A convergence study on the relationships of alcohol consumption patterns to mental distress and self-rated health (성인 음주자의 음주 패턴에 따른 정신건강과 주관적 건강상태에 대한 융합적 연구)

  • The purpose of this study is to assess the relationships of alcohol consumption patterns to mental distress and self-rated health. Data of 19,856 adult drinkers over 19 years of age and older from the Korea National Health and Nutrition Examination Survey were evaluated by multivariate logistic regression analysis. Risks of experiencing perceived high stress, depressive symptoms and suicidal thoughts were higher in alcohol use disorder group in male, whereas the risks were higher in both alcohol use disorder and hazardous drinking groups in female, reflecting higher susceptibility to mental distress. In both male and female, former drinkers presented poor self-rated health compared with low-risk drinkers, although no difference was found in the prevalence of mental distress. Specialized mental health management measures for female drinkers are required, and former drinkers are suggested to be assessed separately from never or low-risk drinkers for their physical and mental health status.

  • 음주습관과 심혈관계 질환 위험요인사이의 관련성을 파악하기 위하여, 1996년 한국 경북에 있는 도시지역 한 철강회사에서 40-50대(40-59세) 남자 근로자 3444명으로부터 공복시 혈액을 10m1 채취하여 혈청지질, 혈당, Uric acid를 측정하고 신장, 체중, 혈압(10-20분 휴식후 측정)과 EKG를 측정하였다. 과거병력, 현재 건강상태, 음주량과 기간 및 음주종류를 포함한 생활습관, 흡연량/일, 운동시간/주 등의 설문지를 작성하였다. 알콜 소비량은 양-빈도의 Index에 의해 측정되었다. 한국 중년 남자에 대한 음주량, 음주기간, 알콜종류등에 따른 심혈관계 질환의 위험요인 유무 정도를 알아보기 위해, 비음주자를 기준으로하여 비교대상군은 금주자와 음주량(2군) 등에 의해 3군으로 분류하였으며, 음주기간에 따른 심혈관계 질환의 위험요인 유무정도를 알아보기 위해 비음주자을 기준으로하여 비교 대상군은 금주자와 음주기간(4군)에 따라 5군으로 분류하였다. 알콜종류에 따른 심혈관계 질환의 위험요인 유무정도를 알아보기 위해 비음주자을 기준으로하여 비교 대상군은 금주자와 음주알콜종류(3군)에 따라 4군으로 분류하였다. 심혈관계 질환 위험요인은 기준되는 범주를 2개(Table 1)로 분류하여 2*2표를 이용하여 odds ratio(dummy variable로 처리), ${\beta}$-coefficient와 표준오차를 이용해서 신뢰구간(95% C.I.)을 구하여 유의성 검정을 하였다. 금주자의 정의는 과거 3달 이상 술을 마시지 않는 사람을 말한다. 연구 집단의 평균 연령은 $44.4{\pm}3.9$세 이며, 비음주자는 649명으로 18.9%, 금주자는 70명(2.0%)이며 음주자는 2725명(79.1%)이다. 음주자중 에타놀 섭취를 평균 100.5g/주(median) 미만인 군은 1239명으로 연구집단 3444명중 35.7%이며, 평균 100.5g/주(median) 이상 음주군은 1276명으로 44.5%이었다. 3444명중 1532명(44.5%) 이 맥주와 소주를 섞어서 마시는 사람이었다. 음주 양에서, 연령, 흡연 양, 흡연 기간, 운동, 음주기간, 알콜종류 등을 통제하여 logistic regression한 결과, 비음주자에 비해 중정도 음주자(${\ge}$100.5gm/week ethanol)의 확장기 혈압의 Odds ratio는 1.33배로 유의하게 높았다(P<0.05). 다른 요인들은 통계적으로 유의한 차이를 보이지 않았다. 알콜종류에서도 연령, 흡연 양, 흡연 기간, 운동, 음주양, 음주기간, 등을 통제한, 비음주자에 비해 맥주와 소주를 섞어 마시는 음주자의 확장기 혈압의 Odds ratio는 1.38배로 유의하게 높았다(P<0.01). 다른 요인들은 통계적으로 유의한 차이를 보이지 않았다. 반면에 음주 습관 중 음주기간에서는 연령, 흡연양, 흡연 기간, 운동, 음주양, 알콜종류 등을 통제한 logistic regression 분석 결과 유의한 차이를 보인 것은 없었다. 그러나 음주기간과 심혈관계 질환 위험요인중 수축기와 확장기 혈압과의 관계는 J 모양을 나타내었다 . 수축기 혈압의 금주자군은 비차비가 1.0보다 약간 낮고(Odds ratio=0.88) 음주기간이 2-10년인 군의 Odds ratio=1.14, 음주기간이 11-20 년인군의 Odds ratio =1.18, 21년 이상인 군의 Odds ratio=1.20로 비음주자의 odds ratio=1에 비해 점차적으로 위험이 높아졌다. 확장기 혈압의 금주자군은 비차비가 1.0보다 약간 낮고(Odds ratio=0.91), 음주기간이 1년 이하인 군의 Odds ratio=1.18, 음주기간이 2-10 년인 군의 Odds ratio=1.23, 음주기간이 11-20 년인 군의 Odds ratio=1.27, 21년 이상인 군의 Odds ratio=1.27로 비음주자의 odds ratio=1에 비해 점차적으로 위험이 높아졌다.

