The Effects of Nutritional Education Program on Nausea and Vomiting, Anorexia, Food Intake, and Nutritional Status of GI Cancer Patients Undergoing Chemotherapy (영양교육프로그램이 항암화학요법을 받는 소화기암환자의 오심구토, 식욕부진, 식이섭취 및 영양상태에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of nutritional education program (NEP) on nausea and vomiting, anorexia, food intake, and nutritional status among GI cancer patients undergoing chemotherapy. Methods: A nonequivalent, non-synchronized posttest design was used. Study subjects were conveniently selected 30 patients (15 subjects for each group) who were undergoing chemotherapy. The subjects of the experimental group received NEP which consisted of 2 educations and 1 counselling while they were hospitalized, and telephone counselling with nutritional supports after discharge for four times. Results: The score of anorexia was lower and the amount of food intake was higher in the experimental group than those of the control group. However, there were no significant differences in levels of nausea and vomiting, weight, hemoglobin, albumin, and total protein level between the experimental and the control group. Conclusion: The findings demonstrate that the NEP was effective in alleviating anorexia and improving subjective food intake of GI cancer patients undergoing chemotherapy. These findings suggest that NEP can be a useful nursing intervention for preventing nutritional disorders for patients with GI cancer after chemotherapy.

A Meta-Analysis of Intervention Studies on Nausea and Vomiting in Cancer Patients (국내 암환자의 오심구토에 적용한 비약물적 중재효과의 메타분석)

  • Purpose: This study was designed to analyze the characteristics and effect size of published intervention studies related to nausea & vomiting among cancer patients. Methods: A total of 1,083 studies were retrieved. From these studies, 20 studies met the inclusion criteria with a total of 698 participants. Two authors independently extracted data from the selected studies and assessed the methodological quality. The data were analyzed by the RevMan 5.0 program of Cochrane library. Results: The twenty studies utilized various non-pharmacological interventions: Nine studies on acupuncture (45%), Five studies utilized massage (25%), Two studies used oral cryotherapy (10%) and Four studies used other therapies. In the twenty studies the effect size of the intervention studies reported a higher effect sizes for massage (d=-1.62) and acupuncture (d=-0.89). Conclusion: This study suggests that non-drug therapy can reduce the levels of nausea and vomiting intensity. Massage and acupuncture interventions studies were more numerous and this may account for the higher effect rate.

Effects of the Nei-Guan Acupressure by Wrist Band on Postoperative Nausea and Vomiting after Middle Ear Surgery (손목밴드를 이용한 내관지압이 중이수술 후 오심과 구토에 미치는 효과)

  • Purpose: This study was to identify the effects of Nei-Guan accupressure using a wrist band on postoperative nausea and vomiting after middle ear surgery. Methods: The study design was a nonequivalent control group non-synchronized posttest design. Forty patients were assigned into two groups of 20 patients, the wrist band group and the control group. In the wristband group, acupressure was applied with a wrist band on the P6 point at both wrists from 30 min before to 24 hr after anesthesia. Results: The 1st hypothesis 'Applied wrist band groups will have a greater reduction in the nausea & vomiting grades by INVR than the control group' was accepted (t=2.303, p=.028). The 2nd hypothesis 'The Applied wrist band groups will have a greater reduction in the occurrence of nausea & vomiting than the control group' was partly accepted. The 3rd hypothesis 'The applied wrist band group will have a greater reduction in injection of antiemetics for 24 hr. post anesthesia than the control group' was rejected. Conclusion: Acupressure on the P6 point using a wrist band is effective to alleviate nausea for middle ear surgery patients.

