Loss of multiple teeth by multiple dental caries, traumatic influence or hereditable disease can cause many problem in children. Using removable appliance can be used for treatment of this problem. But conventional removable appliance has limits of retention by wire-clasp, non esthetic appearance, difficulty in pronunciation by its volume. Flexible denture using $Valplast^{(R)}$ system could be a excellent partial denture for restorarion in child because of various advantage such as retention from soft tissue, natural esthetic aspect, biocompatibility, durability to compare with conventional acrylic appliance. The present case report describe properties and consideration of flexible denture using Valplast $system^{(R)}$ in comparison with conventional acrylic appliance and report successful restoration of child using Valplast $system^{(R)}$.
Purpose : The purpose of this experiment was to evaluate the diagnostic ability of a CCD-based digital system (CDX-2000HQ) in the detection of incipient dental caries. Materials and Methods : 93 extracted human teeth with sound proximal surfaces and interproximal artificial cavities were radiographed using 4 imaging methods. Automatically processed No.2 Insight film (Eastman Kodak Co., U.S.A.) was used for conventional radiography, scanned images of conventional radiograms for indirect digital radiography were used. For the direct digital radiography, the CDX-2000HQ CCD system (Biomedisys Co. Korea) was used. The subtraction images were made from two direct digital images by Sunny program in the CDX-2000HQ system. Two radiologists and three endodontists examined the presence of lesions using a five-point confidence scale and compared the diagnostic ability by ROC (Receiver Operating Characteristic) analysis and one way ANOV A test. Results: The mean ROC areas of conventional radiography, indirect digital radiography, direct digital radiography, and digital subtraction radiography were 0.9093, 0.9102, 0.9184, and 0.9056, respectively. The diagnostic ability of direct digital radiography was better than the other imaging modalities, but there were no statistical differences among these imaging modalities (p > 0.05). Coclusion : These results indicate that new CCD-based digital systems (CDX-2000HQ) have the potential to serve as an alternative to conventional radiography in the detection of incipient dental caries.

The purpose of this study was to evaluate the specificity, sensitivity, and diagnostic power of caries activity test using laser fluorescence. The subjects of this study were 50 children of $7\sim9$ years old. Fluorescence from initial carious lesion of teeth illuminated by an argon laser(480nm) was observed and photographed with barrier filter. Visual examination for the dDfFtT rate and Streptococcus mutans colony counting was done to evaluate correlation with caries activity test using laser fluorescence. Data analysis was accomplished by Axelsson's method. The results from the present study can be summarized as follows: 1. There was positive correlation $(\gamma=0.48)$ between laser fluorescence test and Streptococcus mutans count. And also positive correlation $(\gamma=0.39)$ exists between laser fluorescence test and dDfFtT rate (P<0.01). 2. Positive correlation $(\gamma=0.27)$ between Streptococcus mutans colony count and dDfFtT rate was found(P<0.05). 3. When dDfFtT rate was defined to standard testing method, the specificity, senstivity, and diagnostic power of laser fluorescence test were 44.4%, 85.7%, and 87.8%. 4. When dDfFtT rate was defined to standard testing method, the specificity, senstivity, and diagnostic power of S. mutans colony counting were 77.8%, 92.9%, 84.8%. 5. When S. mutans colony counting was defined to standard testing method, sensitivity, specificity and diagnostic power of laser fluorescence test were 40.0%, 84.8%, 95.1%. In regard to above results, laser fluorescence test considered to be accurate and reliable method for determining caries activity because of it's close relationship with caries susceptibility test and caries experience measurements. And it was also considered to be practical because it would be simple, inexpensive, and time saving method.