Factors associated with risky drinking of adult women: Focused on convergent implications (성인 여성의 위험음주에 영향을 미치는 요인: 융복합적 함의를 중심으로)

  • This study is aimed to examine effects of sociodemographic, social relation, and health related factors on risky drinking among early, middle, and older adulthoods. Using the sixth(2013-2016) National Health and Nutrition Examination Survey, separate logistic regression analyses were conducted for women's early adulthood (n=1,143), middle adulthood (n=1,306), and older adulthood (n=875) to test factors associated with risky drinking. For study results, as for women's early adulthood, education, marital status, employment, depressive feeling, and smoking were statistically associated with risky drinking. As for women's middle adulthood, only subjective stress level and smoking were associated with risky drinking. As for women's older adulthood, education, suicidal ideation, and subjective health condition were associated with risky drinking. These results found different factors influenced risky drinking according to life stages. Based on these results, convergent implications about reducing risky drinking according to life stages of women were discussed.

Comparison of Female Nursing College Students in the State of Drinking, Drinking Motives, and Problem Drinking (간호대 여학생의 음주실태, 음주동기와 문제음주의 상태 비교)

  • The purpose of this study was to compare female nursing college students with respect to drinking habit, drinking motives, and drinking problems. (Ed note: confirm edited version; original is a bit unclear in intent) Subjects include nursing college students in some areas, and data were collected between April and May, 2018. A total of 181 questionnaires were analyzed. The frequency analysis was used to examine the differences between the normal and drinking-problem groups. Pearson's correlation analysis was used to examine the correlations of drinking problems, and logistic regression analysis was used to analyze the factors related to drinking problems. The findings show that the frequency of drinking, rate of drinking, drinking motives, life stress, and prior experiences with drinking problems recorded a high risk ratio for drinking problems. Specifically, the group with a drinking frequency of 2~3 times per week showed a risk ratio that was 21.53 times higher than the group with a drinking frequency of 1~4 times per year. The group of higher drinking motives recorded a risk ratio that was 5.96 times higher than the group of lower drinking motives. The group of higher experiences with drinking problems showed a risk ratio that was 8.80 times higher than the group of lower experiences with drinking problems. In conclusion, the frequency of drinking, rate of drinking, drinking motives, life stress, and experiences with drinking problems were factors highly related to the drinking problems in female college students. The results highlight that careful evaluation of drinking habit of college students are needed. Moreover, periodic temperance education and programs designed to identify and prevent drinking problems are necessary to lower the risk of developing drinking problems and promote a safe drinking culture.

Gender difference in the effects of Korean youth mental health on binge drinking (한국 청소년의 정신건강이 위험음주에 미치는 영향에서의 성차)

  • The purpose of this study is to explore the effects of mental health (stress perception, depression, suicidal ideation) on binge drinking among adolescents according to gender. Data were collected from the 12th (2016) Korea Youth Risk Behavior Web-Based Survey(KYRBWS) and 65,528 middle and high school students, aged 12-18 years, were selected. Data analysis was conducted by using logistic regression analysis for mental health effects on binge drinking. The results showed that the binge drinking due to mental health of female students was higher in that of male students (stress perception: 1,444 times (95% CI: 1,200-1,737), depression: 1,843 times (95% CI: 1.674-2.029), suicidal ideation: 1.656 times (95% CI: 1.464-1.872)). The development of preventive / therapeutic programs for binge drinking should include not only the management of risk factors for mental health but also various approaches depending on gender.

