The Efficiency of VAC(Vacuum-Assisted Closure) in Non-healing Wound (하지의 난치성 창상치유에 있어 VAC(Vacuum-Assisted Closure)의 유용성)

  • The treatment of wounds of the lower extremity caused by diabetes or vascular dysfunction remains a difficult problem for the plastic surgeon. The use of negative pressure in wound healing is a relatively new method to facilitate chronic wound healing by secondary healing. The use of vacuum-assisted closure(VAC) system is purposed to reduce local edema, increase regional blood flow, enhance epithelial migration, preserve a moist wound environment, reduce bacterial colonization, promote granulation tissue formation, and mechanically enhance wound closure. The VAC also can be used as a dressing for anchoring an applied split thickness skin graft. We reviewed the data from 20 consecutive patients with non-healing wound in lower extremity at Dong-A University from March 2002 to December 2004. We used the VAC in 20 patients and compared the results with the control group. In the VAC using group, mean application duration was about 3 weeks and dressing change was done every other day. The follow-up period of patients ranged from 3 months to 30 months with a mean of 17 months. The points of comparison with control group are wound size, granulation tissue proliferation rate, operation method, preoperative time, postoperative healing time, complication, and cost. With those points, we propose to approve the efficiency of the VAC in non-healing wound. As a result, the VAC used in non-healing wound decrease wound size, accelerate granulation tissue formation, do a wound closure with less invasive operation method, make less postoperative complication, can make operation time shorter. Therefore it is cost effect. Our results demonstrate the usefulness of VAC as an adjunct in management of chronic wounds with other extrinsic factors.

Application of Modified Vacuum Assisted Closure with Silver Materials in Chronic Infected Wound (만성 감염성 창상에 대한 변형 음압요법과 은이온 제재의 이용)

  • Purpose: Chronic infected wounds sustained over 4 weeks with exposed tendon or bone are difficult challenges to plastic surgeons. Vacuum assisted closure (VAC) device has been well used for the management of chronic wounds diminishing wound edema, reducing bacterial colonization, promoting formation of granulation tissue and local blood flow by negative pressure to wounds. But Commercial ready-made VAC device might have some difficulties to use because of its high expenses and heavy weight. So we modified traditional VAC device with silver dressing materials as topical therapeutic agents for control of superimposed bacterial wound infection such as MRSA, MRSE and peudomonas. Methods: We designed the modified VAC device using wall suction, 400 cc Hemovac and combined slow release silver dressing materials. We compared 5 consecutive patients' data treated by commercial ready-made VAC device(Group A) with 11 consecutive patients' data treated by modified VAC device combined with silver dressing materials(group B) from September 2004 to June 2007. Granulation tissue growth, wound discharge, wound culture and wound dressing expenses were compared between the two groups. Results: In comparison of results, no statistical differences were identified in reducing rate of wound size between group A and B. Wound discharge was significantly decreased in both groups. Modified VAC device with silver dressing materials showed advantages of convenience, cost effectiveness and bacterial reversion. Conclusion: In combination of modified VAC device and silver dressing materials, our results demonstrated the usefulness of managing chronic open wounds superimposed bacterial infection, cost effectiveness compared with traditional VAC device and improvement of patient mobility.

The Effect of Microcurrent Stimulation on Histological Structure of Wound in Rat (미세전류전기자극이 흰쥐 창상의 조직학적 구조에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effect of the intensity of microcurrent stimulation (with currents of 50 ${\mu}A$, 100 ${\mu}A$ and 300 ${\mu}A$ using a pulse frequency of 5 pps) on wound healing in rats. Methods: Sixty male Korean rats were randomly divided into four groups of 15 rats that were subjected to four different treatment protocols (control group, no treatment; experimental groups, treated with currents of 50 ${\mu}A$, 100 ${\mu}A$ or 300 ${\mu}A$). An experimental 20 mm linear wound was made in each animal and all animals in the experimental groups received microcurrent stimulation once a day for 20 minutes until the day of sacrifice on day 1, day 3 and day 6. An optical microscope was used to determine any histological changes. Results: The experimental results were as follows. 1. In an examination with the naked eye, all groups showed similar changes until the first day. However, from the third day, a little intercellular fluid soaked through wound region in the control group rats. In the experimental group animals, little intercellular fluid soaked through wound region, and swelling and redness did not appear, from the third day. 2. In an examination with histological evaluation, more significant changes were observed in all of the experimental group rats than the control group animals. Especially, a stimulation intensity of 50 ${\mu}A$ caused a more significant effect than the use of the other intensities by day 6 of wound healing. In addition, rapid recovery was observed. Conclusion: It was determined that microcurrent stimulation had a positive effect on wound healing. A stimulation intensity of 50 ${\mu}A$ was more effective than the other intensities (100 ${\mu}A$ and 300 ${\mu}A$) utilized for wound healing. Furthermore, low-intensity microcurrent stimulation was more effective for the purpose of wound healing.

