Background: Lung cancer is the most common cause of cancer mortality in Korea. The TNM stage at presentation in patients with non-small cell lung cancer (NSCLC) has the greatest impact on prognosis. Patients who undergo a complete resection for NSCLC are likely to develop recurrent and/or metastatic disease. There are several factors influencing the development of recurrence. We explored risk factors of recurrence in patients with stages I and II NSCLC, who had undergone curative resection. Methods: We reviewed patients who had complete surgical resection as definitive treatment for stage I or II. Patients followed up for more than 36 months. We evaluated several factors which might have relationship with recurrence, such as patient's demographic factors, TNM staging, pathologic finding, tumor markers and surgical technique. Results: A total of 75 patients were enrolled for analysis, of whom 58 were men and 17 were women with mean age of 61 (range, 37 to 76) years. The average size of tumors was 3.9 cm (0.7 to 10 cm). There were 64 patients with stage I NSCLC and 11 with stage II NSCLC. Among 64 patients with stage I NSCLC, 35 patients showed recurrences whereas 8 patients have recurred in stage II NSCLC. Grade of differentiation of tumor was closely related to the recurrence. Seventy-five percent of patients who had poor tumor differentiation experienced a recurrence. In contrast, 3 patients of twelve had recurrences, who revealed differentiation in their tissue (p<0.05). Conclusion: Tumor differentiation could be a predictive factor for tumor recurrence in patients who have undergone curative resection for stage I or II NSCLC.
Large soft tissue defects around the knee joint are known to significantly diminish joint function. Severe soft tissue defects on the anterior aspect of the knee joint especially bring on significant joint motion limitation. Although simple split skin grafts can cover the skin defect, the progressing scar contracture of the grafted skin causes joint stiffness. One of the best solutions of large soft tissue defects around the knee joint is covering the defect with a good quality skin flap. Separated flaps with one vascular pedicle are good candidates for covering anterior and posterior aspects of the joint for example. Authors performed 12 cases of combined scapular and latissimus dorsi free flaps from 1984 to 2000. Among them, we experienced 5 cases of knee joint defect covering using the double free flap for coverage of the soft tissue defect with preservation of the knee joint function and satisfactory results. The system of flaps based on the subscapular artery and vein provides a variety of composite free flaps. The possible flaps that can be harvested based on this single vascular pedicle include the scapular and parascapular skin flap, the serratus anterior and latissimus dorsi muscular flap, the lateral scapular bone flap, the latissimus dorsi-rib flap, and the serratus anterior-rib flap. This combined flap is available for multiple tissue defects or complex defects because it can be incorporated with skin, muscle and bone flaps. A main advantage is the independent vascular pedicles of each component, which allow freedom in orientation of each components. Consequently it can be freely applied to any form of three dimensional defects on the upper and lower extremities. The combination of scapular cutaneous flap and latissimus dorsi musculocutaneous flap can be resurfaced for massive cutaneous defects on the extremities. We report the use of the combined scapular and latissimus dorsi free flap in five patients to reconstruct massive defects on the extremities with resultant improved joint function. There was no flap failure and minimal complications and disadvantages. The anatomy of this flap is reviewed and the indication and advantages are discussed. All of the five flaps survived and there was no scar contracture affecting the joint motion.

The T-stub subjected to an axial tensile force shows various behavior characteristics according to the changes in the diameter and tightening force of the fastener, the geometric shape of the T-stub, and the material properties of the T-stub and fastener. Due to the influence of these changes, the T-stub shows three failure modes: plastic failure after the flexural yielding of the T-stub flange, flexural yielding of the T-stub fillet, and fracture of the fastener. In general, a T-stub with a thin flange and where the gauge distance of the fastener is long has a larger energy dissipation capacity than a T-stub with a thick flange and where the gauge distance of the fastener is short, due to the plastic deformation after flexural yielding. In this study, three-dimensional nonlinear finite element analysis was carried out to determine the effect of the fastener used for fastening the T-stub on the energy dissipation capacity of the T-stub. For the fastener of the T-stub analysis model, F10T-M20 high-tension bolts and ${\varnothing}19.05-mm$ (3/4-inch) SMA bars were modeled, and the geometric shape of the T-stub was selected to represent the flexural yielding of the T-stub fillet and the axial tensile failure of the fastener.
