An Investigation of Socio-Demographic Characteristics, Medical Use in Juvenile and Adolescents : Using Korea Health Panel Data (2015) (아동과 청소년의 인구사회학적 특성 및 의료이용 연구: 2015년 한국의료패널 자료를 이용하여)

  • Purpose : The aim of this study was to investigate patterns of medical use among juveniles and adolescents, including for chronic disease, in Korea. The study sought to do the following: (a) investigate the extent to which chronic diseases account for medical expenditures, (b) investigate and the socio-demographic characteristics associated with medical use, and (c) identify the differences in medical use between juveniles and adolescents. Methods : We used data from the 2015 Korean Health Panel and selected 12 variables. The socio-demographic characteristics investigated included, growth period (juvenile, adolescents), gender, family income, national basic livelihood act status, disability registration, and degree of disability. There were five medical factors that were considered: emergency room use, hospitalization use, hospital outpatient use, chronic disease, and medical expenditure. Data were analyzed using stepwise multiple and logistic regression. Results : The prevalence of chronic disability in juveniles and adolescents was 31.1 % and 1 %, respectively. The factors affecting medical expenditures included hospitalization use, hospital outpatient use, family income, disability, gender, chronic disease, and emergency room use ($R^2=.160$, p<.05). For national basic livelihood act recipients, the probability of having chronic disease was about 1.6 times higher (OR=1.597, 95 % CI=1.092-2.335, p=.016), compared with non-national basic livelihood act recipients. People with disabilities were 6.6 times more likely than those without disabilities to suffer from chronic disease (OR =6.571, 95 % CI=2.776-15.556, p<.001). Hospital outpatient user was 2.3 times higher than non-user (OR=2.260, 95 % CI=1.702-3.001, p<.001). Juveniles had a 1.7 times and 6.2 times higher likelihood of emergency rooms user (OR=1.654, 95 % CI=1.270-2.155, p<.001), and hospital outpatient user than adolescents (OR=6.208, 95 % CI=4.443-8.676, p<.001). Conclusion : The findings of this study suggest that health care services for juveniles is needed to manage chronic diseases that have an effect on medical expenditures.

Analysis of Effect of Non-drug intervention on the Elderly with Dementia in Korea: Meta-Analysis (국내 치매노인의 비약물적 중재에 대한 효과분석: 메타분석)

  • This paper aims to guide experts who manage dementia by exploring the various non-drug interventions and analyzing the effective intervention methods applicable according to the functional level of the elderly with dementia. Fourteen studies were analyzed in this study. Meta-analysis was performed using the means, standard deviations, and the number of samples. Subsequent meta-analysis showed that the Holnis program had the largest effect size in cognitive function, the bakery activity program in memory, and the composite intermediation program with ADL was the largest. In addition, client-centric cognitive stimulation interventions showed the most significant effect sizes, while in depression and BPSD, rhythmic movement activities had the most significant effect size. Elderly with dementia exhibit various symptoms depending on their characteristics and the progress of the disease. Therefore, more efficient arbitration could be provided if the effects of each intervention can be applied differently.

Effects of the Whole-body Vibration Exercise Combined with Ankle Joint Mobilization on the Gait Function and Balancing Ability in Stroke Patients: A Preliminary Randomized, Controlled Study (발목관절 가동술과 결합한 전신진동운동이 뇌졸중 환자의 보행 기능과 균형 능력에 미치는 영향: 무작위 대조 예비연구)

  • PURPOSE: This study was performed to investigate the effects of the whole-body vibration exercise combined with ankle joint mobilization on the gait and balancing ability in patients with hemiplegic stroke. METHODS: A total of 19 patients at a rehabilitation hospital who had suffered a hemiplegic stroke were randomly assigned to the experimental group (whole-body vibration exercise combined with ankle joint mobilization, n=10) or control group (whole-body vibration exercise, n=9). All participants underwent 30 min of comprehensive rehabilitation therapy (5 × /week for 6 weeks). Additionally, the experimental group performed the whole body vibration exercise and ankle joint mobilization (15 minutes each, 30 minutes total, 3 × / week for 6 weeks). In the control group, only the whole- body vibration exercise was performed in the same manner and not the ankle joint mobilization. The gait and balancing abilities were measured before and after the 6-week training. RESULTS: Significant improvements were observed in the 10-m walk test, timed up-and-go (TUG) test, center of pressure (COP) path length, and COP path velocity in the experimental group (p < .05). The experimental group showed a larger decrease in the COP path length and velocity than the control group (COP path length, -10.27 mm vs. -3.67 mm, p < .05; COP path velocity, -.33 cm/sec vs. -.13 cm/sec, p < .05, respectively). CONCLUSION: The whole-body vibration exercise combined with ankle joint mobilization could be effective in improving the gait and balancing ability of stroke patients and could also be more effective for improving the static balance ability than the general whole-body vibration exercise alone.

