INTRAVENOUS SEDATION WITH MIDAZOLAM (Midazolam을 이용한 정주진정요법)

  • Oral administration of chloral hydrate with hydroxyzine, nitrous oxide inhalation and intramuscular or intranasal administration of midazolam is the most commonly used pharmacological behavior management methods for the children in pre-cooperative stage or show negative behavior in pediatric dentistry. For oral administration of chloral hydrate with hydroxyzine, it is hard to apply to the patient refused to intake and with overweighted or elderly children and the effect of the drug decrease. Nitrous oxide and midazolam is anxiolytic agents and have limitation for properly managing the behavior of the children refuse to dental treatment. These occasions, deep sedation or general anesthesia can be considered. Although intravenous sedation with midazolam has many advantages such as, rapid onset and recovery, possible to titration and few side effect, no rebound effect by metabolites, the reports for, intravenous sedation with midazolam is insufficient in pediatric dentistry. We report the case in pre-cooperative stage. He is treated successfully under intravenous sedation with midazolam.

  • The way of thinking of rural residents(Izuk-myun, Anseong-gun, Kyeonggi-do) was surveyed in order to develop the basic model of new rural setlement planning. From the data collected in this survey, rural life-style, farmers' view on traditional rural life, agriculture, farming, agri-policy, social attitude, and degree of satisfaction of rural life, etc. were analyzed. Firstly, most of rural residents interviewed in this survey have the opinion of not leaving their home village if living conditions improved to a reasonable level in the near furture. In decision- making process of village affairs, they want to participate actively and equally. Secondly, in spite of their acceptance of agriculture as a vital sector destined to national security, most of them think that farming should be little of promise. Thirdly, they have the negative attitude to urban-oriented social value and also to the agricultural policy, while seeking consolation through human and traditional viewing of rural life style. Therefore, in the development of new rural settlement model, idea-making should be directed to the betterment of communal value of village life, stability of rural economy, rationalization of rural life style and enhancement of rural-type happiness.

Impact of Dental Treatment under General Anesthesia or Intravenous Sedation on Oral Health-related Quality of Life in Pediatric Dental Patients (전신마취 또는 정주진정 하 치과치료가 소아 환자의 구강 건강과 관련된 삶의 질에 미치는 영향)

  • The present study aimed to assess the perception of a child's primary caregiver on the oral health-related quality of life (OHRQoL) of dental treatment under general anesthesia (GA) or intravenous sedation (IV-SED) in pediatric dental patients. Self-administered questionnaires were completed before dental treatment under GA or IV-SED by 52 primary caregivers of healthy pediatric patients (10 years old or younger), and 43 (84%) of these caregivers completed the same questionnaires within 6 months after treatment. The Korean version of Child Oral Health Impact Profile (COHIP) and the Family Impact Scale (FIS) were used to assess OHRQoL in the questionnaires. The scores of COHIP and FIS improved after dental treatment. Demographic variables such as gender did not affect the improvement of COHIP and FIS, and age did not affect that of COHIP. However, baseline FIS score of younger patients was better than that of older patients before dental treatment. Regardless of the treatment variables, all COHIP and FIS scores were improved after dental treatment. COHIP score improved more in cases with pulp treatments compared to those without the treatments. On the other hand, FIS score improved less in cases with posterior stainless steel crown restorations compared to those without them. Anterior esthetic restorations and anterior teeth extractions did not affect the degree of improvement. Based on the primary caregiver's perceptions, the OHRQoL of healthy pediatric patients was improved by dental treatment under GA or IV-SED.
