The Development of Vital Sign Web Viewer Systems using HL7 Protocol (HL7 프로토콜을 이용한 생체정보 웹 뷰어 시스템 개발)

  • HL7 is well-Known standard protocol for text data generated in hospital information systems. Vital sign information web viewer systems is also the standard protocol for medical image and transfer. In order to embrace new technologies as telemedicine service, it is important to develope the standard protocol between different systems in the hospital, as well as the communication with external hospital systems. In this paper, we proposed integration method between vital sign web viewr systems and hospital information systems. Through the proper data exchange and modification of information management, HIS will offer better work flow to all hospital employee.

  • The healthcare service industry has become one of the business industries in South Korea where service design is most actively being researched on and applied. In accordance with the recent upsurge of the interest in health, healthcare service is expanding its area including disease prevention, patient management, and rehabilitation treatment as well as cure and nursing care. The health manpower is the supplier, and their professional knowledge and ability and the patients' trust in medical technology are the most important factors for their customers. In addition, service design has come into the spotlight given that the medical institute system, health manpower attitude, and information delivery system and touch point are considered important factors contributing to customer satisfaction. It is very hard to satisfy customers only through professionalism, the environment, and product improvement because healthcare service deals with much more sensitive and emotional customers compared to other service industries. This means that a change in the service mind-set and the attitude of the health manpower as emotional labourers have practical effects. Therefore, the fundamental solution is to establish a system that provides related education with manpower and that settles various problems by itself. This paper introduces several solutions, such as education for health manpower and a service design system applied to a national-university-affiliated hospital in South Korea, and takes a close look at its effects.

  • HL7 is well-known standard protocol for text data generated in hospital information systems. Vital sign information web viewer systems is also the standard protocol for medical image and transfer. In this paper, we have to design to obtain useful vital sign information, which is generated at data receiver modulor of HIS, that is offered by the central monitor. Vital sign informations of central monitor is composed of the row data of several bedsite patient monitors. We are willing to maintain vital sign information of real time and continuity that is generated from the bedsite patient monitor. It is able to apply to remote medical examination and treatment. we proposed integration method between vital sign database systems and hospital information systems. Through the proper exchange and management of patient vital sign information, real time vital sign information management will offer better workflow to all hospital employee.

  • According to the recent increasing trend of the ages, numbers of patients with chronic diseases are increasing and issues for health care are importantly emerged. In this thesis the research implements U-health care system for health care of patients with chronic diseases. The suggested system for health care of patients with chronic diseases composes bio measurement system, mobile gateway and medical information management server, and bio-signals are composed with modules such as electrocardiogram, blood pressure, blood sugar, oxygen saturation if configured as client. Blood sugar check was considered and implemented to be chosen the ways to transmit through bio measurement system or through gateway. Suggested bio measurement system and mobile gateway are transmitted through Bluetooth. The transmitted biodata is searched by observing health check through mobile gateway, by transmitting through network server, and by using client. By implementing bio signal observation system of patients with chronic diseases, present health check is available by monitoring measured bio data, and various bio signals are transmitted in the mobile environment.

  • Management of biosignal data in mobile devices causes many problems in real-time transmission of large volume of multimedia data or storage devices. Therefore, this research paper intends to suggest an m-Health system, a clinical data processing system using mobile in order to provide quick medical service. This system deployed health system on IP network, compounded outputs from many bio sensing in remote sites and performed integrated data processing electronically on various bio sensors. The m-health system measures and monitors various biosignals and sends them to data servers of remote hospitals. It is an Android-based mobile application which patients and their family and medical staff can use anywhere anytime. Medical staff access patient data from hospital data servers and provide feedback on medical diagnosis and prescription to patients or users. Video stream for patient monitoring uses a scalable transcoding technique to decides data size appropriate for network traffic and sends video stream, remarkably reducing loads of mobile systems and networks.

