The Stress Perception, Depressive Symptoms and Medical Comorbidity in Healthcare Center (대학병원 건강검진센터 내원자의 스트레스 지각 정도, 우울 증상 및 신체질환 이환율)

  • Objectives : We surveyed this study for knowing the relation within stress, depression and medical comorbidities, and finding the risk factors of major depression. Methods : 1764 subjects were enrolled from Jan. 2009 to Dec. 2009 who visit Korea University Guro hospital healthcare center. The subjects answered the questionnaire of PSS(Perceived Stress Scale), PHQ-9(Patient Health Questionnaire-9) and the demographic data. We categorized them as the stress group, depressive group, medical comorbidity group and analyzed the correlation analyses and logistic regression analyses. Results : 198 of 1764 subjects(11.8%) were applied to major depression, and the depressive group showed the higher mean stress score(23.19) and mean depression score(12.95) than the normal group. The total PHQ-9 score was increased by perceiving more stress, having more medical comorbidities. The subjects with female, visiting due to recent health problems, irregular exercise, current smoking, history of angina and cerebrovascular disease showed the increased risk of major depression. Conclusions : In this study, we find the PSS, PHQ-9 were valuable for mental health screening in healthcare center. As perceiving more stress and having more medical comorbidity, risk of major depression were increased. Accordingly the individuals with medical diseases or unhealthy lifestyle would need the mental health screen.

Job Satisfaction of Psychiatric and Community Mental Health Nurses in Community Mental Health Centers (정신보건센터에 근무하는 정신보건간호사의 직무만족도)

  • Purpose: The purpose of this study was to investigate the relationship of psychiatric and community mental health nurses' job satisfaction. Methods: This was a descriptive and correlational study in which 151 psychiatric and community mental health nurses (PCMHNs) took part between September and October 2009. Results: The mean score for job satisfaction of the participants was 3.25. In job satisfaction, there were significant differences between psychiatric and community mental health nurse practitioner (PCMHNP) and nurse in community mental health center. All variables except role conflict were positively correlated with job satisfaction. As a result, factors influencing job satisfaction were reward system (${\beta}$=.46, p<.001), self-esteem (${\beta}$=.35, p<.001), leadership (${\beta}$= .27, p<.001), career as a PCMHNP (${\beta}$=-.12, p<.007), job-related characteristics (${\beta}$=.11, p<.24), and career as a nurse in community mental health center (${\beta}$=.09. p<.045). These factors explained 70.3% of the total variance. Conclusion: The results indicated that the comprehensive understanding of job satisfaction to the independent variables could suggest the way how to increase the job satisfaction for psychiatric and community mental health nurse who are a catalysts promoting mental health in community mental health center.

A Study on the Floor Plan Type and the Spatial Composition of Standard Mental Health Centers in Seoul (서울시 기초정신건강증진센터의 평면유형 및 소요공간구성에 관한 연구)

  • Purpose: The purpose of this study is to provide data on the basis for architectural planning of floor plan type and spatial composition by analysing standard mental health centers in Seoul. Methods: Data were collected through literature research, field surveys, and expert interviews to 22 standard mental health centers in Seoul. Results: The results of this study could be summarized as follows; Firstly, Mental Health Center is divided into five types according to the location. Location types of mental health center were 'Director type', 'Health center connection type', 'Public facilities connection type', 'Complex center connection type' and 'Commercial Facilities connection type'. Secondly, Depending on the type of management a mental health center is divided into two types. Types are 'Complex type' and 'Independent type'. Average area of 'Complex type' is $192.99m^2$ and 'Independent type' is $266.87m^2$. This difference affects the various spaces. Implications: It is necessary to give and architectural suggestion of mental health center in response to the proposal of the system.

