신생아의 지속성 폐동맥 고혈압증에서 Sildenafil 치료 경험 (Oral Sildenafil in Persistent Pulmonary Hypertension of the Newborn)

  • 목적: 최근 PPHN의 치료를 위해 iNO와 함께 많은 약물적 치료가 사용되고 있다. Sildenafil은 PDE5의 억제제로 선택적인 폐혈관 확장제로 알려져 있으며 iNO에 비하여 접근성이 뛰어나 iNO에 대안적인 약제로 연구되어 왔다. 저자들은 본원에서 sildenafil로 PPHN을 치료한 경험을 분석하였다. 방법: 심초음파를 통해 PPHN으로 진단된 재태 기간 35주 이상, FiO2 1.0을 필요로 하는 32명의 환아를 대상으로 하였으며 의무기록을 후향적으로 조사하였다. sildenafil은 0.5 mg/kg로 시작하여 6시간마다 1 mg/kg로 투약하였으며, 투약 전과 후 6, 12, 24, 48시간의 MAP, $FiO_2$, OI, MBP를 비교하였다. 부작용을 확인하기 위하여 환아들의 위장관 증상, 뇌초음파, 안저검사와 청성뇌간유발반응 검사의 결과를 조사하였다. 결과: 대상 환아 32명 중 태변 흡인 증후군 9명, 호흡곤란증후군 8명, 폐렴 3명이었으며, 12명은 특발성이었다. 이 중 31명이 생존하였으며 28명의 환아는 본원에서 sildenafil만으로 치료되었고 3명의 환아는 전원되어 iNO 치료 후 회복하였다. Sildenafil 단독으로 치료된 28명의 환아에서 $FiO_2$와 OI는 치료 6시간째부터 MAP는 48시간째부터 유의하게 감소하였다. 모든 환아에서 특별한 부작용은 관찰되지 않았다. 결론: Sildenafil은 35주 이상의 PPHN 환아에서 비교적 효과적이고 안전하게 시도해 볼 수 있는 대안 약제로서, 특히 iNO가 가능하지 않은 병원에서 유용할 것으로 생각된다.

양수과다증 산전력이 있는 미숙아의 가성저알도스테론혈증 1예 (Pseudohypoaldosteronism in a premature neonate with severe polyhydramnios in utero)

  • 가성 저알도스테론혈증은 신장 세뇨관의 알도스테론 수용체의 이상으로 알도스테론의 분비는 증가되지만 반응성은 감소되어 저알도스테론혈증의 증상을 보이는 질병이다. 가성 저알도스테론혈증은 다뇨증, 과도한 소변 내 염분소실, 저나트륨혈증, 고칼륨혈증, 대사성 산증을 특징으로 하며, 산전부터 다뇨 증상이 시작되면 양수과다증을 초래할 수 있다. 미숙아에서는 생후 초기 불감 수분 손실량이 많으며, 폐 기능과 신장 기능이 미숙하여 전해질 이상을 흔히 동반하는데, 몸무게의 소실에도 불구하고 지속적인 다뇨와 저나트륨혈증, 고칼륨혈증의 증상이 있다면 전해질 불균형을 초래하는 기저질환을 확인하여야 한다. 저자들은 양수 과다증의 산전력이 있는 산모에게서 출생한 재태주령 32주 미숙아에서 지속적인 몸무게 감소에도 불구하고 계속되는 다뇨와 저나트륨혈증, 고칼륨혈증이 있어 17수산화프로게스테론을 조기에 선별 검사하여 선천성 부신 과형성증의 가능성을 배제함으로써 불필요한 스테로이드 치료를 피하고, 지속적인 전해질 보충으로 점차 전해질 이상의 호전을 보인 가성 저알도스테론혈증 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

대구지역 사업체급식소에 대한 HACCP 적용 효과 (Effects of HACCP Implementation on an Industry Foodservice Operation in Daegu)