Nausea/Vomiting and Anxiety of Hospitalized Cancer Patients Receiving Chemotherapy (항암화학요법을 받는 입원환자의 오심·구토 및 불안 정도에 관한 조사연구)

  • Purpose: The purpose of this study was to describe the pattern of changes on the score of nausea/vomiting and anxiety during one cycle of chemotherapy. Method: A total of 53 subjects who were admitted to C University Hospital for a period of 3 days and 2 nights for chemotherapy were selected from February to April, 2003. Total scores of nausea/vomiting were measured twice a day 3 days for a total of 6 measurements. Anxiety, anorexia, and fatigue were also measured at the first and last measurement points. Data were analyzed by one-way repeated measures, ANOVA, t-test, paired t-test, & Pearson's correlation. Result: The score of nausea/vomiting increased over time except for the 4th measurement point but no changes were significant over time. There were the significant differences between 1st and 2nd, and 2nd and 3rd nausea/vomiting score at p < 0.05. The scores of anxiety, anorexia, and fatigue between the first and 6th points were significantly different(t=-5.69, p=.001; t=6.25, p=.0001; t=3.65, p=.0007). Conclusion: Further studies are needed to identify the relationship between anxiety, and anticipatory and acute nausea/vomiting respectively.

Effects of Aroma Oil Inhalation on Nausea·Vomiting and Anorexia in Cancer Patients Receiving Chemotherapy (향기흡입이 항암 화학요법 환자의 오심·구토와 식욕부진에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of aroma oil inhalation using peppermint and bergamot on nausea vomiting and anorexia in cancer patients receiving chemotherapy. Method: Study subjects were 30 patients who had experienced nausea and vomiting when they had been hospitalized in K university hospital located in D city after receiving more than two Cisplatin combination chemotherapy treatments. Among them 15 patients were in the experimental group doing aroma oil inhalation and the other 15 patients were in the control group without aroma oil inhalation. The data were collected from February 1, 2002 to May 17, 2002. The data were analyzed with SPSS WIN 10.0 program using frequency, percentage, $x^2$-test, t-test, Repeated Measures ANOVA. Result: 1) The degree of nausea and vomiting in the experimental group with aroma oil inhalation using peppermint and bergamot were significantly lower than that of the control group. 2) The degree of anorexia in the experimental group with aroma oil inhalation using peppermint and bergamot were significantly lower than that of the control group. Conclusion: aroma oil inhalation was effective for relieving patients' nausea vomiting and anorexia receiving chemotherapy. Therefore, it is proposed that aroma oil inhalation should be applied as a supportive nursing arbitration method to relieve patients' nausea vomiting and anorexia who are receiving chemotherapy.

  • Purpose: The present study was to investigate the effect of music therapy on anxiety, nausea, and vomiting in patients undergoing total body irradiation (TBI) for hematopoietic stem cell transplantation (HSCT). Method: A untreated control group with pretest and posttest was used in this study. A total of 35 patients receiving TBI for HSCT were recruited from a medical center at a university in Korea. The music therapy taking for 15-20 minutes per a time was performed twice a day for three days while the patients in the experimental group were undergoing the TBI. Results: The scores of state anxiety, nausea and vomiting tended to increase in the control group as compare with those in the experimental group. The patients in the control group had been administerd antiemetics more often than those in the experimental group. The levels of systolic and diastolic blood pressure and the pulse rates in the experimental group tended to decrease after the experiment. However, there was no statistically significant differences in anxiety, nausea, vomiting, blood pressure, and pulse between the groups. Conclusion: It is suggested to repeat this study with a enough sample size.

Combination Effects of Capsicum Plaster at the Korean Hand Acupuncture Points K-D2 with Prophylactic Antiemetic on Postoperative Nausea and Vomiting after Gynecologic Laparoscopy (고추파스를 이용한 수지요법 침점(K-D2) 자극과 예방적 항구토제의 병용요법이 부인과 복강경 수술 후 오심과 구토에 미치는 효과)