QLF-D system composed with DSLR(digital single-lens reflex) camera, and the images of natural enamel caries and artificial caries was developed from 2 days to 14 days captured by QLF-D system. The correlation between lesion depth of the polarized microscope and luminosity ratio of QLF-D image was analyzed and the results were summarized as follows: 1. The Pearson correlation value between the lesion depth of polarized microscope images and luminosity ratio of QLF-D images was 0.969(p<0.01). 2. From Regression analysis of lesion depth from polarized image by demineralized period, the equation was y = 8.67x - 1.16(p<0.05). 3. From Regression analysis of luminosity ratio from QLF-D image by demineralized period, the equation was y = 3.53x + 6.42(p<0.05). From the results, QLF-D system can detect the enamel caries at the very early stage and can monitor the progression of demineralization and remineralization. For the convenient use of QLF-D system in the laboratory, the image analysing software was needed to analyze of interest site of enamel caries lesion.
The purpose of this study was to evaluate the detection ability of secondary caries using qunatitative light-induce fluorescence-digital (QLF-D) device. Twenty bovine teeth with cavity on surface were demineralized during 21 days for secondary caries lesion of cavity wall. After 21 days, cavity was filled using composite resin and cut the specimen in half with disc. Fluorescence loss of lesion on surface by time flow, cross sectional lesion, and lesion of filled or unfilled surface were analyzed using analysis software. ${\Delta}F$ (value of fluorescence loss) of the lesion on surface assessed by the QLF-D increased significantly over time up to 21 days. And ${\Delta}F$ value of lesion of filled surface is significantly lower than that of unfilled surface (p<0.001). ${\Delta}F$ of filled surface is 1.31 times of cross section lesion. The correlation of between ${\Delta}F$ of filled surface lesion and ${\Delta}F$ of cross section lesion was showed low agreement (0.026) and correlation of between ${\Delta}F$ of unfilled surface lesion and ${\Delta}F$ of cross section lesion was showed high agreement (0.613). In conclusion, secondary caries can be detected on surface using QLF-D. However, interference of fluorescence of filling material is the points to be especially considered for exact analysis of secondary caries lesion.

Objectives: The usage of fluoride varnish for a moderate to low caries-risk group has not been well validated. This study aimed to evaluate the preventive and therapeutic efficacies of fluoride varnish on the initiated root caries. Materials and Methods: Ten premolars were sectioned into quarters, further divided into two windows, one of which was painted with Fluor Protector (1,000 ppm fluoride, Ivoclar Vivadent). An initial lesion with a well-preserved surface layer was produced by pH cycling. Scanned line analysis using energy dispersive spectrometry determined the weight percentages of Ca and P in the demineralized layer. Scanning Electron microscopy and confocal laser scanning microscopy (CLSM) evaluated the varnish-applied root surfaces. Results: The mean lesion depth (SD) was 12.3 (2.6) ${\mu}m$ (single cycling) and 19.6 (3.8) ${\mu}m$ (double cycling). Double cycling extended the lesion depth, but induced no more mineral loss than single cycling (p < 0.05). The mean weight percentages of Ca and P between groups with and without varnish were not significantly different (p < 0.05). A CLSM showed varnish remained within 15 ${\mu}m$ of the surface layer. Conclusions: When a mild acid challenge initiated root tissue demineralization, the application of low-concentration fluoride varnish did not influence the lesion depth or the mineral composition of the subsurface lesion.
The purpose of this study was to provide the basic data for preventing dental caries, and maintaining and enhancing oral health. The subjects of this study were 138 male and female students who were in the 6th grade of elementary schools in Gimpo city. This study investigated the number of decayed tooth and the factors related to the number of decayed tooth, by using the results of questionnaire and oral health survey over such subjects. So, this study obtained the following conclusions. 1. For the number of decayed tooth, 'nothing(D = 0)' was the highest as 37.7%, and 'from two to three' was 23.9%, 'more than four' was 21%, and 'one' was 17.4% in order. 2. For the frequency of visiting dental clinics within the last one year, 'one to two times' was the highest as 52.2%, and the children who have not visited dental clinics even one time during one year were 23.9%. 3. For average daily tooth brushing frequency, 'two times' was the highest as 71.7%. For the use of oral health devices besides toothbrushes, 'I don't use' was the highest as 54.3%. For the experience of sealant and fluoride application, 'nothing' was the highest as 86.9% and 71.3% respectively. For the recognition on the use of fluoride dentifrice, 'I don't know' was the highest as 66.9%. 4. The socio-demographic factors related to the number of decayed tooth were mother's age, mother's background, mother's employment, and after-school fosterer(p > 0.05). 5. The oral health care factors related to the number of decayed tooth were average daily tooth brushing frequency, the use of oral health devices besides toothbrushes, and the experience of fluoride application.(p > 0.05) 6. The snack intake factors related to the number of decayed tooth were tooth care foods intake frequency and decaying foods intake frequency(p > 0.05). 7. The oral health belief item related to the number of decayed tooth was susceptibility(p > 0.05). 8. According to the results of regression analysis, the less mother was employed, the more the average daily tooth brushing frequency was, the more the tooth care foods intake frequency was, the less the decaying foods intake frequency was, and the higher susceptibility was, the lower the number of decayed tooth. 9. In order to prevent and cure early the dental caries which occur frequently in elementary school students, the establishment of oral health centers within schools should be expanded to promote tooth brushing instruction, fluoride solution rinsing, diet control, periodic oral examination.

