Fabrication of palatal lift prosthesis using thermoplastic resin for a patient with velopharyngeal insufficiency (구개인두부전증 환자에서 열가소성레진을 이용해 연구개거상장치를 제작한 증례)

  • Recently, flexible removable prosthesis with thermoplastic resin clasp has increasingly become popular. In comparison with conventionally used acrylic resin, thermoplastic resin has lower flexural strength and elastic modulus. Thus, flexible removable prosthesis has low risk of fracture, so denture base can be made thin and light, increasing patient comfort. Also, it can passively sit at tooth undercut during rest, so abutment teeth need minimum or no preparation. In this case report, a 44 year old female patient with mild velopharyngeal insufficiency was treated with a palatal lift prosthesis made of polyester thermoplastic resin. Since the patient had no missing tooth and desired conservative treatment, the flexible removable prosthesis provided relatively satisfactory results.

Nonthermal plasma on the shear bond strength of relining resin to thermoplastic denture base resin (열가소성 의치상 레진과 첨상용 레진의 접착 강도에 저온플라즈마가 미치는 효과)

  • Purpose: This study evaluated the effect of nonthermal plasma treatment on the bond strength of autopolymerizing relining resin to the injection molded thermoplastic denture base resins (TDBRs) with different surface treatments. Materials and methods: Acrylic Resin (Acrytone), Polyester (Estheshot-Bright), Polyamide (Valplast) and Polypropylene (Weldenz) were subjected to various surface treatments: No treatment, Nonthermal plasma, Sandblasting, Sandblasting and nonthermal plasma. Specimens were bonded using an autopolymerizing relining resin. Shear bond strength was tested using universal testing machine with crosshead speed of 1 mm/min. Statistical analysis by two-way analysis of variance with Tukey's test post hoc was used. Results: Acrytone showed significantly higher shear bond strength value among other TDBR group while Weldenz had the lowest. The sandblasting and nonthermal plasma condition had significantly higher shear bond strength value in all of the resin groups (P < .05). Conclusion: The use of nonthermal plasma treatment showed limited effect on the shear bond strength between TDBRs and relining resin, and combination of nonthermal plasma and sandblasting improved the shear bond strength between TDBR and reline material.

  • For the rehabilitation of fully edentulous patients, implant-supported removable partial dentures can be considered as one of the treatment options with complete dentures or implant-supported overdentures. If removable partial dentures are used in combination with a small number of implants placed in strategically important positions, it can offer additional stability, retention and support through implants and reduce a burden of surgical procedures compared with fixed implant-supported prostheses with extensive implant placement. Moreover, the economical benefit can be expected as well. The purpose of this case report is to present a treatment in which an implant-supported removable partial denture was fabricated considering residual alveolar bone status and demands after teeth loss in a patient who had been using a distal extension removable partial denture for a long period of time. In anterior area, fixed prostheses were fabricated with implant placement and in posterior area, short implants provide only support for the removable partial denture. In addition, denture base and clasp were made of thermoplastic acrylic resin. Finally, functionally and aesthetically satisfying treatment results can be achieved.

  • Removal of excess cement is important to prevent biological complication in cementation of implant restoration with subgingival margin. It can be difficult to completely remove excess cement. Several techniques have been introduced to minimize excess cement using abutment replica. In this case report, a simple method for making abutment replica with hot melt adhesive material in dental office was described. This technique is simple and effective because it can be used for pre-fabricated or custom abutment without additional laboratory procedure. In addition, it can minimize excess cement after cementation of implant restoration.

  • The purpose of this study was to evaluate the bond strength of reline resin to pressure injection type thermoplastic denture base resin. The denture base resins used in this study were $Hi-polycarbonate^{(R)}$(High Dental Co., Japan), Acetal $dental^{(R)}$(Pressingdental s.r.1., Repubblica di San Marine) of thermoplastic resin and Acron $MC^{(R)}$(GC Dental Industrial Co., Japan) of heat cured resin. The reline resins used were Lucitone $199^{(R)}$(Dentsply international Inc., USA), Tokuso $rebase^{(R)}$(Tokuyama Corp., Japan), and $Lightdon-U^{(R)}$(Dreve-Dentamid-Gmbh, Germany). The reline resins are representative of heat-cured, self-cured, and light-cured resin respectively Bond strength was examined by use of a three-point transverse flexural strength test. The results were as follows 1. The bond strength of Lucitone 199 to Acron MC was the highest. 2. The bond strengths of Lucitone 199 and Tokuso rebase to Hi-polycarbonate resulted in a value of approximately one half that of Lucitone 199 to Acron MC and there were no significant differences between these and the bond strength of Tokuso rebase to Acron MC(p<0.05) 3. The bond strengths of reline resins to Acetal dental were lower than those of reline resins to Hi-polycarbonate. 4. For all base resins Lightdon-U showed lower bond strength than the other reline resins.

  • The maxillary defects lead to anatomical and functional deformity of the maxillofacial region. As far as functions are concerned, such defects can produce difficulty in speech, mastication, and deglutition. Obturator prostheses play a very important role in functional recovery for post-maxillectomy patients. To achieve rigidity of the obturator, appropriate retention should be given, and the weight of the prosthesis be reduced. There are two types of hollow bulb obturator: open and closed. A closed type has many advantages. Nevertheless, some problems, including complexity of fabrication and water leakage into the bulb, have the closed hollow obturator not be widely used. The one-step polymerization technique described in this case overcomes the shortcomings by easily constructing a small hollow bulb with two thermoplastic resin sheets.

Evaluation of retrievability using a new soft resin based root canal filling material (레진 계통의 근관 충전재의 제거 용이성에 대한 평가)

  • The aim of this study was to evaluate the retrievability of Resilon as a root canal filling material Twenty-seven human single-rooted extracted teeth were instrumented utilizing a crown down technique with Gates-Glidden burs and ProFile system. In group1 (n = 12) canals were obturated with gutta percha and AH-26 plus sealer using a continuous wave technique and backfilled. In group 2 (n = 15) Resilon was used as a filling material. Then teeth were sealed and kept in $37^{\circ}C$ and 100% humidity for 7 days. For retreatment, the samples were re-accessed and filling material was removed using Gates-Glidden burs and ProFiles. Teeth were sectioned longitudinally to compare the general cleanliness and amount of debris $({\times}\;75)$ using SEM. Chi-square test was used $({\alpha}\;=\;0.05)$ to analyze the data. The total time required for removal of filling materials was expressed as mean ${\pm}SD$ (min) and analyzed by the Student t-test $({\alpha}\;=\;0.05)$, Required time for retreatment was $3.25\;{\pm}\;0.32$ minutes for gutta percha/ AH 26 plus sealer and $3.05\;{\pm}\;0.34$ minutes for Resilon. There was no statistically significant difference between the two experimental groups. There was no significant difference between the groups in the cleanliness of the root canal wall. This study showed that Resilon was effectively removed by Gates-Glidden burs and ProFiles.
