Correlation of motor and non-motor symptoms in Parkinson's disease: a factor-analytic convergence study (파킨슨병 환자의 운동과 비운동 증상의 상관관계: 요인 분석 융합 연구)

  • Parkinson's disease (PD) is clinically characterized by a variety of motor and non-motor symptoms, including cognitive and neuropsychiatric symptoms. Integrating a large variety of symptoms into a small number of clinical subtypes could be valuable for appropriate and early therapeutic intervention. As a first step toward this aim, this study attempted to identify correlation patterns among motor, cognitive and neuropsychiatric symptoms in PD without dementia. One hundred four non-demented patients with PD underwent a comprehensive motor, neuropsychological, and neuropsychiatric assessments. Factor analysis was performed to identify correlation patterns among demographic, motor, cognitive and neuropsychiatric variables. The eight factors were extracted: 1 motor-related, 3 cognitive-related and 4 neuropsychiatric factors. We indentified that characteristics of correlation can have associated symptom pattern in the disease process of Parkinson's disease. The current results suggest that a broad range of motor and non-motor symptoms in PD may be reducible to a small number of clinical parameters, which may be useful for identifying clinical subtypes of PD for individual patients.

  • Virtual Reality(VR) is a new technology which makes humans communicate with computer. It allows the user to see, hear, feel and interact in a three-dimensional virtual world created graphically. In this paper, we introduced VR into psychotherapy area and developed VR system for the exposure therapy of acrophobia. Acrophobia is an abnormal fear of heights. Medications or cognitive-behavior methods have been mainly used as a treatment. Lately the virtual reality technology has been applied to that kind of anxiety disorders. A virtual environment provides patient with stimuli which arouses phobia, and exposing to that environment makes him having ability to over come the fear. In this study, the elevator stimulator that composed with a position sensor, head mount display, and audio system, is suggested. To illustrate the physiological difference between a person who has a feel of phobia and without phobia, heart rate was measured during experiment. And also measured a person's HR after the virtual reality training. In this study, we demonstrated the subjective effectiveness of virtual reality psychotherapy through the clinical experiment.

  • 4차 산업혁명 시대가 도래해 인간 뇌와 기계 간 인터페이스 기술 개발이 한창이다. BMI(Brain-Machine Interface)는 뇌의 신경계로부터 신호를 측정하고 분석해 기계와 같은 외부 기기에 연결해 제어함으로써 사용자의 의사나 의도대로 기기를 움직이는 인터페이스를 만드는 것이다. 뇌-기계 인터페이스 기술은 뇌질환 치료, 장애인을 위한 로봇 팔과 로봇다리 같은 인체 결합기술, 인간과 기계와의 직접적인 정신 교류의 개발을 위한 필적인 기술이다. 본 논문에서는 4차 산업혁명의 핵심기술 중 하나인 뇌 기계 인터페이스에 대한 3P 정보분석을 수행함으로써 BMI의 R&D 및 시장진입을 위한 전략을 제시하였다.

Management of Weight Gain and Obesity Associated With Antipsychotics (항정신병약물 사용과 연관된 체중 증가와 비만의 관리)

  • Objectives : The risk of weight gain is high when using antipsychotic drugs, and the prevalence of obesity in people with mental illness is high. Obesity management in psychiatric patients is important because obesity causes various complications and lowers treatment adherence and quality of life. Methods : In this review, we summarized the management strategies for obesity that can occur when using antipsychotic drugs through a web search. Results : Evaluate obesity-related risk factors and related indicators from the beginning of treatment, and conduct regular monitoring. If an antipsychotic drug is used and obesity is induced, a change to a drug with a low metabolic risk may be attempted. Sufficient interventions are also needed on the need to manage obesity, a healthy diet, and exercises in patients and their families. If weight loss is not achieved and obesity-related complications are associated, the use of anti-obesity drugs may be considered. Pharmacological treatment approaches should be carefully considered. Conclusions : Non-pharmacological and pharmacological therapies can be applied to manage weight gain and obesity caused by the use of antipsychotic drugs. When using anti-obesity drugs, the characteristics of mental disorders, drug safety, and drug interactions should be considered.

Combination & Augmentation Strategies in the Treatment of Depressive Disorder (우울증 치료에 있어서 약물의 Combination과 Augmentation 전략)

  • Even the pharmacotherapy is more effective than placebo for the treatment of depression, the outcome of pharmacoltherapy remains unsatisfactory for many patients. Apart from side effects, there are two major limitations of antidepressant therapy. One is the delayed onset of improvement and another is partial response. In order to address these clinical dilemmas, many psychiatrists more commonly employ add-on therapy. In past, the practice of using multiple drugs to enhance treatment response was called polypharamcy, and was disparaged as poor clinical practice. However, with improved understanding of how drugs affects the central nervous system and increased communication in journals and on computer networks about the relative merits of specific combinations, the scientific basis for the combining drugs is being defined. Indeed, the use of multiple medications as a stratege to enhance response has become both acceptable and widespread now a days. It is now referred to more positively as add-on therapy, co-medication, combination therapy, or drug augmentation. Thus, as the methods of practical strategies for treatment of depression, switching classes antidepressant drugs, combination therapy, augmentation strategies and brief treatment algorithm will be presented with items of considerations. However, when combination of drugs being tried, knowledges about the action of mechanism, pharamcokinetics, and pharmacodynamics are essential to cope with the possible adversive reactions and to get the appropriate responses for the treatment of depressive symptoms.

