Correlation of Caries Experience and Activity between Mothers and Their Children Using a Colorimetric Test (비색법을 이용한 모자간 우식 경험 및 활성의 상관성)

  • The purpose of this study was to compare the validity of $Cariview^{(R)}$, a new colorimetric caries activity test, with $CRT^{(R)}$ bacteria, a conventional bacterial culture method. In addition, this study assesses the correlation between the dental caries experience and activity between mothers and their children. 34 pairs of mothers and their children under the age of 6 years participated in this study with informed consent. After filling out a questionnaire and oral examination, the two caries activity tests above were performed on each subject. In the results, $Cariview^{(R)}$ scores were statistically significant with children's caries experience (r = 0.598, p < 0.01) and showed higher correlation than $CRT^{(R)}$ bacteria scores. $Cariview^{(R)}$ scores showed statistically significant correlation with the number of decayed teeth in both mothers and children (p < 0.05). In both $Cariview^{(R)}$ and $CRT^{(R)}$ bacteria tests, there was no statistically significant correlation between caries experience and caries activity (p > 0.05). $Cariview^{(R)}$ colorimetric test will be clinically useful for predicting future caries risk and establishing a preventative strategy in pediatric dentistry.

Development of caries risk assessment for children and adolescents using Delphi survey (델파이 조사를 이용한 소아·청소년의 우식위험평가 문항 개발)

  • Objectives: This study aimed to design a CRA questionnaire specifically for Korean children and adolescents based on the results of two Delphi survey completed by dental professionals. Methods: Based on the CAMBRA® CRA tool, updated in 2019, we constructed a questionnaire on the oral environment and behavior of children and adolescents. Then, the CVR and I-CVI were evaluated using the results of the first and second Delphi surveys involving 20 professionals. Results: After the first Delphi survey, six items were considered invalid because their CVR and I-CVI were below the recommended values of 0.42 and 0.78, respectively. After receiving feedback from the professional panels, nine items were removed. In the second Delphi survey, the questionnaire contained a total of 21 items, all of which showed good CVR and I-CVI. After modifications based on the opinions of professional panels, the questionnaire was finalized to have 21 questions. Conclusions: The finalized questionnaire with 21 items, based on the opinions of professional panels, showed excellent CVR and I-CVI for all the items. Structured programs with updated guidelines must be developed for Korean children and adolescents in further studies.

Dental Caries Status of 14-16 Year Old Adolescents in Yangsan Area (양산시 거주 14-16세 청소년의 치아우식증 상태에 대한 조사연구)

  • Although adolescents usually show peculiar aspects in oral health status, there have been not so many studies about it, while there have been innumerable studies for children. This study aimed at evaluating the prevalence of dental caries of adolescents in Yangsan in Republic of Korea, detecting its contributing factors, and distinguishing the high-risk group by epidemiological mass survey. Besides, the significance including early caries lesion in diagnosis criteria was assessed with minimal invasive concept. This survey was performed on 1,371 adolescents aged 14 to 16 in Yangsan as subject examinees by a single trained examiner. The dental caries status was checked by oral examination at schools and questionnaire survey was accompanied for some relevant risk factors. The data were analyzed to yield the results as follows: Regarding the caries experience, the DMFT score by WHO criteria with ECL including or not was 4.79 and 3.97 respectively. In both the criteria, female showed higher values and that of high-risk group was almost twice compared with whole examinees by Sic Index. About pit & fissure sealed rate, it revealed the tendency that the more sealed teeth, the lower DMFT values. Among the studied risk factors, only the habit of brushing before going to bed showed positive relationship (p < 0.05).

  • The aim of this study was to investigate the caries management by risk assessment (CAMBRA) development process and the latest updated caries risk assessment for the domestic settlement of the CAMBRA program and the activation of dental clinics. A system for CAMBRA was published in California Dental Association (CDA) in 2007. To investigate whether CAMBRA can be successfully implemented outside a university setting, a practice-based research network (PBRN) was created in the San Francisco Bay Area. Based on the CAMBRA-PRAN clinical studies, the caries risk assessment (CRA) tool was updated for patients aged 6 through adults in 2019. The CAMBRA system is expected to be widely and actively used in the dental field in Korea to contribute to oral health care.

  • A person with neurological disorder is defined as a person with limitation of ambulation or basic life activity due to the cerebral palsy or traumatic brain damage, stroke. Recently, the DMFT of the children with neurological disorders is similar or lower than the DMFT of the children with no disability. But, this article is about the severe-early childhood caries in children with grade 1 neurological disorder. It is supposed that the primary motor impairment and secondary physical weakness, frequent hospitalization, sugar- rich food and drugs, lack of oral care, delayed dental visit made high caries susceptibility. In case of treatment of the children with severe neurological disorders, dentists should make a thorough caries preventive plan based on individualized caries risk assessment. Also parents and medical doctors should recognize the importance of oral care and do the early dental visit and home care.

