  • The purpose of this study was to investigate the effect of aquatic gait training on plantar foot pressure, foot kinesiology and gait speed in right hemiplegic patients. The subject were 20 stroke patients who elapsed from 12 month to 24 month after stroke(aquatic gait training group(n=10), land gait training group(n=10)). This study measured plantar foot pressure, toe out angle, subtalar joint angle, gait speed from data of gate on 2m long measuring apparatus for RS-scan system(RS scan Ltd. German). This experiment performed in twice, before and after the aquatic gait training and land gait training. Collected data were statistically analyzed by SPSS Ver. 12.0 using descriptive statistics, paired t-test. Aquatic gait training group had more variety pressure area on their foot such as T1(Toe 1), HM(Heel medial), and HL(Heel lateral). But motion of subtalar joint flexibility and toe out angle decreased considerably and gate speed also increased. According to the result, aquatic gait training is considered as more effective way in foot stability and normal gait pattern than land gait training.

  • A healthy, 14-year-old boy presented with right hip pain and consequent fever after falling out of bed while sleeping. The patient could not walk and complained of severe pain with active and passive motion, which consisted mainly in extension and internal rotation of the right hip. Laboratory analysis of the peripheral blood identified leukocytosis and increased levels of acute phase reactants. Magnetic resonance imaging of the hip, which was performed with the expectation of right hip pathology, revealed cellulitis and abscess in the right psoas muscle and associated inflammatory changes in the adjacent presacral fat plane but showed no abnormal lesions in the adjacent pelvic bone and spine. Staphylococcus hominis was cultured from the blood. With empirical antibiotic therapy, the patient recovered fully. We report a case of primary psoas abscess confused with hip pathology in an immunocompetent child without underlying disease.

  • For this research, 27 out of 52 patients with chronic low back pain involving sacroiliac joint pain were classified into the experimental group to conduct pelvic exercise program and traditional physical therapies in parallel, and the remaining 25 ones were classified into the control group to only apply traditional physical therapies for 6 weeks. After that, their clinical samples were randomly extracted. Before the experiment, both of the experimental group and the control group had the Oswestry disability index test to see how big their pain was and their hip joint angles were analyzed during walking. After the post-test, finally, the results of the Oswestry disability index test and hip joint angles while walking before and after the experiment were compared between two groups to know the effect of pelvic stability exercise program. Within-group results and between-group results both displayed significantly reduced low back pain, and when comparing hip joint angles of the experimental group, there were differences between mid stance phase, terminal stance phase, pre swing phase and early swing phase of the right hip joint, and mid stance phase, terminal stance phase, pre swing phase and early swing phase of the left hip joint. In the control group, there were significant differences between mid stance phase, terminal stance phase and early swing phase of the right hip joint, and loading response phase, mid stance phase, terminal stance phase and pre swing phase of the left hip joint. as a result, pelvic stability exercise program is helpful to gait rhythm on stance phase and swing phase although effective to decrease Oswestry disability index including pain.

  • While rural highways carry on lower traffic volumes, the deviations of running speeds between vehicles appear to be higher on rural highways than urban highways. The speed characteristic of rural highways is adding to pedestrian-related accidents which occur on a crosswalk with poor sight distance due to the limits of car headlights and lighting. Specially, the aged was mostly occupied in nighttime-related accidents on crosswalks, and pedestrians crossing on the far side of approaching vehicles appear to have the probability of fatality higher than the near side. An alternative is needed to resolve the compounded accidents, and then this study is to establish a new approach to an optimum lighting environment on a crosswalk to improve pedestrian safety. This study was conducted by a survey and a field study on the lighting of existing crosswalks. The field study shows that the existing lighting has the problem of wasting energy and impeding walking due to glare. The survey shows that nighttime sight distance on a crosswalk is required to improve and that road users prefer to be brighter pedestrian waiting space together with the crosswalk. Thus, a lighting environment that is not too bright and illuminates the crosswalk and the pedestrian waiting space is needed to implement.

