A Case Report on Reflux Esophagitis Treated with Korean Medicine in a Patient with Spinal Stenosis (한방병원에 입원한 역류성 식도염을 동반한 척추관협착증 환자에 대한 한방 치료 1례)

  • Objectives: This study investigated the efficacy of Korean medical treatment for reflux esophagitis. Methods: We used Korean medical treatment (herbal medicines, acupuncture, herbal acupuncture, and therapy) to treat a hospitalized patient with reflux esophagitis. To evaluate the treatment, we measured the symptoms using the Reflux Symptom Index (RSI) and the Visual Analogue Scale (VAS) for heartburn, acid reflux, chest pain, foreign body sensations in the throat, chronic cough, hoarseness, and bronchial asthma. The generic health status was evaluated using the European Quality of Life Five-Dimension (EQ-5d) Scale. Results: The RSI and EQ-5d scores improved following treatment, but the VAS scores showed that only heartburn, acid reflux, and chest pain symptoms were relieved. Conclusions: This study suggests that Korean medicine may be effective for treating reflux esophagitis.

  • The study reports the clinical case of a patient with lower extremity weakness and gait disturbance treated with Korean medicine treatments including Chuna therapy. The patient suffered lower extremity weakness and gait disturbance with diagnosis of lumbar disk herniation and spinal stenosis. As a treatment, the doctor applied Chuna therapy, herbal medicine, acupuncture, pharmacopuncture, cupping and moxa. The effect of treatment was evaluated by numeral rating scale (NRS), self-walkable distance, functional independence measure (FIM), Oswestry disability index (ODI) and manual muscle test (MMT). NRS decreased from 6 to 4 at the leg. Self-walkable distance increased from 0 m to 10 m, FIM increased 85 to 96 points while ODI decreased 64% to 54%. MMT of hip flex and knee extension improved from grade 3+, grade 3 to grade 4, respectively. Korean medicine treatment can be effective for patients who suffer lower extremity weakness and gait disturbance due to lumbar disk herniation and spinal stenosis. Further clinical studies are required to verify these findings.

  • Objectives: The purpose of this study is to report the efficacy of applying Korean medical treatment to chronic gastritis. Methods: We treated a chronic gastritis patient with Korean medical treatments (herbal medicines, acupuncture, and herbal acupuncture therapy). To evaluate the results of this treatment, we took measurements using the Gastrointestinal Symptom Rating Scale and Visual Analogue Scale for abdominal pain, heartburn, nausea, and abdominal distension. The patient's generic health status was evaluated using the European Quality of Life 5 Dimensions Scale. Results: Improvements in gastrointestinal symptom ratings and the European Quality of Life 5 Dimensions Scale were observed after the treatments. Relief of symptoms was observed in abdominal pain, heartburn, nausea, vomiting, and abdominal distension using the Visual Analogue Scale. Conclusions: This study suggests that Korean medicine may be effective in the treatment of chronic gastritis.

  • Objectives: The purpose of this study is to investigate the effectiveness of using Korean medical treatment for peripheral facial nerve palsy. Methods: We primarily treated the patient's peripheral facial nerve palsy during hospitalization (06/28/21~07/13/21) without any other steroids or antiviral drug treatments and additionally treated their spinal stenosis, using Korean medical treatments in both cases (herbal medicine, acupuncture, and herbal acupuncture therapy). The House-Brackmann grading system and Yanagihara's unweighted grading system were used to assess facial symptom changes, and the Numerical Rating Scale was used to evaluate the back pain caused by the spinal stenosis. Results: Following treatment, the patient's House-Brackmann and Numerical Rating Scale scores decreased, while their Yanagihara unweighted score improved. Conclusions: This study suggests that Korean medicine may be effective in the treatment of facial nerve palsy.