Association between Candidiasis and Early Childhood Caries : Analysis Using Healthcare Big Data (보건의료 빅데이터를 이용한 유아기 우식증과 칸디다증의 연관성 분석)

  • The aim of this study was to identify the association between candidiasis and early childhood caries and to investigate whether the experience of candidiasis or oral candidiasis before age 1 can be considered as a risk factor for early childhood caries. The database used in this study was provided by Health Insurance Review and Assessment Service. Medical records of children born from January 2010 to December 2012 were obtained, and those without dental records were excluded. Subjects were divided into several groups based on the experience of candidiasis or oral candidiasis before age 6: candidiasis group and non-candidiasis group; oral candidiasis group and non-oral candidiasis group. Another categorization was done according to the experience of candidiasis or oral candidiasis before age 1. The incidence rate of early childhood caries in each group were compared. The prevalence of dental caries in children who have been diagnosed with candidiasis or oral candidiasis before age 6 was significantly different from those who have not experienced candidiasis. Similarly, children who have suffered from candidiasis or oral candidiasis before age 1 had significantly different incidence of caries from the children without candidiasis experience.

  • This article reviewed theological background and historicak development in dental caries management by risk assessment. Dental caries prevention and treatment according to CAMBRA model is patient-centered, risk-based, evidence-based practice. Team approach is necessary and clinician need to integrate science, practice and product. Dental hygienist take a important role in implementing CAMBRA. CAMBRA model could be incorporated into dental and dental hygiene education. Dentist and dental hygienist able to provide scientific and ethical care managing dental caries by risk assessment.

  • The new paradigm of dentistry require the detection of caries in their earlier stages. To achieve this, a high technology detection device and systematic and organized caries management system are needed. Caries management by risk assessment (CAMBRA) model is representative caries management system that satisfied new paradigm. Dental caries prevention and treatment according to CAMBRA model is patient-centered, risk-based, evidence-based practice. Therefore, individual caries management such as CAMBRA should be performed through accurate assessment of caries disease indicators and comprehensive assessment of caries risk factors and protective factors. Based on the CAMBRA better effectiveness of comprehensive dental caries management including non-surgical treatment will be accomplished.

An Analysis on the Understanding of Korean Caries Risk Assessment for Children and Adolescents (소아·청소년을 위한 한국형 우식위험평가 문항의 이해도 분석)

  • Purpose : This study aimed to analyze the level of understanding of Korean children and adolescents of caries risk assessment items in order to develop caries risk assessment tools. Methods : A total of 52 parents and 108 middle and high school students were analyzed for their understanding of nine items of the CAMBRA CRA for Korean children and adolescents. The nine questions were analyzed through a Likert five-point scale. Centralized distribution analysis was conducted to compare significant differences in the three groups' understanding of the questions. Results : The evaluation of the middle school and high school students' understanding showed that four items scored less than 3.0 points. The items were "Hyposalivatory medications (antihistamines, antipsychotics, asthma, etc.)," "Brushing below once a day," "F varnish last six months," and ".12 % chlorhexidine gluconate mouthrinse daily seven days monthly." The understanding of elementary school parents of ".12 % chlorhexidine gluconate mouthrinse daily seven days monthly" was less than 3.0 points. In addition, as a result of comparing the averages of the three groups, a statistically significant difference (p<.05) was found for a total of five items: "Hyposalivatory medications (antihistamines, antipsychotics, asthma, etc.)," 'Not used oral products such as floss and interdental toothbrushes," "F toothpaste 2× daily or more," "F varnish last six months," ".12 % chlorhexidine gluconate mouthrinse daily seven days monthly." Conclusion : This study was conducted to provide basic data for the development of a caries prevention program by identifying whether the caries risk assessment reconstituted in Korean style can be used for children and adolescents. More than half of the questions scored 3.0 points or higher and were generally completed. Nonetheless, a systematic program can only be completed if a plan is secured to improve repondents' understanding of the questions before a caries prevention program is developed.

  • Caries Management by Risk Assessment (CAMBRA), published by California Dental Association in 2003, is a customized caries care system that classifies individuals' caries risk into 4 risk groups based on objective evidences and provides chemical treatments targeted for each caries risk level. However, this system was not only developed but also optimized for situation in the United States, resulting into many limitations to be used in Korea, and thus Korean CAMBRA (K-CAMBRA) that considers the clinical situation in Korea needs to be developed. K-CAMBRA includes various techniques that are newly developed in order to overcome the limitations. First, Q-ray, a new optical technology, is utilized in order to avoid the subjectivity of visual inspection during assessment of disease indicators and risk factors. Moreover, Cariview? that reflects the paradigm shift in cariology as a new form of caries assessment kit is used. In addition, considering the situation in Korea, where it is impossible to use high concentration fluoride product, Oral pack with a customized tray is added to increase the contact time of chemical substance. CAMBRA is believed to be the key clinical tool that overcomes the limitations of the paradigm of the conventional restoration-based surgical model of dentistry. Furthermore, it can be expected that Korean dentists can act as oral physicians who are able to control and care individuals' caries risk rather than operative experts who only care about the outcome of caries.