Effects of the Korean Herbal Bathing Candidates on Allergic Contact or Wound Induced Dermatitis Mice Model (알레르기성 접촉 피부염 및 외과적 창상 모델에서 한방 입욕제 원료 처방의 효과)

  • Allergic contact or wound induced dermatitis are common environmental health issue and seriously affect the patient's quality of life. In now, people are interested in the korean herbal bathing that get better the skin conditions. This study was performed to investigate the effect of korean herbal bathing candidates extract on allergic contact or wound induced dermatitis. ICR mice were divided into four groups: naive, control, experimental group A and B. Control and experimental groups induced allergic contact or wound induced dermatitis by DNFB and TMA. Since then, each experimental group was pasted with corresponding korean herbal bathing extract into the skin. All groups were observed after 5 days. When the korean herbal bathing extract was applied, arbitrary skin wound index and epidermal thickness were significantly decreased compared to that of control. These data suggest that these korean herbal bathing candidates may be an effective therapeutic supplements for the treatment of skin.

Experimental Studies on Induced Traumatic Pericarditis and Myocarditis in Ruminants (반추수(反芻獸)의 창상성심낭심근염(創傷性心囊心筋炎)에 관(關)한 실험적연구(實驗的硏究))

  • This study was performed to obtain an accurate methods of diagnosis in early stage of traumatic pericarditis and myocarditis in ruminants. Korean native goats were used as experimental animal and traumatic pericarditis and myocarditis were induced by surgical intervention. The changes in clinical signs, hematological values, blood enzymes and electrocardigrams were observed for 14 days after operation. The results obtained were as follows: 1. Body temperature in experimental group of induced traumatic pericarditis and myocarditis was slightly increased than control group that was rumenotomized. Heart rate was increased in both groups but the increased rate was noticed remarkably in experimental group. There was no change in respiratory rate in experimental and control group. 2. Total leucocytes counts and neutrophil percentage maintained higher level after operation and lymphocytes percentage noticed reversed proportion to neutrophil in both groups. 3. Serum total protein concentration and alkaline phosphatase activities after operation showed a little lower levels than these values prior to operation. Serum glutamic oxaloacetic transaminase and lactic dehydrogenase activities in control group showed slight increase through all the experimental period, while in experimental group these activities were markedly elevated and an increase in serum lactic dehydrogenase activities was more remarkable comparing to serum glutamic oxaloacetic transaminase. 4. Electrocardiograms in control group did not show any abnormalities except for slight increase in heart rate. In experimental group the amplitude of QRS complex was increased in duration of experimental period. The transference from QRS complex to T wave was uncertain and S-T segment moved upward obliquely through electrical zero line.

Studies on Experimentally Induced Traumatic Hepatitis and Splenitis in Ruminants (반추수(反芻獸)의 창상성간염(創傷性肝炎) 및 비염(脾炎)에 관한 실험적(實驗的) 연구(硏究))

  • The traumatic hepatitis and splenitis were experimentally induced in Korean native goats. General examinations, hematological and blood biochemical findings were investigated up to 10 days after induction of traumatic hepatitis and splenitis. Body weight of goats with traumatic hepatitis and splenitis showed no change or only a slight decrease. Changes in body temperature and respiratory rate were not noted in both experimental groups. Heart rate hart not been changed in traumatic splenitis group but were a little increased in traumatic hepatitis group. Erythrocytic series such as erythrocyte counts, hemoglobin concentration and packed cell volume showed same variation within normal range during the period of observation. Total leucocyte counts showed the considerable increase in both experimental groups. The increase of the percentage of neutrophil and the decrease of the percentage of lymphocyte were more remarkable in traumatic hepatitis group than that of traumatic splenitis group. Serum aspartate transferase activity was increased in both experimental groups after surgery, while serum alkaline phosphatase activity has been decreased conciderably up to 7 days after surgery. Serum lactic dehydrogenase activity has been increased up to 3 days after operation and thereafter the activity has returned to normal level in traumatic hepatitis group and the activity increased only the first day after surgery in traumatic splenitis group. There were no changes in total protein and albumin contents in both experimental groups during the period of observation. Serum total cholesterol contents showed a slight increase in both experimental groups and the range of increase in traumatic hepatitis group was more wide. Serum bilirubin contents were net changed in traumatic hepatitis group, but showed a slight increase in traumatic splenitis group.