Recent Domestic and International Trends on Non-Surgical Treatment of Lumbar Spinal Stenosis (요추 척추관 협착증의 비수술적 치료에 대한 최근 국내·외 연구 동향)

  • Objectives The present study examined the recent domestic and international trends of non-surgical treatment of lumbar spinal stenosis (LSS). Methods The studies on non-surgical treatment of LSS were investigated via searching Korean web databases and PubMed. As a result, 39 studies were analyzed according to the authors, the types of study, the relationship to surgical treatment and the method of treatment. Results The first authors' occupations were identified as 17 doctors, 12 Korean medicine doctors, six physiotherapists, three chiropractic doctors and one doctor of science. The most common type of study in Korea was the case report and most of the studies were retrospective. Overseas, there were many prospective or experimental studies including randomized controlled trials (RCT). In Korea, non-surgical treatment was performed independently of surgical treatment in most cases. Among non-surgical treatments, the most researched treatment was traditional Korean medicine (TKM), followed by injection therapy, exercise therapy, and physical therapy. Conclusions Through this review, we can determine the effectiveness of TKM and its research direction. TKM should be studied experimentally including RCT and it should be conducted not only on acupuncture, but also on other treatment methods such as acupotomy, chuna, and herbal medicine. It is also necessary to conduct studies on TKM before and after surgery to compare the effects of surgery and TKM together, as well the effects on non-surgical treatments.

Electroacupuncture for Lumbar Spinal Stenosis: A Systematic Review and Meta-Analysis (요추 척추관 협착증에 대한 전침 치료의 효과: 체계적 문헌고찰 및 메타분석)

  • Objectives To evaluate the evidence supporting the efficacy and safety of electroacupuncture for lumbar spinal stenosis. Methods We searched eight electronic databases (PubMed, EMBASE, Cochrane Library, Chinese Academic Journals, Research Information Sharing Service, ScienceOn, KMBASE, DBpia) and related two journals up to March 2023. We included randomized controlled trials of testing electroacupuncture for lumbar spinal stenosis patients. The methodological quality of relevant randomized controlled trials assessed by the Cochrane risk of bias tool. Results Among 90 articles that were searched, seven randomized controlled trials involving 474 participants were finally selected in this systematic review. Electroacupuncture was more effective on lumbar spinal stenosis compared with other treatments including analgesics, acupuncture, bed rest and exercise therapy, but showed ambiguous effect compared with physical therapy. Most of the side effects and adverse reactions were reported as minor and temporary. Conclusions Electroacupuncture for lumbar spinal stenosis was more effective than analgesics, acupuncture, bed rest and exercise therapy. In terms of safety, it was limited because there are many papers that do not mention side effects and adverse reactions related to electroacupuncture. Additional studies are needed to determine the effect of electroacupuncture on lumbar spinal stenosis.

The Value of Additional Cervicothoracic Spine Sagittal T2-weighted Images Included in Routine Lumbar Spine MR Imaging (요추 MR영상에 포함된 경흉추 시상T2강조영상의 효용성 평가)

  • Purpose : To evaluate the usefulness of cervicothoracic spine sagittal T2-weighted images (CT SAG T2WIs) included in routine lumbar spine MRI. Materials and Methods: Institutional review board approval was obtained and informed consents were waived for this retrospective study. The study group comprised 2,113 patients who underwent lumbar spine MRI from January 2005 to December 2005. CT SAG T2WIs were added in the routine lumbar spine MRIs. Radiologic reports were reviewed retrospectively for pathologic lesions on CT SAG T2WIs by one radiologist. Information of additional cervical or thoracic spine MRI and/or CT for further evaluation of positive findings on CT SAG T2WIs and their treatment were collected by retrospectively reviewing medical records. Results: The CT SAG T2WIs revealed 142 pathologic lesions in 139 (6.58%) of the 2,113 patients. They were easily obtained without positional change in a scan time of less than 2 minutes. Additional cervical or thoracic spine MRI and/or CT for positive findings on CT SAG T2WIs were performed in 13 patients. Seven patients underwent surgical treatment. Conclusion: CT SAG T2WIs included in routine lumbar spine MRI were useful in finding the pathologic lesions in cervicothoracic spine for the patients who assumed to have lesions in lumbar spine.

