A Study on the Analysis of the Related Factors to Distinguish High Risk Group of Female Suicidal Attempts (여성의 자살시도 고위험군 선별을 위한 관련요인 분석)

  • This study aims to determine the current status of experiences of suicidal attempts of Korean women and to evaluate related factors and provide basic sources for distinguishing high risk group. The primary data of the 6th period of national health and nutrition survey by KCDC, between 2013 and 2015,wereutilized in this study.Women were selected;general characteristics, psychological characteristics, and relevant information of daily activities were collected. The collected data wereanalyzed by applying the complex sample analysis method,using the SPSS 20.0 program with an application of weighted values. The resultsof this study revealed that 1% of the study population had previously attempted suicide; the suicidal attempts in subjects with prior psychological counselling within 1 year was 5.25 times higher than those without prior counselling.Subjectswith too much stress were 14.92 times more likely to attempt suicide than those without stress. Moreover, subjects with higher-intensity work in daily lives was 4.85 times more likely to attempt suicide than those with less intense work. In addition, subjects who sat for longer period of time was 3.76 times more likely to attempt suicide. All of these were statistically significant. Therefore, these subjects should be considered as high risk for suicidal attempt.In order to prevent female suicide, guidelines that considers the psychological health status and daily activities are required. In addition, it is necessary to approach this issuewith individualized prevention programs as part of social policies.

ANXIOUS-DEPRESSIVE ATTRIBUTES TO SUICIDAL IDEA OF ADOLESCENTS (청소년의 자살사고와 관련된 우울-불안 성향)

  • Objectives:This study was aimed at examining the emotional predictors of adolescent suicidal ideas in a community. Methods:The subjects were 1909 middle and high school students(936 males, 973 females) in Koyang-shi. We evaluated all subjects using Center for Epidemiologic Study for Depression(CES-D), Revised Children's Manifest Anxiety Scale(RCMAS), Korean-Youth Self Report(K-YSR), Psychological Self-1 of Offer's Self Image Questionnaire(PS-1 of OSIQ) and Suicide Ideation Questionnaire (SIQ). We compared the PS-1 of OSIQ scores of Anxious-depressive group(above cut-off points of CES-D, RCMAS and K-YSR) to those of control group, and conducted correlation analysis and stepwise multiple regression analysis to all subjects. Results:The number of anxious-depressive group was 126(6.58%) among 1909 subjects with significant gender difference(M:F ratio 1:1.5). Both PS-1 of OSIQ and SIQ scores were significantly higher in Anxious-depressive group than in control group. There were significant correlations among the scales:CES-D, RCMAS, A/D of K-YSR, PS-1 of OSIQ and SIQ. Through stepwise multiple regression analysis, we found that significant predictors of suicidal idea were CES-D, RCMAS and A/D of K-YSR. Conclusion:Anxious-depressive adolescents may be the high-risk group for suicidality and need preventive interventions of suicidality in a community.

Geriatric Depression and Suicidality According to Residence Type among the Elderly in a Rural City Area (거주형태에 따른 노인 우울증과 자살경향성 비교 연구)

  • Objective : This study aimed to investigate and compare the prevalence of depression and suicidality among the elderly in a rural city according to their residence type. Methods : Participants were 311 elderly people (109 male and 202 female) in Icheon City, whom trained researchers interviewed and examined Geriatric Depression Scale, Social Support Scale, Instrumental Activities of Daily Living, Activities of Daily Living, Quality of Life Scale, and Mini International Neuropsychiatric Interview (MINI), module C. Using these data, we analyzed participants' risk factors associated with depression and suicidality. Results : The prevalence of depression was 28.0%. The prevalence in the elderly living alone and those admitted to residential facilities was significantly higher than that among the elderly living with family. Suicidality frequency was 19.6%. The suicidality frequency among the elderly living alone and those admitted to facilities was significantly higher than that among the elderly living with family. The risk factors for depression were age, admission to a facility, and low economic status. The suicidality risk factors were living alone, admission to a facility, poor social support, and a history of headache. Conclusion : These results showed the prevalence of depression and frequency of suicidality among the elderly in such a city was quite high. The results of this study remind psychiatrists of the importance of early detection and therapeutic intervention for the elderly with a high risk of depression and suicidality.

