DENTAL MANAGEMENT OF A PATIENT WITH KABUKI SYNDROME : A CASE REPORT (가부키 증후군(Kabuki syndrome) 환자의 치과적 관리의 치험례)

  • Kabuki syndrome is characterized by typical facial features (elongated palpebral fissures with eversion of the lateral third of the lower eyelid; arched and broad eyebrows; short columella with depressed nasal tip; large, prominent, or cupped ears), minor skeletal anomalies, persistence of fetal fingertip pads, mild to moderate intellectual disability, and postnatal growth deficiency. A 6-year-old male with kabuki syndrome was referred from the local dental clinic for left facial swelling and dental caries on all primary molars. He was treated for acute periapical abscess with incision and drainage under physical restraint, and left maxillary second primary molar was extracted. Other caries treatment was performed under general anesthesia. As the syndrome involves many different medical problems, special cares should be considered. Dental treatment should be carried out in comprehensive consultation system.

Bond Strength of Band on Zirconia Crown with Compomer and Resin Cement (지르코니아 기성관에 컴포머와 레진 시멘트로 접착한 교정 밴드의 결합력)

  • The aim of this study was to evaluate the compomer cement and resin cement as an orthodontic band cement on zirconia crown. A total of 30 specimens were prepared. Preformed stainless steel crowns and zirconia crowns of upper right second primary molar were used. Orthodontic bands were cemented on stainless steel crowns (Group I, n = 10) and zirconia crowns (Group II, n = 10) with compomer cement. The other bands were cemented on zirconia crowns with resin cement (Group III, n = 10). The tensile loads were applied to band to measure the bond strength. The mean of bond strengths of group I, II and III were 0.79 MPa, 1.09 MPa and 1.56 MPa respectively. Bond strength of group II is significantly higher than group I. There was no significant difference between group II and III. Compomer cement and resin cement containing functional monomers showed favorable bond strength of orthodontic bands.

THE INFLUENCE ON THE ERUPTION OF MANDIBULAR 1st PREMOLARS AFTER PREMATURE LOSS OF MANDIBULAR 1st DECIDUOUS MOLARS (하악(下顎) 제1유구치(第一乳臼齒) 조기상실(早期喪失)이 하악(下顎) 제1소구치(第一小臼齒) 맹출(萌出)에 미치는 영향(影響))

  • The purpose of this study is to determine what influence the lesion and the premature loss of mandibular 1st deciduous molar have on the eruption of its successor, and the author devided 580 cases of orthopantomograms from age 3 to 9 years old children into 3 groups: Control Group: both side normal Study I Group: one normal and the other lesion. Study II Group: one normal and the other premature loss before 8 years old. and observed the amount of differences and the relative position in eruption between study-tooth successor and normal-tooth successor, The following results were obtained: 1. The differences in eruption between right and left mandibular 1st primary molars of Study Group were greater than those of Control Group. 2. The successors of lesioned-teeth showed more accelerated eruption than the antimeres in 40.9% and more delayed, in 22.7%. 3. The successors of premature lost-teeth showed more accelerated eruption than the antimeres in 61.7% and more delayed, in 4.3%.

  • Ectopic eruption should be understood as a change in the course of the normal eruption path of a dental bud at any moment its origin. An example of this alteration is the dental transposition, a rare and more specific dental anomaly that may be defined as a change of position between two teeth. This case shows ectopic eruption of transposed mandibular lateral incisor beneath primary first molar at the first transitional period of the mixed dentition The crown of the lateral incisor has tipped distally, compelling root resorption and exfoliation of the adjacent primary cuspid and primary first molar. The reason for such eruption is not clearly understood, but it may involve; (1)trauma history, (2)prolonged retention of the deciduous teeth, (3)premature exfoliation of the deciduous teeth, and (4)genetic factor. Treatment is divided into interceptive and definitive treatment. Ectopically erupting mandibular incisor tends to become transposed with the adjacent cuspid and thus seems to warrant early orthodontic intervention. Early treatment may obviate later extraction or transposition of the incisor and canine in the permanent dentition. Timing is an important factor to be considered regarding in the correction of the lateral incisor transposition. This case advocates treatment with an active orthodontic therapy at the early stage of the mixed dentiton, before the eruption of the permanent cuspid.

  • In the mixed dentition, many children have mild crowding of anterior dentition on the mandibular arch and mesial tilting of mandibular molar. Lip bumper have been used to gain arch length for the alignment of mild to moderate crowded dental arches. As such, they may provide an alternative to extraction therapy. The claimed therapeutic effect of the lip bumper is bodily forward incisor movement, flaring of the lower incisors, and distal tipping of the molar. The dental changes can be attributed to removal of lip pressure on the lower anterior dentition and the distal forces exerted at the molar abutment. The purpose of this study is to show more easily method of treatment for mild anterior crowding of mandible.

