Effect of Unmet Healthcare Needs on Quality of Life (미충족 의료 경험이 삶의 질에 미치는 영향)

  • The purpose of this study was to investigate the potential causal relationship between unmet healthcare needs and metrics related to quality of life. To examine the relationships between these variables, the study conducted an analysis by processing additional irradiated raw materials of the Korean Medical Panel. The metrics related to quality of life included six variables: athletic ability, self-care, daily activities, pain/trouble, uneasiness/depression, and subjective health conditions. Linear regression analysis revealed that the unmet healthcare needs had statistically significant negative effects on all six quality of life variables. A review of the magnitude of the linear regression β values for those variables showed that the relative level of influence on the quality of life variables decreased in the following order: pain/trouble, subjective health conditions, uneasiness/depression, daily activities, athletic ability, and self-care. Based on the results, practical applications related to strengthening working-level links between health and welfare is considered an effective policy response to the continued presence of unmet healthcare needs; such applications could contribute to improving the quality of life of those with unmet healthcare needs.

  • Medical institutions remodel existing hospital buildings rather than construct buildings to satisfy patient satisfaction and enhance competitiveness with other medical institutions. Medical institution remodeling is gradually increasing due to enhanced laws and systems to improve the quality of medical services and the level of patient safety. However, prior studies were discussed only within a limited range, including architectural elements, about medical institution remodeling. Therefore, this study aims to provide basic data to medical institutions planning future remodeling by examining the legal issues of medical institution remodeling in various ways. Medical institution remodeling extends the construction period due to restrictions on construction conditions, and causes damage such as noise and dust to patients. So it is necessary to prevent damage to patients in advance and to support appropriate remodeling of medical institutions. Therefore, it is necessary to prepare an Act on promotion of and support for remodeling of medical institutions. In addition, medical institutions should prepare a checklist for voluntary compliance with construction standards, preventive measures and post-improvement measures, and measures for infection and radiation exposure in hospitals.

  • The efficiency and quality of healthcare services rely heavily on the secure protection and transparent management of individuals' medical information, which is becoming increasingly important in the digital age. To address this issue, we propose a distributed identity management (DID) and electronic medical record (EMR) integration system based on Hyperledger Indy, which aims to ensure the ownership of medical information to individuals and increase the accessibility and utilization of medical information. The system will allow individuals to manage their own medical information and share it transparently when necessary, which will improve the efficiency of healthcare services. In addition, the system will securely protect and transparently manage medical information, increasing the transparency of medical services and strengthening individuals' control over their medical information. Thus, the system will contribute significantly to improving the quality of medical services, protecting individuals' medical information, and improving the efficiency of medical services.

국가적 수준의 치과의사 역량을 위한 치과마취과학의 교육역량과 치과의사국가고시 제고

  • 치과마취학의 교육목표는 치과치료 시 적절한 환자평가와 함께 동통 및 불안을 관리하기 위한 약품선택과 치과치료를 위한 흡입법과 정맥주사를 이용한 의식하진정법과 언제, 어떻게, 어디로 전신마취를 의뢰하는지를 알아야 할 사항과 특히 의학적 병력을 가진 환자와 특별한 관리가 필요한 소아 노인 장애인의 전반적 이해와 치료 전 전신적 환자평가를 할 수 있는 능력과 치과진료 중 일어날 수 있는 의료적 응급 상황과 치과응급상황을 식별하고 인공소생술을 포함한 치과 치료 과정에서 발생할 수 있는 의학적 응급상황들에 적절한 관리를 할 수 있어야 한다. 치의학교육의 최소한의 질 보장을 위한 국가적 수준의 치과의사 역량체계를 구축함으로써 치의학교육의 질 향상을 위한 치과대학(원)의 변화를 자극하고, 치과의료와 의료인력의 질적 증진 및 치과의료행위의 최소수준을 보장하는데 있다. 하지만 현행 국가고시에 있어 치과마취과학은 구강외과 과목에 포함되어 위 기술한 치과마취과학의 교육목표를 달성하는 데 어려움이 있다. 그러므로 국가수준의 치의학교육의 질에 대해 제고하고 학교의 교육과정 변화와 개선에 적용하고 국시원의 새로운 과목의 도입과 실기시험을 통하여 치과마취과학의 교육목표를 달성하는 데 목적이 있다.

Service Quality of Dental Institutes Measured by SERVQUAL Model and Intention to Re-visit (SERVQUAL 모형으로 측정한 일부 치과의원의 서비스 질과 재이용의도)