  • Purpose: This study was done to evaluate the combination effects of capsicum plaster at the Korean hand acupuncture points K-D2 with prophylactic antiemetic on Postoperative Nausea and Vomiting (PONV). Methods: An experimental research design (a randomized, a double-blinded, and a placebo-control procedure) was used. The participants were female patients undergoing gynecologic laparoscopy; the control group (n=34) received intravenous prophylactic ramosetron 0.3mg, while the experimental group (n=34) had Korean Hand Therapy additionally. In the experimental group, capsicum plaster was applied at K-D2 of both 2nd and 4th fingers by means of Korean Hand Therapy for a period of 30 minutes before the induction of anesthesia and removed 8 hours after the laparoscopy. Results: The occurrence of nausea, nausea intensity and need for rescue with antiemetic in the experimental group was significantly less than in the control group 2 hours after surgery. Conclusion: Results of the study show capsicum plaster at K-D2 is an effective method for reducing PONV in spite of the low occurrence of PONV because of the prophylactic antiemetic medication.

The Meta-Analysis of the Effect of Acupressure for Nausea and Vomiting in Cancer Patients Receiving Chemotherapy (지압이 항암요법환자의 오심과 구토에 미치는 효과 관련 연구 분석)

  • Purpose: This study was to analyze the characteristics and effect size of intervention studies that used acupressure for the control of chemotherapy-induced nausea and vomiting (CINV). Methods: Nine electronic databases including Korea Education Research and Information, the National Assembly Library, KISS, Korea Med, NDSL, PubMed, EBSCO, Cochrane library, and OVID (to December, 2009) were searched. Thirteen studies of randomized controlled trials (RCTs) or controlled clinical trials (CCTs) were selected. Data were analyzed by SPSS/WIN 18.0. Results: Overall effect size of acupressure was moderate (0.684). Nausea had a moderate effect size (0.770) and nausea and vomiting had a high effect size (0.899). Acupressure using sticker needles had a high effect size (1.368) among finger acupressure, si-acupuncture, and wrist band treatment. Conclusion: This study suggests that acupressure using sticker needles on the spots of nei-guan, chok-samni and hap-kok can reduce the levels of nausea and vomiting in cancer patients receiving chemotherapy. For improving the reliability of the meta-analysis results, further randomized controlled trials with better study methodology are needed.

관해유도 항암요법을 받는 백혈병 환자에서 예방적 항진균제 투여의 효과 -fluconazole과 nystatin의 비교-

  • 대상환자는 모두 23명이였으여, 이중 12명은 fluconazole을, 11명은 nystatin을 투여받았다. 두 군사이에 연령, 남여비, 기저질환, 진균감염의 위험요소는 차이가 없었다. 투약기간은 fluconazole군(F)이 24일, nystatin군(N)이 23일이였다. 중도탈락은 3례로 F중 MRSA ?혈증 1례, N중 오심, 구토 1례외 Stevens-Johnson증후군 1례였다. 평가가능한 20례중, F 11례중 8례와, N 9례중 4례에서 진균감염중의 예방에 성공하였고, F 3례와 N 5례는 실패하였다. 실패한 F 3례는 코의 피부조직에서 aspergillus가 중명된 1례와 경험적으로 amphotericin을 투여한 2례였고, N 5례는 혈액배양에서 Candida tropicalis가 분리된 1례와 경험적 amphotericin을 받은 4례였다. 진균의 colonization은 F중 3균주는 치료중 소실되었으나, 2례는 증가하였다. N은 1균주가 소실된 반면, 3례는 증가하였다. Colonization이 증가한 F 2례는 C. albicans와 Trichosporon beigelli가 대변에서 분리된 경우이고. M 3례는 대변에서 C. albicans가 분리된 1례, 구강에서 aspergillus가 분리된 1례, 혈액배양에서 C. tropicalis가 분해된 1례였다. 부작용은 오심 구토가 F중 1례, N중 2례 였으며, 이중 N의 1례는 증상이 심하여 투약을 중지하였다. 간기능 검사상 SGOT/SGPT의 상승은 F 12중 3례, N 11중 2례에서 관찰되었으나, 두 군사이에 통계학적으로 유의한 차이는 없었다.
