A Study on the Analysis of Factors Affecting Patients' Way-Finding in the Hospital - Focused on a Case of 'A' General Hospital - (병원건축 길찾기 요소의 중요도 분석에 관한 연구 -'A'종합병원 외래진료부를 중심으로-)

  • Upon reviewing the tendency of the large-scale hospital buildings constructed so far, we may well know that the architectural space of the hospital buildings has been designed for users' conveniences in terms of functional elements fulfilled, efficiency, uniform colors and lighting, which means that patients may well feel difficult to find their ways in the hospital. The result may be that the patients who are weak mentally and physically feel uneasy or tensed when they wander in the hospital, which may lead to their increased physical fatigue, higher blood pressure or other harmful effects. Thus, patients' way-finding emerges as a new design challenge for the hospital building. The purpose of this study was to define the environmental variables and factors affecting patients' way-finding in the hospital and thereby, determine the correlations between the variables/factors and way-finding to provide for some framework of analysis useful to solutions of patients' way-finding problems, while identifying the causes for difficulty of their way-finding due to differences of perception between hospital building designer and user groups.

Medical Service Factors Associated with Patient Satisfaction: The Difference between Inpatient and Outpatient of A Medium-sized Hospital (환자만족도에 영향을 미치는 의료서비스 품질 요인: A 중소병원 입원.외래환자 간 차이)

  • This study attempts to analyze medical service factors associated with patients' satisfaction, and its difference between inpatients and outpatients in A medium-sized hospital located in rural area. Questionaries have been collected from 411 patients of 96 inpatients and 315 outpatients. The survey was carried out for 4 weeks, from August 20, 2011. The study found that hospital awareness was only factor related to patients' satisfaction in inpatients. In the case of outpatients, several factors were related to patients' satisfaction. They are included qualified health care providers, infirmary accommodations, staff kindness, hospital awareness, and convenient procedure of treatment and admission. Based on the findings, it is desirable to differentiate the strategy in improving patients' satisfaction between patient type; either inpatient or outpatient to being competitive.

  • In recent, hospital information systems are widely used to electronically record, manage and share the data collected in hospitals. Such systems have contributed greatly to improving the work efficiency in modern hospitals, however, the collected data concerning the patients should be appropriately processed and reused to provide the healthcare service providers with decision supports. Especially, this paper proposes the patient flow monitoring system for the operations management of the outpatient department for patients with chronic diseases, and discusses the related issues. The proposed system visualizes the standard process model extracted from the patient history data and various performance measures, and this enables the managers to evaluate and enhance the operations of the outpatient clinic. In this paper, the patient flow monitoring system is applied to the rheumatology clinic, and the prototype system optimized for I-pad is illustrated.

A STUDY ON THE DENTAL TREATMENT UNDER OUTPATIENT GENERAL ANESTHESIA (서울대학교치과병원 장애인진료실에서의 치과치료 분석)

  • Seoul National University Dental Hospital opened The Clinic for The Disabled to provide dental treatment under outpatient general anesthesia. 432 patients underwent general anesthesia for dental treatment. The informations such as the pattern of dental treatment, the reasons of general anesthesia, the characteristics of the patients were analyzed. Followings are concluded. 1. The patients below 15 years old made up 50.60%. 2. The reasons for general anesthesia included mental and physical disabilities, systemic disease, dental phobia, and so on. 3. Restorative treatment was the most common procedure with the average of 9.7 teeth treated per one patient. 4. 62(14%) patients underwent general anesthesia for dental treatment more than once. 5. The efforts for better multidisciplinary system to provide more comprehensive and effective dental treatment including periodic recall check, preventive treatment for handicapped patients are necessary.

Determinants of Patients Satisfaction and Intent to Revisit Oriental Medical Hospitals (한방병원 환자 만족도 및 재이용 의사 결정요인)

  • This study aimed to investigate the determinants of patients satisfaction and intent to revisit oriental medical hospitals. The level of overall patient satisfaction was used as an intervening variable and the level of intent to revisit was used as a dependent variable. The sample used in this study consisted of 578 patients from 3 oriental medical hospitals located in Chungnam Province. Data were collected from October 2014 to December 2014 with a structured and self-administrated questionnaire and analysed using path analysis. The results of the study indicate that oriental medical hospitals should make an effort to improve the overall satisfaction of patients, in the case of outpatient, especially focusing on the doctors' service and medical procedure, and in the case of inpatient, especially focusing on the facilities convenience, medical price and hospital ward life which will lead to high level of intent to revisit of patients.

Informally Patients Prediction Model of Admission Patients (입원환자 데이터를 이용한 예약부도환자 이탈방지 모형 연구)

  • The aims of this study is to medical record data warehouse which had been collected from hospital information systems. continuous patient 2,118 60.5%, informally patient 1,385 39.5%. In using survival factors sex, age, area, insurance, admission-course, medical treatment, out-patient lesson, out-patient form, conference diagnosis, operation, cancer, medical reservation. As a result of making a predictive modeling using the logistic regression, the fitness of the predictive modeling of informally patient was 66.0% and neural network, the predictive was 66.72% and CHAID, the predictive was 63.25%, which is a data mining. The expected modeling of the informally patients, the hospital through the continuous patient management and trust of hospital.

