Effects of Vitamin C on Oxidative Stress Due to Anesthesia and Surgical Trauma in Dogs (개 마취와 수술 창상에 따른 산화스트레스에 대한 비타민 C의 효과)

  • The study evaluated the antioxidant effects of ascorbic acid (AA) against oxidative stress during laparotomy in dogs under general anesthesia using isoflurane. Twelve dogs were randomly assigned to either the ascorbic acid group (AAG) or the sham group (SG). AAG received intravenous dosage of 100 mg/animal of standard AA 10 minutes before anesthesia. Plasma levels of cortisol, glucose, total oxidant status (TOS), total antioxidant status (TAS), and oxidative stress index (OSI) were measured. Cortisol level increased significantly with time in both groups (p < 0.05). The change of glucose levels was not significantly different between both groups. TOS and OSI increased significantly with time in the SG (p < 0.05), whereas it did not significantly change with time in the AAG. The TOS and OSI of the AAG were significantly lower than in the SG (p < 0.05). However, TAS did not significantly change between both groups. The results suggest that the decrease of TOS in the AAG might be related to the conversion of antioxidants to oxidants. The decrease of OSI indicates that the decrease of reactive oxygen species (i.e., oxidative stress) produced at the site of injury is able to decrease surgical side effects of abnormal blood circulation, organ failure, and inflammation. Therefore, AA can be used to protect a surgical patient from oxidative stress in canine laparotomy.

Anatomical Comparison of Compression, Opposite, and Lateral Woods in a Branch of Pinus parviflora S. et Z. (섬잣나무(Pinus parviflora S. et Z.) 지재의 압축이상재, 측면재, 대응재에 관한 해부학적 특성 비교)

  • Compression, lateral, and opposite woods in a branch of Pinus parviflora S. et Z. were described and compared in the qualitative anatomical aspects through light and scanning electron microscopy. Tracheid transition from earlywood to latewood in the compression wood appeared to be relatively more gradual than in the opposite or lateral wood. Growth ring width and proportion of latewood were thought to be greater in the compression wood than in the lateral or opposite wood. The latewood tracheids of compression wood in transverse surface were mostly round, differently from those of lateral and opposite woods with square to angular shapes. Also, intercellular spaces, helical cavities and checks, and slit-like pit apertures were observed only in the compression wood tracheids. Cross-field pitting in the compression wood appeared not to be used as diagnostic guide because of their severe alteration from normal fenestriform or window-like type to cupressoid to taxodioid types. In tangential surface, fusiform rays in the compression wood were wider but lower than those in the lateral wood or opposite wood. In conclusion, compression wood was different from lateral and opposite woods but lateral and opposite woods were almost identical in qualitative anatomical features.

Development of CO2 Emission Factor for Wood Chip Fuel and Reduction Effects (목질계 바이오매스 중 대체연료 우드칩의 온실가스(CO2) 배출계수 개발 및 저감 효과)

  • Technology for energy recovery from waste can reduce the greenhouse gas emissions. So recently, there are several companies using RDF, RPF, WCF instead of using only coal fuel and it's part of the fuel on the increase. In this study, we developed Wood chip fuel $CO_2$ emission factor through fuel analysis. It's moisture content is 23%, received net calorific value is 2,845 kcal/kg, and received basis carbon is 34%. The result of emission factor is $105ton\;CO_2/TJ$, it's 5.9% lower than 2006 IPCC guideline default factor $112ton\;CO_2/TJ$. The gross GHG(Greenhouse gases) emissions of plant A is $178,767ton\;CO_2 eq./yr$, and Net GHG emissions is $40,359ton\;CO_2 eq./yr$. Therefore, the reduction of GHG emissions is $138,408ton\;CO_2/yr$ through using WCF, and I accounts for 77% of all GHG emissions.

