Effects of Operative Technique, Pedicle Screws, or Fusion on Loading of Adjacent Segments after Lumbar Fusion: Finite Element Biomechanical Analysis (요추 유합술 후, 인접분절의 위험을 초래하는 원인들에 대한 생체 역학적 분석)

  • 척추경 나사못을 이용한 요추 유합술은 가장 보편적으로 사용되는 수술 적 치료 방법이다. 과거 여러 연구들에서 이러한 척추 유합술의 임상적 우수성은 이미 입증 되었으며, 척추경 나사못은 시술 부위의 운동을 완전히 제한함으로써 높은 유합율을 얻을 수 있으나, 상대적으로 인접 분절의 조기 퇴행성 변화의 요인 중 하나로 보고되고 있다. 또한 유합술을 수행 할 때 후방조직의 절제 또한 인접 분절의 퇴행을 초래하는 원인으로 보고되고 있다. 따라서 본 연구에서는 유한요소해석 방법을 이용하여 척추경 나사못 과 후방조직을 절제하는 시술 방법을 비교하여 척추체의 운동범위의 증가량을 계산하였고, 척추경 나사못 모델과 후방조직을 제거한 모델을 개발하여 시술하기 전 정상모델과 비교하였다. 개발된 모델과 정상 모델을 생체 역학적 측면에서 분석하여 인접분절의 조기 퇴행성 변화를 일으키는 가장 큰 원인이 무엇인지 정량적으로 분석하였고, 이를 근거 하여 임상적 효과와 그 이론적 근거를 제시하고자 한다.

The Usefulness of Diffusion-weighted MR Imaging for Differentiation between Degenerative Spines and Infectious Spondylitis (퇴행성 척추와 감염성 척추염의 감별에 있어서 확산강조영상의 유용성)

  • Purpose : The differential diagnosis between Modic type I degenerative spine and infectious spondylitis sometimes is difficult, because the affected bone marrows in both disease show similar signal intensity on conventional MR imaging. We evaluate the usefulness of diffusion-wighted MR imaging for differential diagnosis between Modic type I degenerative spine and infectious spondylitis. Materials and methods : The spin-echo and diffusion-weighted MR images of eight patients with Modic type I degenerative spines and 14 patients with infectious spondylitis diagnosed by clinical findings or CT-guided biopsies we re analyzed. The diffusion-weighted imaging sequence was based on reversed fast imaging with steady-state precession (PSIF). Signal intensity changes of the vertebral bone marrow on conventional spin-echo and diffusion-weighted MR imaging were compared between degenerative spine and infectious spondylitis. Results : On T1-weighte d images, the affeted bone marrow in both disease showed hypointense signals. On T 2-weighted images, all of type I degenerative spine and 11 of infectious spondylitis showed hyperintensity, and three of infectious spondylitis showed heterogeneo us mixed signal intensity. On diffusion-weighted MR images, all of type I degenerative spine were hypointense with peripheral high signal intensity to normal vertebral body, but infectious spondylitis was hyperintense (n = 11) and hypointense (n=3). Conclusion : Diffusion-weighted MR imaging is useful to differentiate Modic type I degenerative spine from infectious spondylitis. On diffusion-weighted images, the high singal intensity of bone marrow suggests infectious spondylitis, whereas the low signal intensity of bone marrow with peripheral focal high signal intensity suggests type I degenerative spine.

  • Objectives : The objective of this study is to evaluate the treatment effect of Oriental therapy including exercise therapy in chronic ankylosing spondylitis with total hip replacement. Methods : Oriental therapy and exercise therapy were performed for 64days admission in a patient who had chronic ankylosing spondylitis with total hip replacement and physical tests were examined. Result : Pain and ROM of cervical. lumbar spine, and left hip joint has improved with this treatment. And Anorexia, dyspepsia, and fatigue were also decreased. Conclusions: In this case, oriental therapy was proved to be effective in improving chronic symptoms and general prostration of Ankylosing Spondylitis. And It is necessary to carry out exercise therapy regulary.

The Effects of Spinal Decompression Combined with Therapeutic Modalities for Patients with Lumbar Radiculopathy (치료적 모달리티를 병용한 척추 감압치료가 요추 신경뿌리병증 환자에게 미치는 효과)

  • The purpose of the present study was to determine the effect of 4 weeks course of motorized spinal decompression delivered via SpineMT(mobilization & traction) combined with therapeutic modalities on the treatment of patients with lumbar radiculopathy(LRP). A total of 15 patients with LRP (mean age, 36.63 years; age range 20-50years) participated in this study. 4 weeks course of spinal decompression delivered via SpineMT combined with therapeutic modalities was delivered to the patients for 6 days per week for the first two weeks, and four times per week for two additional weeks. The entire treatment consisted of 20 visits over 4 week period. Comparisons of changes in the muscle strengthening (MS), straight leg raise (SLR), and Oswestry disability index (ODI) at pre-intervention, after 10 treatment sessions, and at discharge (after 20 treatment sessions) were analyzed. There were significant improvements in the outcome measures of MS test, SLR test, and ODI score after 10 and 20 sessions of spinal decompression treatment combined with therapeutic modalities as compared with the pre-intervention(p<0.05). Spinal decompression treatment combined with therapeutic modalities appears to be a safe and efficacious, noninvasive treatment modality for patients with LRP.