Effects of Nurse-led Patient Education for Gout Patients (통풍환자에 대한 간호사 주도 환자교육의 효과)

  • Objectives: This study was performed to analyze the effect of nurse-led education on results for patients with gout. Methods: The face-to-face education was conducted by a specialist nurse. Patient satisfaction and drug adherence, serum uric acid was assessed. Results: Patient satisfaction was significantly higher in the nurse-led education group than in the usual care group. The serum uric acid level on the second visit was lower after patient education. In addition, the mean drug adherence in the nurse-led group was significantly higher than in the usual care group on the second and third visits. Conclusion: The nurse-led education about gout improves patient satisfaction, drug adherence, and serum uric acid level in patients with gout.

The Study on Trends and Factors of inpatient care of the province residents provided in Seoul (지방 환자의 서울 지역 입원진료의 추이 및 요인에 관한 연구)

  • 본 연구의 목적은 지방 환자의 서울 지역 입원진료의 추이를 파악하고 그 요인을 규명하는 데 있다. 이를 위해 2005년 및 2008년 환자조사 입원자료를 이용하였으며, 서울지역 거주 환자를 제외하고 2005년 333,280명, 2008년 419,873명을 연구대상으로 하였다. 자료분석은 기술통계, 카이제곱 검정, 로지스틱 회귀분석을 실시하였다. 2005년 대비 2008년 성별, 연령별, 의료기관 유형 등 일반적 특성의 분포는 유사한 것으로 나타났다. 지방 환자의 서울지역 이용은 다소 증가한 것으로 나타났으며, 서울 지역 입원진료는 남자, 중장년층 건강보험환자가 타기관에서 의뢰되어 외래를 통해 입원하며, 주 거주지가 경기, 강원, 충북, 충남, 제주지역 순으로, 광역시는 상대적으로 낮았다. 질병군별로는 선천성 기형, 신생물, 종양이나 수술후 추후치료, 눈 질환, 혈액 조혈 면역기 질환, 근골격계 질환 순으로 지방환자의 서울지역 의료기관 입원 이용률이 높았다. 그러나 상대적으로 지방 입원진료 확률이 높은 노년층, 의료급여, 응급경유, 질병군별로 중증도가 높은 환자가 혼재되어 있어 있을 가능성이 있어 향후 중증도 보정에 대한 심층 연구가 필요한 것으로 판단된다.

  • 본 연구는 환자의 의료기관 선택기준에 관한 조사를 실시하는데 있다. 외래, 중 병원에서 검사한 사항을 CD로 담아 타 병원 진료 후 현 병원에 내원한 대상자와 본 병원에서 진료 후 CD를 가지고 타 병원으로 전원 하는 환자 및 보호자를 대상으로 2009년 10월 5일부터 10월 26일까지 본 연구에 동의하는 대상자 302명을 대상으로 하였다. 내원 전원 환자의 물리적 특성에서 주차시설, 주위시설, 의료진 서비스 품질에서는 간호사 친절도, 전문성, 설명, 의료 서비스 품질 특성에서는 의료기기 최신설비, 병원 평판에서는 의료인력 의술, 주위 환경, 병원 외적 요인에서는 진료의 권유 항목이 유의하였다(p<0.05). 내원 시 병원 선택 변수 평균치 검정 결과 중 성별에서는 만족도, 연령별에서는 의료진 서비스품질, 거주지에서는 병원외적요인, 병원지역에서는 병원평판, 만족도, 진료기관에서는 병원평판, 만족도, 진료구분에서는 만족도에서 유의하였다(p<0.05). 전원 시 병원 선택 변수 평균치 검정결과 중 연령별에서는 의료진 서비스 품질, 거주지에서는 병원외적요인, 병원지역에서는 병원평판, 만족도, 진료기관에서는 병원평판, 만족도, 진료 구분에서는 만족도가 유의하였다(p<0.05). 본 연구에서는 내원 전원 환자는 물리적 환경, 의료서비스 품질, 병원 평판 등 을 병원 선택에 중요한 요인들로 지각 하고 있으며 간호사와 주위 사람들의 권유에 의하여 병원을 선택한다고 나타났다. 내원 환자는 의료진 서비스 품질, 병원 평판, 만족도에서 평균치가 높게 나타났다. 전원 환자는 물리적 환경, 의료진 서비스품질, 의료 서비스 품질, 병원 평판, 만족도에서 평균치가 높게 나타났다. 자료 분석은 SPSS 12.0 Window Program을 이용하였다.

The Distribution and Treatment of Outpatients with General Anesthesia in Chonbuk National University Dental Hospital for 9 Years (최근 9년간 전북대학교 치과 병원 외래 전신마취 환자 분포 및 치료)

  • General anesthesia (GA) for dental care in handicapped patients is necessary to facilitate the provision of safe, efficient, and effective quality treatment. The aims of this study were to determine the anesthetic characteristics of handicapped patients in need of dental treatment in these day care units, and to establish for plan to provide better services. 325 patients who had outpatient general anesthesia from January 2005 to March 2014 were assessed for this study. Patients' distribution and treatment patterns were examined. The proportion of male patients (202, 62.2%) was higher than female patients (123, 37.8%) and the largest group of patients were 5 to 10 years old (85, 26.2%). The reasons for general anesthesia included mental and physical disabilities (207, 63.7%), behavior management (84, 25.8%), parent needs (14, 4.3%), and so on. Restorative treatment was the most common procedure with the average of 4.2 teeth treated per one patient and 43 (13.2%) patients underwent general anesthesia for dental treatment more than once. To expand and improve access to the dental care of the disabled, improvement of the health care system, enhancement of their training for dental care by professionals, and enlarging caregivers'understanding of the importance of oral care in the early stages are required.
