An Analysis on the Difference of Influential Factors between Metabolic Syndrome Group and Attention Group (대사증후군과 주의군 간의 영향요인 차이 분석)

  • 목적: 본 연구의 목적은 대사증후군 주의군 대비 대사증후군의 위험요소를 식별하여 예방을 위한 중재방안을 마련하기 위한 것이다. 방법: 한국의료패널 2015년도 데이터를 활용하여 당뇨만 앓고 있는 사람을 주의군으로 당뇨, 고혈압, 이상지질혈증을 모두 앓고 있는 사람을 대사증후군으로 구분하여 추출한 다음 가중치를 부여하여 총 1,559,884명을 대상으로 하였다. 주의군 대비 개인적 특성, 건강생활습관 특성, 삶의 질 특성, 의료이용 특성에 대한 대사증후군의 영향요인을 분석하였다. 결과: 개인적 특성에서는 40대미만 연령대에 비해 연령에 비례하여 대사증후군 위험이 증가하다 70대에 40대미만과 유사한 수준으로 떨어졌다. 남성보다는 여성, 고졸이하보다는 대졸이상, 중소지역에 비해 서울시 거주자, 장애가 없는 군보다는 있는 군, 의료급여가입자보다는 건강보험 가입자, 경제활동을 안하는 군보다는 경제활동 군의 대사증후군 위험이 높았다. 건강생활 습관 특성에서는 비흡연자보다는 흡연자, 술을 안마시는 군보다는 주 2-3회 이상 음주자, 운동을 하는 군보다 안하는 군, BMI가 정상인 군보다는 30 이상인 군에서 대사증후군 위험이 높았다. 삶의 질 특성에서는 일상 활동에 지장이 없는 군보다 있는 군, 통증이나 불편이 없는 군보다 있는 군, 주관적 건강상태가 보통이상보다는 나쁜 군, 섭식문제가 없는 군보다 있었던 군에서 대사증후군 위험이 높았다. 의료이용 특성에서는 미충족 의료이용 경험이 없는 군보다 있었던 군, 약제비가 부담이 안되는 군보다 부담이 되는 군에서 대사증후군 위험이 높았다. 결론: 당뇨를 앓고 있는 그룹 중 본 연구를 통해 확인된 대사증후군 위험 요소가 많은 이들에 대해 우선적으로 대사증후군 예방 보건교육을 집중하는 방안을 제언한다.

The Factor Affecting Alcohol Use Disorder in Korean women (한국 여성의 알코올사용장애 영향요인)

  • The purpose of this study was identify the factor affecting alcohol use disorder in Korean women. This study used the data from the 6th Korea National Health and Nutrition Examinations Survey and analyzed the demographic characteristics, health related factors and alcohol use disorder. Data were analyzed using the IBM SPSS 25.0 program. As a result, the prevalence of alcohol use disorder in Korean women was 14.7%. There were significant differences depending on age, marital status, educations level, occupation, smoking, stress, melancholy. By logistic regression, the risk factors of alcohol use disorder were age, marital status, occupation, smoking, melancholy. According to the results of this study, further research on the alcohol use disorder of single-working women should be conducted in considering of various variable.

The Association between cardiovascular disease and Periodontal Disease on Convergence Study in Adults over Age 40. (40세이상 성인의 심혈관질환과 치주질환 관련성에 관한 융합적 연구)

  • The purpose of this study was to investigate the relationship between cardiovascular disease and periodontal disease. The subjects were 3,149 adults over 40 years of age using the third year National Health and Nutrition Examination Survey (2015). Data were analyzed using the SPSS 22.0 program. As a result, the relationship between cardiovascular disease and periodontal disease was 1.27 times higher in obesity group compared to normal group, when adjusted for disturbance variables (age, smoking status, drinking status, income) In hypertensive patients, the hypertension group had a 1.32-fold higher risk of periodontal disease when the disturbance variables (age, smoking status, drinking status, income) were adjusted compared to those without hypertension. Therefore, cardiovascular disease is associated with periodontal disease, and it can be used as a good basis for educational and preventive measures to reduce or prevent the incidence of cardiovascular disease and periodontal disease in the future.