  • Radiation-related practitioners and radiation-treated patients at medical institutions are inevitably exposed to radiation for diagnosis and treatment. Although standards for maximum doses are recommended by the International Commission on Radiological Protection (ICPR) and the International Atomic Energy Agency (IAEA), more direct and available measurement and analytical methods are necessary for optimal exposure management for potential exposure subjects such as practitioners and patients. Thus, in this study we developed a system for real-time radiation monitoring at a distance that works with existing portable device. The monitoring system comprises three parts for detection, imaging, and transmission. For miniaturization of the detection part, a scintillation detector was designed based on a silicon photomultiplier (SiPM). The imaging part uses a wireless charge-coupled device (CCD) camera module along with the detection part to transmit a radiation image and measured data through the transmission part using a Bluetooth-enabled portable device. To evaluate the performance of the developed system, diagnostic X-ray generators and sources of $^{137}Cs$, $^{22}Na$, $^{60}Co$, $^{204}Tl$, and $^{90}Sr$ were used. We checked the results for reactivity to gamma, beta, and X-ray radiation and determined that the error range in the response linearity is less than 3% with regard to radiation strength and in the detection accuracy evaluation with regard to measured distance using MCNPX Code. We hope that the results of this study will contribute to cost savings for radiation detection system configuration and to individual exposure management.

  • u-Health 서비스는 의료기술에 ICT을 접목하여 보다 나은 건강관련 서비스를 제공할 수 있는 패러다임이다. 그러나 이러한 시스템을 도입하기에는 현실적으로 많은 도전이 존재한다. 개인의 건강관리 도구로 존재할 수 있을 것처럼 보이는 이러한 서비스는 그것이 국민의료의 질과 연관되는 이슈로 확대될 때, 환자라는 소비자집단, 의료인이라는 공급자집단, 그리고 국가라는 보건행정을 담당하는 관리집단의 이익이 첨예하게 대립할 수 있는 이슈가 될 수 있다. 이에 본 연구에서는 이러한 서비스의 제공을 이러한 개개 이익집단의 관점에서 탈피하고 순수한 양질의 의료 서비스 제공의 관점에서 살펴보아 과연 어떠한 ICT 관련 서비스 제공이 의료 환경 제고를 위해 필요한지를 파악해 보고자 하였다. 특히, 원격지에서의 자료공유를 통한 의료 서비스의 질 제고에 대해 관심을 가지고 이를 이익집단간의 이해관계가 크지 않은 ODA국가를 대상으로 하여 적용할 수 있는 서비스 모델을 제안하고자 하였다.

Implementation of the Electronic Prescription Security System Using by an If Card (IC 카드에 의한 원외 전자처방전 보안을 위한 시스템 구축)

  • Nowadays, a patient's private medical data which is exposed to the outside world has a severe effect on not only the patient's private life but also his/her social activities and environment. So, it is important to securely protect the patient's private medical data from the illegal manipulation. This paper studies the method to store the electronic prescription information in an IC card. For that, an access control for users, such as a doctor, a nurse, a medical institute member, a pharmacy, a pharmacist, or a patient, is proposed to access the data stored in an IC card. The certificate is issued using the Crypto API of a certificate management model supported by Windows 2000. The public/private key is created by the Cryptographic Service Provider program, and the electronic prescription is signed using the digital signature. The proposed system, therefore, can improve the quality of medical services by securing the safety and integrity of the electronic prescription, stored in an IC card.

  • Recent changes in health care environment including aging population and prevalence of chronic disease encourage the adoption of new innovative technological solutions including wearable vital sensors, wireless networks, and smart phone. In this paper, we present an effective at-home lifestyle monitoring system that can be used for self-management and health intervention of patient himself in the Management-by-Exception perspectives. We implemented the filtering and queuing algorithms as a preprocessor of monitoring system to enhance efficiency of proposed system, and the effective UX design for self-management of patients themselves. The 94,467 actual clinic data was used to test the efficiency of the proposed system. As as a result, 64.8% of the incoming vital data was identified to be filtered out.