The Correlation Between Defense Mechanism and Posttraumatic Stress Disorder in Burn Patients (화상 환자의 방어기제와 외상후 스트레스 장애와의 관련성)

  • Objectives : This study was designed to investigate the correlation between defense mechanism and posttraumatic stress disorder in burn patients. Methods : We recruit 40 burn patients and we evaluate sociodemographic characteristics, characteristics related to burn, korean version of Impact of Event Scale-Revised (IES-R), Korean Version of Center for epidemiologic studies depression scale (CES-D) and Ewha Defense Mechanisms Test (EDMT). Multiple regression analysis was performed to evaluate the correlation between EDMT score and IES-R score. Results : Factor 2 of EDMT which consists of Ego-expansion subjects was significantly correlate with IER-S score. In factor 2, Controlling (β=-0.299, p<0.05), Sublimation (β=-0.276, p<0.05) is significantly correlate with IER-S score. Conclusions : Some subjects of defense mechanism was significantly correlate with IES-R score. it might be helpful to understand defense mechanism for the posttraumatic stress disorder in burn patients.

Mediating Effects of Hope and Therapeutic Relationship in the Relation between General Social Functions and Mental Health Recovery of Community People with Mental Illness (지역사회 정신장애인의 전반적 사회기능과 정신건강 회복 간의 관계에서 희망과 치료적 관계의 매개효과)

  • Purpose: This study tries to test mediating effects of hope and therapeutic relationship in the relation between general social functions and mental health recovery of community people with mental illness. Methods: This study was carried out in a cross-sectional research design. The participants included 217 people with mental illnesses who were enrolled at eight Mental Health Welfare Centers in the Gyeonggi Province. Data were collected from February to May, 2018. The collected data were analyzed using a regression analysis, and SPSS PROCESS macro was used to test the mediating effects. Results: This study analyzed the direct effects of general social functions on the hope, therapeutic relationship and mental health recovery. And general social functions had indirect effects on their mental health recovery via hope and therapeutic relationship. Both hope and therapeutic relationship had dual mediating effects in the influence of general social functions on mental health recovery. Conclusion: The role of hope and therapeutic relationship in the mental health recovery of community people with mental illness is important, and it is confirmed that hope is a powerful factor influencing mental health recovery.

A Cohort Study of Children and Adolescents Victims with Sexual Abuse in Korea and Their Initial Assessment Results (아동청소년 성폭력 피해자들을 위한 코호트 연구 : 코호트 구축과 초기 평가 결과)

  • Objectives : The goals of the study are how to establish the cohort systems for the children and adolescents victims with sexual abuse in Korea and to identify the risk and protective factors that influence mental health in child sexual abuse (CSA). This is initial assessment data based on the analysis of cohort variables for baseline evaluation of subjects. Methods : We constructed the cohort systems for CSA victims recruited by Seoul Sunflower Children Center, CSA victims protection center. The initial assessment data which consisted of demographic and psychological inventories of CSA victims and their parents/families, psychiatric diagnoses were the results of statistical analysis of 65 subjects under 19 years old for 3 years 7 months. Results : The initial data were followings : female participants, N=56; mean age, 11.6 (SD=4.5); the most sexual assault, molestation 71.8%; victims, family and acquaintance 87.1%; 61.5% of the subjects diagnosed with psychiatric disorder; 29.2% diagnosed with PTSD and 23.1% diagnosed with depression. Mean duration for abuse to report is 1.5 years. Mean score of IES-R-K, TSCYC-avoidant and CBCL-problematic behavior were increased above clinical cut-off. Conclusions : CSA victims tend to have high risks in mental health problem. The cohort study could provide the risk and protective factors of CSA in mental health, and construct the predictive model for mental illness in Korea.