  • A 10-year-old, intact female Beagle dog was presented for examination of acute onset of right-sided head tilt and moderate ataxia. Clinical signs were acutely presented 12 days ago and had been progressively alleviated until the admission day. The dog was bright, alert, and responsive. On neurological examinations, mild head tilt to the right and mild ataxia were noted, thus vestibular disorders were suspected. Central vestibular disease was ruled out based on the clinical signs, magnetic resonance imaging(MRI), and cerebrospinal fluid(CSF) analysis. Otoscopic examination showed that the tympanic membranes were intact and normal in appearance. On radiographic and MR examinations, abnormalities were not found in the tympanic bulla and the petrous portion of the temporal bone. Hypothyroidism was ruled out by thyroid-stimulation hormone(TSH) stimulation test. Clinical signs were completely disappeared at 2 weeks after discharge without any therapy. Thus, the dog was definitively diagnosed as idiopathic vestibular disease based on the clinical signs, excluding other causes of vestibular dysfunction, and the alleviation of clinical signs with time.

  • The vestibular function test reveals the objective findings of the impairment of the vestibular labyrinth. It's purpose is based on the analysis of the findings and detect the location and etiology of the labyrinthine impairment. In the vestibular function test, the vestibulo-spinal reflex has the clinical significance upon the tonus of the striated muscles by the labyrithine stimulation and contribute to regulating the posture and the position, at rest as well as in motion. The vestibulo-spinal reflex must performe as one of the routine vestivular function test because it can be evoked in man by such weak stimuli to the labyrinth as cannot induce vestibulo-ocular reflex. Authors performed the vestibular function test such as one leg test, gait test, stepping test and vertical writing test to one hundred of healthy and young male adult and received the following results. Results 1. One leg test: In 30 seconds, the frequency of dropping the leg on the ground was between 0 to 3 times in Rt., and 0 to 5 times in Lt. The mean frequency was 0.48 times in Rt., and 0.68 times in Lt. 2. Gait test: In forward gait; the range of the deviation was distributed 0 to 100 cm and mean range was 22.5cm to the Rt., 26.1cm to the Lt. In backward gait; the range deviation was distributed 0 to 140cm and mean range was 35.4cm to the Rt., 33.0cm to the Lt. 3. Stepping test: In normal head position; forward movement war 93% and backward 5%. The angle of displacement deviated to the Rt. side in 36%, and Lt. in 50%. The angle of rotation deviated to the Rt. side in 53 %, and Lt. in 36%. The mean values: angle of displacement was 22.05 degrees, angle of rotation was 24.40 degrees, distance of displacement was 48.95cm. In backward head position; Forward movement was 94% and backward was 3%. The angle of displacement deviated in 34%, and Rt. in 55%, to the Rt. side The angle of rotation deviated to the Rt. side in 50%, and Lt. in 42%. The mean values; angle of displacement was 29.72 degrees, angle of rotation was 39.53 degrees, distance of displacement was 44.17cm. 44.17cm. 4. Vertical writing test: The angle of deviation was between 0 to 16 degrees in all cases, and was between 0 to 12 degrees in the cases of normal head position. The mean angle of deviation was between 4.15 to 5.76 degrees on each side. The direction of deviation to the Rt. side was 54~69%, Lt. was 25~40% and 3~7% was vertical without deviation.

  • In this study, muscle activity was measured using surface EMG (sEMG) during a voluntary maneuver (ankle dorsiflexion) in the supine position was compared pre and post gait training. Nine patients with incomplete spinal cord injury participated in a supported treadmill ambulation training (STAT), twenty minutes a day, five days a week for three months. Two tests, a gait speed test and a voluntary maneuver test, were made the same day, or at least the same week, pre and post gait training. Ten healthy subjects' data recorded using the same voluntary maneuvers were used for the reference. sEMG measured from ten lower limb muscles was used to observe the two features of amplitude and motor control distribution pattern, named response vector. The result showed that the average gait speed of patients increased significantly (p〈0.1) from 0.47$\pm$0.35 m/s to 0.68$\pm$0.52 m/s. In sEMG analysis, six out of nine patients showed a tendency to increase the right tibialis anterior activity during right ankle dorsiflexion from 109.7$\pm$148.5 $mutextrm{V}$ to 145.9$\pm$180.7 $mutextrm{V}$ but it was not significant (p〈0.055). In addition, only two patients showed increase of correlation coefficient and total muscle activity in the left fide during left dorsiflexion. Patients' muscle activity changes after gait training varied individually and generally depended on their muscle control abilities of the pre-STAT status. Response vector being introduced for quantitative analysis showed good Possibility to anticipate. evaluate, and/or guide patients with SCI, before and after gait training.
