고령위암 환자에서 술 후 처치 및 영양지원

  • 1. 병원에 입원하는 환자들 중 상당수에서 어느정도의 영양실조가 있는 것으로 보고되므로 수술하기 위해 입원하는 환자에서 영양과 관계있는 측면에서 병력청취 및 이학적 검사상 약 6개월간 평소체중의 $10\%$ 이상 감소, 혈장단백(알부민, 트란스페린 등) 저하, 총임파 구수감소 등이 있으면 철저한 영양평가를 시행하여 중등도 이상의 위험군에서는 술 전 약 $7\~10$일간 영양지원을 하는 것이 약 $10\%$의 술 후 합병증을 줄이는 것으로 보고되나 이는 수술의 스트레스 정도, 술 후 영양 섭취 지연의 기간 및 정도 및 수술의 응급여부에의 해 결 정한다. 2. 영양지원이 결정되면 그 경로는 경장이 좋으며 그것이 안될 때 정맥영양을 시행한다. 3. 술 전 영양지원을 받지않은 중등도 이상의 영양실조 환자에서 술 후 조기 영양지원에 대한 연구에서는 TPN군에서 대조군보다 오히려 약 $10\%$의 합병증 발생률이 높다. 4. 영양실조가 없는 환자에서도 술 후 약 7일 이상의 금식이 예상되면 TPN을 시행하는 것이 좋다.

A comparison of nutritional status by intensive nutritional support in enteral nutrition patients (경장영양을 시행한 환자에서 영양집중지원에 따른 영양상태 비교)

  • Purpose: The aim of this study was to determine the effects of nutritional intervention focused on a Nutrition Support Team (NST) in patients receiving enteral nutrition (EN) in general hospital wards. Methods: The electronic medical records of 95 adult patients admitted in C university hospital and received EN supply for more than 3 days at a general ward were analyzed retrospectively. The subjects were classified into the intervention group (n = 40) and non-intervention group (n = 55). Results: The calorie support rate (%) and protein support rate (%) increased significantly only in the intervention group after 2 weeks compared to the rate upon admission. The serum albumin levels increased in the intervention group after 2 weeks compared to the levels at admission, but decreased in the non-intervention group. The glucose levels decreased only in the intervention group compared to that at admission. Conclusion: The nutritional status of the patients was improved by the proper planning of nutrition management from the beginning of hospitalization and systematically managing the nutrition intervention of the NST.

A Study on the Current Status and Needs of Nutrition Education on Children's Sugar Intake Reduction among the Center for Children's Foodservice Management and Child Care Facilities (어린이급식관리지원센터와 보육시설의 유아 당류 섭취 줄이기 영양교육 실태 및 요구도)

  • This study investigated the current status and needs for nutrition education to help reduce children's sugars intake at the Center for Children's Foodservice Management (CCFM, n=115), and Child Care Facilities (CCF, n=646) through an online survey conducted from October $5^{th}$ to $30^{th}$ 2015. A total of 14.8% of CCFM respondents and 31.9% of CCF respondents provided nutrition education on sugars intake to young children as a main topic (p<0.001). A higher percentage (CCFM 47.8%: CCF 42.4%) delivered nutrition education on sugars intake to young children as a sub-component (p<0.001). Over 90% of the CCFM and CCF participants agreed on the necessity of providing nutrition education on sugars intake to children. The most common reasons given for delivering nutrition education on children's sugar intake were "there are many more urgent nutrition education topics" for CCFM, and "insufficient nutrition education information and materials" for CCF. The percentage of nutrition education on children's sugar intake provided to the children's parents was low showing about 20% in the both groups. The percentage of CCFM participants providing nutrition, education on children's sugar intake to the teachers in CCF was also low, showing about 14.8%; however, 68.0% of the CCF participants wanted to received teacher's education on guiding children's sugar intake. Regarding ideas about a nutrition education program on children's sugar intake for young children, most respondents in both groups answered "sugar intake and dental cavities or obesity" for appropriate education contents, "story telling or puppet show" for appropriate education methods, and "dietitian from CCFM and class teacher together" for appropriate educator. For appropriate education time, there was a significantl difference between the CCFM responses (average 2.7 times) and the CCF responses (average 4 times). Based on the above results, we found that implementing nutrition education on children's sugar intake at the CCFM and CCF, was low; however, awareness of the need for nutrition education on children's sugar intake and the program development and supply was very high. Also, the opinions of CCFM and CCF participants about a nutrition education program on children's sugar intake for young children can provide foundation data to develop and implement the CCFM-based nutrition education program.