Factors Influencing Hazardous Drinking of Women who Experienced Intimate Partner Violence (아내폭력 피해여성의 위험 음주 실태와 영향요인에 관한 연구)

  • This study analyzed the state of battered women who drink at a dangerously high level and the factors that influence their hazardous drinking in order to promote their safety and their healthy adjustments. The researchers utilized the data from "Analysis of the Effectiveness of Intimate Partner Violence Offenders Correction and Treatment Program" funded by Ministry of Gender Equality, and thereby, sampled 124 battered women for interview over phone. The results of applying the WHO's standard for hazardous drinking based on frequency and quantity calculations showed that 15.7% of survey participants were classified as hazardous drinkers, either currently experiencing or having the risk of experiencing problems related to drinking. The result of carrying out a logistic regression analysis to identify the factors that influence hazardous drinking showed that household income and self-blame were factors that exerted statistically significant influence. Based on these results, clinical and practical intervention plans for helping battered women who drink hazardously were discussed.

Factors Associated with Drinking Experience, Smoking Experience, and Internet Addiction among Elementary School Students (초등학생의 음주 및 흡연 경험, 인터넷 중독 영향 요인)

  • This study examined the status of and factors associated with drinking experience, smoking experience, and internet addiction among elementary school students. Cross-sectional data were collected by questionnaire from 1,061 children among the fifth and sixth grades in public elementary schools in Gyeonggi province. The prevalence of drinking and smoking experience, internet addiction of participants was 22.7%, 2.9%, and 4.2% respectively. Multiple logistic regressions showed that the likelihood of drinking and smoking experience, and internet addiction was commonly higher with those who was a male and had higher aggression level. The likelihood of drinking experience was lower with those who had higher family support, while was higher with those who had higher friend support. That of internet addiction was lower with having higher friend and teacher support. Prevention strategies to prevent drinking, smoking, and internet addiction among elementary school students should consider student's gender, control aggression, and enhance social support in family and school environments.

The Effects of Driving Behavior Determinants on Dangerous Driving and Traffic Accidents in the Reckless Drivers Group: A Path Analysis Study (사고 및 음주운전자들의 운전행동결정요인 특성이 위험행동 및 교통사고에 미치는 영향: 경로분석 연구)

  • Speeding and drunken driving make drivers fail to detect hazards and cope with various driving situations. These behaviors also raise the possibility of being involved in traffic accidents and tend to increase the number of fatalities. The authors compared the driving behavior determinants of a rockless drivers group, consisting of individuals who have committed traffic accidents or offended regulations through drunken driving, with a normal drivers group. In the results, the reckless drivers group showed high scores of 'speeding' and 'drunken driving', and they also stated that they had more experiences of speeding, drunken driving and traffic accidents. In the path analysis study, it was found that the impacts of the rockless drivers group's 'risk sensitivity' and 'situational adaptability' on traffic accidents were stronger than those of normal drivers. This means 'risk sensitivity' and 'situational adaptability' can explain the origins of traffic accidents better in the reckless drivers group than accidents of the normal drivers group.

A study about factors associated with risky drinking between adolescents in Korean ethnic families and adolescents in multicultural families (일반가정 청소년과 다문화가족 청소년의 위험 음주에 영향을 미치는 요인 연구)

  • The aim of this study is to examine factors associated with risky drinking behavior between adolescents in Korean ethnic families and multicultural families. This study used the used the 13th(2017)Korea Youth Risk Behavior Survey(KYRBS) data and conducted separate logistic regression respectively for 61,320 adolescents in Korean ethnic families and 956 in multicultural families. As for results, first, the common variables associated with risky drinking for both adolescents in Korean ethnic families and adolescents in multicultural families were smoking and drinking alone. Second, variables associated with risky drinking among adolescents in Korean ethnic families were gender, middle-school vs. high-school, depression, having two-parents, access to buy alcohol, residence type, and pocket money. Third, as for adolescents in multicultural families, only father's education level was associated with risky drinking. Based on these results implications about reducing risky drinking were discussed.

Factors associated with depressive mood among problem drinkers by gender (성인 문제음주자의 성별에 따른 우울경험과 관련 요인)

  • The purpose of this study was to assess the relationship between drinking patterns and depressive mood, and to understand the factors associated with depressive mood among problem drinkers. Data of 19,878 adults over 19 years of age and older from the third Korea National Health and Nutrition Examination Survey were evaluated by multivariate logistic regression analysis. The risk of depressive mood was greater with higher level of problem drinking in both genders. For male problem drinkers, the factors associated with depressive mood include older age, lower education level, unemployment, high perceived stress, bad subjective health status, and non-married status. For female, higher risk was found in groups with the age of 40-49, lower education level, unemployment, high perceived stress, and bad subjective health status. By early screening of the identified risk factors, progression to mental health problem can be avoided. For preventative measures, differential approach depending on gender is suggested.