  • An eruption failure can be observed for child and adolescent periods when the primary dentition is changed to the permanent dentition through the mixed dentition frequently. The eruption failure can lead to miss erupting times of the tooth, then it will cause a lot of problems including root resorption, esthetic problem, transposition of adjacent tooth, malocclusoin and etc. Especially, the maxillary first molar is importantly concerned with occlusion and growth and is an essential tooth for development and maintenance of occlusion. So, it is a momentous part of more proper occlusal management to find these abnormal cases at the early stage and solve the problems. The sorts of eruption failures of the maxillary first molars can be divided into delayed eruption, impaction and the primary retention and the secondary retention. When physical obstacles cause impaction, first of all they must be removed then we can treat the impaction with observation after removal, surgical exposure or orthodontic traction. If the source of impaction is an ectopic eruption, the treatment can be a brasswire, a pendulum appliance, a space maintainer or space regainer after the extraction of the second deciduous tooth and etc. These cases are made a diagnosis of eruption failures of the maxillary first molars in mixed dentition period and have good prognosises after my treatments. So I reported them.

MICRO-TENSILE BONDING STRENGTH OF REGIONAL PRIMARY MOLAR DENTIN (유구치 상아질의 각 부위에 적용된 수종의 복합레진 접착제의 미세인장접착강도에 관한 연구)

  • The purpose of this study was to evaluate the micro-tensile bond strength (${\mu}TBS$) of four luting resin to regional dentin of human primary teeth. Dentin from non-carious primary molars were prepared from different regions (s, superficial dentin; d, deep dentin; c, cervical dentin), and divided into groups based on anatomical locations and types of luting resins (Scotchbond Multi-purpose : SB ; One-Step : OS ; AdheSE Bond : ASE ; G-Bond : GB) : SB-s, SB-d, SB-c; OS-s, OS-d, OS-c; ASE-s, ASE-d, ASE-c ; GB-s, GB-d, GB-c. Luting resins were used according to the manufacturers' instructions, to bond $Light-Core^{TM}$ Core Build-Up Composite) to the exposed dentin specimens in the light-curing mode. After storage for 1 day, ${\mu}TBS$ was tested at a cross-head speed of 1 mm/min. Data were analyzed with T-test and two-way ANOVA. The bonding interface and fractography analyses were performed with SEM. The results were as follows : 1. ${\mu}TBS$ to superficial dentin was significantly higher than to deep dentin for SB(p<0.05). But there were no significant differences in regional ${\mu}TBS$ among OS, ASE, GB(p>0.05). 2. There were no significant differences in ${\mu}TBS$ to superficial dentin among each groups. But, in deep dentin, ${\mu}TBS$ of SB-d was significantly lower than those of OS-d, ASE-d, and GB-d(p<0.05). ${\mu}TBS$ of OS-d was significantly higher than those of GB-d(p<0.05), but there were no significant differences in ${\mu}TBS$ of ASEd. There were no significant differences among ${\mu}TBS$ of ASE-d, OS-d, and GB-d.

  • The purpose of this study was to compare the retentive forces of various types of clasps for removable orthodontic appliances for primary and mixed dentition. Seven metal models of a single tooth and two teeth were made, including maxillary left primary canine, first primary molar, second primary molar and first molar. Retentive forces of Adams clasp, circumferential clasp(C clasp), Jackson clasp, Duyzing clasp, arrowhead clasp, ball clasp, eyelet clasp, and triangular clasp were measured by Universal Testing Machine(Zwick Z020, Germany). The obtained results were as follows. 1. Jackson clasp and Adams clasp showed the highest retentive force among single tooth clasps. 2. C clasp showed the lowest retentive force, and there was no statistically significant difference in retentive force between mesial end C clasp and distal end C clasp. 3. Eyelet clasp showed the highest, and ball clasp showed the lowest retentive farce among clasps for interdental undercut. 4. Triangular clasp showed higher retentive force than ball clasp.

  • Ectopic eruption of the first permanent molar is an abnormal positioning of this tooth, causing a premature resorption of the distal surface of the second primary molar. It occurs in approximately 3~4% of the population and the maxillary arch is usually affected. While 66% of ectopically erupting permanent molars are corrected spontaneously without treatment (i.e. a reversible type), active treatment is necessary for irreversible ectopic eruption cases. The treatment modalities have been divided into two categories: interproximal wedging and distal tipping. Interproximal wedging is indicated for minimal impaction and when the impaction is severe, distal tipping techniques are required. Although much has been written about treatment modalities on unilateral ectopic eruption of the first permanent molar, few reports mention bilateral ectopic eruption cases. In this report, two cases of bilateral ectopic eruption of the first permanent molars in young patients are presented. We describe the use of a modified bilateral Halterman appliance for correction of these cases and the clinical results were satisfactory.

철도기준점을 이용한 철도중심선형 좌표변환에 관한연구 - 호남고속철도 계획노선을 중심으로 -

  • In this paper through Honam high-speed railroad which is planned with the north and south axis, we will verify the feasibility of the coordinate conversion using railroad control points after regarding current planned-railroad as the linear central axises. From analysis, distortion of Y axis varies 21cm to 40cm diminishing to a gentle straight line, distortion of X axis varies 14cm to 29cm. Through a revision, the deviation value between the coordinates were 6mm to 9mm and it satisfied the allowable error of national geographic information institute which is following ITRF (International Terrestrial Reference Frame) and cadastral boundary survey(10cm). consequently the coordinate conversion is possible using railroad control points as common control points.