Comparison of the nutritional indicators of critically Ill patients on extracorporeal membrane oxygen (ECMO) (체외막산소화장치 (ECMO) 적용 중환자의 영양지표 비교)

  • Purpose: This study aimed at investigating the nutritional status, nutritional support, and nutritional indicators of critically ill patients on extracorporeal membrane oxygen (ECMO) in intensive care units (ICU). Methods: This descriptive study obtained data from the medical records of 37 patients who were treated using ECMO at a university hospital in Korea. The patients were admitted to the Korea University Anam Hospital ICU with acute or serious damage to vital organs from January 1, 2014 to June 30, 2016. Results: Although 32 patients (86.5%) were at a high risk of malnutrition, 26 patients (70.3%) were considered normal in terms of nutritional status by the nutritionist at the beginning of their ICU stay. However, after two weeks, nine patients had passed away and only one patient maintained normal status. Parenteral nutrition was started first but took 4.25 ± 6.95 days till initiation. Only eight patients (21.6%) were able to meet their requirement for both calories and protein. The group provided with adequate calorie and protein showed significantly longer use of the ECMO and respirator and longer ICU and total hospital stay than their counterparts. Normal levels of serum albumin and protein of the group at low-risk for malnutrition on the day of initiation of ECMO, which were significantly higher than the high-risk group, declined by the last day of ECMO leading to a lack of significant differences between the two groups. Conclusion: Considering that the nutritional indicators of patients deteriorated as the days on ECMO increased, more aggressive nutritional management to ensure adequate nutritional support should be emphasized from the beginning and throughout the ICU stay.

  • Prevalence of allergic diseases is influenced by environment and dietary life. It is key to improve daily food life to relieve them. Food nutrition labeling is useful to do it by offering nutrition information. The purpose of the study is to find relationship between experience of diagnosis of allergic diseases and recognition of food nutrition labeling. The data of 4,928 people with experience on diagnosis allergic rhinitis, asthma, atopic dermatitis of 2016 Korea National Health and Nutrition Survey was used. According to the result of binary logistic regression analysis, those who had experience in being diagnosed with an allergy showed high awareness in food labels. There were differences between allergy diagnosis groups and allergy non-diagnosis in affecting factors of residence, income level, subjective health status and body-shape perception. Support measures are needed to enhance access and convenience to nutrition education and nutrition labeling to support nutrition labeling utilization.

  • The purpose of this study was to examine the experiences of teachers during the development and performance of a nutrition education program based on collaboration between the school organization and local community, as the experiences of teachers who participate in a nutrition education program might be one of major factors to determine the success or failure of their performance. It's basic goal is give some suggestions on school nutrition education and to explore how to assist teachers to ensure their successful performance of a nutrition education program. As a result of analyzing the collected data, the experiences that the teachers had in the program planning stage were subcategorized into two groups: 'finding hope amid complaints' and 'building a learning community'. The experiences that the teachers had in the acting stage were subcategorized ino two groups as well: 'regulating, equalizing and forming a new schema' and 'building stepping stones'. The experiences that they had in the evaluation stage were also subcategorized into two groups: 'finding it rewarding and thankful to teach' and 'new challenge'. The findings of the study are expected to provide essential information on how to assist teachers to offer effective and successful nutrition education.

Nutritional Support, Gastric Residual Volume and Nutritional Status during Enteral Nutrition in Intensive Care Unit Patients (중환자실 경장영양 환자의 영양지원, 위 잔여량 및 영양상태)

  • Purpose: The purpose of this study was to investigate the nutritional support, gastric residual volume, and nutritional status of the intensive care unit (ICU) patients on enteral feeding. Methods: A descriptive longitudinal design was used to collect 5 day data on enteral nutrition of 52 ICU patients in an university hospital. Nutritional support was calculated with actual caloric intake compared to individual caloric requirement. Residual volumes were measured prior to routine feedings, and the serum albumin levels and the total lymphocyte counts were checked to evaluate nutritional status. The data were analyzed using one group repeated measures ANOVA, paired t-test, and Spearman's bivariate correlation analysis. Results: The subjects received their first enteral feeding on the $5.75^{th}$ day of ICU admission. The mean nutritional support rate was 49.1% of the requirement, however prescription rate and support rate were increased as time goes by. Gastric residual volumes were less than 10 cc in 95% cases. A significant negative correlation was found between nutritional support and nutritional status. Conclusion: The nutritional support for ICU patient was low compared to the requirement, and their nutritional status was worse than at the time of ICU admission. Further studies are necessary to develop nursing interventions for improving nutritional support for ICU patients.

Evidence-based Nutritional Support in the Intensive Care Unit (중환자를 위한 근거중심 영양지원)

  • Optimal nutrition serves to maintain normal organ function and to preserve body energy stores to guarantee survival during times of shortage of food. Adequate nutrition of intensive care unit (ICU) patients improves outcome, while malnutrition is strongly associated with increased morbidity and mortality rates among critically ill patients. Previously published researches showed that trials of nutritional support in critical illness rarely fulfill basic quality requirements. Nutrition support plays a vital role in the prevention and treatment of nutritional deficiencies in at-risk, critically ill patients. This paper reviewed the challenges in determining critically ill patients' nutrition requirements including nutrition assessment, determination of caloric requirements then providing them with adequate nutrition support while in the ICU with the guidelines published by Society of Critical Care Medicine and American Society for Parenteral and Enteral Nutrition. Nutrition support can be effectively enhanced by using the guidelines.
