Health Care Utilization Pattern and Its Related Factors of Low-income Population with Abnormal Results through Health Examination (저소득층 건강검진 유소견자의 의료이용 양상 및 관련요인)

  • Objectives: The purpose of this study was to examine the health care utilization pattern and its related factors of low-income population with abnormal results through health examination. Methods: Analysed data were collected through a questionnaire survey, which was given to 263 persons who 30 years or over with abnormal results through health examination at Health Center. This survey was conducted in March, 2003. This study employed Andersen's prediction model as most well known medical demand mode and data were analysed through 2-test, and multiple logistic regression analysis. Results: The proportion of medical utilization for thorough examination or treatment among study subjects was 51.0%. In multiple logistic regression analysis as dependent variable with medical utilization, the variables affecting the medical utilization were 'feeling about abnormal result(anxiety versus no anxiety: odds ratio 2.25, 95% confidence intervals 1.07-4.75)', 'type of health security(medicaid type I versus health insurance: odds ratio 2.82, 95% confidence intervals 1.04-7.66; medicaid type II versus health insurance: odds ratio 3.22, 95% confidence intervals 1.37-7.53)', 'experience of health examination during past 2 years(odds ratio 2.39, 95% confidence intervals 1.09-5.21)' and 'family member's response for abnormal result(recommendation for medical utilization versus no response: odds ratio 4.90, 95% confidence intervals 1.75-13.75; family member recommended to utilize medical facilities with him/her versus no response: odds ratio 19.47, 95% confidence intervals 5.01-75.73)'. The time of medical utilization was 8-15 days after they received the result(29.9%), 16-30 days after they receive the result(27.6%), 2-7 days after they received the result(20.9%) in order. The most important reason why they didn't take a medical utilization was that it seemed insignificant to them(32.4%). Conclusions: In order to promote medical utilization of low-income population, health education for abnormal result and its management would be necessary to family member as well as person with abnormal result. And follow-up management program for person with abnormal result through health examination such as home-visit health care would be necessary.

Validation of the coach-athlete relationship scale of amateur golf players: Rasch rating scale model (아마추어 골프 선수를 위한 코치-선수 관계 척도의 타당화: Rasch 평정척도 모형 적용)

  • The purpose of this research was to develop and validate the coach-athlete relationship scale suitable to amateur golf players by applying the Rasch rating scale model. As the coach-athlete relationship scale, the Korean form of scale developed by Kim and Park (2008), which was revised based on the evidence on the basis of inspection contents, was used to conduct a survey on 217 amateur golf athletes. And the unidimensionality, which is the basic assumption of the Rasch model, was verified using the WINSTEPS program, and the appropriateness of the item category was established through the step calibration. The goodness of fit of each question was tested through the goodness-of-fit index and the differential item functioning (DIF) was estimated according to the golf career. When the goodness-of-fit index estimated for each question was 1.30 or more it was judged unfit and the significance level in the analysis was all set as.05. The results of the analysis showed that the measures variance explained by the Rasch measurement model was more (33.7%) than 20%, so the unidimensionality assumptions of the 11 questions (..hospitable posture when my coach is teaching) were satisfied. The result of analyzing the item category (7 scale) with step calibration was found to be unfit, but in the result of reanalyzing by rescoring into a 5-point scale, it was found to be fit. Particularly, in the result of estimating the goodness-of-fit using the systematized item category (5 scale), Question 10 (...my best when my coach is teaching) and Question 11 were found to be unfit, and as a result of estimating the differential functioning item according to golf career, Question 11 was found to be unevenly differentiated according to golf career. So the 5-point scale of Question 9 after eliminating the two questions which were unfit and differentiated was validated to be the coach-athlete relationship scale suitable to amateur golf athletes.

