치과에서의 의료사고와 의료분쟁의 이해

  • 건강과 의료에 관심이 높아지면서 의료수요는 날로 증가하게 되었고 이에 따르는 의료분쟁 또한 증가하는 추세이다. 본의 아니게 발생한 의료사고는 의료분쟁으로 발전하게 되는데 과실과 악결과 그리고 이에 따르는 인과관계와 책임여부를 따지게 된다. 이를 규명하기 위해서는 의료인은 주의의무와 설명의무를 잘 수행했는지와 환자 또한 의무를 잘 수행했는지를 평가하게 된다. 일단 의료분쟁이 생기지 않도록 최선의 진료와 환자와의 대화가 요구되며 분쟁시 화해, 조정, 소송 등으로 해결하며 이 과정 각각에서의 준비와 대책을 세워야 할 것이다. 현재의 상황에서 언제 어디서 일어날지 모르는 의료분쟁을 방지하려면 진료기술의 숙련도를 높이고 발전하는 새로운 의학 정보를 얻는데 게으르지 말아야하며 특히 환자를 대하는데 진단, 치료과정, 치료의 후유증, 위험성에 관한 자세한 설명을 통하여 환자 스스로 결정할 수 있는 의료환경이 요구되며 가장 기본적인 의료기록부 작성에 관심을 가져야 할것이다.

  • The purpose of the present study is to theoretically assess IT Implementation Model of Cooper and Zmud (1990) in a hospital IS use context. A case study was applied to analogical study by interview from several end-users of the information systems at a university hospital. This study presented an EMR(Electronic Medical Record) systems how is initially implemented at an initial stage, continually adopted, adapted, accepted at an adoption stage, and finally rountinized and infused into an organization. Our study also elaborated IT Implementation Model as defining EMR development and its impact on nature of IS use in a hospital. This case study explained the characteristics of EMR and hospital organization context conceptually.

  • In this paper, we proposed the vehicle data format of digital tachograph to improve the vehicle data format of the MOLIT(Ministry of Land, Infrastructure and Transport). The vehicle data collection and analysis is currently available through digital tachograph mandatory installation for commercial vehicles. And public agency is used the vehicle data format as a transmission format defined by MOLIT's guideline for their vehicle data processing. However, Vehicle data format is different for each manufacturer and MOLIT's vehicle data format is also need to advance. We confirmed the proposed the vehicle data format improved based on the vehicle data format of MOLIT through the system implementation and experiment in this paper.

A Study on the Protection of Personal Information in the Medical Service Act (의료법의 개인정보보호에 관한 연구)

  • There is a growing voice that medical information should be shared because it can prepare for genetic diseases or cancer by analyzing and utilizing medical information in big data or artificial intelligence to develop medical technology and improve patient care. The utilization and protection of patients' personal information are the same as two sides of the same coin. Medical institutions or medical personnel should take extra caution in handling personal information with high environmental distinct characteristics and sensitivity, which is different from general information processors. In general, the patient's personal information is processed by medical personnel or medical institutions through the processes of collection, creation, and destruction. Still, the use of terms related to personal information in the Medical Service Act is jumbled, or the scope of application is unclear, so it relies on the interpretation of precedents. For the medical personnel or the founder of the medical institution, in the case of infringement of Article 24(4), it cannot be regarded that it means only medical treatment information among personal information, whether or not it should be treated the same as the personal information under Article 23, because the sensitive information of patients is recorded, saved, and stored in electronic medical records. Although the prohibition of information leakage under Article 19 of the Medical Service Act has a revision; 'secret' that was learned in business was revised to 'information', but only the name was changed, and the benefit and protection of the law is the same as the 'secret' of the criminal law, such that the patient's right to self-determination of personal information is not protected. The Privacy Law and the Local Health Act consider the benefit and protection of the law in 'information learned in business' as the right to self-determination of personal information and stipulate the same penalties for personal information infringement such as leakage, forgery, alteration, and damage. The privacy regulations of the Medical Service Act require that the terms be adjusted uniformly because the jumbled use of terms can confuse information subjects, information processors, and shows certain limitations on the protection of personal information because the contents or scope of the regulations of the Medical Service Law for special corporations and the Privacy Law may cause confusion in interpretation. The patient's personal information is sensitive and must be safely protected in its use and processing. Personal information must be processed in accordance with the protection principle of Privacy Law, and the rights such as privacy, freedom, personal rights, and the right to self-determination of personal information of patients or guardians, the information subject, must be guaranteed.

