우리나라 보건의료 정보체계와 정보화사업의 현황 및 문제

  • 국가 보건의료체계(health care system)가 올바르게 기능하기 위해서는 의료체계를 구성하는 각 요소들간의 유기적인 상호 작용이 대단히 중요하다. 정보체계는 이러한 유기적인 상호 작용을 위해 보건의료체계의 각 영역에서 꼭 필요한 시스템이다. 보건의료 서비스를 효율적으로 생산하기 위해서 투입하는 자원의 개발과 배치 체계는 물론 생산된 보건의료 서비스를 소비자가 효과적으로 이용할 수 있도록 하는 서비스 전달체계(delivery system)의 전과정에서 보건의료정보체계는 중요한 인프라가 되고 있다. 정보체계는 궁극적으로 보건의료체계가 지향하는 목적을 달성할 수 있도록 짜여져야 한다. 다시 말해, 의료체계의 각 하부영역에서 의료이용의 접근성(accessibility), 형평성(equity), 서비스의 의과학적 질(scientific technical quality), 포괄성(comprehensiveness) 등 보건의료체계가 추구하는 가치를 정보체계에서도 그대로 담아 낼 수 있어야 한다. 또한 정보체계를 구축하기 위한 요건으로 각 사업 영역별 표준화, 사업 영역간의 상호관계, 공공과 민간의 역할 분담, 정보 관련 시선과 인력, 정보기술 등의 인프라, 각 사업 영역별 정보화에서 담아야 할 정보 내용(contents, DB) 등에 대한 사전 기획이 수립되어야 한다. 그 동안 보건의료분야의 정보화사업은 1996년부터 보건소기관의 행정, 진료, 보건 업무지원을 위한 보건소정보화사업과, 1995년부터 초고속공공응용서비스 사업의 일환으로 추진해 온 전염병, 혈액유통, 장기이식, 외래진료예약, 원격진료, 응급의료, 원격치매진료 시스템 구축사업 등이 있으며, 응급의료, 혈액관리, 장기이식, 전염병정보, 외래진료예약시스템을 통합한 보건의료정보통합서비스시스템(1998년) 구축사업 등이 있었다. 이러한 사업들에 대해 현재 평가되고 있는 문제점들로는 정보화 사업에 대한 전략과 비젼이 부재한 상태에서 지나치게 정보기술 중심으로 추진되다 보니 보건의료 정보의 공유나 활용이 전혀 이루어지지 않거나 매우 제한적이었고, 시범사업의 경우는 초기 사업을 추진하기 위한 주체가 명확하지 않았고, 관련 인력의 훈련과 양성이 안 되었으며, 사업 확대를 위한 법적 기반의 취약, 정보화 사업이 부실한 데 대한 평가 기준과 대안 제시 등이 매우 취약한 것이 문제점으로 나타났다. 특히, 정보 내용과 DB 구축을 위한 표준화 등에 대한 사전 설계 부재는 개발된 정보시스템의 운영을 결정적으로 어렵게 만든 요인이 되고 있다. 전반적으로 기존의 정보화사업은 사업 추진을 위해서 꼭 필요한 비젼 수립, 비젼에 따른 목표 설정, 전략 개발, 관리체계 구축, 운영방안 수립 등 단계적인 정보화 계획이 부재한 것으로 평가되고 있어 보건의료정보체계 구축을 위한 종합 비젼 기획(ISP)이 시급한 실정이다.

Convergence Evaluation of Indoor Air Measurement in Medical Institution (의료기관 실내공기 측정의 융합적 평가)

  • The purpose of this study was to investigate the indoor air quality of medical institutions and to present the detailed management plan by analyzing the actual situation of management and analyzing the difference according to the place in a convergent. ($PM_{10}$), carbon dioxide ($CO_2$), formaldehyde (HCHO), total aerobic bacteria (TAB) and carbon monoxide (CO) were investigated in 153 hospitals in Busan. As a result, The overall output average did not exceed the maintenance criteria, but the maximum values for each item were 91% for $PM_{10}$, 97% for $CO_2$, 96% for HCHO, and 99% for bioaesol. And It is confirmed that this is very close to the standard. In addition, the results of each place showed different values according to each feature, but there was no significant difference in the results of five measurements. In conclusion, Indoor air in medical institution considering the characteristics should be performed systematic and detailed management of ventilation facilities and energy problems.