Although traumatic thoracic aortic rupture is potentially a fatal condition requiring surgical attention, the presence of concomitant injury involving other parts of the body may greatly increase the risk of cardio-pulmonary bypass. We report our experience of treating associated injuries prior to the thoracic aortic rupture in these patients. Material and Method: From 1997 to 2003, the medical records of 24 traumatic aortic rupture patients were retrospectively reviewed and checked for the presence of associated injury, surgical method, postoperative course, and complications. Surgical technique comprised thoracotomy with proximal anastomosis under deep hypothermic circulatory arrest followed by side arm perfusion to reestablish cerebral circulation. CSF drainage was performed to prevent lower extremity paraplegia. Result: Major concomitant injuries (n=83) were noted in all of the reviewed patients, Of these, there were 49 thoracic injuries, 18 musculoskeletal injuries, and 13 abdominal injuries, Operations for associated injuries (n=16) were performed in 12 patients on mean 7.6$\pm$12.6 days following the injury. The diagnosis of aortic rupture at the time of injury was detected in only 18 patients. Delayed surgery of the thoracic aorta was performed on average 695$\pm$1350 days after injury and there were no deaths or progression of rupture in any of these patients during the observation period. There were no operative deaths and no major postoperative complications. Conclusion: Treating concomitant major injuries prior to the aortic injury in traumatic aortic rupture may reduce surgical mortality and morbidity.
Linear regression technique has been used widely in water resources field as well as various fields such as economics and statistics, and so on. Using fuzzy regression technique, it is possible to quantify uncertainty and reflect them to the regression model. In this study, fuzzy regression model is developed to compute design floods in any place in Pyeongchang River basin. In ungaged basins, it is usually difficult to obtain data required for flood discharge analysis. In this study, basin characteristics elements are analyzed spatially using GIS and the technique of estimating design flood in ungaged mountainous basin is studied based on the result. Fuzzy regression technique is applied to Pyeongchang River basin which has mountainous basin characteristics and well collected rainfall and runoff data through IHP test basin project. Fuzzy design flood estimation equations are developed using the basin characteristics elements for Pyeongchang River basin. The suitability of developed fuzzy equations are examined by comparing the results with design floods computed in 9 locations along the river. Using regional regression method and fuzzy regression analysis, the uncertainties of the design floods occurred from the data monitoring can be quantified.

Purpose: The purpose of current study was to review the surgical treatment results in patients with a ganglion cyst involving peripheral nerves and to suggest the poor prognostic factor. Materials and Methods: Twelve patients having neurologic symptoms caused by ganglion cyst were treated operatively between 1995 and 2000. The peripheral nerves involved were the tibial nerve in three patients, suprascapular nerve, common peroneal nerve, radial nerve, and ulnar nerve in two patients each, and median nerve in one patient. Pain was present in six patients, sensory disturbance or motor weakness was seen in seven patients each; and sensory disturbance and motor weakness were concurrently present in four patients. Results: In all six patients who complained of preoperative pain, the pain was resolved after surgery. Improvements were seen in five of seven patients who had preoperative sensory disturbance and in all patients who had preoperative motor weakness. Complete sensory recovery was obtained in only two of four patients with preoperative sensory disturbance and motor weakness, indicating a poor prognosis factor. Conclusion: Early accurate diagnosis and early excision of these ganglion cysts causing compression neuropathy could produce excellent clinical results.