  • The main purpose of this study was to identify the service quality of some dental clinics located in Gwangju and find an association between service quality and patient's intention to re-visit the same dental clinic. The search objects were 197 patients from five dental clinics, and data collected by self-administered questionnaire which was composed general characteristics, characteristics relating to visit of clinics, quality of service, patients satisfaction and intentions to re-visit. The used statistical analysis to find the association between quality of service and intention to re-visit was multiple regression analysis. This study looked at relations between quality of dental service, service satisfaction and intentions to visit again, and found that quality of service including six service areas had a statistically significant positive correlations with satisfaction and intentions of re-visit. When this study conducted a multiple regression analysis to identify variables that influence intentions of re-visit, it found that patients revisited the dental clinic only when quality of service was included. In conclusion, quality of service evaluated by patients influenced on their intention of re-visit and especially. Therefore, this study concludes that improvement in satisfaction and intentions of re-visit relies on Accessibility, that is, providing of medical information, consultant telephone, how delivery the dental medical system is.   • This study was to investigate the relationship between safety & quality management changes of patient and changes in management activities based on hospital workers in five mental hospitals and five geriatric hospitals which should be required medical certifying authorities. As a result of the research study, participation whether or not of certification service of mental hospital & geriatric hospital workers was positive correlation to improve change of 'Performance level of Safety Activities for the patient' 'Provide the High Quality Medical Service for the patient' 'Respect the Rights and Responsibility of the patient' 'Performance level of Infection Control Activities' out of contents of Patient Safety & Medical Service Quality. Also developmental changes of Safety Activities for the patient Hospital Quality for the patient Rights and Responsibility of the patient out of contents of Patient Safety & Medical Service Quality need to the Capacity Management Activities through Education and training, and Medical System & Evaluation of Management Provide the High Quality Medical Service for the patient out of contents of Patient Safety & Medical Service Quality need to the need to the Customer Orientation Process.

The Impact of Medical Utilization on Subjective Health and Happiness Index and Quality of Life according to the Economic Level of the Elderly (노인의 경제적 수준에 따른 의료이용이 주관적 건강수준과 행복감 지수 및 삶의 질에 미치는 영향)

  • The purpose of this study was to find concrete measures to improve the subjective health level, happiness and quality of life of the elderly according to economic level and to propose social and policy alternatives accordingly. As a research method, 63,929 elderly people aged 65 or older were surveyed using the Community Health Survey (Indicator Bank) _v09, and the frequency of health use by economic level, subjective health level, euphoria and quality of life Analysis and Chi square analysis and independent t-test. Multi variate logistic regression analysis was performed with subjective health level as a dependent variable and multiple linear regression analysis was performed to determine the factors affecting euphoria and quality of life. The results of the study are as follows. In the case of recipients, medical use was lower than that of non-recipients, lower education level, female age of 75 years or older, and less stress, In case of present or past recipients, the result of non - receipt increased as the subjective health level was worse, and the non - recipient had higher euphoria and quality of life. As a result, there is a need for alternatives to increase opportunities for medical use among the recipients, with particular attention being paid to women and elderly people over 75 years old. It is expected to be used as a basic data to effectively improve the health promotion, happiness and quality of life of the elderly people of low income group.

  • 문제: 환자가 수술장 내에서 수술을 기다리면서 느끼는 불안감을 최소화할 수 있도록 해야 하지만 수술실의 효율성 및 의료진 편의성 위주로 운영되고 있어 수술실 내 대기시간이 연장되고 있다. 목적: 수술장 내 대기시간을 단축시키기 위해 수술환자의 이동경로에 따른 지연요인과 문제점을 파악하고 개선하여 환자가 수술을 기다리면서 느끼는 불안감을 최소화하고자 한다. 의료기관: 부산시에 소재한 481병상의 종합병원 수술실 질 향상 활동: 수술환자의 대기시간 지연요인과 문제점을 파악하고 개선을 통해 질 향상을 도모하였다. 개선효과: 대기시간 수행율에서 수술장 도착까지의 수행율이 개선전 95%에서 개선후98%로, 수술방 입실까지의 수행율이 개선전 88%에서 개선후 94%로, 마취시작까지의 수행율이 개선전 93%에서 개선후 96%로 수행율이 높아졌다.

Attitudes toward Quality Improvement Activities of QA Committee Member Physicians in Korean University Hospitals (대학병원 질 향상 관련 위원회 소속 의사들의 의료 질 관리 활동에 대한 인식 및 태도)

  • Background : The purpose of this study was to understand general attitudes of physicians toward hospital quality improvement activities who have been members of QA committee in 32 Korean university hospitals. Methods : A postal survey about opinions of hospital quality improvement activities and desirable policy directions was sent to 328 QA committee member physicians. The questionnaires were composed of 48 items. In total 152 physicians responded to this survey(response rate: 49.3%). This study was carried out from January to February 1996. Results : Most physicians(97.5%) recognized the necessity of hospital QA activities. The most dominant supporting reason for quality activities was to improve clinical outcome. Two thirds of physicians regarded their own hospital activities for quality improvement as inactive. They considered that the obstacles were too little concern(33.6%), unclear objectives(28.9%), lack of human resources(14.3%), and insufficient education and training for quality improvement(10.1%). The most favoring policy among respondents was to give health care providers economic incentives. Provision of education and training for implementing quality improvement was the next to it. Physicians revealed their preferences for professional society, government, health insurance societies, consumer groups, hospital labor unions, and mass media in sequence as sponsoring agencies for hospital accreditation program. Conclusion : These study suggested that the concrete means for motivating physicians and promoting constructive partnership among related parties should be developed in order to activate current hospital quality activities in Korea.

  • 우리나라에서 노령화가 급속히 진행됨에 따라 근골격계(뼈 및 관절부분)의 기능 장애는 개인의 삶의 질을 심각하게 저하시킬 뿐만 아니라 개인 및 국가 경제에도 큰 손실을 초래하고 있다. 현재 국내에서 사용되고 있는 근 골격계의 재생/재건/재활 관련 의료기기 대부분은 수입에 의존하고 있다. 고가의 수입제품의 사용은 심각한 경제적 손실 뿐 만 아니라 환자의 의료비 부담에 따른 양질의 의료혜택에도 지장을 주고 있다.(중략)