  • The purpose of this study was to make public oral health plan more effective and the improvement of the hospital management system for better clinical dental practice. Distribution and trends were examined in all patients who had been examined and diagnosed at Department of Pedodontics, Dental Hospital, Chosun University over 10 year-period from 1990 to 1999 Results were as follows, 1. The number of patients per year was increasing trend after 1996 and higher visiting rate in male(55.9%) than in female(44.1%). 2. Age distribution had shown $3\sim4$ year-old cup being the largest (23.4%) and each percentage of $5\sim6,\;7\sim8,\;0\sim2,\;9\sim10,\;11\sim12,\;13\sim14,\;above\sim15$ year-old group was 19.9%, 17.7%, 14.6%, 12.6%, 8.0%, 3.3%. 0.5%. 3. Geographic distribution showed a majority of patients in Kwang-Ju (83.0%). Group in the suburbs of Kwang-Ju(Jang-Sung group) was 5.4%. 4. Dental caries showed the highest percentage(40.5%) in chief complaints and percentage of oral exam, orthodontic problem, oral pain, trauma, supernumerary teeth, swelling was 13.9% 12.6%, 8.8%, 7.4%, 5.5%, 4.9%. 5. In all patients with traumatic problem, crown fracture showed the highest percentage(41.4%) and percentage of subluxation was 19.4%. 6. Majority(78.7%) of patients were not refered, and percentage of patients refered from local clinic was 20.5%. 7. Patients who had periodic check-up comprised 19.6%, and percentage of after 3, 6, 9, 12, 18, 24month was 36.7%, 22.2%, 13.5%, 11.3%, 5.4%, 11.0%.

  • The purpose of this research is to clarify the elements of the function of the role of industrial high schools that its experts perceived. The content of this research is verify the function element required for the performance of the role of specialized education through content validity ratio(CVR). This research adopted the method of literature research and Delphi method, which is to collect and come to an agreement of the opinions of the 26 research panels. The first round is constructed by the semi-constructed questionnaire for the analysis of the opinions of the panels by inductive method. The second round is to categorize the result of the first one into 7 domains, and asked each category by Likert's 5 scale checklists, and statistically analyzed mean, medium, standard deviation, and quartile. The third round is to statistically analyze Mean, standard deviation, medium, and validity ratio(CVR) to reassure the opinions of the panels on the basis of the result of the first one. The categorized contents of the function required for the performance of the specialized education in this research is 'in-service visit and in-service training', 'licence acquiring education', 'employment counseling and job employment information', 'custom-made education connected with industry', 'career education' and 'enhancement of basic career competency'. The panels are divided into professors, teachers, professionals, and policy administrators, and they verified the validity rate of the function role and priority of emphasis. The result showed that the tendency of the education is converting from physical function-centered education to education of emotional attitude and competence of thought.

Assessment and Prediction of Stand Yield in Cryptomeria japonica Stands (삼나무 임분수확량 평가 및 예측)

  • The objective of this paper is to look into the growth of Cryptomeria japonica stand in South Korea along with the evaluation on their yields, followed by their carbon stocks and removals. A total of 106 sample plots were selected from Jeonnam, Gyeongnam, and Jeju, where the groups of standard are grown. We only used 92 plots data except outlier. As part of the analysis, the Weibull diameter distribution was applied. In order to estimate the diameter distribution, the growth estimation equation for each of the growth factors including the height, the diameter at breast height, and the basal area was drafted out and the verification for each equation was examined. The site index for figuring out the forest productivity of Cryptomeria japonica stand for each district was also developed as a Schumacher model and 30yr was used as a reference age for the estimation of the site index. It was found that the site index for Cryptomeria japonica stand in South Korea ranges from 10 to 16 and this result was used as a standard for developing the stand yield table. According to the site 14 in the stand yield table, the mean annual increment (MAI) of the Cryptomeria japonica reaches $7.6m^3/ha$ on its 25yr and its growing stock is estimated to be at $190.1m^3/ha$. This volume is about $20m^3$ as high as that of the Chamaesyparis obtusa. Furthermore, the annual carbon absorptions for a Cryptomeria japonica stand reached the peak at 25yr, which is 2.14 tC/ha/yr, $7.83tCO_2/ha/yr$. When compared to the other conifers, this rate is slightly higher than that of a Chamaecyparis obtusa ($7.5tCO_2/ha/yr$) but lower than that of the Pinus koraiensis ($10.4tCO_2/ha/yr$) and Larix kaempferi ($11.2tCO_2/ha/yr$). With such research result as a base, it is necessary to come up with the ways to enhance the utilization of Cryptomeria japonica as timbers, besides making use of their growth data.

