The Recurrence Pattern of Primary External Auditory Canal Cancer (원발성 외이도암의 재발 양상)

  • Background/Objective: The survival and recurrence pattern of the patients with primary cancer of the external auditory canal was evaluated. Materials & Methods: Seventeen patients with primary cancer of the external auditory canal from 2002 to 2013 was analyzed retrospectively. Overall survival, recurrence free survival, local recurrence free survival and distant metastasis free survival was calculated by Kaplan-Meyer's method. Results: Five year overall survival rate, 5 year recurrence free survival rate, 5 year local recurrence free survival rate and 5 year distant metastasis free survival rate were 49.3% 54.9%, 64.7% and 69.6% respectively. The recurrence pattern is different to the pathologic type, squamous cell carcinoma or adenoid cystic carcinoma. The patients with squamous cell carcinoma showed local recurrence and the patients with adenoid cystic carcinoma showed distant metastasis mainly. Conclusion: Primary cancer of external auditory canal showed different clinical course depend on the pathologic diagnosis.

Traumatic Asphyxia due to Blunt Chest Trauma with External Auditory Canal Bleeding (흉부 둔상으로 발생한 외이도 출혈이 동반된 외상성 질식 증후군)

  • Traumatic asphyxia, also called 'Perthes syndrome', is characterized by subconjunctival hemorrhage, cervicofacialpetechiae and cyanosis resulting from venous hypertension caused by an abrupt, severe, compressive force to the thoracoabdominal region. A 37-year-old male patient who was transferred to the emergency room due to chest trauma by overturning of a forkcrane. His head, neck, and shoulders showed severe ecchymosis, and his upper chest was cyanotic. There was bilateral subconjunctival hemorrhage and bilateral ear bleeding without tympanic rupture. Perthes syndrome is a rare condition and we treated a patient with typical and atypical symptoms; thus we report this case of Perthes syndrome.

Use of a Combination Imidacloprid and Permethrin for Psoroptic Mite Infestation in Three Pet Rabbits (세 마리 애완 토끼의 귀진드기 감염증에 imidacloprid와 permethrin 합제의 이용)

  • Psoroptic mites can cause severe pruritic otitis externa with crusts in rabbits. In this case report, three rabbits with a psoroptic mite infestation were treated with a formulation containing 10% imidacloprid and 50% permethrin at a dose of 0.4 ml regardless of body weight. One week after treatment, the rabbits showed mild pruritus with no crusts, mites or eggs. Four weeks after treatment, there were no clinical signs, mites or eggs observed in the rabbits. The rabbits were treated successfully with this combination without adverse reactions for 8 weeks after treatment. This case report suggests that a single topical application of a combination of imidacloprid and permethrin may be an effective and practical treatment for psoroptic mite infestations in pet rabbits.

Blocking Effect of Natural Hemp Fabrics on Mouse Skin (쥐의 피부에 미치는 천연 대마 섬유의 UV-B 차단 효과)

  • 천연 대마섬유의 UVB 차단 효과를 조사하고자 mouse를 4개의 그룹으로 구분하여 자외선을 조사한 뒤 외이의 조직절편을 제작하여 비교 분석하였다. 자외선을 직접 조사한 실험군[U], 대마섬유로 차단한 뒤 자외선을 조사한 실험군[H], 폴리에스테르 섬유로 차단한 후 자외선을 조사한 실험군[P], 자외선을 조사하지 않은 대조군[C]으로 나누어 실험한 후 광학현미경하에서 피부 조직의 변화를 관찰하였다. 대조군[C] 외이조직의 상피세포층 두께는 $16.3\;{\mu}m$이었지만, 자외선을 직접 조사한[U]군의 상피세포층은 $65.4\;{\mu}m$, [H]군은 $33.3\;{\mu}m$, [P]군은 $41.0\;{\mu}m$의 두께 변화를 보였다. 또한, 정상적인 외이의 상피세포의 단면적은 $22.0\;{\mu}m^2$이었으나, 자외선을 직접 조사한 [U]군의 상피세포층은 $37.7\;{\mu}m^2$, [H]군은 $30.1\;{\mu}m^2$, [P]군은 $30.2\;{\mu}m^2$이었다. 따라서 천연 대마섬유와 합성 폴리에스테르섬유 모두 UVB를 차단하는 능력을 갖고 있지만 전자가 후자보다 더 효과적으로 UVB를 차단함을 알 수 있었다.

