응급실에 내원한 만성 폐쇄성 폐질환 환자의 호흡곤란 속성에 관한 연구 (A Study on Characteristics of Dyspnea in Patients with Chronic Obstructive Pulmonary Disease in the Emergency Department Visits)

  • Purpose: The purpose of this study was to examine descriptors of dyspnea and clinical characteristics in patients with COPD at ED visit. Method: The study design was a descriptive survey, and 46 patients with COPD complaining of dyspnea were participated during their visits to the ED of C university hospital in G city, from October 1, 2004 to April 8, 2005. Result: The qualitative characteristics of dyspnea at the time of decision to the ED visit were short of breath (71.7%), tight (32.6%), hard to breathe (15.2%), smothering or suffocating (10.9%), gasping (8.7%), couldn't breathe (6.5%), constricted (2.2%), and hunger for air (2.2%). In the item of dyspnea checklists, My chest felt tight was the most common description among subjects. As the result of factor analysis of dyspnea checklists, the first factor was characterized rapid and shallow, the second was suffocating/smothering, the third was hunger for air, and the fourth was constricted. Conclusion: This study suggests that a checklist of dyspnea descriptors based on this findings would be utilized as a tool of initial and ongoing assessment for dyspneic patients with COPD in the ED after identifying the validity and reliability of the checklist.

응급실 간호사의 환자 의료 정보 보호행위에 대한 인식 및 실천 정도 (Perception and Performance of Emergency-room Nurse's Protection Behavior for the Consumer Health Information)

  • Purpose: The purpose of this study was to investigate the emergency-room nurse's perception and performance about the protection behavior of the consumer health information. Method: The participants were 191 emergency-room nurses in secondary, tertiary medical center nationally. The data were analyzed by SPSS 12.0 program. Result: First, the total average scores of the degree of the perception and performance by emergencyroom nurses on protection behavior of the consumer health information were $4.24{\pm}0.30$ and $3.51{\pm}0.26$. Second, the degree of the perception and performance on protection behavior of the consumer health information was heavily dependent on the following general characteristics of the participants: age, nurse position, career, and education experience on protection behavior of the consumer health information. Third, there was a positive correlation between the degree of perception and performance about the protection behavior of the consumer health information. Conclusions: The results showed the statistically significant partial correlation between the degree of perception and performance. Therefore, it is suggested to apply the concrete education program to enhance the perception in order to improve the performance.

중환자실과 응급실 간호사의 표준주의 수행에 영향을 미치는 요인 (Factors Influencing Compliance with Standard Precautions in Intensive Care Unit and Emergency Room Nurses)

  • Purpose: The purpose of this study was to investigate the level of knowledge, attitude and compliance with standard precautions and to identify predicting factors of compliance with standard precautions in ICU and ER nurses. Methods: The participants were 228 ICU and ER nurses working in one of three university hospitals and one general hospital. Collected data included general characteristics, features related to infection risk, and knowledge, attitude and compliance with standard precautions. For the final analysis, 218 questionnaires were used. Results: Within the previous one year, 103 (47.3%) nurses experienced pricking injury from syringe needles or other sharp materials, 111 (50.9%) nurses reported exposure to patients' blood and body fluid. In general, the scores for knowledge, attitude and compliance were all high. Compliance scores for nurses in intensive care units were significantly higher than those of nurses in emergency. There were significant correlations of knowledge, attitude, and compliance with standard precautions. Attitude and work place were significant factors predicting compliance with standard precautions. Conclusion: The results indicate that to increase nurse's compliance with standard precautions, continuous efforts to enhance positive attitudes and at the same time, improve work environments and use individualized approaches based on the work units are needed.

