Dual Plate Fixation for Displaced Transverse Fracture of the Lower Sacrum (전위된 하부 천골 횡골절에 대한 이중 금속판을 이용한 내고정술)

  • Isolated fractures of lower sacrum are commonly treated conservatively in most cases because of low energy damage and stable fracture. However, surgical treatment is required in displaced unstable fracture. But the surgical technique is not established and even case reports are not common. We reported a case of displaced transverse fracture of the lower sacrum that was treated with an open reduction and dual plate fixation.

  • Developmental venous anomalies (DVAs) are common intracranial vascular malformations and they are generally do not cause clinical complications. In cases showing DVA and hemorrhage, the hemorrhage is usually associated with adjacent cavernous malformations. Very few cases of intracerebral hemorrhage (ICH) caused by thrombosis in DVA have been reported in the literature. In this case report, we present an interesting case of a large ICH caused by thrombosis within a DVA with an unusual structure that may have potentiated the thrombosis.

  • Coronavirus disease 2019 (COVID-19) is spreading rapidly around the world. Several articles have so far reported on radiological findings of COVID-19 pneumonia. Herein, we present three cases of COVID-19 pneumonia in South Korea, and provide clinical information as well as chest radiograph and chest CT findings.

  • Langerhans cell histiocytosis (LCH) is a rare condition that usually occurs in children and commonly affects the skeletal system. It is extremely rare in adults, especially in the clavicles. In this report, we describe a pathologically confirmed case of LCH in the clavicle of a 50-year-old male. We report various radiological findings, such as plain radiography, CT, MR, and PET-CT, along with a review of the literature.

  • Extraskeletal osteochondroma, a variant of chondroma, typically arises in the para-articular location of hands and feet. It is a rare disease and is particularly uncommon when joint components are not involved or localized away from joints. Herein, we report a case of extraskeletal osteochondroma in the posterior neck of a 66-year-old female. The characteristic radiologic finding of our case is presented, along with the typical findings of the disease and review of related literature reports.

The Physical and Mental Symptoms of Normal Subjects with Mild Life Stress (생활 스트레스가 경미한 일반 성인에서의 신체적 및 정신적 증상 - 종합병원 건강증진센터 내원자들을 대상으로 -)

  • Objectives : Aims of the study were to investigate the physical and mental symptom profiles related with perceived life stress in normal populations. Methods: The study subjects were 186 subjects who visited the Health Promotion Center at National Health Insurance Corporation Ilsan Hospital between July and December, 2000. We measured Life Stress Assessment Scale (LSAS), SCL-90-R (Symptom Check List-90 Revised), and Physical Health Check Questionnaires to evaluate perceived life stress for one year, and physical and mental symptoms. The physical markers related with stress were also measured. According to LSAS scores, we selected two groups from all subjects : Group I (LSAS score with lower 25 percentile) and Group II (LSAS score with higher 75 percentile). Each number of subjects was 46 in Group I and 47 in Group II. We analyzed stasistical significances of study variables between Group I and II. Results : Group II subjects showed higher scores in all subscales of SCL-90-R compared to Group I subjects. Comparing to Group I, Group II subjects complained more physiological symptoms related with anxiety and depression. But there were no statistical differences in the physical markers related stress between two groups. Conclusion: We found that past year-life stress caused various mental and physical symptoms, which didn't develop any physical illness related life stress in normal populations yet.

Effect of Helicobacter pylori Eradication on the Prevention of Gastric Cancer in the General Population with or without Precancerous Lesions: A Meta-analysis (일반 인구 집단에서 전암성 병변 유무에 따른 헬리코박터 제균 치료의 위암 예방 효과 연구: 메타분석)