  • Atherosclerosis has more than 60% of the causes of arterial occlusive diseases. The abdominal aorta and lower extremity arteries are the most common sites of occlusion. We have treated surgically 2 cases who had intermittent claudication and were diagnosed as simultaneous aortobifemoral and bilateral femoropopliteal obstruction by angiography, but had ineffective results from medical treatment or angioplasty. Simultaneously aortobifemoral bypass using Hemashield Y graft and bilateral femoropopliteal bypass using autologous greater saphenous vein were done. After operations, the symptom disappeared and there were no specific post-operative complications except abdominal wound dehiscence. In postoperative angiography, we had obtained good patency of bypass graft. We are following up patients through the out patient department without recurrence up to 16 months.

Acute Aortic Injury after Nuss Bar Removal -A case report- (너스바 제거 후 발생한 급성 대동맥 손상 -치험 1예-)

  • Nuss operation as a method for correction of pectus excavatum is safe and satisfactory, but its complication presents pneumothorax, displaced bar, wound infection, pericarditis, pleural effusion, hemothorax, cardiac injury etc. We report a rare case of acute ascending aortic rupture after displaced and adhered Nuss bar removal. The patient was a 30-years old man who received Nuss operation 3 years ago. Nuss bar was removed without resistance but massively bled at both operation wound, so immediately femoro-femoral CPB and median sternotomy was done and repaired proximal aortic arch under deep hypothermic total circulatory arrest. The patient was discharged without complication at postoperative 13 day.

Electrostatic Characteristics of the Washer-dryer Combination under Various Drying Conditions (건조 일체형 드럼세탁기의 건조 조건에 따른 정전기 특성)

  • This paper suggests the optimal conditions for the laundry to prevent from the electrostatic charge by the evaluation of the electrostatic characteristics in the full process of washing and drying. Cotton, nylon, and polyester fabrics were used as test washing specimens. Detergent and softener were used under the standard washing cycle, and then the electrostatic characteristics of laundry were measured. The results showed that the moisture regain decreased and the electrostatic charge increased with the drying time. It was observed that the specimen fabrics were already dried up before the standard drying cycle was finished. Consequently, the excessive drying caused a generation of electrostatic charge due to the removal of the trace of moisture remaining and the excessive friction. Especially, the softener played an important role to prevent from a generation of electrostatic charge, whose insertion was more effective in the drying than in the rinsing process. It was also shown that the electrostatic charge could be decreased by drying the fabrics of one kind. On the other hand, for drying the mixed kinds of fabrics, the electrostatic charge increased remarkably. Therefore we suggest that the laundry be classified according to the kinds of fiber, and then be washed and dried before excessive drying to reduce electrostatic charge And further, a proper use of softener is effective to reduce electrostatic charge.

The Effect of Lonicera japonica Extract in Wound-induced Rats (창상을 유발한 흰쥐에서 금은화(金銀花) 추출물의 치료 효과)

  • Objectives This study is carried out to investigate the effects of Lonicera japonica in wound-induced rats. Methods Rats were divided into 5 groups; normal (Nor), control (Veh), positive comparison (PC), Lonicera japonica 100 mg/kg (LL), Lonicera japonica 200 mg/kg (LH), each n=8. Total polyphenol and flavonoid were quantified. 2,2-diphenyl-1-picrylhydrazyl (DPPH) and 2,2'-azino-bis(3 ethylbenzothiazoline-6-sulfonic acid) (ABTS) free radical scavenging activation were measured. Reactive oxygen species (ROS) was measured in serum. Antioxidant factors and inflammatory factors were measured in skin tissue, and also hydroxyproline content. Skin tissue was analyzed by Hematoxylin & Eosin and Masson's trichrome staining method. Results Total polyphenol and flavonoid were 32.86±0.14 mg/g and 67.17±0.57 mg/g. The IC50 values of DPPH and ABTS free radical scavenging activation were 26.69±1.50 ㎍/mL and 49.33±4.52 ㎍/mL. ROS was significantly lower in LL and LH groups. Nuclear factor-erythroid 2-related factor 2 (Nrf2) was significantly higher in LH group and higher in LL group but not significant. Superoxide dismutase 1 (SOD-1), catalase, and heme oxygenase 1 (HO-1) were significantly higher in LL and LH groups. Nuclear factor kappa-B p65 (NF-κBp65), phosphorylated iκBα (p-iκBα), cyclooxygenase 2 (COX-2), and tumor necrosis factor alpha (TNF-α) were significantly lower in LL and LH groups. Hydroxyproline was significantly higher in LL and LH groups. The histopathologic analysis showed that skin tissue had recovered further more in LL and LH groups than in Veh group. Conclusions These results suggest that Lonicera japonica has the anti-oxidant, anti-inflammatory and healing effects in wound-induced rats.