Evaluation for Loss Rate of Carbon Dioxide by Sample Storage Method (시료 보관 방법에 따른 이산화탄소 시료의 유실율 평가)

  • The study evaluated the loss rate of carbon dioxide, which is the prevalent greenhouse gas, according to the method of keeping specimens using standard gases. In order to examine the difference generated by a bag of storing specimens, it checked the loss rate according to concentration and time by making a 2nd set of standard gases in a Tedlar bag and an Aluminium bag. The containers lost gases at a rate of 5% or more after 6 day, and 10% or more after 15 days in both bags. In time, it was confirmed that the loss rate of the Aluminium bag was higher than that of the Tedlar bag. Specifically, there was no significant difference between the two bags after 12 days had passed, but by the 15th day, it was confirmed that the loss in an Aluminium bag was more severe than that in the Tedlar bag. The study went on to check the relations between the humidity in storage and the loss rate. The results showed that there was no significant effect of humidity on the loss rate until 72 hours had passed, when the loss rate in the Tedlar bag with high humidity was about 5% higher than that of the Tedlar bag in dry conditions. This study examined the loss rate of carbon dioxide, a typical greenhouse gas. It is important to note that the loss rate was determined by calculating highly reliable carbon dioxide emission and emission coefficient data, with consideration to the method of keeping specimens.

Analysis of the Effect of Intralesional Steroid Injection on the Voice During Laryngeal Microsurgery (후두 미세수술 중 병변 내 스테로이드 주입이 음성에 미치는 효과 분석)

  • Background and Objectives Vocal fold (VF) scar is known to be the most common cause of dysphonia after laryngeal microsurgery (LMS). Steroids reduce postoperative scar formation by inhibiting inflammation and collagen deposition. However, the clinical evidence of whether steroids are helpful in reducing VF scar formation after LMS is still lacking. The purpose of this study is to determine whether intralesional VF steroid injection after LMS helps to reduce postoperative scar formation and voice quality. Materials and Method This study was conducted on 80 patients who underwent LMS for VF polyp, Reinke's edema, and leukoplakia. Among them, 40 patients who underwent VF steroid injection after LMS were set as the injection group, and patients who had similar sex, age, and lesion size and who underwent LMS alone were set as the control group. In each group, stroboscopy, multi-dimensional voice program, Aerophone II, and voice handicap index (VHI) were performed before and 1 month after surgery, and the results were statistically analyzed. Results There were no statistically significant differences in the distribution of sex, age, symptom duration, occupation and smoking status between each group. Both groups consisted of VF polyp (n=21), Reinke's edema (n=11), and leukoplakia (n=9). On stroboscopy, the lesion disappeared after surgery, and the amplitude and mucosal wave were symmetrical on both sides of the VFs in all patients. Acoustic parameters and VHI significantly improved after surgery in all patients. However, there was no significant difference between the injection and control group in most of the results. Conclusion There was no significant difference in the results of stroboscopy, acoustic, aerodynamic, and subjective evaluation before and after surgery in the injection group and the control group.

Relationship between Ischemia-Modified Albumin and the Healing Period of Amputation Wounds in Patients with Diabetes Mellitus Following Non-traumatic Below-Knee Amputation (당뇨족으로 인한 비외상성 하퇴부 절단 환자에서 시행한 절단부 창상의 치유 기간과 Ischemia-Modified Albumin과의 관계)

  • Purpose: The present study examined the effectiveness of the preoperative ischemia-modified albumin (IMA) levels in predicting the healing period of amputation wounds in patients with diabetes mellitus following a non-traumatic below-knee amputation (BKA). Materials and Methods: This study enrolled 41 diabetic foot ulcer patients who underwent BKA at the authors' hospital diabetic foot center from April 2016 to April 2022. Among the 41 patients, 29 (70.7%) were male and 12 (29.3%) were female. Their mean age was 64.54±11.38 years (41~81 years). The mean follow-up period was 19.48±5.56 weeks (14~48 weeks) after BKA. The patients were divided into two groups (high IMA group and normal IMA group), which evaluated the healing period, wound dehiscence, and revision operation rate using a Fisher's exact test and Mann-Whitney U test. Three orthopedic surgeons performed stump wound evaluation, and they were evaluated as healing when all sutures were fused without oozing. Results: Thirty patients (73.2%) (group A) showed a high level of IMA (median: 91.2 U/mL), and 11 (26.8%) patients (group B) showed a normal range of IMA (median: 82.7 U/mL). In group A, the median period for wound healing took 1.4 weeks longer, which was significant (p=0.001). No statistical relationship was observed between wound dehiscence, revision operation rate, and IMA value. There was no correlation between the other risk factors (estimated glomerular filtration rate, HbA1c) and the wound healing period. Conclusion: Although there was a limitation in using IMA as the sole factor to predict the healing period of amputation wounds in patients after BKA, this study revealed a significant positive correlation between IMA and the period of stump healing after BKA. Therefore, the preoperative IMA levels may help predict the period of stump healing after BKA.