  • Due to the spread of Corona 19 around the world, Infectious Disease Medicine and New Medical Devices such as Diagnostic Agent are being rapidly developed and launched, and for the fast supply and demand of these, each country has eased import regulations or has implemented policies for fast approval(NIDS, 2020). On the other hand, New Developed Medical Devices that are not related to New Infectious Diseases, they are still entering the market through strict licensing and licensing regulations, such as delay and cancellation in the test inspection process, etc. Therefore, This Study specialized in the government-managed laws encountered when New Medical Devices enter the market, derive Factors influencing the Strength of Regulations, analyzes the Strength of Regulations, and proposes a Regulatory Response Framework. The Research Method was conducted by Literature Research, was applied by Failure Mode and Effects Analysis(FMEA) Method, Expert Interview(1st): Idea Collection, Expert Interview(2nd): Validation, and Priority through the Application Process of FMEA Method. A Method of Quantifying the Intensity of Regulation was proposed by multiplying the Impact of the Influencing Factors for each stage of regulation and the Burden Impact for each type of Regulatory Affairs to find the Importance of the Regulatory Factors and multiplying the Severity of the Regulatory Impact. The Implications are that major overseas countries and the Korean government are actively responding with Special Regulatory Policies and Mitigation Policies for fast licensing of New Developed Medical Devices in accordance with Corona 19. It is expected that the direction for improvement of regulations and measures to respond to regulations will be implemented so that a more proactive and preemptive response to the regulatory process of the licensing policy for New Devices can be achieved.

A Study on the Oral Health and Oral Health Related Quality of Life of Stroke Patients in a Rehabilitation Ward: A Pilot Study (재활병동에 입원한 뇌졸중 환자의 구강건강 및 구강건강관련 삶의 질에 관한 연구: 예비연구)

  • The aim of this study was to examine the geriatric status, oral health, and oral health related quality of life of stroke patients hospitalized at a rehabilitatio ward and assess the relationships among these factors. A total of 78 stroke patients participated in this cross-sectional study. The following oral examinations were performed by a dentist and dental hygienist: Decayed, Missing, Filled Teeth, gingival, debris, calculus, and community periodontal index (CPI). The patients' sociodemographic and geriatric status were evaluated using the Barthel index and a mini-mental state examine (MMSE) according to their medical record and a questionnaire survey. Oral health related quality of life was assessed using the Oral Health Impact Profile (OHIP)-14 questionnaire. Except for three participants, most stroke patients had a gingival index of 2 or more, which means probing results in bleeding gums. Additionally, 53.8% of participants had a CPI code of 3 or 4, which indicates a probing depth of 4 mm or more. CPI index was significantly related to debris, calculus, and gingival index. The OHIP-14 score significantly related to the Barthel index and MMSE (p<0.05). The oral health of stroke patients in the rehabilitation ward was relatively poor and related to poor oral hygiene. Oral health related quality of life was related not to oral health but to geriatric status significantly.

Effects of the Health Management Importance Awareness on Occupational Performance and Basic Fitness for the People with Intellectual Disabilities Participated in the Muscle Strengthening Exercise (건강관리 중요도 인식이 시설 거주 지적장애인들의 작업수행과 기초체력에 미치는 영향)

  • This study performed to provide the basic information about the effects of the health management importance awareness on the occupational performance and basic fitness for the people with intellectual disabilities. 16 people with intellectual disabilities participated in muscle strengthening exercise. To assess health management importance awareness, the Canadian Occupational Performance Measure was used to identify the importance of the health management. According to the results of the importance, they were divided into two groups: Extreme and Neutral Importance Group in order to analyze the changes of the occupational performance and basic fitness. In-group analysis of the performance, satisfaction level, strength, and flexibility found that all measures improved in both groups following completion of the program. Between-group analysis following completion of the program indicated group differences in the performance and satisfaction level, but no group differences were found in the muscle strength and flexibility. Thus, this study showed the positive effects from the results of the program participation targeting the people with intellectual disabilities, and highlighted the importance of the awareness for them regarding the need for the health management (as evidenced by the improvement in occupational performance). It is expected that effective health management through participation in programs such as this will help maintain a healthy lifestyle and contribute to improve the quality of life.