Effect of Stabilization Exercise with Abdominal Breath on Balance and Oswestry Disability Index for Low Back Pain Patients (복식호흡을 병행한 척추 안정화운동이 요통환자의 균형능력과 오스웨스트리 장애지수에 미치는 영향)

  • 본 연구는 복식호흡을 병행한 척추 안정화 운동이 요통환자의 균형능력과 통증에 미치는 영향을 알아보기 위해 시행되었다. 20대 요통환자 18명을 대상으로 하였으며 실험군은 척추안정화 운동을 실시하는 동안 복식호흡을 함께 적용하였으며 대조군은 척추안정화 운동만을 시행하였다. 운동은 8주간 적용하였으며 운동 시작 전과 후에 오스웨스트리 요통장애지수 설문지를 사용하여 통증을 비교하고 BioRescue를 사용하여 신체중심이 이동한 길이와 면적을 측정하여 균형능력을 비교하였다. 실험결과, 두 그룹 모두 운동 후, 요통장애지수가 유의하게 감소하였으나 그룹간의 유의한 차이는 없었다. 신체중심이동면적 비교에서 실험군은 운동 후 유의한 감소가 있었으나 그룹간의 유의한 차이는 없었다. 신체중심이동길이의 비교에서 눈을 감은 상태에서 실험군은 유의한 차이가 있었으나 대조군은 유의한 차이가 없었으며 그룹간 비교에서도 유의한 차이가 있었다. 이러한 결과를 통해 요통환자에게 안정화운동과 복식호흡을 함께 병행하여 실시한다면 더욱 효과적일 것이라고 생각된다.

  • A 3-D finite element model of human thoracolumbar spine (T12-L2) was reconstructed from CT images. Various anterior and posterior instrumentation techniques were performed with long cage after corpectomy. Six loading cases were applied up to 10 Nm, espectively. The rotations of T12 with respect to L2 were measured and the stiffnesses were calculated as the applied forces divided by the segmental rotations. The posterior fixation technique increased the stiffness of the spine the most. The addition of anterior rod from 1 to 2 increased the stiffness significantly without posterior fixation, but no effect was found with posterior fixation. We found that different fixation techniques changed the stiffness of the spine.

Value of Bone Scan in Addition to F-18 FDG PET/CT and Characteristics of Discordant lesions between F-18 FDG PET/CT and Bone Scan in the Spinal Bony Metastasis (척추골전이에 있어 F-18 FDG PET/CT에 대한 골스캔의 추가적 역할 및 F-18 FDG PET/CT와 골스캔간에 불일치 병소에 대한 연구)

  • Purpose: Our purpose was to evaluate spinal bony metastasis which could be missed on an F-18 FDG PET/CT (FDG PET/CT) alone, and to characterize discordant metastatic lesions between FDG PET/CT and bone scan. Material and Methods: FDG PET/CT and bone scans of 43 patients with spinal bony metastasis were analyzed retrospectively. A McNemar test was performed comparing the FDG PET/CT alone to the FDG PET/CT plus bone scan in the spinal bony metastases. A one-way chi-square test was performed to characterize the metastases that were missed on the FDG PET/CT alone. To evaluate discordant lesions between FDG PET/CT and bone scan, we performed logistic regression analyses. The independent variables were sites (cervical, thoracic, and lumbar), size (large and small), and maximum SUVs, and the dependant variable was bone scan uptake (positive and negative MDP uptake). Results: A significant difference was found between the FDG PET/CT alone and the FDG PET/CT combined with the bone scan (p < 0.01). Using the FDG PET/CT only, diffuse osteoblastic metastasis was missed with a significantly higher frequency (p = 0.04). In the univariate analysis, cervical vertebra and small size were related to negative MDP uptake, and thoracic vertebra and large size were related to positive MDP uptake. However, in the multivariate analysis, only the large size was related to positive MDP uptake. Conclusion: A bone scan in addition to the FDG PET/CT increased the ability to evaluate spinal bony metastases, especially for diffuse osteoblastic metastasis. Large metastasis was related to positive bone scan uptake in spinal bony metastasis.

Effects of Spinal Stabilization Training Program on Muscle Function and Gait Ability for Private Security Guard (민간경호원의 규칙적인 척추안정화운동 프로그램 참여가 근기능 및 보행능력에 미치는 영향)

  • The purpose of this study was to analyze the variation of muscle function and gait ability according to regular spinal stabilization exercise program for private security guards and the conclusion as follows. First, it increased lumbar extension strength and lumbar flexion strength both exercise and non exercise group according to the variation of muscle function with the spinal stabilization exercise program. There was significant difference between exercise and non exercise group after the program but not before the program. Second, it appeared the functional vanishment of lumbar extensor before the program both exercise and non exercise group according to the variation of muscle function with the spinal stabilization exercise program for lumbar flexor extensor ratio. Third, it analyzed for the spinal stabilization with 8 ways according to the variation of muscle function with the spinal stabilization exercise program for the change of the spinal stabilization. The spinal stabilizaton increased each angle for the execise group but decreased or not changed for non exercise group. Fourth, it used 10m gait test for the speed and step length according to the variation of muscle function with the spinal stabilization exercise program. It appeared the increasement of gait ability for exercise group and there was significant difference between exercise and non exercise group. In conclusion, there was a positive effect for lumbar extension strength, lumbar flexion strength, lumbar flexor extensor ratio, spinal stabilization, and gait ability with regular spinal stabilization exercise program. This means that spinal stabilization exercise program give positive effect to the decrease of back pain and the development of muscular strength for private security guard, also will help to recover and return to work more faster. Also, it seems that the exercise prescription program may be applicable to prevent or to improve the function for private security guard.