  • Pseudomonas aeruginosa and Staphylococcus aureus are the two most frequently encountered pathogens responsible for chronic wound infections, often coexisting in such cases. These infections exhibit heightened virulence compared to single infections, leading to unfavorable patient outcomes. The interaction among microorganisms within polymicrobial infections has been shown to exacerbate disease progression. Polymicrobial infections, prevalent in various contexts such as the respiratory tract, wounds, and diabetic foot, typically involve diverse microorganisms, with Pseudomonas aeruginosa and Staphylococcus aureus being the most commonly identified pathogens. This study aimed to compare the growth patterns of bacteria under a concentration gradient of toxic chemicals, focusing on a Gram-negative strain of Pseudomonas aeruginosa and a Gram-positive strain of Staphylococcus aureus. The minimum inhibitory concentration (MIC), which signifies the concentration at which bacterial growth is inhibited, was determined by performing broth microdilution and assessing the bacteria's growth curves. The growth curves of both Pseudomonas aeruginosa and Staphylococcus aureus were confirmed, and the exponential growth phases were applied to calculate the doubling times of bacteria. The MIC value for each toxic chemical was determined through broth microdilution. These results allowed for the identification of disparities in growth rates between Gram-positive and Gram-negative bacteria, as well as differences in resistance to individual toxic substances. We expect that this approach has a strong potential for further development towards the innovative treatment of bacteria-associated infections.

The Effect of Dangguijakyak-san on Wound Healing (당귀작약산의 창상 회복에 대한 효과)

  • Objectives We evaluated the wound healing effects of Dangguijakyak-san (DJ) using C57BL/6 mice that were generated open wound. Methods The study was conducted with seven C57BL/6 mice assigned to each group, divided into the normal group, control group, vitamin E group, DJ low-dose group, DJ high-dose group. We measured total polyphenol, flavonoid contents, the size of the wound, liver function, pro-inflammatory cytokine activity in serum, inflammation-related proteins, adhesion molecules and chemokine proteins, collagen-related proteins in skin tissue and histopathological changes by H&E and Masson's staining. Results DJ treatment significantly reduced the area of the wound compared to the control group. Also, inflammatory cytokines were reduced and the expression of anti-inflammatory-related factors (interleukin-4 [IL-4] and IL-10) was significantly increased in the DJ treatment group. We identified that DJ treatment inhibits both pathways of inflammation, the mitogen-activated protein kinases and nuclear factor-κB pathway. Moreover, the protein expressions of Sirt1 (sirtuin 1), MCP-1 (monocyte chemoattractant protein 1), ICAM-1 (intercellular adhesion molecule 1), and VCAM-1 (vascular cell adhesion molecule 1) were decreased by DJ administration. Also, the expression of α-smooth muscle actin and collagen type I alpha 1, collagen-related proteins, that help skin recovery was significantly increased in the DJ treatment group. Histopathologically, a relatively thin epithelial layer could be observed in the DJ administration group, as well as an increase in fibroblasts and collagen fibers. Conclusions These data suggest that DJ treatment is effective in wound healing, suppressing inflammatory proteins, increasing skin repair factors and improving histopathological changes caused by wounds.

The Effects of Sibjeondaebotanggamibang on the Treating of Wound (십전대보탕가미방(十全大補湯加味方)의 창상(創傷) 치유(治癒) 효과(效果))