  • Computed tomography (CT) has been established as an important diagnostic tool in clinical medicine and has become a major source of medical exposure. A nationwide survey regarding CT examinations was carried out in 2007. Thanks to the appeasement policy regulating the import of CT scanners, there are 1,825 CT scanners across the country as of the end of March 2010, which means that we have 36.8 CT scanners per one million people. The annual number of examinations was 3.29 million, the number of examinations per 1000 population was 68. The most part of examinations was abdomen and pelvis. and the collective effective dose was in these parts. The effective dose per one population was evaluated as 0.952 mSv.

The Analysis of Hospital Characteristics affecting Blood Transfusion to the patients under Knee or Hip Total Replacement Arthroplasty (슬관절과 고관절 전치환술 환자의 수혈에 영향을 미치는 병원특성 요인 분석)

  • The problems related with blood supply shortage and the stability of blood transfusion are on the rise, as it is expected that the blood doners will decrease but the blood use amount for aged population will increase, owing to low birth and aged population increase. The purpose of this study is to analyze the hospital characteristics which affect patients receiving and non-receiving blood transfusion for the knee and hip total replacement arthroplasty. Data were collected from Health Insurance Review and Assessment Service's 2011 sample data, and 5,370 inpatients were abstracted from them. Logistic regression analysis was performed, using SPSS 20. Independent variables used are hospital characteristics variables and patient characteristics variables. Hospital characteristics variables are hospital type, ownership, residence and the number of usable beds, and patient characteristics variables are gender, age, severity, type of anesthesia, main diagnosis, whether or not of anemia and insurance class. At the result of this study, it was found that hospital type, region, gender, age, severity, main diagnosis and whether or not of anemia were the factors that mostly affected the blood transfusion for knee arthroplasty. And hospital type, residence, gender, age, severity, type of anesthesia and whether of not of anemia were the factors that mostly affected the blood transfusion for hip arthroplasty. In addition to that, it is expected that this research which analyzed the present state of blood transfusion and its influence factors are cost effective, and would make a good use of preliminary data for good quality of medical service.

Dose Customized Apron Micro Functional Design Using Convergence Shielding Sheet (융합 차폐시트를 이용한 선량 맞춤형 에이프런 마이크로 기능성 디자인)

  • Radiation shielding clothing for medical institutions is used based on lead equivalent of 0.25 mmPb. However, this study intends to study the shielding suit that can guarantee the user's activity while considering the sensitivity of each part of the body. By manufacturing based on eco-friendly shielding material, it was attempted to solve the weight problem and environmental problem of existing lead aprons, and to present the same shielding performance as lead equivalent in thickness. The fabric of the produced shielding sheet was manufactured through a calendar process that adjusts the thickness of the shielding sheet from lead equivalent 0.12 mmPb to 0.32 mmPb. In addition, the usability evaluation of the manufactured shielding clothes was conducted for the subjects who were workers in medical institutions. As a result, the activity became easier and the weight was reduced by 0.26 kg. In the future, it is thought that it is necessary to improve the shielding suit design considering the activity.

Improving the Performance of Blood Transfusion Management Division (수혈관리실의 역할 수행에 따른 개선 효과)

  • The effect of improvement by keeping the former as the priority was examined through an evaluation of the role of the blood transfusion management (BTM) division. This division manages the status of blood for transfusion, evaluates the adequacy of blood transfusion (EABT), monitors and responds to the occurrence of transfusion-related side effects, and performs other tasks necessary for BTM. Although the establishment and operation of the BTM division can lead to the efficient evaluation of transfusion adequacy, there are disadvantages in that it takes time for EABT and it is difficult for the staff in charge of the BTM division to evaluate the adequacy of all the blood. In the future, it is essential to introduce a BTM division and committee specific to Korean patients to implement safe and appropriate BTM in medical institutions, and to assist medical institutions in training their personnel.