The Effect of Use of Nutrition Labelling on Knowledge and Perception of Nutrition Labelling, and Awareness of Nutrition Labelling Usefulness with among College Students (대학생의 영양표시 확인 여부에 따른 영양표시에 대한 지식, 인식 및 유용성 자각 비교)

  • This study describes the prevalence of nutrition labelling use and its association with demographic factors, knowledge and perception of nutrition labelling, and awareness of nutrition labelling usefulness among 282 college students, aged 21~23 years, in Busan Gyeongnam area. The subjects consisted of 150 of male (53.2%) and 132 of female (46.8%) students. 31.3% of male students and 50% of female students reported reading nutrition labelling always or usually, while 34.7% of male and 13.6% of female subjects reported that they don't use nutrition labelling. Although most of the subjects perceived the need of nutrition labelling, their knowledge and understanding of nutrition labelling and their awareness of nutrition labelling usefulness were not enough to apply the information from the nutrition labelling in their daily life, even in the subjects who reported reading nutrition labelling always or usually. This statement might be one of the reasons why there were no significant differences among the groups divided by nutrition labelling use in regards to the dietary habit or snack eating habit in this study. Therefore, a practical nutrition education program on nutrition labelling should be provided for the college students to increase nutrition labelling use and to apply the information from nutrition labelling in their daily life in order to maintain healthy dietary habit.

Mushroom consumption patterns in the capital area (수도권 도시가구의 버섯 소비양상)

  • Profitability of farmers has decreased mainly owing to low price while the gross amount of mushroom production has increased continuously in South Korea. In this regard, analyzing patterns of mushroom consumption is believed to be meaningful. We used a panel data set consisting of 667 families, from 2010 to 2015. Based on the panel data, mushroom consumption patterns of people living in city areas were examined. Multiple descriptive analysis methods and frequency analysis approaches were adopted in this study in terms of time and space dimensions, demographic properties, and purchase behaviors. The findings of this studyshow that mushroom purchase is highly dependent on seasonal events, which implies that the product consumption timing is predictable. In addition, yearly purchase amount patterns reflect that superstores have become the major mushroomtrading venues. This directly supports the need to establish supply chain capabilities for mushroom farmers so that they gain more bargaining power against enterprise-type groceries. Finally, functional features of mushroom can be linked with marketing promotion because purchase patterns demonstrate potential needs for healthcare food in mushroom categories. Based on the analyzed patterns, this paper provides insightful implications for policy makers who want to promote mushroom consumption.

The Effect of Early IASP and Reperfusion Therapy in Patient of Post MI Cardiogenic Shock (Post MI Cardiogenic Shock 환자에서 조기 IABP 및 Reperfusion Therapy의 효과)

