with regards to their specific features as emotional state, color, and time order. In the following day, the subjects were tasked to recall again the images they reproduced in the day's test order to observe of memory. Results showed that all 3 patients failed to arrange the retrieved images in time order and their images of the previous day were unclear in color and were stationary like photographs, even when they reproduced the mental images at much quantity as controls. patients could not remember particular events of yesterday, and only recalled the general occurrences of every day life. These results suggest that in the early stage of Dementia of Alzheimer Type, difficulties in the retrieval of recent episodic memory begin to primarily occur, and qualitative impairment happens earlier than quantitative.