응급실 간호사의 업무 스트레스, 강인성 및 소진 간의 관계 (Relationship of Job Stress, Hardness, and Burnout among Emergency Room Nurses)

  • Purpose: The purpose of this study was to examine the relationship of job stress, hardness and burnout among emergency room nurses. Methods: The subjects were 110 emergency room nurses from three university hospital and three general hospital, with more than 500 beds, in G city and J region. Data collection was done from October 11 to November 12, 2010. Results: The score of job stress was 3.8 out of 5 point, the score of hardness was 2.6 out of 4 point, and the score of burnout was 3.3 out of 5 point. Burnout was found to be in a significant positive correlation with job stress and significant negative correlation with hardness. The main predictors of burnout were to hardness, job stress and night duty, and these variables explained 45.2% about burnout. Conclusion: In conclusion, the higher job stress was in higher burnout and the higher hardness was in the lower burnout. It is necessary to create improvement ways and programs to reduce and prevent burnout of emergency room nurses.

  • Purpose: The purpose of this study is to verify the factors that determines burnout of nurses in emergency department. Methods: The survey was given to 170 ED nurses which are located in Busan from July 1st to August 31st of 2012. The results were analyzed by descriptive statistics, t-test, ANOVA, pearson correlation coefficients and stepwise multiple regression using SAS 9.2 program. Results: There were statistically significant differences in burnout depending on gender, position, workplace satisfaction, job satisfaction. Significant positive correlation between compassion fatigue and burnout was found. Also significant negative correlation was found between self-esteem, compassion satisfaction and burnout. Factors influencing burnout were self-esteem, compassion satisfaction and compassion fatigue with $R^2$ value 58.0%. Conclusion: Considering these results, it seems that great efforts will be needed for reducing burnout by mitigating the emergency nurses' compassion fatigue and introducing programs to increase the self-esteem and compassion satisfaction.

응급실 간호사의 외상후 스트레스, 직무 스트레스, 이직의도의 관계 (The Relationship of Post-traumatic Stress, Job Stress and Turnover Intention in Emergency Department Nurses)

  • Purpose: This study was done to identify relationships among post-traumatic stress (PTS), job stress and turnover intention in emergency department (ED) nurses. Methods: Participants were 250 ED nurses who had worked for one month or longer in the ED one of three university hospitals or seven general hospitals in Daegu City or Gyeong Buk Province in Korea. Structured questionnaires were used to measure PTS, job stress, and turnover intention of participants. Data were analyzed using descriptive statistics, one-way ANOVA, path analysis, and stepwise multiple regression with the SPSS program. Results: There were significant relationships between PTS, job stress and turnover intention. PTS influenced turnover intention directly and was indirectly mediated by job stress. The experience of traumatic events influenced PTS, job stress, and turnover intention. Indirect experience of traumatic events in the ED was an important predictor, explaining 20.1% of PTS in high-risk post-traumatic participants. Conclusion: PTS can be an important factor for job stress and turnover intention. The direct and indirect experience of traumatic events can influence PTS, job stress, and turnover intention in ED nurses. Based on these results, strategies for managing PTS and relating job stress are recommended to reduce turnover intention in ED nurses.

응급실 간호사의 전문직 자아개념, 역할갈등 및 업무만족도 간의 관계 (The Relationships of Professional Self-Concept, Role Conflict and Job Satisfaction on Emergency Department Nurses)

  • Purpose: The purpose of this study was to investigate the relationship between professional self-concept, role conflict and job satisfaction of emergency department (ER) nurse. Method: Data were collected from a convenience sample of 200 ER nurses who worked in one of 9 hospitals in 5cities. The instrument for this study was a structured questionnaire on professional self-concept, role conflict and job satisfaction. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and multiple regression. Results: The mean score for professional self-concept was 2.52 (0.28), for role conflict, 3.37 (0.55), and for job satisfaction, 2.71 (0.35). There were significant differences on three variables according to total career, and satisfaction with nursing. There was a significant positive correlation between professional self-concept and role conflict, job satisfaction. Role conflict showed a significant negative correlation with job satisfaction. Conclusion: The results of this study indicate that the important role-related variables of professional self-concept, role conflict and job satisfaction of ER nurses are significantly related, and that as, role conflict is an important factor for job satisfaction of ER nurses, strategies to decrease role conflict need to be developed.