Research Trends in Occupational Therapy Intervention for Children in Korea (국내 작업치료의 아동 중재 연구 동향)

  • Objective : The purpose of this study was to identify the status and trends of Korean child occupational therapy intervention studies according to the International Classification of Functioning, Disability, and Health, Children and Youth Version (ICF-CY). Methods : In this research, 47 studies on occupational therapy interventions for children that were published between January 2017 and December 2021 in the Journal of Occupational Therapy, registered in the Korea Citation Index, and analyzed the classification of the study type and evidence level to understand the trends. Moreover, intervention objectives and approaches were analyzed on the basis of the ICF-CY. Results : The outcomes of the analysis of the articles published in the Journal of Occupational Therapy were as follows: (1) Level IV was the highest evidence level (53.19%). (2) Among the studies, most (53.7%) included school-age children as subjects. Autism spectrum disorders and developmental delays were the most common diagnoses (14.8%). (3) As for the purpose of intervention according to ICF-CY, activity and participation factors were the most common (48.94%), and a sensory approach was frequently used. Conclusion : This study reviewed articles on occupational therapies for children that were published in the Journal of Occupational Therapy to understand the trends in occupational therapy interventions for children in South Korea. For the development of occupational therapies for children in the future, more qualitative research types and studies on various intervention approaches are needed.

A Study on the Appropriateness of Community Rehabilitation Job Knowledge and Education for Community Care : Focusing on the Job as an Occupational Therapist (커뮤니티케어를 위한 지역사회 재활 직무 지식과 교육의 적절성 조사연구 : 작업치료사 직무 중심으로)

  • This study was conducted to find out the appropriateness of knowledge and education of occupational therapists' community rehabilitation jobs for community care. A questionnaire was developed to find out the job knowledge level of occupational therapists working in the community and the appropriateness of school education and maintenance education, and the validity and reliability were verified through a preliminary survey. Based on the verified questionnaire, a survey was conducted on occupational therapists working in the community. As a result, overall, the average score was low in the level of knowledge about the job of occupational therapists engaged in the community and the appropriateness of school education and maintenance education. Through this study, we were able to know the necessary knowledge in the practice of community occupational therapy, understand the level of knowledge retention of occupational therapists working in the community, and learn about the appropriateness of school education and maintenance education. Referring to the results of this study, if a systematic education program related to community occupational therapy jobs is developed in existing medical institution-centered education, the job knowledge level of occupational therapists in the community can be raised. Amid the rapidly changing times and the pandemic of infectious diseases, additional research is needed on job-related knowledge and education corresponding to various occupations in charge of rehabilitation in the community.

A Literature Review of Tongue Movement and Measurement Tools for Dysphagia (연하장애 환자의 혀 운동 및 측정 도구에 대한 고찰)

  • Objective : This review aimed to provide information for clinical application by confirming the principles and characteristics of the tool through a review of tongue movement and measurement tools for patients with swallowing disorders. Results : We identified 15 tools used as tongue exercises and measurement tools in the field of dysphagia. According to principle, the tools were classified as either a bulb sensor, resistive sensor sheet, mouthpiece with sensor, or other techniques. The bulb sensor was easy to use but had limitations in fixing the position when measuring tongue pressure. The resistive sensor sheet could be measured at a more stable position than the bulb sensor. A mouthpiece with a sensor could be used in an individual's oral cavity such that the position was fixed when measuring the tongue pressure. Other techniques had the advantage of being wireless and capable of sensing light. Conclusion : Based on this literature review, it is necessary to facilitate the selection of the best tool for quantitative tongue measurement in dysphagia. The review can also be used to develop a Korean tongue movement tool model that can be used in hospitals and community centers.

Effect of Oral Motor Facilitation Technique (OMFT) and Neuromuscular Electrical Stimulation (NMES) Applied to a Patient With Wallenberg's Syndrome: A Case Study (발렌버그 증후군(Wallenberg's Syndrome) 환자에게 적용한 구강운동촉진기술(OMFT)과 신경근전기자극치료(Neuromusclular Electrical Stimulation; NMES) 효과: 단일 사례 연구)

  • Objective : This study aimed to confirm the possibility of the clinical application of oral motor facilitation technique (OMFT) protocol and neuromuscular electrical stimulation (NMES) in patients with Wallenberg syndrome. Methods : One patient with Wallenberg syndrome was treated with OMFT and NMES applied 40 times each, 5 days a week, twice a day for 4 weeks. The Comprehensive Oral-Facial Function Scale (COFFS), Korean-Mann Swallowing Ability Assessment (K-MASA), and Penetration-Aspiration Scale (PAS) were used to compare the changes before and after the intervention. Data analysis was used to compare the score changes before and after the intervention. Results : Orofacial function and swallowing ability improved after the intervention in the individual who participated in this study. Among oral motor functions, relatively greater functional improvement was observed in tongue movement compared to other functions, which was evaluated to the extent that pharyngeal swallowing was possible. Conclusions : Early swallowing rehabilitation using systematic OMFT and NMES of exercise intensity confirmed the possibility of improving oral motor function and dysphagia. In the future, complementary studies on the effects of interventions applying the OMFT and NMES will be needed.