  • Objectives This study was aimed to investigate the effects of Sibjeondaebotanggamibang (SJT) on the wound-induced rats. Methods It was observed the effects of anti-oxidation and anti-inflammation by using of lipopolysaccharide (LPS)-treated RAW 264.7 cells. For the observing on SJT anti-oxidation, it needed to mesure the total amount of polyphenol, DPPH scavenging ability, ABTS scavenging ability and the value of ROS production. In order to observe on the anti-inflammation of SJT, it was mesured the value of No and Cytokine (TNF-${\alpha}$, IL-$1{\beta}$, IL-6). It needed to make a scar (around $2{\times}2cm^2$) on the top of the fascia in the back of the rats and then the rats were divided into 4 groups (n=6). Control group was not treated at all, whereas SJ group was orally medicated SJT, Terra group was per-cutaneously applied Terramycin, and SJ+Terra group was both orally medicated SJT and percutaneously applied Terramycin per day for three weeks. The size of wound was measured with Digimatic Caliper and the blood samples (WBC, neutrophil, monocyte, lymphocyte) were analyzed using Minos-ST, which were collected by cardiac puncture. The effect on inflammatory cytokine (TNF-${\alpha}$, IL-$1{\beta}$, IL-6), immunological cells in synovial fluid was measured. To measure the wound factor expressed by wounded skin sample, we extracted RNA and to investigate MMP-1,2,9 we used RT-PCR. For performing histopathological examinations, we paralyzed the rats by ether, and extracted wounded skin tissues, which were measured by H & E, and monitored on the optical microscope. Results 1. DPPH and ABTS scavenging activity of SJT was increased concentration-dependantly, and ROS scavenging activity was significantly increased (10, $100{\mu}g/ml$). 2. NO production was significantly reduced in SJT treated cells ($100{\mu}g/ml$), both TNF-${\alpha}$ and IL-6 in SJT treated cells (1, 10, $100{\mu}g/ml$), and IL-$1{\beta}$ in SJT treated cells (1, $100{\mu}g/ml$). 1. The size of wound was significantly decreasing in SJ group, Terra group, SJ+Terra group. 2. WBC was significantly reduced in SJ and SJ+Terra group, monocyte in SJ+Terra group. Neutrophil was also reduced in SJ, SJ+Terra group but meaningless. 3. TNF-${\alpha}$ and IL-6 were significantly reduced in SJ group, Terra group, SJ+Terra group, and IL-$1{\beta}$ in SJ+Terra group. 4. mRNA expression in MMP-1 was significantly reduced in SJ group. 5. Collegan production and chronic inflammation were significantly decreased in SJ group, Terra group, SJ+Terra groups. Re-epithelization on the skin in Terra group, SJ+Terra groups was decreased. Conclusions According to this in vitro experiment, Sibjeondaebotanggamibang (SJT) has the effects of anti-oxidative and anti-inflammatory. By in vivo experiment, SJT has the effects of anti-inflammatory. Moreover, the progress of recovery was found visually, heamatologically, genetically and histopathologically. In conclusion, it could be thought that SJT has effect on the treating of wound.

Vein Injury and Wound Complications Associated with Techniques of Saphenous Vein Harvest (복재정맥 수확방법에 따른 이식편의 손상과 수술창의 합병증의 빈도)

  • Although arterial grafts are widely used due to the advantage of long-term patency in coronary bypass surgery, greater saphenous vein is still an important additional conduit. It was reported that preservation of the adventitia of vein graft and the adjacent tissues may bring the improved long-term graft patency. The aim of this study is to look for a harvest technique that can reduce vein injury and wound complications. Material and Method: In thirty-four patients that vein grafts were used for coronary bypass surgery, 50 harvest sites were included for the study. In 25 harvest sites in calf below knee (group 1), vein was exposed through a long incision and then clearly dissected from the adjacent tissue. Ten endoscopic vein harvests were performed in the thighs (group 2). Fifteen other vein grafts that were bluntly dissected were harvested from the thighs through three separate incisions (group 3). Result: Vein harvest time was longest in endoscopic harvest group (44.7$\pm$9.8 minutes) and shortest in group 3 (24.2$\pm$5.9 minutes) (p=0.000). Most avulsion injuries of vein branches happened in the endoscopic group. Sequential grafting numbers per vein were 1.72$\pm$0.98 with thigh vein graft and 1.16$\pm$0.37 with calf vein (p=0.02). Swelling of foot and/or leg, which was the most common wound complication after vein harvest, was most commonly presented in group 1 (20/25 sites; p=0.000). Tingling, the most common neurologic complication, was also most prevalent in group 1 (7/25 sites; p=0.013). The risk factor of the wound complication was vein harvest from calf, and the vein harvest technique was not a risk for wound complication. Conclusion: Vein harvest technique through three separate incisions from thigh presented shorter harvest time and less vein injury and wound complication compared with the endoscopic harvest technique from thigh or the harvest through a long incision from calf.
