The Relationship of Caregiving Appraisal and Family Function among Senile Dementia Patients' Families using In-home Services for the Long-Term Care Insurance (노인장기요양보험 재가보호서비스를 이용하는 노인성 치매환자 가족의 돌봄평가와 가족기능과의 관계)

  • The aim of this study was to identify the relationship of caregiving appraisal and family function among senile dementia patients' families using in-home services for the Long-Term Care Insurance(LTCI). The participants were 121 family caregivers of senile dementia patients using in-home services for the LTCI. The participants' mean age was $53.80{\pm}10.86years$, all middle-aged women(75.2%). When negative cognition's burden and neutral one's demand in their caregiving appraisal were low and positive cognition's satisfaction and mastery in their caregiving appraisal were high, emotional function in their family function was high. There is a need to develop nursing interventions for strengthening positive cognition's satisfaction and decreasing neutral one's demand in their caregiving appraisal to improve emotional function and communication of senile dementia patients' families.

The Effect of Choice and Service Quality on User Satisfaction among Long-Term Home Care Service Recipients (장기요양 재가서비스 서비스 품질과 선택권 실현이 이용자 만족도에 미치는 영향)

  • The purpose of this study is to examine the moderating effect of choice realization between service quality and user satisfaction to elderly home care services. For this purpose, we analyzed 258 respondents who received elderly home care service in Jeollabuk-do. Considering the assumption that the moderating effect of choice realization would be different by rural and urban areas because of the disparity of the elderly welfare infrastructure, the urban and rural areas were analyzed separately. The analyses showed that choice realization had a moderating effect on the relationship between service quality and user's service satisfaction for the service clients resided in urban areas. However, there was no significant moderating effect of choice realization for the service clients in rural areas. This result implies that the moderating effect of selection realization is different in rural and urban areas because of the poor elderly home care facilities in rural areas. In order for aged people to actively express the effect of service user choice, it is necessary to solve welfare imbalance between regions through expansion of welfare infrastructure and policy support to rural area.

In-depth interview with the center managers for home visit dental hygiene services in the long-term care insurance (노인장기요양보험 구강위생서비스의 활성화를 위한 재가장기요양기관 관리책임자 심층면담)

  • Objectives: This study was carried out to identify plans to activate home visit oral care services by dental hygienists in Long-term care insurance. Methods: In-depth interviews were carried out with 21 Long-term Home Care Center Managers as target. A total of 21 (27%) Home Care Centers were selected through convenience sampling among 78 Home Care Centers that are located in Won-ju city. The Managers were presented with questions and answered in 20-30 minutes in accordance with the interview instructions. The interview results were analyzed through content analysis, and their experiences and perceptions were classified into two themes and categorized again into four components. Results: The Home Care Center Managers suggested that dental hygienists should activate home-visit oral hygiene services. It is necessary to improve the management process and awareness of the elderly. Conclusions: To activate oral hygiene services, it is necessary to improve the service guideline and enhance the efficiency of the service process. This should be acceptable both to the elderly who need the services and the dental hygienists who provide them.

  • The UK is famous for being the first country in Europe to adopt the policies of marketisation regarding social welfare. Numerous other countries, including social democratic countries, have followed suit, and South Korea has also adopted the marketisation of care through the introduction of long-term care insurance. The aim of this study is to examine recent reforms concerning adult social care market in the UK, and to determine policy recommendations to further develop the Korean long-term care insurance market. Findings show that the UK has actively regulated and managed the care market. In particular, after the sudden bankruptcy of nursing homes, the CQC systematically analyzes the risks of bankruptcy of big service providers in terms of financial conditions and quality of services according to the six steps detailed in the Care Act 2014. If some service providers experience high levels of risk, the CQC reports results to local authorities in order to manage the risk of bankruptcy of these service providers. Such reforms in the UK suggest a number of policy measures for South Korea in which the problems of long-term care market are prevalent, including increased system management, introduction of a new inspection system, the expansion of public-based inspection organizations, and disclosure of information by the National Health Insurance Corporation.

The effect of market attributes on Long-term Care(LTC) Service

  • In this research, a survey was done on two hundred and forty-six elders who receive care through LTC to verify the effect of market attributes on the user satisfaction of the Long-therm Care Service. The collected data were analyzed through SPSS 23.0 statistics program. This study reveals the following results: First, in service tangibility, competitiveness, administrative regulation, organizational process, and autonomy in choice all have a positive relation with user satisfaction. Second, in the aspect of credibility, house ownership, competitiveness, administrative regulation, and freedom in choice show a positive-relation with user satisfaction. Third, in responsiveness, the scale of the organization, monthly income, competitiveness, administrative regulation, organizational process, and autonomy in choice have a positive effect on user satisfaction. Forth, in assurance ability, monthly income, competitiveness, administrative regulation, organizational process, and autonomy in choice also positively affected user satisfaction. Lastly, in the aspect of perceptual openness, competitiveness, administrative regulation, organizational culture, organizational process, and autonomy in choice show a positive relationship with user satisfaction.