  • 담낭용종의 잘 알려진 위험요소는 대사증후군, 연령, 이상지질혈증 등이다. 제주도는 한반도에서 가장 큰 섬으로 한국의 본토에서 약 80km 떨어져 있으며 행정구역상 제주시와 서귀포시로 나뉘어 있지만 기업들과 편의시설들, 학교들 때문에 자연스럽게 시내와 시외로 더 나누어진다. 많은 이주민들과 젊은 성인들은 상업 활동이 활발한 시내 지역에 사는 경향이 있다. 고령 성인들은 어업이나 농업에 종사하기 때문에 시외 지역에 사는 경향이 있다. 고령의 성인들은 전통적인 식이를 유지하고 있고, 그들 중 다수는 힘든 노동으로 인해서 고위험 알코올 음주자의 비율이 높다. 이러한 요인들로 인해 두 지역간의 담낭용종 유병률에도 차이가 있을 것이다. 따라서 이 연구의 목적은 제주도 두 지역간에 담낭용종의 유병률을 알아보고 그 유병률과 관련된 다양한 요인들을 비교해 보고자 하는 것이다. 이 연구는 2009년 1월부터 2019년 12월까지 제주대학교병원 단일 건강검진센터에서 건강검진을 시행한 21,734명을 대상으로 하였다. 시내 지역과 시외 지역 거주민들의 담낭용종 유병률은 각각 10.1%와 8,7% (P = 0.006)로 의미있게 차이가 있었다. 평균 총콜레스테롤 수치가 시내 지역 거주민들에서 시외 지역 거주민들보다 높았고, 음주와 관련된 평균 알카라인 인산분해효소와 감마-글루타밀전이효소 수치들과 평균연령은 시외 지역 거주민들이 시내 지역 거주민들보다 더 높았다. 나이, 알코올 섭취와 평균 콜레스테롤 수치가 이러한 두 지역 간에 담낭용종 유병률의 차이를 만들었을 것이다.

A Study on the Change of Bone Mineral Density(BMD) by Life Habit and Physical Condition (성인의 신체조건 및 생활습관에 따른 골밀도 변화에 대한 연구)

  • Purpose: To evaluate the correlation between BMD and life habit such as drinking. exercise smoking or physical condition such as age, sex, height, weight, body mass index(BMI). Materials and Methods: I evaluated the BMD of the femoral neck and L2-L4 spines of 321 persons who took a regular health screening in Woosuk university oriental medical hospital from February to April in 2006 by dual energy bone mineral densitometry. Results: The age of persons ranged from 20 years to 75 years(mean $45.10{\pm}11.54$) and there were 160 males and 161 females. In males, BMD of the femoral head was highest at 2nd decade, BMD of the spine was highest at 4th decade, and BMD of both femoral head and lumbar spine was lowest at 6th decade. In fenales, BMD of both femoral head and lumbar spine was highest at 4th decade and lowest at 6th decade. Among the various physical conditions, only height of persons showed significant correlation with BMD in both males and females. BMD was increased according to increasing height. In males, BMD of persons who had habit such as drinking, exercise or smoking did not show significant change statistically. But in females, drinking group showed high BMD relative to non-drinking group in both femoral head and lumbar spine. Conclusion: BMD was different according to age, sex, height and life habit. Especially aged people showed osteoporotic change progressively. More persistent effort is needed to find out the factors decreasing BMD for prevention of problems by osteoporosis.

Effects of Abdominal Obesity and Risk Drinking on the Hypertension Risk in Korean Adults (복부비만과 위험음주가 성인의 고혈압에 미치는 영향)

  • Purpose: This study was conducted to investigate the combined effects of abdominal obesity and alcohol drinking on the risk of hypertension in Korean adults (aged ${\geq}30yrs$). Methods: Data of 13,885 subjects from the sixth Korea National Health and Nutrition Examination Survey were analyzed. The multiple logistic regression tests were used for the analysis, including potential covariates of the model. Results: Frequency of drinking, typical drinking quantity, and frequency of binge drinking had a positive relation to hypertension. The odds ratio of hypertension for risk drinkers with abdominal obesity was 4.81 compared to non-risk drinkers with normal waist circumstance, whereas the odds ratios of hypertension for risk drinkers with normal waist circumstance and non-risk drinkers with abdominal obesity were 1.58 and 2.37 respectively. Conclusion: Both abdominal obesity and alcohol drinking patterns were strong risk factors of hypertension in the Korean adults. Risk drinkers with abdominal obesity showed a marked high risk in hypertension compared to those with a single condition alone.