  • 사업체급식소와 HACCP 적용효과를 평가하기 위해 대구지역 특정 위탁급식소의 급식종사자 21명을 대상으로 2회에 걸친 HACCP 교육과 수시 현장지도를 통하여 HACCP을 적용한 후 미생물적 품질, 조리 및 배식온도와 소요시간을 측정하였다. 미생물 검사를 위한 음식물의 채취는 비가열조리식품, 가열후처리식품, 가열조리식품의 세 가지 조리 공정별로 CCP를 정하고 CCP별로 검수단계, 전처리단계, 조리단계, 배식단계에서 각각 취하였다. 미생물검사는 표준 평판균수와 대장균군수를 측정하였다. HACCP 적용 전에 비해 적용 후, 조리 및 배식단계에서 급식종사자들이 식품내부온도를 74$^{\circ}C$ 이상으로 유지하고, 위험온도대인 5~6$0^{\circ}C$ 에 보관하는 것을 피함으로써 가열조리식품에서 미생물적 품질은 향상되었으며, 조리기구의 미생물적 품질도 HACCP 적용 후 향상되었다. 그러나 HACCP 적용 후에도 비가열 조리식품과 가열후처리식품에서의 미생물적 품질은 기준에 적합할 만큼 향상되지 않았다. 비가열조리식품과 가열후처리식품은 검수단계에서의 식재료 및 양념류의 철저한 품질 검사부터 배식단계의 배식온도까지 세심한 주의가 요구되며, 조리종사자의 개인위생과 조리기구의 교차오염도 신경을 써야 한다. 그러므로 식품안전성이 보장된 급식을 위해서는 단체급식소의 조리종사자를 대상으로 하는 HACCP 교육을 통하여 HACCP을 적용할 뿐 아니라 식품제조업체에서의 HACCP 적용 또한 절실히 필요한 것으로 사료된다.

치위생(학)과 학생의 치과감염관리에 관한 인식현황 (Current conditions regarding dental infection management recognition of students in the department of dental hygiene)

  • This research was performed to provide basic data for the development of infection related dental hygiene studies by surveying the current condition of recognition among students in the department of dental hygiene toward hospital infection management while receiving the following results by using a personal self-administered survey method targeting 303 students in the department of dental hygiene from certain areas. 1. With the current condition of recognition on the sanitization and sterilization of instruments among students in the department of dental hygiene(study), the rate at which surgical instruments are to be sterilized with autoclaves was 79.9% which was relatively higher than other instruments while it was shown that prosthetic instruments for treatment was 56.4%, conservative instruments for treatment was 51.8%, and ultrasonic scaler tip was 51.1% while the way syringe tips(36.1%) and the dental anesthetic apparatus(27.9%) were revealed to require sanitization by alcohol. 2. The 'hand wash' area was the highest with 4.71 while the 'materials and environment management' area and 'equipment management' area appeared high respectively with 4.43 and 4.41. 3. With the current condition of recognition on equipment management, 'equipments used for contagious patients are separately washed after a one-time use and must be sterilized or separated-and-discarded' was the highest with 4.82 while 'sterilization equipments with humidity or water on it are considered contaminated and are not used' showed the lowest recognition level with 3.90. 4. Regarding the current condition of materials and environment management, 'contagious and general trash are separated and discarded' was the highest with 4.70 while 'the refrigerator for medicine storage is cleaned on a regular basis once a month' was revealed as the lowest with 4.11. 5. With the current condition of recognition on hand washing, 'one must wash their hands after coming in contact with contagious patients, was the highest with 4.90 while washing hands after taking off gloves' appeared as the lowest with 4.51 point. To conclude department of dental hygiene there is to infection management and necessary about organization disinfecting and pasteurization to strengthen an education in order raising a stamp helping practical ratio about the infection management which whole, is from presence at a sickbed and connection does and about the infection management which is substantial and educational program development leads feed with the fact that deepening studying which is continuous must become accomplished becomes.

일부 노인의 일반적 특성 및 구강보건지식과 행태에 따른 주관적 구강건강상태 연구 (Research on the subjective status of oral cavity's health following senior citizens' characteristics)

  • To identify subjective health status of senior citizens' oral health following senior citizens' general characteristics and knowledge and behavior for the oral hygiene, this research conducted individual interview with 237 senior citizens in some parts of Seoul. The results are as follows. 1. Degree of senior citizens' knowledge on the oral hygiene is about Middle. Among the categories on the senior citizens' knowledge on the oral hygiene, teeth's brushing was the highest while knowledge on fluorine was the lowest. 2. Senior citizens who feel that their oral health is healthy when it comes to the subjective health state of senior citizens' oral health following behavior for the oral hygiene, brush their teeth, three times in a day, for more than three minutes (p<.01). Moreover, senior citizens who feel that their oral health is healthy brush different corners of the tongue when brushing (p<.01). 3. In case of knowledge on the oral hygiene following general characteristics, knowledge on the oral hygiene was higher when economic status was higher (p<.01). In case of living expenses, knowledge on the oral hygiene was higher for the senior citizens with pay or income (p<.01). 4. In case of behavior for the oral hygiene, women tended to act for the oral hygiene more than men. As for the method for raising living expenses, senior citizens who receive basic social security check or those at the highest tier tended to act less for the oral hygiene (p<.01). 5. Senior citizens who answered that their oral health is healthy when it comes to the subjective health state of senior citizens' oral cavity tended to have high knowledge on the oral hygiene (p<.01). In conclusion, subjective health status of senior citizens' oral health is higher when the knowledge on the oral hygiene and behavior for the oral hygiene are higher. Accordingly, it is necessary to develop and execute oral hygiene training program to change senior citizens' behavior incrementally and the dental hygienists who can conduct this training should be actively attracted into the senior citizens' oral hygiene training.