Effects on Pressure Pain and Tactile Threshold by Auricular TENS at Shenmen Point (신문 반응점에 적용한 외이 경피신경전기자극이 압통각 및 촉각역치에 미치는 영향)

  • The purpose of this study were to determine the changes between pre, during, intermed, post of each two groups of 16 persons and to compare the effect of transcutaneous electrical nerve stimulation(TENS) at shenmen of auricular point on experimental pressure pain and tactile threshold measured at both ulnar styloid process and medial malleoli. Sixteen healthy adult men and women, aged 20 to 28 years, were assigned randomly to eight of one groups. Control group received TENS to exception of auricular point. Experimental group received TENS to shenmen of auricular point. Experimental pressure pain and tactile pain threshold at the both ulnar styloid process and medial malleoli was determined with algometer and von frey filament before 10 minute, during 10 minute, intermediate and post 30 min of treatment. In pressure pain and tactile threshold showed a statistically significant increase(p<0.05) ipsilateral and contralateral of treatment group. These results suggest that TENS at shenmen of auricular point has the capability to higher pressure pain and tactile threshold in whole body.

  • Relapsing polychondritis is a rare and sometimes fatal disease of unknown etiology, manifestating as an episodic and progressive inflammatory disorder, affecting predominantly the cartilage of the ears, nose and tracheobroncheal tree as well as internal structures of the eyes and ears. We experienced a case of relapsing polychondritis involving the larynx, tracheobroncheal tree, eye, nose and ear in a 20 year old male, required tracheostomy due to severe dyspnea.

  • Successful middle ear surgery requires the availability of al safe, effective bonding material. Side effect caused by synthetic materials have led to the use of biologic adhesive. However, they carry the risk of transmission of infectious diseases if they are prepared from pooled human blood. The adhesive strength of ammonium sulfate fibrin adhesive produce an adhesive strength that is half that of the homologous commercial product. It is, however, good enough for use in several otolaryngological operations, tympanoplasty, facial nerve repair, reconstruction of ossicles, reconstruction of posterior wall of ear canal and obliteration of frontal sinus and mastoid antrum using bone dust.

  • In this thesis, a nonlinear compression fitting method was studied for each frequency channel of a 64 channel digital hearing aid. Unlike conventional fitting formula method done from the result of the hearing loss test, the present fitting method uses the auditory threshold of sound pressure measured near the tympanic membrane while ITE (In-The-Ear) hearing aid is fitted into the user's ear canal. Also, the spectral distribution of the voice sound pressure was used for realizing of output sound pressure compression curves against input sound pressure level. Theoretical research results of FFT-iFFT compression algorithm has been evaluated by experimental gain measurements at each different input sound pressure level 50 dB, 70 dB, 90 dB respectively.

Effects of Transcutaneous Electrical Nerve Stimulation and Laser at Auricular Points on Experimental Cutaneous Pain Threshold (외이에 대한 경피신경 전기자극과 레이저가 실험적 피부 통증역치에 미치는 영향)

  • The purposes of this study were 1) to determine the changes between pre-treatment and post-treatment of four groups of 15 persons each and 2) to compare the effect of conventional transcutaneous electrical nerve stimulation(TENS) and laser at auricular acupuncture points on experimental pain threshold measured at the wrist. Sixty healthy adult men and women(M:32, F:28), aged 20 to 28 years, were assigned randomly to one of four groups. Group 1 received TENS to the appropriate auricular point for wrist pain, Group 2 received laser to the appropriate auricular point for wrist pain, Group 3 received placebo TENS to the appropriate auricular point for wrist pain, Group 4 received no treatment and served as controls. Experimental pain threshold at the wrist was determined with a painful electrical stimulus before and 20 minutes after ear stimulation. Group 1 was the only group that showed a stastically significant increase (p<0.05) in pain threshold after treatment whereas the Group 2,3 and 4 did not. These results suggest that TENS has the capability to higher pain threshold but laser does not.

Utilizing Pedicled Muscle Flap for Defect Coverage after External Auditory Canal Cancer Resection (외이도에 발생한 악성종양에서 유경근육피판을 이용한 결손 재건)

  • Purpose: Cancer arising from the external auditory canal is a rare disease. A lesion that seems harmless in someway, can be lethal when inadequately excised, the tumor may infiltrate nerves, the parotid and auditory tissues before re-invading the skin. Wide resection of the lesion surrounding the structure and reconstruction with an adequate plan is crucial for the treatment of this disease. Methods: Two patients with external auditory canal cancer were treated with muscle flaps and skin grafts. Lateral temporal bone resection (LTBR) was performed for complete resection of the cancer. The defect cavity was obliterated with highly vascularized tissue using pedicled sternocleidomastoid muscle, and temporalis muscle individually, combined with full thickness skin graft for covering the skin defect of the ear. Results: Clear resection margin was obtained, and both patients showed disease free survival during the follow up. There was no complications of hematoma, infection, flap loss, or wound problem in both patients. Both patient received radiation therapy, there was no osteoradionecrosis or any other complication related to radiation therapy. Conclusion: Utilizing pedicled muscle flaps for managing defects after wide resection of the external auditory canal cancer is an effective method for managing this difficult disease.
