Environmental Risk Factors for Attention Deficit Hyperactivity Disorder and Implications for Clinical Practice (주의력결핍 과잉행동장애의 환경요인과 임상적 의미)

  • In this review, we have provided an overview of the environmental risk factors for attention deficit hyperactivity disorder (ADHD), focusing on the major environmental toxicants related to the disorder. Researchers have indicated that since the characteristics of ADHD are complex, the disorder’s etiology involves multiple genes of moderate effect interacting with environmental factors. The possible roles of prenatal and perinatal exposure have been the main focus of research on environmental risk factors for ADHD. Among environmental toxicants, we reviewed the potential effects on the development of ADHD of exposure to lead, nicotine, alcohol, polychlorinated biphenyls (PCBs), and dioxin. Further, for the each neurotoxicant, clinical prevention or intervention strategies aimed at reducing a child’s risk from environmental toxic insults have been presented.

The Literature Study for Attention Deficit Hyperactivity Disorder(ADHD) in Traditional Chinese Medical Journals (주의력결핍 과잉행동장애 치료에 대한 문헌적 고찰 -최근 중의 잡지를 중심으로-)

  • Objective : The aim of this study was to investigate treatment of Attention Deficit Hyperactivity Disorder(ADHD) in current Traditional Chinese Medical Journals. Methods : From January 2000 to December 2005, we searched Traditional Chinese Medical Journals. Results : 21 articles were selected and classified into two large groups, case report and comparison study. Totally 1137 cases, complete cures are 276, significant improvements and some improvement 1028, complete cure rate 24.27%, total improvement rate 90.41%. Conclusion : Oriental medical treatments for ADHD were investigated to be effective in most trials. But there remains a debate about outcome criteria. Further studies are needed. Treatment methods were herbal medicine, acupuncture, auricular acupuncture, family consult.

  • Attention Deficit Hyperactivity Disorder(ADHD) is a common disease in children. This study is clinical case reports of 3 ADHD Children treated with acupuncture and herbal medicine. Two boys-one was 10 years old and the other was 8, and one girl-8 years old were treated with acupuncture and herbal medicine from March 2003 to May 2004. Acupoints-CV20(百會) CV24(上星) L7(列缺) LI4(合谷) Liv3(太衝) K3(太谿) was used and herbal medicine-Yukmijihwangtang(六味地黃湯) and Guibitang(歸脾湯) was used. T-score in ADS test(Continuous Performance Tests) checked after these treatments was lowered to the normal range, Conners' parent rating scale and Home Situations Questionaire score were lowered compared with those checked bfore treatment. These result suggest that acupuncture and herbal medicine could be one of helpful treatment to the ADHD children.

  • This paper first reviewed the definition, criteria, and neurological theories concerning the etiology of AD/HD, and the empirical studies dealing with the comorbidity of AD/HD with other psychiatric disorders. Secondly, results of studies using various neuropsychological tests for assessing the cognitive and behavioral problems in AD/HD children were examined, which suggest the possibility that dysfunction may exist in neural pathways involving many areas of the brain in AD/HD. However, because most of neuropsychological test used in Korea for ADHD children had been developed abroad, further study involving AD/HD, normal control, and other psychiatric control groups is needed to obtain developmental norms for interpreting the results, and to make more accurate diagnosis, and to clarify comorbidity of AD/HD with other disorders.

  • Attention-deficit hyperactivity disorder (ADHD) is a common, childhood-onset, neuropsychiatric disorder with an estimated prevalence of 2-7.6% in Korean children. Although the etiology of ADHD is not well understood, evidence from genetic factor and environmental factor studies suggests that ADHD results from a gene environmental interaction. In the current study, we reviewed the evidence for and clinical implications of the hypothetical roles of organophosphate pesticides, organochlorine pesticides, polychlorinated biphenyls, phthalate, bisphenol, polyfluoroalkyl chemicals, polycyclic aromatic hydrocarbons, mercury, lead, arsenic, cadmium, manganese, tobacco, alcohol as harmful risk factors in the development of ADHD.