The Relationship between Possibility of Bipolar Disorder and Suicidal Attempt in Emergency Room (자살시도로 응급실에 내원한 환자의 자살 위험성과 양극성 장애 가능성의 연관성)

  • Objectives : The aim of this study was to investigate the factors including possibility of bipolar disorder that are related to intensity of suicidal idea. Methods : The study subjects consisted of 50 patients who did suicidal attempt and treated at the Inje University Haeundae Paik Hospital Emergency Room. All participants underwent psychiatric interview and underwent Columbia Suicide Severity Rating Scale(C-SSRS), Beck Depression Inventory(BDI), Korean Version of Mood Disorder Questionnaire(MDQ) and Bipolar Spectrum Diagnostic Scale(BSDS) to evaluate patient's suicide attempt, severity of depression and possibility of bipolar disorder. Results : Compared to non-high risk group, suicide high risk group showed significantly higher BDI(p<0.001) and intensity of ideation(IOI) in C-SSRS(p<0.001). Also intensity of ideation(IOI) was correlated with Mood Disorder Questionnaire(MDQ) bipolarity positive(p=0.033). Conclusions : The present study indicated that possibility of bipolar disorder plays a significant role in suicide attempters. Assessment of suicide ideation severity and possibility of bipolar disorder should be considered when suicide attempters come to emergency room.

Effects of a Nurse Presence Program on Suicide Prevention for Elders with a Chronic Disease (간호사의 함께있음 프로그램이 만성질환 노인의 자살예방에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of a nurse presence program on suicide prevention for elders with chronic disease. Method: The subjects were recruited from two different elderly institutions located in D city and K province, Korea. Twenty subjects in the control group received no intervention and nineteen subjects in the experimental group received a nurse presence program. Result: There were more significant decreases in suicide ideation, and the cortisol level and increases in life satisfaction in the experimental group compared to the control group. Conclusion: According to the above results, a nurse presence program for elders with a chronic disease decreased stressful events like suicide ideation and increased self esteem through therapeutic interaction. These findings suggest that this program can be used as an efficient independent nursing intervention for elders in a critical situation.

Development and Application of a Multidimensional Suicide Prevention Program for Korean Elders by utilizing a Community Network (지역사회 네트워크를 이용한 한국노인의 다차원적 자살예방 프로그램 개발 및 적용)

  • Purpose: The purpose of this study was to develop a multidimensional suicide prevention program for Korean elders by utilizing a community network and to evaluate its effect. Methods: A non-equivalent control group pretest-posttest design was used. The subjects were recruited from two different elderly institutions located in D city and K province, Korea. Nineteen subjects in the control group received no intervention and 20 subjects in the experimental group received a multidimensional suicide prevention program. Results: There were more significant decreases in depression, suicide ideation, and increases in life satisfaction in the experimental group compared to the control group. Conclusion: According to the above results, the multidimensional suicide prevention program for Korean elders decreased stressful events like depression, and suicide ideation and increased life satisfaction through the community network. These findings suggest that this program can be used as an efficient intervention for elders in a critical situation.

The Study of the Two-Dimensional Suicidal Type Based on Psychological Autopsy: A Focus on Suicidal Behaviors and Suicidal Risk Factors (한국형 심리부검 기반 이차원적 자살유형 연구: 자살행동과 자살위험요인을 중심으로)

  • The current study aimed to explore the suicidal behaviors and risk factors of completed suicides using psychological autopsy and use them as index variables to classify suicidal types. In addition, this study looked into the influential factors that affect each suicidal type. related to suicidal behaviors and suicidal risk factors by psychological autopsy. In addiction, the distinctions among the classes were analyzed. For this, psychological autopsies were conducted on the families and the close ones of 128 completed suicides. Then, the index variables were finally chosen for classifying suicidal types. The selected index variables for suicidal risk factors were mental disorders, suicide/self-harm, significant changes in physical appearance, marital conflict, adjustment and relationship issues at work/school, unemployment/layoff, jobless status and serious financial problems. The selected index variables for suicidal behaviors were expressing their suicidal attempts, writing suicidal notes, asking for help, the time/place/method of suicidal behavior, past suicidal/self-harm experience and the first person who witnessed the suicide. The Latent Class Analysis(LCA) and the 3-step method were used for classifying suicidal types. Then external variables(financial changes, cohabitation, existence of stressors, changes in stress level or relationships and family members with mental disorder/alchohol problems/ physical disorders, and work/school stisfaction) were applied for distinguishing classes. As a result, 5 classes(financial problems, adjustment problems, complex problems, psychiatric problems, and response to event[s]) were revealed on suicidal behaviors and 3 classes(residence- suicidal attempt- found by family, nonresidence- nonsuicidal attempt- found by acquaintances, residence- nonsuicidal attempt- found by family) were presented on suicidal risk factors. External variables such as gender, marital status, cohabitation, changes in relationships significantly differentiated among the 3 classes. Especially, class 3(residence- nonsuicidal attempt- found by family) tended to cohabit with others, were married, and had a significantly high level of interpersonal conflicts. When comparing the 5 classes of suicidal risk factors, auxiliary variables such as economic changes, cohabitation, stress, relationship changes, and family-related problems, and school/work satisfaction significantly differentiated the 5 classes. Especially class 3 (complex problems) experienced comparatively less family-related problems, but showed an aggravating level of personal stress. Suicial prevention strategies should be provided considering the characteristics of each class and the influential factors.