Workplace Friendship and Organizational Effectiveness of Dental Hygienists (치과의료기관 근무자들의 프렌드십과 조직효과성 관계 연구)

  • This study purports to measure the level of work friendship in dental clinic and examines the friendship's effect on the organizational effectiveness. Data were collected from workers who worked in dental clinic located in Seoul and Gyeonggi areas by self-administered questionnaires from early in October till lately in September, 2009 through direct interview and e-mail. Among 250 questionnaires, 240 responses were returned, and 17 copies with an inaccurate answer were excluded. Finally 223 responses were analyzed through SPSS program. The study revealed that the work friendship in dental clinic has enormous influence on job satisfaction, occupational commitment, intent to leave, stress etc. The results imply that the managers of the dental clinics need to create an organizational climate which emphasizes on a good relationship among members and have them take part in various committees or informal activities.

Effect of Organizational Support Perception on Intrinsic Job Motivation : Verification of the Causal Effects of Work-Family Conflict and Work-Family Balance (조직지원인식이 내재적 직무동기에 미치는 영향 : 일-가정 갈등 및 일-가정 균형의 인과관계 효과 검증)

  • This study aims to analyze the influence of organizational support perception of workers in medical institutions on intrinsic job motivation, and to check whether there is significance in the mediating effect of work-family conflict and work-family balance factors in this process. The results of empirical analysis through the questionnaire are as follows. First, it was confirmed that organizational support recognition had a significant positive effect on work-family balance as well as intrinsic job motivation, and work-family balance had a significant positive effect on intrinsic job motivation. Second, it was confirmed that organizational support recognition had a significant negative effect on work-family conflict, but work-family conflict had no significant influence on intrinsic job motivation. Third, in order to reduce job stress for medical institution workers, it is necessary to reduce job intensity, assign appropriate workload for ability. And in order to improve manpower operation and job efficiency, Job training and staffing in the right place are needed. Fourth, in order to improve positive organizational support perception and intrinsic job motivation, It is necessary to induce long-term service by providing support and institutional devices to increase attachment to the current job and recognize organizational problems as their own problems with various incentive systems. The limitations of this study and future research directions are as follows. First, it is believed that an expanded analysis of medical institution workers nationwide by region, gender, medical institution, academic, and income will not only provide more valuable results, but also evaluate the quality of medical services. Second, it is necessary to reflect the impact of the work-life balance support system on each employee depending on the environmental uncertainty or degree of competition in the hospital to which medical institution workers belong. Third, organizational support perception will be recognized differently depending on organizational culture and organizational type, and organizational size and work characteristics, working years, and work types, so it is necessary to reflect this. Fourth, it is necessary to analyze various new personnel management techniques such as hospital's organizational structure, job design, organizational support method, motivational approach, and personnel evaluation method in line with the recent change in the government's medical institution policy and the global business environment. It is also considered important to analyze by reflecting recent and near future medical trends.

A Study on the Evaluation of the Emergency Medical System in Seoul (서울시 응급의료체제에 대한 평가 연구)

  • The purpose of this thesis is to lay groundwork for the development of emergency care system in metropolitan area. It compares the performance and outcome of the current system with foreign counterparts and investigates the changing aspects of future medical environment. Emergency medical system can be divided into two parts of both pre-hospital care, which refers to the emergency measures taken before arriving at a hospital, and hospital care that is given within a hospital. Pre-hospital care includes on-the-spot expedients, information system and delivery system, whereas hospital care is related to the classification and specialization of medical care facilities. This research focuses on the evaluation of the performance of a rescue party, which is part of pre-historical care system. As a result, it provides valuable material for the development of the emergency medical system in Seoul.