Effectiveness of Evaluation for Visiting Care Service Institution: From the User's Point of View (방문요양서비스 기관 평가의 효과성 : 이용자 관점에서)

  • The purpose of this study is to confirm the effectiveness of the evaluation of long-term care insurance visiting care service institutions for the elderly from the user's point of view. To this end, data from a survey of 266 users collected through allocation sampling by region(14 cities and counties) in Jeollabuk-do and public data from 47 institutions were combined and analyzed using a multi-layered model. The main research results are as follows. First, among the five evaluation areas, the higher the score in the 'Rights·Responsibility' area, which is to give users a sense of rights and respect users, the higher the service quality and satisfaction. Second, among the five evaluation areas, the 'Rights·Responsibility' area had an effect on loyalty. In addition, it was found that the 'Outcome' area for satisfaction with institutions and employees and changes in users had an effect on loyalty. Third, it was found that 'Institutional Management', 'Environment·Safety', 'Process' did not affect service quality, satisfaction, and loyalty. Based on these results, it is proposed to reorganize the evaluation scale that users can recognize and that is faithful to the purpose.

A Study on the Perception of Quality of Care Services by Care Workers using Big Data (빅데이터를 활용한 요양보호사의 서비스질 인식에 관한 연구)

  • Background: This study was conducted to confirm the service quality management of care workers, who are direct service personnel of long-term care insurance for the elderly, using unstructured big data. Methods: Using a textome, this study collected and analyzed unstructured social data related to care workers' service quality. Frequency, TF-IDF, centrality, semantic network, and CONCOR analyses were conducted on the top 50 keywords collected by crawling the data. Results: As a result of frequency analysis, the top-ranked keywords were 'Long-term care services,' 'Care workers,' 'Quality of care services,' 'Long term care,' 'Long term care facilities,' 'Enhancement,' 'Elderly,' 'Treatment,' 'Improvement,' and 'Necessity.' The results of degree centrality and eigenvector centrality were almost the same as those of the frequency analysis. As a result of the CONCOR analysis, it was found that the improvement in the quality of long-term care services, the operation of the long-term care services, the long-term care services system, and the perception of the psychological aspects of the care workers were of high concern. Conclusion: This study contributes to setting various directions for improving the service quality of care workers by presenting perceptions related to the service quality of care workers as a meaningful group.

A systematic Review on the Effects of Occupational Reminiscence Therapy in Dementia Offered National Long Term Care Insurance (국내 장기요양보험서비스를 제공 받는 치매 환자 대상 작업 중심 회상치료 효과에 대한 체계적 고찰)

  • Objective : The purpose of this study was to look at a systematic review on the effects of occupational reminiscence therapy in Dementia offered National Long term care Insurance. Through this comprehensive study, we have to compare the studies. Methods : We systematically examined papers published in journal from 2009 to 2013, using KERIS. Main words to examine are Dementia, Long term care service, Day care center, reminiscence therapy, occupational centered, etc. Results : 6 studies were selected, All of them were occupational reminiscence therapy. Reminiscence therapy can be devided into communication centered reminiscence therapy and occupational reminiscence therapy. The results demonstrated that the intervention significantly affected the maintenance cognitive skills and reminiscence skills, decreased depression, behavioral changes, improvement of communication and interaction skills and quality of life etc. of elderly people with dementia (p<.05). Conclusions : If occupational therapists can obtain knowledge and make a program for occupational reminiscence therapy, research in this field will be further developed. In the future study, the use of occupational reminiscence therapy applied to a variety of interventions and majority of patients is needed on occupational therapy.

A Study on the Improvement of In-Home Care Service Quality through Evaluation of Services and Agency by Long-term Care Workers (요양보호사의 기관 및 서비스 평가를 통한 재가노인서비스 품질 향상 방안)

  • The objective of this study is to suggest methods to improve in-home service quality through service evaluation by long-term care workers. To achieve this objective, general characteristics of 223 long-term care workers, evaluation of service and agency, and retraining needs have been surveyed. An assessment of the survey results have resulted in the following conclusions. Though long-term care workers are not uneducated, the majority face unstable employment. And the content of supervision hoped for in producing improved long-term care services has been found to be based on the service-user's relationships. Moreover, among topics needing to be addressed for retraining, much attention has been shown for understanding of the elderly and their families, health care knowledge about geriatric diseases, and counseling techniques directed towards the affected person and their family. Findings from the research are as follow: enhancing the quality of long-term care requires a structural reassessment; upgrading the quality of care agencies requires the improvement of methods used to raise awareness of users and their guardians and the expansion of opportunities for education programs for professionalism.

The Effect of Experience of Abuse on Life Satisfaction of the elderly in Long-term Care Facilities (노인요양 시설내 학대경험이 이용 노인의 삶의 만족도에 미치는 영향)

  • With the introduction of long-term care insurance, the number of long-term care facilities and the elderly using it is increasing. Nevertheless, there was a lack of concern and research on elderly abuse and how to affect the life of the elderly in long-term care facilities. The study analyzed the data collected from 278 senior citizens of 44 long-term care facilities in two regions. The analysis was conducted to analyze the relationship among elderly abuse, service quality, service satisfaction and life satisfaction. An analysis showed that some of the elderly experienced elderly abuse, especially sexual abuse. The experience of abuse affect the perception of the service quality, service satisfaction and life satisfaction of the elderly. The elderly experienced abuse was undervalued in service quality, service satisfaction and life satisfaction. Based on these findings, the implications to prevent elderly abuse of long-term care facilities are suggested.