Incidence of Colon Cancer Related to Cigarette Smoking and Alcohol Consumption in Adults with Metabolic Syndrome: Prospective Cohort Study (대사증후군 성인의 흡연 및 음주 상태가 대장암 발생 위험에 미치는 영향: 전향적 코호트 연구)

  • Purpose: The purpose of this study was to identify the impact of cigarette smoking and alcohol consumption on the incidence of colon cancer in adults with metabolic syndrome. Methods: This study employed a longitudinal study design and utilized secondary data drawn from the Korean Genome and Epidemiology Study (KoGES). The data of a sample of 2,327 adults with metabolic syndrome tracked every two years from 2001 to 2014 were used in this study. Statistical data analyses of the frequency, number of cases per 100,000 person-years, log-rank test, Kaplan-Meier curve, and Cox's proportional hazards regression were performed using IBM SPSS statistics version 24. Results: During the observation period, the number of colon cancer cases was 46, and the total person-years were 252,444. The incidence of colon cancer was higher in current, over 10 pack-year smokers when compared to non-smokers (hazard ratio=3.38, 95% confidence interval=1.09~8.42). Conclusion: Excessive and long-term smoking should be avoided to prevent colon cancer, especially in adults with metabolic syndrome, since it might exacerbate the risk factors of colon cancer. Particularly, health professionals need to provide individualized smoking cessation interventions to those at high risk of colon cancer.

The impact of alcohol consumption and binge drinking on the risk of dental trauma in South Korean adolescents (우리나라 청소년의 음주 및 만취 경험이 치아 외상 위험성에 미치는 영향)

  • Objectives: This study aimed to investigate the impact of alcohol consumption and binge drinking on the occurrence of dental trauma in South Korean adolescents. Methods: Data were analyzed from the South Korean Youth Risk Behavior Survey, which used a complex sampling design to ensure representativeness. A total of 52,875 adolescent were included in this study. Logistic regression analysis was conducted to assess the association between alcohol consumption and tooth fractures. Confounders such as gender, education, city, academic performance, economic status, etc. were adjusted. Results: Logistic regression analysis revealed that alcohol consumption was significantly associated with tooth fractures in adolescents. After adjusting for all factors in Model 3 of binge drinking, the odds ratio remained statistically significant at 1.331 (95% CI: 1.064-1.664). Conclusions: Alcohol consumption and binge drinking increase the risk of tooth fractures in adolescents. Preventive measures targeting alcohol-related behaviors in this population are crucial for improving oral health outcomes.

Youth Crisis Forecasting by Youth Counseling Data Analysis (청소년상담데이터 기반 위기청소년 예측)

  • The main purpose of study is to identify relevance between nature and types of risk factors that delinquent teenagers are exposed and types of methodologies implemented to prevent committing school violence, domestic violence, and suicide or to help recovering from violent activities and suicide attempts. The results show that school dropout has much relevance in risk factors such as probation, lawbreaking, smoking, drinking, runaway, domestic violence victim, and suicidal attempt. Risk rate of school dropout for those teenagers who smoke and drink in the period of runaway is 2.76 times higher than those teenagers who do not smoke or drink. More specifically, drinking increases more risk of school dropout than smoking. Contribution of this study is to identify empirical evidence that calls for comprehensive risk management for delinquent teenagers encompassing home, school, and community rather than focusing on risk itself.

Prevalence of Arthritis and Health Behavior Related Risk Factors of the Older Residents in Andong Rural Area (안동 농촌지역 중년 및 노인 주민의 관절염 유병율과 건강습관 관련 위험요인 분석)

  • This study was performed to estimate the prevalence and the risk factors of arthritis in Andong rural area in 2003. The subjects were 1,462 people (544 males, 818 females) aged 45 years and over. The arthritis group was composed of 322 people (72 males, 250 females), diagnosed by doctor or self-diagnosed by the symptoms. Prevalence of arthritis was higher in female (27.2%) compared to male (13.2%). Mean age of arthritis group was significantly higher than that of normal group (male 66.2 vs 62.8, female 62.6 vs 60.1, p<0.001). The anthropometric measurements, such as % body fat, body mass index (BMI), and waist circumference, and the biochemical measurements, such as plasma triglyceride and total cholesterol level, were significantly higher only in female (not in male) arthritis group than those in normal group. Health related lifestyle factors, such as smoking, alcohol drinking, and exercise, were not different between both groups. The risk factors for arthritis were analyzed using the multiple logistic regression method and expressed as odds ratio (OR). The results showed that female compared to male (OR=1.983), over 65 years compared to 45$\sim$64 years (male OR=2.769 and female OR=1.461), and obese female subjects (not for male) by % body fat ($\geq$32% OR=2.035) or BMI ($\geq$25 kg/$m^2$ OR=1.556) showed significantly higher risk factors for arthritis. Regarding nutrient intakes, higher intakes of fat (OR=1.443), calcium (OR=1.503), iron (OR=1.518) and vitamin A (OR=1.390) in female seemed to be risk factors. In contrast to female, higher intakes of vitamin A (OR=0.526) and riboflavin (OR=0.582) seemed to decrease the risk for arthritis in male. This study revealed that the prevalence of arthritis was significantly higher in female and aged individuals. Also, in order to decrease the prevalence and/or prevention of arthritis, female should prevent overfatness and decrease some nutrient intakes, while male should increase their intakes.