A Study for Clinical and Radiological Correlation between Lumbar Lordotic Angle, Lumbar Intervertebral Disc Angle and Lumbar Spinal Stenosis (요추 전만 각도 및 요추 추간판 각도와 요추 척추관 협착증의 상관관계에 관한 연구)

  • Objectives The purpose of this study is to find out clinical and radiological correlation between lumbar lordotic angle, lumbar intervertebral disc angle and lumbar spinal stenosis. Methods Total 250 patients' who had visited Bu-Chun Jaseng Hospital of Korean Medicine lumbar lordotic angle, intervertebral disc angle of L4/5 and dural sac dimension of L4/5 were measured by X-ray and MRI films. We analysed correlation between lumbar lordotic angle, intervertebral disc angle of L4/5 and lumbar spinal stenosis in terms of clinical and radiological aspect. Results 1. The mean intervertebral disc angle of L4/5 were $10.72{\pm}3.98^{\circ}$, the mean lumbar lordotic angle were $41.97{\pm}11.73^{\circ}$ and the mean dural sac dimension of L4/5 were $133.18{\pm}45.46mm^2$. 2. This study shows that dural sac dimension of L4/5 was inversely reated to intervertebral disc angle of L4/5 by statistically (p<0.05). 3. There was visible difference regarding intervertebral disc angle of L4/5 between patients who had been diagnosed with lumbar spinal stenosis by clinically and patients who had not been diagnosed with lumbar spinal stenosis by clinically; The former's angle was relatively higher than the latter's (p<0.05). Conclusions There was a statistical significance between intervertebral disc angle of L4/5 and lumbar spinal stenosis in single-segment.

Arthroscopic Anterior Acromioplasty for the Treatment of Chronic Impingement Syndrome of the Shoulder (관절경적 전방 견봉 성형술을 이용한 만성 견관절 충돌 증후군의 치료)

  • Purpose : The purpose of this study was to evaluate clinical efficacy of the arthroscopic anterior acromioplasty for the treatment of chronic impingement syndrome of the shoulder. Materials and Methods : Between July 1995 and December 1997, twenty seven consecutive shoulders of 26 patients with chronic impingement syndrome of the shoulder were treated by arthroscopic anterior acromioplasty. The patients who had severe osteoarthritis of the shoulder full thickness tear of the rotator cuff, and nonoutlet impingement were excluded. The clinical results were evaluated by using UCLA shoulder rating scale. The average follow-up was 2years 3months(range, 1year 7months to 3years 1 11months). Results : Twenty three patients$(85.2\%)$ were rated as excellent or good results, while four patients$(14.8\%)$ were fair. Twenty six cases$(96.3\%)$ were satisfied with the results of the operations, while one case$(3.7\%)$, who had Parkinsonian syndrome, ossification of posterior longitudinal ligament(OPLL) of the cervical spine, and spinal stenosis of the 5th and 6th cervical spine was not satisfied. Conclusion : Arthroscopic anterior acromioplasty was an effective treatment method, especially for relief of pain, for the treatment of chronic impingement syndrome of the shoulder. If the patient has the combined lesions in the cervical spine and the shoulder, and systemic lesions, these lesions may influence the results of treatment after operation, and cause the unpredictable results.

  • Objectives: The purpose of this study is to report the effect of Postural Yinyang Correction of Temporomandibular Joint (Functional Cerebrospinal Therapy, FCST) on a patient with Osteoarthritis of the hip caused by Hip dysplasia and Lumbar spinal stenosis. Methods: A patient with Osteoarthritis of the hip caused by Hip dysplasia and Lumbar spinal stenosis was treated at Dept. of Acupuncture & Moxibustion, ○○ University Korean Medicine Hospital from Nov 8th, 2019 to Dec 6th, 2019 and received a Korean-Western medical treatment mainly managed with FCST. This study was measured with VAS (Visual Analogue Scale), ODI (Oswestry Disability Index) and questionnaire. Results: After treatment, the patient's pain was controlled and gait ability was improved, also VAS, ODI and questionnaire score were improved. Conclusions: Korean-Western Medical Treatment mainly managed with FCST may be helpful in controling pain with Osteoarthritis of the hip caused by Hip dysplasia and Lumbar spinal stenosis, but the further researches are needed.