돈분액비 토양침투율과 토양내 분포 특성 (Infiltration Rates of Liquid Pig Manure with Various Dilution Ratios in Three Different Soil)

  • BACKGROUND: Increase in application of liquid pig manure(LPM) in agriculture as nutritional source has become a social issue due to its influence on water quality. Also, proper application methods have not been developed with respect to indigenous properties of LPM and soil physical properties. Therefore, we conducted this experiment to observe the infiltration characteristics and distribution of dissolved organic compounds of LPM in soils having different soil textures. METHODS AND RESULTS: To do this experiment, we collected three different soils and LPM. We analyzed the physical and chemical properties of both soils and LPM to determine the dilution ratios of LPM. The LPM diluted to 4 different ratios with distilled water was applied to the top of soil column. Infiltration rates were observed by time and depth until the amount of effluent collected from the bottom of the soil columns were stabilized while maintaining the hydraulic head 3 cm above the soil column. The results showed that infiltration rates increased with increasing dilution ratios in the order of sandy, loamy sand, and sandy loam. The time required to reach steady state was increased with decreasing sand contents clay. CONCLUSION: The size and amount of the dissolved organic compounds in LPM that can determine the efficiency as fertilizer and environmental problems as nonpoint pollution source in water quality have not been investigated with respect to behavior and transport of them in soil. Therefore, it requires further research how we can properly apply LPM as valuable fertilizer substitute for inorganic fertilizers.

노인시설 종사자들의 노인에 대한 태도 및 노인장기요양보험제도에 대한 인식조사 (A study on the elder care facility workers' attitude towards senior citizens and on the perception towards long-term care insurance system for senior citizens)

  • Objectives : This research identified the attitude towards senior citizens targeting the workers of the facility for senior citizens to establish Long-Term Care Insurance System early on, and to identify the degree of perception on the importance of senior citizens' oral health and on the need for dental sanitation specialists. Methods : This research targeted 233 workers of the facility for the senior citizens located in some parts of the Gyeongnam area. Survey was conducted from December 1, 2009 to January 31, 2010 to identify the facility workers' attitude towards senior citizens and on the perception towards Long-Term Care Insurance System. Results : 1. As for the awareness towards senior citizens, 50.2% said that they are comfortable around senior citizens while degree of kindness towards senior citizens was 48.5%. 21.0% had experienced training for senior citizens' oral health management. As for the question on who should serve as the trainer, dental sanitation specialists were citied the most with 47.6%. 29.2% was very interested in the senior citizens' oral health while 83.7% said that oral cavity managers are needed in the facility for the senior citizens. As for the question on who should be in charge, if necessary, dental sanitation specialist was cited the most with 57.9%. 2. Awareness on the Long-Term Care Insurance System, treatment for the oral cavity sanitation by visiting homes and information on the activity of long-term care specialists was high following the experience of training for the senior citizens' oral health management in case of the workers at the senior citizens' facility. This manifested statistically significant difference(p<0.05)(p<0.01). Moreover, treatment for oral cavity sanitation manifested statistically significant difference was manifested in the treatment for oral cavity sanitation following the need for oral health managers and who should be in charge, if necessary(p<0.01). Conclusions : These results advocate the need to develop diverse senior citizens' oral health management methods and to continue to carry out actions for senior citizens' oral health improvement that can be carried out by the long-term caretakers for the management of senior citizens' oral cavity health.