Giant cell tumor is most frequently found in juxtaarticular region, and difficult to treat because of local recurrence. Although primary resections reduce recurrence, the joint function will be markedly impaired. Techniques involving physical adjuncts(high speed burr and electric cauterization), acrylic cement or en bloc resection with VFG(vascularized fibular graft) have been employed to reduce local recurrence. From October 1984 to April 1994, twenty-nine patients diagnosed as giant cell tumor were treated at department of Orthopaedic Surgery, School of Medicine, Kyung Hee University. There were eleven men and 18 women, ranging in age from 17 to 52 years(mean: 34 years). The average follow-up period was four years and five months. The location of the lesion was around the knee in 15, distal radius in three, femoral head in three, and others in eight patients. Fifteen patients around the knee joint were treated with several modalities; curettage with bone graft in five, curettage with cement filling in three, curettage with bone graft and physical adjuncts in five, en bloc resection with VFG in one and en bloc resection with arthroplasty in one patient. The functional results, according to the Marshall's knee score, were excellent in one, good in two, and fair in two after the curettage with bone graft, good in three after the curettage with bone cement filling, excellent in one, good in four after the curettage with bone graft and physical adjuncts, and good in two after the en bloc resection with VFG or arthroplasty. Three patients had local recurrence among 15 patients with giant cell tumor around knee. Vascularized fibular graft around wrist joint provided good functional restoration without local recurrence in all three patients who had giant cell tumor in distal radius. Although there is no statistical significance, it seems that curettage with bone graft using physical adjuncts or acrylic cement reveals better results than simple curettage with bone graft. Excellent functional result were obtained without local recurrence by using vascularized fibular graft after en bloc resection.

This study investigates the relationships among learning motivation, learning immersion, and academic achievement in college-level nursing students who give non-face-to-face online lectures. The data for this study were collected using questionnaires given to 147 students attending four-year nursing colleges at two universities. For the data analysis method, descriptive statistics, t-test, ANOVA, the Scheffe test, Pearson's correlation analysis, and multiple regression analysis were used. As a result of the study, the average motivation was 101.29±12.18 points, learning commitment was 72.02±8.03 points, and academic achievement was 31.31±4.06 points. The relationships among learning motivation, learning commitment, and adaptation to college life showed significant positive correlations. In the results of multiple regression analysis, factors influencing the academic achievements of nursing students were found to be learning motivation and learning commitment. Therefore, in order to increase academic achievement in nursing college students, it is necessary to develop a specific strategy to improve learning motivation and to develop various activities and programs to immerse students in learning.
Currently, the signalized intersections in Korea are operated without providing an emergency vehicle preemption control strategy. Thus, it might threaten the safety of the pedestrians and drivers on highways when an emergency vehicle faces congested traffic conditions. The existing preemption control is activated when an emergency vehicle is detected along a path. This enables emergency vehicles to progress uninterrupted, but it also increases the delay of other vehicles. In this paper, a revised preemption control strategy considering queue length restrictions is proposed to make both a progressive movement of an emergency vehicle and reduce delay of other vehicles simultaneously. By applying the preemption control strategy through a simulation study, it was shown that delay of an emergency vehicle decreased to 44.3%-96.1% and speed increased to 8.8%-42.0% in all 9 cases as compared with a conventional signal control. The existing preemption control is superior for oversaturated conditions (v/c >1.0) or a link length less than 200m. However, the preemption control considering queue length constraints shows better performance than the existing preemption control when the v/c is less than 0.8 and a link length is longer than 500m.
The extracted phenolic compounds from Pinus densiflora root were examined biological activities for beauty food. The tyrosinase inhibitory activity which was related to skin-whitening was observed. The tyrosinase inhibitory activity was confirmed to be 92% in ethanol extract at $50{\mu}g/mL$ phenolic. The elastase and collagenase inhibitory activity as anti-wrinkle effect were showed 61 and 78% in ethanol extract at $200{\mu}g/mL$ phenolic, respectively. Astringent effect of ethanol extract was showed to be 82% at $50{\mu}g/mL$ phenolic. Hyaluronidase inhibitory activity of ethanol extract as anti-inflammation effect was confirmed to be 94% of inhibition at $200{\mu}g/mL$ phenolic. These results demonstrated that isolated phenolic compounds from P. densiflora root could be expected to use as a functional cosmetic materials.