  • Medical record is part of the personal information that values the dignity and value of an individual, and can lead to serious social prejudice and disadvantage to an individual when it is breached illegally. In addition, the medical record has been highly threatened because its value is relatively high, and external threats are continuing. In this paper, we propose a medical record sharing framework that guarantees patient's privacy based on blockchain using ciphertext policy-based attribute based proxy re-encryption scheme. The proposed framework first uses the blockchain technology to ensure the integrity and transparency of medical records, and uses the stealth address to build the unlinkability between physician and patient. Besides, the ciphertext policy attribute-based proxy re-encryption scheme is used to enable fine-grained access control, and it is possible to share information in emergency situations without patient's agreement.

Overweight & Obesity in Children & Adolescents with Autism Spectrum Disorders - Retrospective Chart Review - (자폐스펙트럼장애(Autism Spectrum Disorder) 소아청소년에서의 과체중 및 비만 연구 - 후향적 의무기록 조사 -)

  • Objectives : In children and adolescents, the prevalence of overweight has increased in the last 20 years. little research is available on the prevalence of obesity in children with autism spectrum disorders(ASD). The purpose of our study was to determine the prevalence of overweight among a clinical population of children diagnosed with autism spectrum disorders(ASD). Methods : Retrospective chart review of children ages 3-18 years seen between 2012 and 2015 at a Konyang University hospital psychiatric clinic. Diagnostic, medical, and demographic information was extracted from the charts. Body mass index(BMI) was calculated from measures of height and weight recorded in the child's chart. The Center for Disease Control's BMI growth reference was used to determine an age and gender-specific BMI z-score for the children. Results : In our study, children with ASD compared to non ASD group had significantly higher BMI percentiles (p=0.032). The prevalence of overweight($BMI{\geq}85th$ to 95th percentiles) and obesity($BMI{\geq}95th$ percentiles) was 35% and 19% respectively in children with ASD and without ASD. Conclusions : Despite noted limitations, our data suggest that overweight and obesity in children with ASD are more prevalent than without ASD. Longitudinal and further study is needed to examine the factors associated with obesity in this population.

  • According to the article of inventory in the act of public records management from $29^{th}$ January on 1999, record repository, special repository and the archives have been doing inventory totally once every two years. As the archives have acquisited incredibly and explosively lots of archives, this has induced a lot of problem in inventory activity due to legacy inventory acts. To solve these problems, the author have revised and legislated on $29^{th}$ Oct. 2016. The purpose of this study is to point out the problems that prior uniformal inventory cycle and to explain in detail that the contents of the newly revised article, and a benefit and expectations of practical inventory activity. The major revision of article is inventory cycle of at least totally once every four years and the act of obligation on a after-process of inventory.

  • The LEX system is a XML-based framework for medical information consolidation. The Lex makes it possible for heterogeneous HISs(Hospital Information Systems) exchange and share HL7 messages by storing the messages into a single Central Clinical Database. In this paper, we propose a HL7 message server independently interoperable from existing HIS to generate HL7 messages, and design an XML database schema suitable for storing and manipulating such data. We also propose a new DTD for efficient transformation of HL7 messages to XML documents for storage saving as well as supporting patient-oriented information retrieval.

  • SNOMED CT is a standard clinical terminology to provide a standardized way to record and manage clinical records for EMR or EHR. The structure of SNOMED CT is very complex because of huge expressive power and its internal design mechanism. Although there are some SNOMED CT browsers for browsing and searching SNOMED CT concepts, they are less effective because they do not consider such complexity of SNOMED CT. This paper depicts problems of current SNOMED CT browsers and analyze SNOMED CT dataset. Then, this paper analyze requirements of SNOMED CT browser which improves searching and selecting of appropriate clinical terms.

A Study on the Health Screening Solution by Using Electronic Medical Record (전자의무기록을 활용한 건강검진 솔루션에 관한 연구)

  • The Electronic Medical Record(: EMR) is to store medical data not in the form of document, but in the data storage. Such EMR can not only solve various problems of document use such as storage/arrangement of and securing space for document, but also make it possible to provide customized-treatment based on large quantity of customer data, so that hospitals can reduce the management cost and also improve the work efficiency. Customers also can receive the great quality of medical service. Owing to such strengths, the EMR has been rapidly introduced and applied to many hospitals and clinics since 1990s. In case of the current health screening system, however, paper forms used for health screening is also stored, on top of EMR. There would be various reasons why it is stored in the form of document. While the EMR used in hospitals is comprised of a unit program performing medical record, the health screening system is comprised of a unit program performing logics related to health screening. For this reason, it might be unavoidable for the health screening system to store document forms. If the EMR function is applied to the health screening system, it is expected to be able to operate more efficient health screening solution.