응급실 중증도 분류 간호사의 업무 경험: 질적 연구 (Triage Nurses' Work Experiences in Emergency Department: A Qualitative Research)

  • Purpose: The purpose of the study was to describe work experiences of triage nurses working at emergency departments in Korea. Methods: A qualitative descriptive research method was used. Twenty-one nurses of emergency department participated in the first interview and 16 of them participated in the second interview. Data were collected through in-depth interviews and field notes. Qualitative content analysis was utilized to analyze the data. Results: Forty-eight codes, 12 categories, and six themes emerged from the data analysis: caring on the frontline, burden as an initial evaluator, handling things alone, lack of others' understanding, becoming an expert, and needs for improvement. Participating triage nurses faced many challenges and difficult situations in emergency departments. They felt burden and loneliness as initial evaluator. Also, they felt pride in their work and struggled to become a professional. Conclusion: This study is the first qualitative study that has focused on exploring the experiences of triage nurses in emergency departments in Korea. The results provide description and in-depth understanding of working experiences of triage nurses. The results provide valuable fundamental data for regulation, clinical practice, education, and research for triage nurses in Korean emergency departments.

중환자실과 응급실 간호사의 소진과 공감역량이 직무만족도에 미치는 영향 (Burnout and compassion competency to explain job satisfaction among nurses working in intensive care units or emergency rooms)

  • Purpose : This study aimed to examine the relationships of burnout and compassion competency with job satisfaction among nurses working in the intensive care units and emergency rooms. Methods : A correlational survey was conducted from August 1 to August 14, 2018 and involved in 102 nurses working at four university hospitals for more than 1 year. A structured questionnaire was used to measure burnout, compassion competency, and job satisfaction, and data were analyzed using hierarchical multiple regression and mediation effect analysis. Results : The mean scores of burnout, compassion competency, and job satisfaction were 2.58, 4.35, and 2.95, respectively. Burnout and compassion competency along with working night shifts and a wish to transfer explained 35.3% of variance in job satisfaction. Compassion competency showed a small but significant mediating effect in the relationship between burnout and job satisfaction (indirect effect -.0446, 95% CI -.1133, -.0075). Conclusion : Burnout and compassion competency were the significant influencing factors of job satisfaction among nurses working in intensive care units and emergency rooms. Since compassion competency showed a significant mediating effect, further studies should focus on intervention strategies for compassion competency to improve job satisfaction in this population.

간호사의 치매케어 관련 지식, 치매에 대한 태도 및 치매간호 접근법과의 관계 (The Relationship among Dementia care Knowledge, Attitudes toward to dementia and Approach to dementia care of Nurses)

  • 본 연구는 간호사의 치매케어 관련 지식, 치매에 대한 태도 및 치매간호 접근법에 대해 조사하고 관련 변수를 파악하기 위한 서술적 조사연구이다. P시에 소재한 G 병원의 남 녀 간호사 202명을 대상으로 구조화된 설문지를 이용하여 자료를 수집하였다. 연구결과 대상자의 치매케어 관련 지식은 11.02점, 치매에 대한 태도는 40.15점이었으며 치매간호 접근법은 7.90점이었다. 치매케어 관련 지식은 성별(t=-2.424, p<.05)과 교육수준(F=2.932, p<.05) 그리고 근무병동(F=2.268, p<.05)에서 통계적으로 유의한 차이가 있는 것으로 나타났다. 치매케어 관련 지식은 치매에 대한 태도와 유의한 양의 상관관계(r=.162, p<.05)가 있었으며, 치매케어 관련 지식에 영향을 미치는 변수는 응급실(${\beta}=-1.568$)과 중환자실(${\beta}=-1.682$)이었다. 본 연구결과 치매환자에게 질 높은 간호를 제공하기 위해서는 치매케어 지식의 향상과 치매에 대한 긍정적인 태도를 향상시키기 위한 융복합적인 교육프로그램의 개발이 필요하다.