Topical fluoride application for children is a widely peformed procedure in the field of pediatric dentistry to prevent dental caries. However, it is recently recognized as having some unwanted effects on several esthetic restorative materials as it roughens the surface of the restorative materials. The aim of this study was to evaluate the surface changes in composite resins to topical fluoride. Composite resins(Z $250^{(R)}$, Ultraseal $XT^{(R)}$ Filtek $flow^{(R)}$$Revolution^{(R)}$, $Denfil^{(R)}$) in topical fluoride agents were immersed and their surface roughness, weight loss and SEM were evaluated. The results were as follows : 1. The 4 minutes-immersion groups showed more roughened surface than 1 minute-immersion groups and the control groups showed the smoothest surface among all the materials, and there was statistically significant difference except the revolution between the groups. 2. There was no significant difference between the 1 minute-immersion groups and 4 minutes-immersion groups in weight loss. 3. The experimental group treated with topical fluoride gel showed the generally mere roughened surface than control group in the SEM findings.
This study was designed to investigate the using status and awareness of pit and fissure sealant, and how it affects on secondary caries when performed on above of initial caries. After classifying 446 occlusal surface into four groups, performed pit and fissure sealant, induced artificial caries, used DIAGNOdent (Kavo) to measure degree of secondary caries. Also, distributed a questionnaire on dental clinic in metropolitan area to find out using status and awareness of pit and fissure sealant, the following results were obtained. 1) On inspection and percussion, Group 4 corresponding to the enamel caries showed the highest secondary caries after sealant and was statistically significant difference in the order of initial group, stained group, sound group (p<0.05). 2) Inspection showed the highest percentage on tooth fissure caries diagnostic methods before sealant. 3) 56.6% didn't know about DIAGNOdent, 91.6% didn't have it. 4) In clinically, the most cause of secondary caries after sealant was a broken sealant, making caries on the downward. Based on the results of above study, degree of caries under sealant could affect on secondary caries, needs publicity about the use and necessity of objective fluorescence device.

Many operations have been carried out using the fluoride ion to prevent and reverse dental caries. It certainly encourages remineralization and also prevents dental caries. However, the remineralization developed by these means is superficial only leaving lower levels of demineralized lesion with a degree of porosity and incomplete mineralization. We must consider its toxic effects when it is ingested for overdose. The CPP-ACP paste is able to remineralize the incipient lesion and has no harmful effects when it is ingested, because it was made form casein which is from the protein of milk. The purpose of this article is to compare the remineralization effects between APF gel and the CPP-ACP paste. we applicated the APF gel and CPP-ACP paste on the artificial enamel carious lesion. After 14 days, we measured the surface microhardness and observed the remineralized lesion under polarized light microscope. The results were as follows : 1. The surface microhardness of group III was the highest, followed by group II, and I(p<0.05). 2. The surface microhardness of group III was significantly higher than those of group I, and II(p<0.05). 3. We could observe thin and irregular remineralization layer of group II, and regular and moderate remineralization layer of group III under polirized light microscope.