  • Background: We sought to examine the use and outcomes of early intraaortic balloon counterpulsation(IABP) combined with early reperfusion therapy in patients presenting cardiogenic shock complicated acute myocardial infarction. The usc of IABP in patients with cardiogenic shock is widely accepted. However there is not ample information on the use of this technique in patients with cardiogenic shock who arc treated with reperfusion therapy in Korea. Materials and Methods: Twenty-eight patients presented with cardiogenic shock were classified into two groups: the early IABP group (insertion within 12 hours after AMI onset time) and the late IABP group (insertion after 12 hours). We compared In-hospital mortality between the two groups (early IABP group vs late IABP group). Results: Two groups showed no significant difference in clinical feature and coronary angiographic results. Among total 28 patients, 7 patients were treated with thrombolytic therapy and 21 patients with PTCA. Insertion site bleeding, fever, thrombocytopenia were reported as some of the complications of IABP insertion. In-hospital mortalities in the early IABP group and late IABP group were 4 patients(25%) and 8 patients(66%), respectively(p<0.05). Early IABP insertion and early PTCA showed lower hospital mortality rates. There was significant difference in the time to PTCA after AMI onset between the two groups(p<0.05). Conclusion: IABP appears to be useful in patients presenting cardiogenic shock unresponsive medical therapy. Early IABP insertion and early reperfusion therapy may reduce in-hospital mortality rates of post-MI cardiogenic shock patients.

The effect of subject awareness level of oral health on the quality of life for adult patients who visit hospitals and neighborhood clinics in Busan (부산지역 병의원을 내원하는 성인의 구강건강인식수준이 삶의 질에 미치는 영향)

  • The purpose of this study was to determine the quality of life according to the level of subjective perception as well as to analyze the influencing factors and improve the subjective perception to improve the quality of life. The results of the survey for adult patients in hospitals and neighborhood clinics in the region of Busan for nine months from December 2015 were as follows. Regarding the total symptoms, the women had a score of 1.41, and the younger respondents felt they suffered from no symptoms. An analysis of the quality of life by the general characteristics showed that the respondents who were better educated and had no systemic diseases had a higher score of 54.71 on the OHIP-14. The respondents who were better educated and had no systemic diseases had a higher score of 6.48 when a single item was used to determine their happiness index, and the respondents who were better educated and had no systemic diseases had a higher score of 3.82 when five items were used to rate their happiness index. The factor with the greatest influence on their scores on the OHIP-14 was the total symptoms, followed in order by the happiness index and self-rated oral health status. The variable that exerted the largest influence on the happiness index was the OHIP-14, followed by the self-rated health status, high-school or lower education, and the presence or absence of one or more chronic disease. Therefore, an improvement in the subjective awareness level is required to boost the quality of life, and a variety of health education programs should be prepared to raise the awareness level. In addition, there should be a chance to convey accurate information.

A Study of Food Attitudes at the Community Level (지역집단(地域集團)의 식품태도(食品態度)에 관(關)한 연구(硏究))

  • The aims of this study are, 1) to describe the attitudes of the community towards each food item and 2) to provide the information necessary for the solution of nutritional problems due to the different food intake of the community. This study is based on the Food Consumption Survey conducted by the Ministry of Health and Social Affairs in a series of National Nutrition Survey in August, 1973. The sample included 8 communities, in which 160 households were selected at random for this study. The procedures and methods used in this study to compute the acceptability index were adopted from the study of H. Toyokawa, University of Tokyo. Food attitudes of the sample communities are clear in terms of the mean acceptability index which is presented in Table 3. The following order is indicative of the relative strength of acceptability of the respective food items: 1. Seasonings (-1.20) 2. Oils and fats (-1.12) 3. Pulses(- .60) 4. Processed vegetables (- .45) 5. Seaweeds (- .32) 6. Sugars (- .19) 7. Fresh vegetables (- .15) 8. Rice (- .13) 9. Cereals and grain products (- .07) 10. Starchy roots (- .06) 11. Fresh fish and shells (.44) 12. Processed fish and shells (.56) 13. Eggs (1.06) 14. Fruits (1.26) 15. Meats and poultry (1.33) 16. Milk and milk products (2.87) There are highly significant difference in the acceptability index between food items and the mean intake of rice, cereals and grain products, pulses, starchy roots, processed vegetables, fruits, sugars, seaweeds, seasonings, meats and poultry, eggs, fresh and shells, and oils and fats between the communities at the .01 level of significance.
