Infected Sclerosing Lipogranuloma after Hernioplasty: Ultrasonographic and MRI Findings (탈장성형술 후 음낭 내에 발생한 감염성 경화지방육아종의 초음파 및 자기공명영상 소견: 증례 보고)

  • We report the ultrasonographic and MRI findings of an infected sclerosing lipogranuloma after scrotal hernioplasty. Sclerosing lipogranuloma is a rare foreign-body reaction of fat tissue, with most cases being associated with the genital and urinary tracts. To the best of our knowledge, MRI findings in sclerosing lipogranuloma in the scrotal sac have not yet been published and this is possibly the first study to report the case of an infected sclerosing lipogranuloma in the English literature.

  • 옛날 유럽에서는 굴뚝 청소부의 음낭에서 암이 자주 발생했다. 1775년 영구의 의사 포트는 음낭암에 대해 이런 보고를 했다. 굴뚝 청소 중에 그을음이 음낭의 주름에 끼어 암이 발생한다고. 암을 연구하던 의사와 과학자들은 피부에 타르를 바르면 인공적으로 암을 발생시킬 수 있을 것이라고 생각했기 때문이다. 이렇게 다소 허무맹랑하게 암에 대한 접근이 시작된 지 200여년이 지났지만 암은 여전히 정복대상이다. 암에 대한 연구로 암에 대해서 한 걸음 다가갈 수 있는 계기를 만들었던 암 개척자들을 알아본다.

  • Acute scrotal pain is a common clinical symptom of urinary system and there are various causes. Among them, thrombosis of testicular vein is rare, but one of the causes of acute scrotal pain. Thrombosis is a common disease that can occur anywhere in our body. But thrombosis of testicular vein has not been reported in Korea. We report a case of thrombosis of testicular vein in a 26-years-old man with acute scrotal pain.

  • Kawasaki disease (KD) is a systemic vasculitis that occurs predominantly in infants and young children. The etiology of KD is unknown and coronary heart disease is a major complication of KD. Acute scrotum is a rare complication of acute KD, and not as well recognized as other manifestations of the disease. We report a 2-month-old boy with acute scrotum in the acute phase of KD. He was treated with intravenous immunoglobulin (total 2 g/kg) and aspirin (50 mg/kg/day). The treatment was effective in resolving his fever and other clinical symptoms, but 2 days after starting treatment he experienced scrotal swelling. Scrotal ultrasound and transillumination were used in the diagnosis of acute scrotum. After 2 months, a follow-up testicular ultrasound revealed a remission of the acute scrotum. Subsequently, he has been followed up for KD.

  • MRI는 주로 부인과 질환의 진단에 이용되어 왔으나 뛰어난 연조직 대조도와 다면 영상 능력으로 남성 골반강의 평가에서도 그 활용이 점차 확대되고 있다. 남성 골반강 MRI는 전립선을 중심으로 발전되어 왔으나 음경과 음낭, 요도 등을 침범한 외상, 종양 등의 질환에 매우 유용하므로 이들 부위에서의 MRI 이용이 더욱 증가할 것으로 보인다. 여기서는 전립선, 음경, 음낭 등의 남성 골반강 장기를 평가하는데 필요한 검사기법, 정상 해부학, 이상 질환의 MRI 소견 등에 대해 기술하고자 한다.

A Case of Scrotal Swelling Induced by Tunica Vaginalis Abscess (고환초막강의 농양에 의한 음낭 부종 1례)

  • Scrotal swelling may be acute or chronic, painful or painless. Common causes of scrotal swelling in newborns are hydrocele, inguinal hernia, testicular torsion, testicular tumor, scrotal hematoma, meconium peritonitis and epididymitis. Abrupt onset of a painful scrotal swelling necessitates prompt evaluation. Testicular torsion and incarcerated inguinal hernia require urgent surgical management. We report a case of scrotal swelling caused by a tunica vaginalis abscess in a 20-days-old boy. He was admitted to the hospital due to fever, irritability and left scrotal swelling with local heat, tenderness and redness. Exploratory laparotomy was performed to rule out testicular torsion. On the operative field, congestive erythematous inflammation on the left tunica vaginalis was noted and it was filled with a pus like discharge. The cultured organism was Streptococcus agalactiae(group B). He recovered quickly after debridement and administration of empirical antibiotics.

Imaging of Scrotal Tumors (음낭 종양의 영상 소견)

  • Ultrasonography is effective for imaging superficial organs, such as the scrotum. Using a highfrequency transducer, ultrasonography can identify the location and characteristics of scrotal lesions with high accuracy. The primary role of ultrasound (US) in the evaluation of a scrotal mass is to determine if it is intratesticular or extratesticular. Additional clinical information and other imaging options may be needed to diagnose benign tumors and pseudo-tumors. MRI is an effective problem-solving tool in cases with nondiagnostic US findings. CT is helpful for staging testicular cancer and localizing undescended testis. This review covers the imaging features of testicular and extratesticular tumors.

Acute Epididymitis in Two Children with Henoch-Sch$\ddot{o}$nlein Purpura (Henoch-Sch$\ddot{o}$nlein 자반증에서 동반된 급성 부고환염)

  • Scrotal involvement has been reported from 2% to 38% of males with Henoch-Sch$\ddot{o}$nlein purpura (HSP). It may present before the rash occurs or even long time after it has disappeared. Scrotal involvement of HSP usually results in pain, tenderness, swelling or discoloration of scrotum. Scrotal pain sometimes mimics testicular torsion to various degrees, which can be followed by unnecessary operation. In our 2 cases, one was a 5-year and 11-month-old boy who came to our emergency department due to scrotal pain before the diagnosis of HSP, and the other was a 5-year and 1-month-old boy who came to the outpatient clinic due to scrotal pain after the resolution of HSP about 1 month before the visit. We performed Doppler ultrasonography (USG) to evaluate the acute scrotal pain in the two boys. On Doppler USG, epididymis showed increased blood flow, and testis showed normal blood flow. These findings enabled the diagnosis of acute epididymitis and excluded testicular torsion. Epididymitis was improved by conservative management including shortterm steroid therapy within 5 days. It is important to perform adequate evaluation with tools such as Doppler USG in the early course of acute scrotum of HSP, to avoid unnecessary scrotal exploration.

  • In children and adolescents, inguinal and scrotal diseases are relatively common, and imaging is very useful for the diagnosis and differential diagnosis of these diseases. Therefore, it is important to understand the imaging findings of these diseases. In this article, we classify these diseases into small testes, cryptorchidism, patent processus vaginalis, acute scrotum pain, trauma, testicular tumors, and others and describe their characteristic findings.