  • Although obsessive-compulsive disorder(OCD) affects children, adolescents, and adults whether its juvenile(dhild and adolescent) and adult forms are different subtype of a disorder remains unknown. But there are increasing findings that suggest that juvenile OCD may be unique subtype of the disorder. One proposed subtype is the childhood OCD associated with high comorbidity of tic disorder and increased familial loading for OCD or tic disorder. The other proposed subtype is the childhood OCD and/or tic disorder occuring in association with streptococcal infection(PANDAS). These two subtypes of OCD are unlikely to respond to SSRI due to possible different pathphysiological mechanism. So this paper reviews the characteristics of OCD and therapeutic approaches for treatment resistant OCD in childhood and adolescence. Considering the likely heterogeneity of OCD, the possibility that juvenild OCD may be a variant of the disorder can have important clinical and scientific implications because it may further our understanding of this disorder, its etiology, and perhaps its treatment.

Comparison of Alexithymia among Patients with Psychosomatic Disorders, Anxiety Disorders and Depressive Disorders (정신신체장애, 불안장애 및 우울장애 환자들 간의 Alexithymia의 비교)

  • A comparison was made regarding the degree of alexithymia among patients with psychosomatic disorders, anxiety disorders and depressive disorders. The author examined the degree of alexithymia in three groups : 100 psychosomatic patients(including 47 patients with tension headache), 52 outpatients with anxiety disorden, and 50 outpatients with depressive disorders. Alexithymia was assessed by Alexithymia provoked Response Questionnaires(APRQ) developed as a semi-structured interview form. No significant difference was found in the degree of alexithymia among Patients with Psychosomatic disorders, anxiety disorders, and depressive disorders. On the other hand, patients with tension headache were significantly more alexithymic than patients with anxiety disorders and depressive disorders, respectively. However, there was no significant difference in degree of alexithymia between patients with anxiety disorders and those with depressive disorders. Multiple regression analysis revealed that demographic variables such as set age, education level, and marital status did not make a significant influence on alexithymia scores. These results suggest a greater degree of alexithymia in patients with a specific group of psychosomiatic disorders such as tension headache than in patients with emotional disorders, unlike the previous report that in general, psychosomatic patients are alexithymic. Thus, it is necessary to develop special forms of interview which can induce and encourage expression of emotion as a therapeutic strategy for patients with tension headache.

  • When a traumatic subject experiences extreme stress, various psychiatric problems, including PTSD, can occur if they do not respond appropriately. In addition, non-war soldiers who experience life-threatening events while serving in the military may experience severe and chronic PTSD symptoms. However, there are still insufficient domestic studies on what traumatic events soldiers are exposed to and how much PTSD symptoms they experience. Therefore, this study provides a method that can be used as basic data for PTSD and psychological support of military service personnel by identifying trends in domestic research. Furthermore, it studies PTSD not only of soldiers but also of 'civilians' who have experienced combat, and provides symptomatic treatment and relief programs to civilians according to the difference in PTSD between general trauma and war trauma. It is expected that these studies will increase military confidence and enable the military to play a leading role in war trauma PTSD research and prevention, treatment and mitigation.

Clinical Subtypes of Delirium (섬망의 임상적 아형)

  • Delirium is an organic psychiatric syndrome characterized by an acute onset, prominent disturbance of consciousness and cognitive impairment with fluctuating course. Although there is not a clear consensus concerning the optimal classification system for delirium subtypes, Lipowski(1983) firstly classified delirium by psychomotor activity, namely hyperactive, hypoactive, and mixed. According results of several following studies, prevalence of hypoactive delirium were not less than that of hyperactive delirium. But a diagnosis of hypoactive delirium often missed, which is most frequently misdiagnosed as depression and dementia. Hyperactive delirium can be caused by alcohol or benzodiazepine withdrawal, would be related with excessive dopamine and cholinergic deficiency, and is more responsive to high-potency antipsychotics therapy. Hypoactive delirium would be caused by metabolic encephalopathy, and tends to present a less responsiveness to antipsychotics and poorer overall prognosis with a prolonged duration of admission than hyperactive delirium. Delirium is not a homogenous syndrome. Because of different subtypes, it may have dissimilar underlying pathogenetic pathways. So different treatment strategies between various subtypes may be needed.

  • The purpose of this study is to investigate the state projects related to psychological sentiment for child and youth in Korea, examine the current status of art treatment-related projects for child and youth, and seek ways to improve them, focusing on projects supported by the Ministry of Health and Welfare, the Ministry of Gender Equality and Family, and the Ministry of Culture, Sports and Tourism. The results examined through this study were firstly, mental health promotion projects conducted in the public sector in Korea are mainly carried out on consignment to affiliated organizations, and are promoted not only for diagnosis, mediation, and therapeutic intervention but also for care and prevention. Second, psychological and emotional support projects are being attempted with various approaches such as reading, playing, gardening, and art, and art therapy is actively used as a part of art programs. In addition, it is suggested that the government should provide efficient welfare and utilize it as a common goal through linkage and cooperation with other ministries for the continuous and stable implementation of state-led psychological support projects. In addition, it is hoped that this research will be used as a research material to explore specific strategies for effectiveness of national policy projects on the overall psychological sentiment of child and youth.