  • Attention deficit hyperactivity disorder (ADHD) is a common childhood psychiatric disorder. Recently, it has been suggested that brain-derived neurotropic factor (BDNF) may play a role in the pathogenesis of ADHD. Our aim of this review is to understand the physiological functions of BDNF and its potential relationship with ADHD and therapeutic approaches of ADHD. Searches were conducted in Pubmed and Research Information Service System (RISS). In this review, we summarized important literatures for the physiological functions of BDNF in neurodevelopment, change of serum BDNF level in ADHD, association of BDNF polymorphism and ADHD and potential association of treatment of ADHD with serum BDNF level. Further studies are required to more clearly understand the source and the role of BDNF in ADHD and to develop BDNF based-ADHD treatement.

  • The purpose of this study was to detect early children with hyperactivity which is one of the symptoms of Attention Deficit-Hyperactivity Disorder (ADHD). This study used two methods: K-CBCL and observation of children's behavior. K-CBCL was done online by parents at home. For observation of children's behavior, the school asked children to wear a 3-axis accelerometer on their wrists. The data from K-CBCL and 3-axis accelerometer were analyzed and clustered to separate hypersensitive children from ordinary children. This experiment confirmed that 3-axis accelerometer which is one of Ubiquitous techniques and the K-CBCL questionnaire were helpful for detection of hypersensitive children.

  • ADHD(과활동성 주의력 결핍 장애) 환자 수가 증가하며 주의력 집중이 사회적 문제로 대두되고 있다. 그러나 ADHD에 대한 이해나 요인에 대한 연구는 미흡하다. 본 연구에서는 아동기 전신마취가 ADHD 발생에 영향이 있다는 연구를 기반으로, 상관관계 분석과 선형회귀분석, Lasso Regression, Support Vector Regression, Deep Neural Network, Ensemble, Random Forest Regression을 활용하여 ADHD 증가 요인에 대해 탐구했다. 분석 결과는 전신 마취에 노출될 가능성이 높은 아동의 경우 ADHD에 노출될 가능성 역시 높을 수 있음을 시사한다.

  • The purpose of the present study is to investigate cognitive characteristics of ADHD children by comparing their performances on KEDI-WISC according to age and EEG variables. Subjects were 56 ADHD children who visited Seoul National University Children's Hospital during the period from January, 1988 to March, 1989. Group differences on age and EEG variables were tested by ANOVA, and Hierarchical Cluster Analysis was performed to investigate how ADHD children were classified based on their performances on KEDI-WISC. The results Indicated that ADHD children showed low scores on Coding, Digit Span, and Comprehension subtests, suggesting their attention deficits and impulsivity. ADHD children were clustered Into three groups based on only FSIQ. In post-hoc tests three groups showed different cognitive strengths and weaknesses on KEDI-WISC. Group differences on age were not significant, and abnormal EEG group showed lower PIQ than normal EEG group, suggesting the possibility that their attention deficits were related to neurological factors.

  • Objectives : This review examines the extent of harmonization between the Diagnostic and Statistical Manual (fourth edition) (DSM-IV) attention deficit hyperactivity disorder (ADHD) and International Classification of Diseases (tenth edition) (ICD-10) hyperkinetic disorder (HKD). Methods : PubMed review of the clinical literatures from 1980 to March 2005 was undertaken using the search term of ADHD, HKD, DSM, ICD, diagnostic criteria, algorithm, and validity. Results : There continues to be a substantial difference in the prevalence of ADHD and HKD. Based on the limited studies that directly compare the most recent two algorithms of ADHD and HKD, several implications for clinical practice and research are discussed. Conclusion : To understand further if the criteria for DSM-IV and ICD-10 for ADHD and HKD are converged or not, more researches on the validity of ADHD and HKD subtypes should be undertaken.