Korean Treatment Guideline on Pharmacotherapy of Co-existing Symptoms and Antipsychotics-related Side Effects in Patients with Schizophrenia ('2019 한국형 조현병 약물치료 지침서'에 따른 조현병에서 동반증상 및 부작용의 치료)

  • Objectives: The current study covers a secondary revision of the guidelines for the pharmacotherapy of schizophrenia issued by the Korean Medication Algorithm for Schizophrenia (KMAP-SCZ) 2001, specifically for co-existing symptoms and antipsychotics-related side-effects in schizophrenia patients. Methods: An expert consensus regarding the strategies of pharmacotherapy for positive symptoms of schizophrenia, co-existing symptoms of schizophrenia, and side-effect of antipsychotics in patients with schizophrenia was retrieved by responses obtained using a 30-item questionnaire. Results: For the co-existing symptoms, agitation could be treated with oral or intramuscular injection of benzodiazepine or antipsychotics; depressive symptoms with atypical antipsychotics and adjunctive use of antidepressant; obsessive-compulsive symptoms with selective serotonin reuptake inhibitors and antipsychotics other than clozapine and olanzapine; negative symptoms with atypical antipsychotics or antidepressants; higher risk of suicide with clozapine; comorbid substance abuse with use of naltrexone or bupropion/varenicline, respectively. For the antipsychotics-related side effects, anticholinergics (extrapyramidal symptom), propranolol and benzodiazepine (akathisia), topiramate or metformin (weight gain), change of antipsychotics to aripiprazole (hyperprolactinemia and prolonged QTc) or clozapine (tardive dyskinesia) could be used. Conclusion: Updated pharmacotherapy strategies for co-existing symptoms and antipsychotics-related side effects in schizophrenia patients as presented in KMAP-SCZ 2019 could help effective clinical decision making of psychiatrists as a preferable option.

School Based Intervention with Mental Health Problem in the Elementary School : Systematic Review (정신건강 어려움이 있는 국내 초등학생의 학교-기반 중재 : 체계적 고찰)

  • The purpose of this study was to find more effective mental health intervention for school based intervention with mental health problem in an elementary school. A systematic review was conducted according to the PRISMA checklist. We accessed databases including RISS, KISS, and the National Assembly Library of Korea. Subject studies were classified according to type of intervention, subject characteristic, and evaluation. Finally, twenty-five studies matched the inclusion criteria. According to types of intervention, it was classified into four types which are psychoeducation(self-management skills, social skills, social emotional learn ing), behavior intervention(positive behavior support, modeling), psychotherapy(art therapy, counseling and parent coaching), and recreation intervention(robot intervention, horticultural therapy). The most of subjects were intellectual disability, the following subjects were ADHD. The most areas of evaluation were emotional change and attention. The findings of this review support school based intervention and provide evidence. And it can be used as an important basic data for preparing more effective mental health-related school-based interventions.

Relationship of Participation in Jazz Dance to Body Image and Mental Healt (재즈댄스 참가와 신체상 및 정신건강과의 관계)

  • The purpose of this study was to examine the relationship of participation in jazz dance to body image and mental health in an attempt to make a contribution to the spread of jazz dance. The subjects in this study were 183 people who were selected by purposive sampling from a population of Seoul and Gyeonggi province residents who were at the Western age of 18 and up. The selected people took general education courses at colleges, attended cultural centers or fitness clubs, or learned dance for all. One instrument used in this study to assess their body image was Jourard and Secord(1954)'s Body Cathexis Scale, which was modified into six factors and 39 items. The other was Kim Gwang-il, et. al.(1978)'s 46-item inventory, which translated Rickles and Rock(l976)'s SCL-90 with 90 items to suit Korean circumstances. That inventory was modified into 31 items and four factors. For data handling, factor analysis, reliability analysis, regression analysis and path analysis were utilized. The findings of the study were as follows: 1. The extent of jazz dance participation had an impact on their body image. Those who performed jazz dance more frequency had a better image about their health and the lower part of the body. 2. The level of jazz dance participation exerted an influence on their mental health. A longer participation term led to more interpersonal sensitivity, and a higher participation frequency was followed by more somatization. 3. Concerning the causal relationship of participation in jazz dance to body image and mental health, the participation frequency affected health, the image of the lower part of the body and somatization in the firsthand manner, and the image of the lower part of the body had a firsthand impact on interpersonal sensitivity, hostility, anxiety and depression. Accordingly, the image of the lower part of the body played a crucial role as a parameter in the relationship between the jazz dance participation frequency and mental health.
