육계를 사육하고 있는 농장에서 '저혈당증'은 이제 모르는 농가가 거의 없을 정도로 잘 알려진 용어가 되었다. 많은 농가가 이로 인한 피해를 경험하고 있거나 이와 유사한 증상이 있을 경우 가장 먼저 저혈당증을 의심할 정도로 인식이 확산되어 있다. 그러나 오히려 이러한 인식의 확대가 증상이 유사하다는 것만으로 저혈당증으로 판단하여 그보다 더 심각한 피해요인을 간과하는 경우도 많고, 그 치료 방법도 어떤 지역이나 농가에 따라 매우 다양하여 오히려 피해를 가중시키는 경우도 있다. 따라서 본고는 크게 두 가지로 구분하여 저혈당증에 대한 이해를 돕고자 한다. 첫째는 지난해에 저혈당증으로 진단되었던 가검물을 기초로 하여 저혈당증에 대하여 알아보고, 둘째는 2001년 올해 현재까지 저혈당증을 의심하여 의뢰되었던 가검물에 대하여 고찰해 봄으로써 저혈당증에 대한 올바른 접근을 유도하고자 한다. (저혈당증과 관련하여 저혈당-급성폐사증후군(HSMS : Hypoglycemia-Spiking Mortaility Synotrome : HSMS)이라는 명칭이 사용되고 있으나 본고에서는 '저혈당증'으로 하였다.)

2009년 부산의 한 고속도로에서 8중 추돌사고가 발생했다. 40대 남성이 운전 도중 저혈당 상태에 빠져 일어난 사고였다. 당뇨병환자라면 흔하게 저혈당을 경험하지만 그 위험에 대해서는 모르고 있는 경우가 많은 것으로 드러났다. 한독약품과 한국 노바티스에서 실시한 저혈당 인식 실태 조사결과{2010년)에 따르면 당뇨병환자 10명 중 7명 (74%)이 저혈당에 대해 잘 모르고 있는 것으로 나타났다. 또한 주의가 필요한 야간 저혈당증 및 저혈당 무감지증을 겪은 당뇨병환자도 각각 37%와 44%를 차지했다.

Hepatomegaly and liver dysfunction might develop in patients with diabetes mellitus due to glycogen deposition or nonalcoholic steatohepatitis. We experienced a case of hepatic glycogenosis in a patient with type 1 diabetes mellitus who presented with recurrent hypoglycemia, suggesting impairment of glycogenolysis and gluconeogenesis. A 10-year-old girl with a 4-year history of type 1 diabetes mellitus was admitted because of recurrent hypoglycemia and abdominal pain in the right upper quadrant. She had Cushingoid features and hepatomegaly that extended 6 cm below the right costal margin. Laboratory data and radiologic examination revealed elevated liver enzyme levels due to fatty liver. Periodic acid-Schiff (PAS) staining revealed intense glycogen deposition in the cytoplasm of the hepatocytes and PAS reactivity was lost with diastase treatment. At 2 months after administration of glucagon injection and uncooked cornstarch between meals and at bedtime, the hypoglycemic episodes and liver dysfunction improved. It is important to distinguish hepatic glycogenosis from steatohepatitis, because it is possible to prevent excessive hepatic glycogen storage in hepatic glycogenosis cases by strictly controlling blood glucose level and by glucagon administration. To prevent severe hypoglycemic symptoms accompanied by hepatic glycogenosis, we suggest that uncooked cornstarch, which is effective in maintaining blood glucose level, can also be administered.

Purpose : In a previous report, it took several days for white matter lesions to regress in hypoglycemic encephalopathy. We present a case of rapid diffusion-weighted image (DWI) changes in hypoglycemic encephalopathy. Case Report: A 58-year-old male patient was found semi-comatous with the only abnormality in his laboratory tests showing hypoglycemia (44 mg/dL). After rapid correction of glucose level, immediate brain DWI showed bilateral subcortical white matter lesions. After about 5 hours, follow-up DWI showed resolved subcortical white matter lesions, with newly-appeared bilateral fronto-temporo-parietal cortical lesions. Conclusion: Both white matter and cortex involvement in hypoglycemic encephalopathy has been shown in several reports, but rapid regression of white matter changes in hypoglycemic encephalopathy has been rarely reported. It is important to know that MR imaging changes in hypoglycemic encephalopathy can be made as quick as just a few-hour-long.

대부분의 당뇨병이 있는 개와 고양이는 쉽게 진단할 수 있고 초기에 적절한 식이관리와 중간형 또는 지속형 인슐린을 하루에 1회 또는 2회 투여함으로써 적절히 관리할 수 있다. 당뇨병의 장기간 관리는 체중, 물섭취량과 연속적인 혈당치의 감시에 의하여 적절히 수행할 수 있다. 연속적인 혈당치 측정은 인슐린 치료와 관련된 잠재적인 문제점(저혈당증, 인슐린 유발성 과혈당증, 인슐린 내성)을 확인하거나 피할 수 있는 가능성을 높여 주며, 혈당을 적절한 수준으로 유지시켜 만성 당뇨병의 합병증 발생율을 줄이거나 지연시킬 수 있게 해준다.