Relationships among Knowledge and Skills about Suicide Prevention, Attitudes toward Suicide, and Burnout of Suicide Prevention Work of Nurses at Mental Health Welfare Centers: A Mixed Methods Study (정신건강복지센터 간호사의 자살예방 지식 및 기술, 자살에 대한 태도와 자살예방업무 소진의 관계: 혼합연구방법의 적용)

  • Purpose: This study aimed to identify relationships among knowledge and skills about suicide prevention, attitudes toward suicide, and burnout of suicide prevention work of nurses at mental health welfare centers. Methods: An explanatory sequential mixed-method research was conducted. For the quantitative study, the subjects (nurses) were 133 nurses executing suicide prevention work. The data were analyzed using the SPSS 25.0 program. For the qualitative research, 13 nurses with high burnout scores were interviewed. The data were analyzed using theme analysis method. Results: Quantitative results showed the average burnout of suicide prevention work was 54.62 ± 12.51. The burnout of suicide prevention work had significant correlations with attitudes toward suicide (r = .30, p < .001) and suicide prevention skills (r = - .18, p = .037). Qualitative results showed six themes related to burnout of suicide prevention work. They were 'feeling a lack of confidence in one's suicide counselling skills', 'feeling of the limits of one's ability to cope with a suicide crisis', 'feeling regret for not being able to help the clients', 'being over-empathetic to the clients', 'Not being able to understand the clients because the subjects (nurses) opposes committing suicide', and 'thinking that the suicidal thoughts of suicide attempters do not improve'. Conclusion: To reduce burnout of nurses' suicide prevention work at mental health welfare centers, there is a need to develop an educational program considering nurses' attitudes toward suicide and one to enhance their confidence in suicide prevention skills.

  • 얼마 전 성추행 당한 사병이 그 고통을 견디다 못해 자살한 사건이 있었다. 군대라는 밀폐된 공간에서 성범죄가 자행된다는 사실에 온 국민은 경악을 금치못했다. 인권의 사각지대라는 멍에를 벗기 위해 노력하던 군은 또 다시 성범죄라는 큰 짐을 떠안게 되었다.

Prevalence and Related Risk Factors of Suicidal Ideation in Urban Adolescents (일 도시지역 청소년의 자살사고 유병률과 위험인자에 대한 연구)

  • Objective: The aim of the present study is to assess the prevalence and related risk factors of suicide ideation among middle and high school students in an urban area. Methods: We surveyed 3,691 middle and high school students (2,159 male, 1,532 female, 11-19 years of age) using a self-report questionnaire that covered basic socio-demographic data, academic achievements, presence of physical or psychiatric illness, sleep duration on weekdays, time spent at private academies on weekdays and weekends, and subjective needs for counseling. The Beck Depression Inventory (BDI) and Reynolds' Suicidal Ideation Questionnaire (SIQ) were included in the survey. Results: The prevalence of students with high suicide ideation ($SIQ{\geq}62$) was 4.6%. In a logistic regression model, female sex (p = 0.002), younger age (p < 0.001), poor academic achievement (p = 0.043) and higher score of BDI (p < 0.001) were associated with a higher SIQ score. In addition, younger age (p = 0.045) and a higher BDI score (p < 0.001) were associated with a higher SIQ score adolescents having high suicide ideation ($SIQ{\geq}62$). Conclusion: Related risk factors of suicide ideation in adolescents were female sex, younger age, poor academic achievement, and a depressive mood. It would be especially helpful to pay more attention to younger adolescents and a depressive mood as a high-risk group. The understanding of these factors will be helpful for providing an effective suicide screening and prevention program for adolescents.
