Carcinoma of the Uterine Cervix Treated with External Beam Irradiation Alone (자궁경부암의 외부방사선 치료 성적)

  • A retrospective analysis was peformed on 42 patients with carcinoma of the uterine cervix who were treated with external beam (EB) radiation therapy alone at the Department of Therapeutic Radiology, Seoul National University Hospital from March 1979 to December 1988. After whole pelvic field irradiation of 50Gy, all the patients received additional booster dose of 12-22Gy to the primary tumor Thirty one received EB radiotherapy alone because of poor geometry for intracavitary application,5 because of medical problems and 6 because of other reasons. Five year locoregional control rate and five year survival rate were $34.5\%\;and\;35.4\%,$ respectively. Five year survivals were $66.7\%,\;36.4%,\;32.8\%\;and\;25.0\%$ for stage IIA, IIB, IIIB and IVA, respectively. The response one month after treatment well correlated with prognosis. The incidence of grade 2 and 3 complication was $12\%\;and\;10\%,$ respectively. There was tendency of increased complication with advanced stage.

The Theory of Meaning in Hospice Care

  • Care for the human spirit is a core component of quality end-of-life care. Logotherapy, based on the premise that the primary motivation of human beings is to find meaning and purpose in life, can be helpful in providing care for patients, families, and loved ones in hospice care. The use of Socratic dialog in posing questions about one's life experiences, values, and attitudes is a useful method of evoking reflection. Guidance for finding meaning, even until one's last moments, can be found in the three categories: (a) tasks or deeds, (b) experiences of love and beauty, and (c) attitudes chosen in spite of a fate that cannot be changed. Self-transcendence, defined as getting outside the self for the good of others, can add meaning to life. A growing body of research concerning meaning-centered therapy is promising for improving spiritual well-being and a sense of meaning and purpose in life.

The Characteristics of semantic association task performance in elderly with subjective memory impairment and mild cognitive impairment (주관적 기억장애 및 경도인지장애 노인의 의미연상과제 수행 특성)

  • The loss of semantic knowledge and impairments in semantic associations by semantic category is gaining increasing attention, as indicators of early-stage cognitive decline. As such, we assigned semantic association task (SAT) to normal elderly (NE) and those with subjective memory impairment (SMI) or mild cognitive impairment (MCI) to examine their performance by semantic subcategories and the differences in error patterns. We found a significant difference in the number of correct response and reaction time under the SAT categories among the three groups, with the highest performance observed in 'function' and the lowest performance in 'superordinate' and 'part/whole'. Moreover, the error frequency was the lowest in NE, followed by those with SMI and MCI, with the latter two groups showing a significant increase in no-response. Our findings demonstrate the varying extent and process of impairments in the semantic network by category over different stages of cognitive decline. Thus, we proposed SAT performance as an indicator to detect and follow-up on cognitive decline in elderly with cognitive disorder.

Effects of a Logotherapy-Based Music and Imagery Program on the Self-Worth of Personal Assistants for the Disabled (의미치료에 기반한 음악과 심상 프로그램이 장애인 활동보조인의 자기가치감에 미치는 효과)

  • Purpose: This study has proved the intervention effects of the proposed logotherapy-based music imagery (LBMI) program on the self-worth of the personal assistants for the disabled. The program, in the image activities and at the stage of postlude after listening to music, unites logodrama of logotherapy, paradoxical intention and dereflected techniques. The intervention effects of the LBMI program were tested for the 28 personal assistants for the disabled who found themselves without the value and meaning of life and were randomly equally divided into the experimental and control groups. This intervention was carried out for the six different terms, each of which was alloted 60 minutes. To evaluate the intervention effects, the self-worth scale were examined before and after the intervention for both experimental and control group. The result shows that the self-worth improved significantly in the experimental group(p < 0.001), whereas it remained unchanged in the control group(p > 0.459). This finding proves that the proposed LBMI program ameliorates the self-worth of the personal assistants for the disabled.

  • This research is to study the psychological, emotional, physical pains of the spouses of gambling addicts and the recovery process from the suffering. From this, we expect to propose a supportive program to help the recovery. Nine of the spouses of gambling addicts joined this research and the hermeneutical grounded theory methodology was applied to interpret it. Data were collected by the depth interview and various written records and they were analyzed by Rennie(2007)'s methodology. We could construct 20 categories by analyzing and categorizing meaningful units of the data. The core theme of the recovery experience was presented as "Stopping rolling the Sysyphus's boulder, and returning to the routine peace." Based upon the research outcomes, we suggested a supportive program considering male and female differences and a way to improve the accessibility to the therapy system for the spouses of gambling addicts and the need for a meaning therapy.