  • We performed a comprehensive meta-analysis on whether Helicobacter pylori (H. pylori) eradication can prevent gastric cancer in individuals with precancerous lesions (chronic atrophic gastritis, intestinal metaplasia, and dysplasia). The studies evaluating the effect of H. pylori eradication on gastric cancer were searched from PubMed, KoreaMed, EMBASE, and Cochrane Library. Subgroup analyses by precancerous lesions, regions and follow-up time were performed. Six randomized clinical trials were included. In the overall population, H. pylori eradication significantly decreased the risk of gastric cancer (risk ratio [RR]=0.56; 95% confidence interval [CI]: 0.41-0.77, p<0.01). Eradication therapy significantly reduced the risk of gastric cancer in Asian and group who followed up more than 10 years (RR=0.51; 95% CI: 0.35-0.73, p<0.01; I2=0%, p=0.46). However, subgroup analysis according to the precancerous lesions did not show any significant results (p>0.05). In the general population especially in Asian population, H. pylori eradication can reduce the risk of gastric cancer. The effect of the eradication treatment is remarkable upon the long-term follow-up. In contrast, there is insufficient evidence whether the eradication should be recommended for the individuals with precancerous lesions.

Specialty Hospital and Keyword Searching Ads Regulation (전문병원과 키워드검색광고 규제)

  • The (Korean) Medical Services Act revised in 2009 introduces the accreditation of specialty hospital. When a hospital meets prescribed standards, passes a board review, and is accredited as a specialty hospital by the Minister of Health and Welfare, then it may use 'specialty hospital' in its name and certification mark of specialty hospital. The problem is that the (Korean) Fair Trade Commission and the (Korean) Ministry of Health and Welfare, both of which have authorities to regulate advertising in general and in health care service in turn, announced the guidelines to prohibit internet (portal) service providers to provide keyword search ads service using key-words such as 'specialty' or 'specialized in' for those who are not accredited by the Minister of Health and Welfare. In this article, whether these guidelines can be justified by the current regime and whether the current specialty hospital policy is agreeable would be examined. To do this, the legal nature of accreditation of specialty hospital, the limit of advertisement regulation, the law of keyword search ads, and the liability of internet service providers also would be analyzed.

Relationship Between Life Style and Obesity Indices in Adults Using Data from Health Examination (건강검진자료에 의한 일반 성인의 생활습관과 비민지표와의 관련성)

  • The purpose of this study was to obtain the relationship between life styles and obesity indices, and to investigate the influence of life styles on obesity indices among adults who examined health checkup in an university hospital. The subjects for this study were 4,112 adults who underwent medical examinations at a health center of a university hospital in Daejeon city from Jan 2012 to Dec 2013. We surveyed the life styles and obesity indices of study subjects from self-recorded questionnaires and medical examination charts after confirmed IRB of the hospital. As a results, the levels of obesity indices of subjects was closely correlated with increasing age, and life styles as alcohol drinking, smoking, regular exercise, overeating, meat consumption. The influence of life styles on obesity indices was incident in female than in male. And the main risk factors were regular exercise factor in obesity indices in both sex.

Market share of specialty hospitals in the region and out of the region (전문 질환에 대한 전문병원의 권역내·외 시장점유율 비교)

  • Purposes: The Specialty hospital designation policy had launched in 2011 and 110 designated specialty hospitals have been operating nationwide in 2022. This study was to estimate the market share of specialty hospitals for the specific diseases compared to other types of hospitals. Methodology: Data were derived from the National Health Insurance Claim data from 2018 to 2019. Subjects were all the inpatients with MDC(Major Disease Category) that specialty hospitals specialized in. A total of 34,231,387 claims were analyzed to estimate the market share. Findings: 90 specialty hospitals were responsible for 2.4 percent of inpatient care with specific diseases for specialty hospitals. There were regional variations in the market share of the specialty hospitals as the number of specialty hospitals in regions. Specialty hospitals' market shares were relatively high in burn(31.3%), ophthalmology(16.4%), obstetrics and gynecology(7.1%), alcohol(6.0%), joint(3.7%), spine(2.7%). After adjusting the number of inpatients per hospital, hospitals specialized in burn, alcohol, ophthalmology, breast, joint, obstetrics and gynecology, and hand replantation had treated more patients than tertiary hospitals. Practical Implications: Although specialty hospitals' market share was small, some types of specialty hospitals had an impact on the regional market as well as the national level market. To improve patients' accessibility to a specialty hospital, it is necessary to government supports non-specialized hospitals to change into specialty hospitals in certain fields and regions where the number of specialty hospitals is insufficient.