전국 권역별, 연령별 여자 성인과 노인의 짠맛 미각판정치, 식태도 및 식행동 비교 (A Comparison of Salty Taste Assessment, Dietary Attitude and Dietary Behavior among Adult and Senior Women by Region and by Age in Korea)

  • Objectives: The purpose of this study was to compare and analyze the results of salty taste assessment, dietary attitudes, and dietary behaviors among adult and senior women by region and by age. The results generated from this was expected to provide fundamental data for implementing a nationwide salt reduction education program. Methods: The salty taste assessment tool was applied to 4,064 subjects from 15 areas in Korea. Also, a survey of dietary attitude and dietary behavior related to salt intake was conducted for all subjects participated in this study. Results: The salty taste assessment scores by region and by age were the lowest in capital (p < 0.01) and was highest among the 70+ year age group (p < 0.01). The dietary attitude scores and dietary behavior scores showed that Gyeongsang was the highest (p < 0.001) and the capital was the lowest. The dietary attitude scores were highest at 20&30's followed by 40's and 70's group (p < 0.001). Dietary behavior scores showed that 20's~30's and 40's groups were higher than the other age groups (p < 0.001). The score of 'I like kimchi' was $3.46{\pm}0.88$, which was the highest among 10 dietary behavior questions. The score of 'I eat a lot of kimchi' was $3.30{\pm}0.90$, which was the highest among 10 dietary behavior questions related to salt intake. The scores of salty taste assessment had significant positive correlations between the scores of dietary attitude (p < 0.001), dietary behavior (p < 0.001) and self-awareness (p < 0.001). Conclusions: Based on the results of salty taste assessment, we observed a tendency that older people and regions except the capital had higher preference for salty taste. Our results suggested the necessity for a nationwide salty reduction education program tailored for regions and for different age groups.

식품 중 곰팡이독소 안전기준 관리 (Administration of Mycotoxins in Food in Korea)

  • 식품 중 아플라톡신 ($B_1+B_2+G_1+G_2$)의 기준은 곡류, 두류, 땅콩, 견과류 및 그 단순가공품 (분쇄, 절단 등), 곡류 가공품 및 두류가공품, 과자류 (땅콩 또는 견과류 함유식품), 장류, 고춧가루, 팝콘용 옥수수가공품, 기타 식품류 (찐쌀)에 대하여 15 ($B_1$은 10)${\mu}g/kg$ 이하로, 원유 및 우유류 중 아플라톡신의 기준은 $M_1$으로서 0.5 ${\mu}g/kg$ 이하로 설정되었다. 사과주스, 사과주스 농축액 중 파튤린의 기준은 50 ${\mu}g/kg$ 이하로 설정되어 있으며, 오크라톡신 A의 기준은 밀, 호밀, 보리, 커피콩, 북은 커피에 5 ${\mu}g/kg$, 인스턴트커피, 건포도 중 10 ${\mu}g/kg$, 포도주스, 포도주스 농축액, 포도주 중 2 ${\mu}g/kg$ 이하로 설정되었고, 푸모니신($B_1+B_2$)은 옥수수 중 4 mg/kg, 옥수수 단순가공품 및 옥수수 가루 중 2 mg/kg, 옥수수 가공품 중 1 mg/kg 이하로 설정되었다. 현재 식품 중 곰팡이독소에 대한 안전관리는 노출량 조사와 위해평가를 통해 합리적이고 과학적으로 설정되어 관리하고 있다.

초등학교 영양교사의 영양교육 실시를 위한 교육자, 학습자 및 그 부모 대상 요구도 조사 (A Survey on the Needs of Educators, Learners and Parents for Implementing Nutrition Education by Nutrition Teachers in Elementary Schools)

  • The purpose of this study was to investigate the needs of educators, learners and parents in elementary schools for implementing a systematic nutrition education program by nutrition teachers to begin in 2006. The subjects were 833 dietitians, 808 principals, 3,141 teachers, 7,577 students and 6,003 parents in elementary schools. A fairly large proportion of the principals (98.4%), teachers (95.5%), parents (96.6%) and students (62.8%) responded that nutrition education is urgently needed. Every dietitian, principal and teacher has recognized that for the enforcement of a nutrition program in the future, the placement of one nutrition teacher in every school is needed without a moment's delay. Many subjects responded that elementary low grades are the most proper time for nutrition education for students and it is also needed to extend this not only to students, but also parents and teachers who have a direct influence on them. In the education hour, they responded that it is proper one hour a month for teachers and one hour a week for students. Fifty-six percent of dietitians, 58.3% of principals and 67.0% of teachers responded that the best educational way to heighten the effect of nutrition education was education through discretional activities. It was found that 46.4% of students have wanted nutrition education to be enforced during special activities. Education through their school homepage or in-school broadcasting system, while it has been most utilized, was low in a degree of preference with 10.9%. In regards to a nutrition program, principals and parents have attached much importance to the dining etiquette, dietary attitudes, and relations of foodstuffs with health, while dietitians and teachers have made much of a balanced eating habit, and the relations of dietary life with health. Finally, for a nutrition program to be enforced towards a direction that the educators and students want, it is judged that first of all the role of nutrition teachers is the most important, furthermore the support of manpower and the budget should be made.