  • This study aims to analyzed the treatment factors in the reading activities in order to examine the possibility for such factors to applied to the actual life of the old, and sought for the utilization and direction of the reading program that is presented as the treatment and prevention of the emotional problems of the old people. Considering such circumstances, involving in reading activities improves the quality of the old people's life by meeting their internal desire by themselves and giving the meaning to their life with creativity. In addition, the reading activities helps the old people to express in-depth conflicts, emotion, desire and thinking that they are routinely experiencing, thus leading to the treatment of their agony, trauma, and pain. In such sense, it is very meaningful for the elderly to be joined in any reading activities.

A Phenomenological Study on Students' Experiences of Flipped Learning-Based Class of Sensory Integration Therapy (대학생의 플립드 러닝 기반 감각통합치료 수업 경험에 관한 현상학적 연구)

  • Objective : The purpose of this study is to investigate the meaning of students' experience participating in the flipped learning based instruction in sensory integration, and to understand the demand and introspection of participants. Methods : This study used a phenomenological approach of qualitative study. The participants were 10 students in 3rd year of the occupational therapy program in K Univeristy. Data information was collected by one-to-one interview and analyzed through phenomenological research method. Results : Through the interview, 20 units of meaning, 8 central meanings, and 3 themes were drew. The information collected were analyzed into three themes; Learning Experiences in Online and Offline Courses, Request and Introspection of Learners on Flipped Learning. The result showed that online courses brought learners convenience and satisfaction with repeatable learning in every time and space the learner want. However, the learners appealed issues of communication and concentration due to the absence of face-to-face instruction by their instructor. For the offline courses, students showed interest in various practical classwork of sensory integration and changes in their attitude to actively engage in the practical classes. Conclusion : Flipped learning based instruction was effective for the sensory integration classes which require practice in terms of time securement and immersion in practice. The learners requested for adopting flipped learning based instruction to other subjects in occupational therapy curriculum, and introspected that they needed to actively engage in classes through the experience of flipped learning-based classes of sensory integration. The results of this study can be used as a basic resource when flipped learning classes are planned in occupational therapy education.

새로운 항생제 치료에 관해서

  • 최근 수의학에서도 여러 항생제에 내성이 있는 세균 감염이 확인되면서 기존에 사용되었던 항생제에 의한 치료 반응이 떨어지고 새로운 항생제가 도입되고 있다. 그러나 항생제 내성을 가진 세균의 출현을 방지하고 항생제에 의한 부작용을 최소화하기 위해 적절한 항생제 사용은 필수적이다. 세균 배양과 항생제 감수성 검사를 기본으로 항생제에 있어서의 약동학(pharmacodynamics)과 약역학(pharmacokinetics)을 바탕으로 항생제 치료에 대해 보다 정확히 접근할 수 있다. 세균에 대한 최소억제농도(minimum inhibitory concentration, MIC)와 최고혈장약물농도(peak plasma drug concentration, C max)를 비교하는 것은 항생제 선택의 기준이 된다.또한 MIC가 C max에 가까울수록 효과적인 치료를 위해서는 가능한 높은 용량을 사용해야 함을 의미한다.

  • 결핵은 적절한 처방에 따른 약제를 일정기간동안 규칙적으로 복용하면 완치되는 질환이다. 약을 제대로 복용하지 않을 경우에는 치료실패와 재발이 되며 약제 내성균의 전파, 경제적 손실 그리고 사망까지 초래한다. 따라서 결핵환자의 순응도는 치료처방에 따라 약을 규칙적으로 주워진 기간동안 복용하는 환자의 행동을 의미하며 이것이 결핵치료의 성공여부를 결정짓는 가장 중요한 요인이다. 그래서 결핵치료에 있어서 보다 관심을 기울여야 할 사항은 결핵약제보다는 환자의 순응도를 높이는 데 있다. 이번 달에는 환자에게 교육을 시키므로 내원 약속일을 지키게 하는 등 환자의 순응도를 높이기 위한 방안들을 서술한다
